Y'all joke but those three vertical cylinders are actually exactly how mega container ship engines are designed for exactly that reason: extreme efficiency. I've heard that the wartsilla RTA96 2 stroke ship engine is the most efficient motor ever built. It gets over 50% thermal efficiency, and has a theoretical peak thermal efficiency near 60%
You'd need an aircraft hangar's worth of space to change the conrods, and you wouldn't be able to fit under most bridges. At least it sounds great though
The two cylinders on the bottom go to the normal exhaust location, and the three on top have a pace style header that makes them wine like a supercharger.
BTW, if the inline 3 portion is a regular 120 degree crank, you can turn this into a far less scuffed inline 5. All you'd have to do is set cylinders 1 and 5 to reach TDC or BDC together, but be on opposite strokes (IE one on compression, the other on exhaust) while the middle 3 cylinders run as a normal inline 3 (but probably with a 2-4-3, 4-2-3, or 3-4-2 firing order instead of 2-3-4 or 4-3-2 for smoothness or whatever)
To make it even worse you could try to make the piston rods of the side cylinders only a fraction longer than the excentricity of the crank. That way it should run nice and uneven and might even get stuck under the right conditions. If both has the same length it will definitely get stuck.
There were engines somewhat similar to this, the small cylinder was called a “helper cylinder” and contributed to starting the engine since compression was very high. Typically in heavy Diesel engine equipment, sometimes they were on steam engines also
Id just imagine the lower part being mounted flat into the engine bay like how it's done in a Subaru. But with there being a useless thin but long tower with intake, exhaust, coolant pipes and fuel lines. A driveable eyesore.
People build crazy engine all the time and it seems to work just fine, I couldn't even get my I2 to run without stalling whenever put into gear, or even idle smoothly, I need a guide sooo bad lol

I think what makes it sound V6/Flat 6 like is how the middle three cylinders horizontally are firing exactly like a three cylinder engine..Then along with the configuration and the other two cylinders, it adds a sort of a V8 engine effect..at low RPMs
