I feel validated. I am obese type 2 diabetic and was a sugar addict. I have complex-PTSD and one of my coping mechanisms was sugar and carbs. I am on keto and use sugar substitute. It hasn't caused me to crave "real" sugar. It's helped me manage consuming no additional sugar and total carbs below 40 grams a day. Maybe one day I'll work on cutting back the sugar substitute but right now, it's helping me stop consuming sugar.

The problem with fake sweeteners is that they prolong the cravings and contribute to cravings. I started with not eating any of it. I didn’t do keto treats until holidays. I decided I didn’t even like them. I do think it is a progress staller for most people. Keep in mind the carb content in the artificial sugar is pretty high. It may not raise glucose in you but it could raise insulin. I do understand why you are recommending it to obese patients. I think fake sweeteners prolong the addiction to sugar though and the sooner you give up sweetness in your mouth, the sooner you will be free of the need for sweetness at all. I don’t crave it at all anymore.

  • @gratefulketo2211

Great info as usual! I know that artificial sweeteners do not raise your blood sugars but I’ve often wondered if they spike your insulin and slow down your weight loss. Does anyone know? I suppose that, in the end, it doesn’t matter as eventually all the weight comes off easily...if perhaps a bit slower.

    • @helenabell5616

For most people, sweeteners will spike your insulin. I drink diet coke, just like the doc said but then I do 18hrs of fasting daily. It's the fasting that helps with weight loss and I can enjoy my diet coke.

    • @gratefulketo2211
      @gratefulketo2211 4 роки тому +1

Helena Bell ... Fasting is definitely the way to go. Also, I have recently decided to drink any beverages that have sweetener during my eating window only. It seems to make sense to only incur any possible insulin spikes during this time in order to maximize my weight loss. Don’t know if it’s true but that’s what I’m trying.

    • @tracyandrirs8863

@@helenabell5616 how do you know that? People are not testing their insulin levels at home and there are very few studies showing that they do spike insulin. We should try not to generalize that something does something to most people.

    • @HuFlungDung2
      @HuFlungDung2 4 роки тому +5

@@crishamilton6678 Dr. Cywes is speaking primarily to those who have already gone off the rails and have attempted to recover from obesity or full blown type 2 diabetes. If that hasn't happened to you, then you have more freedom to experiment. Determine if you are triggered to binge, and what caused it.
The natural design of a human shows that we are meant to be able to process carbohydrates quite effectively and efficiently. But this ability needs to handled with conscientiousness in today's world of temptation. Get on top of fatty liver by keto or carnivore diet. Your liver can't buffer very much carb intake at once, but it can handle some. Exercise your muscles to drain the glucose stores so that they can help buffer your carbs. Intermittent fasting is also effective for this. Typically, I make more of an effort to do a longer intermittent fast if I am taking on some carbs. For me, some starchy vegetables are more satiating than they used to be before I went keto. As some have said, you become more sensitive to milder sweetness when you stop mainlining carbs.
Most of all, I think is to take in a high percentage of fat early in the feeding cycle to get satiety working for you. It is MUCH easier to control carb intake when your hunger is subdued. I use a tablespoon sized 'fat bomb' as a hunger killer, not an in-between meal snack. Fat bombs are typically sweetened artificially, because cacao or cocoa powder is on the bitter side.
Put butter or olive oil or coconut or MCT oil on everything. Get fats working for you. That's where satiety comes from.
My goto dessert is a sugar free naked cheese cake (almond flour or chopped nut base) and sugar free whipped cream. Portion size is still small, unlike what they'd serve you at Olive Garden. Their portion size is essentially about 8 times the size it should be, but of course theirs is loaded with sugar. Cheese cake toppings can be a sugar free pudding, or some cooked berries sweetened artificially. This is not low carb particularly, but it is much reduced from regular cheesecakes. If a small piece is not enough, then you didn't fat load sufficiently beforehand. Fat, fat, fat. Get enough fat.

Thank you so much for talking about this from the addiction model and harm reduction angle. Your videos ring so true for my own lived experience. I keep coming back to the experience that when I'm off sugar or starchy foods entirely, I'm like a 'normal' person. When I'm off sugar, starch, and grain I have no cravings and I'm not a fiend. When the carb count goes up, the energy peaks and valleys start and the binging on ice cream follows. It's very black and white for me. I do use liquid stevia in my coffee, but have been thinking that this too is a "crutch". If I buy Keto ice cream, I'll eat all the bars in the box. BUT, while I'm off sugar I can actually not buy them in the first place! That's not the case when I'm in high carb eating/addiction mode. I was JUST at the grocery and forced to stand waiting and the line was in the ice cream isle. I looked at the low carb/ice cream bars and said to myself "yeah, no, that's just not a good idea for me". That's alone is revolutionary. I've been on the yo-yo. I've done the relapse. I've been through this before. It's black and white. In one mode, I'm a junkie, and in the other I have the modicum of self control the "normal" people seem to have. So that's my truth. I'm not an alcoholic, but I'm a carboholic. But I could shrug my shoulders and not bother with the low carb ice cream bars tonight BECAUSE I'm eating non starchy raw vegetables (ie salad) every day with lots of healthy fat and protein. And that it. My idea of a binge tonight is basket of raspberries (and I don't don't do this every week), but I know Dr. Cywes, that's the junkie in me doing that. Thank you for helping all of us recognize this for what it is. If I binge on raspberries once in awhile, it's better than the half gallon of ice cream that pulls me into a two week bender. But a fix is still a fix.

As always, great advice Dr. Cywes. Like with so many things in life, whether to use artificial sweeteners or not, is a highly individual decision. For some, relying on artificial sweeteners maybe the crutch they need to get over for sugar addiction. For others, it maybe a gateway that makes them revert back to their old sugar eating habits. I belong to the latter category. Whether it is straight sugar or a sugar substitute, a little taste of something sweet puts me on the road to recidivism. So, while I am not a religious man, I do heed the statement made by St. Augustine: " To many, complete abstinence is easier than perfect moderation". Even a man who lived nearly 1700 years ago understood the nature of addiction, and how it exists on a spectrum. Hence, that is why he qualified his quote with "To many", indicating that this admonishment does not apply to all people. The purists within the dietary community, regardless of what side of the artificial sweetener debate they fall on, could learn a lesson from that.

The longer I am off sugar the less sweetner I need. Erythritol gives me stomach upset and raises my insulin a bit. I like the sparkling water with light flavor 0 calorie. However it has taken me a long time to get to a point where I don't like sweet stuff that much. If I do eat something with sugar the craving will start to return. There is also a lot of psychology connected with food. If I eat one thing with sugar I acknowledge it, enjoy it and don't worry or guilt myself about it. That kind of takes the power out of the sugary food in my mind so I don't binge and can move on.

I lost 20 pounds that year. I have never since went back to soda with sugar. I don't like it. I don't like how it coats the tongue. I don't like how it feels in the body. I no longer have a need for candy or anything packed with sugar.
If someone think it's the taste of "sweet" that is the issue, they are simply an idiot. Sugar is an energy packed stimulant that makes you high in a rush. It contributes to obesity which is linked with 11 forms of cancer. A sweetener have no effect.

I used vaping to get off real cigarettes. I had a 35 year smoking habit and couldn't laugh without nearly choking to death as I coughed. It was gross and embarrassing. I used vaping for about a year, then weaned off it and then was free. I hate the smell of cigarettes now and have no desire to vape. The freedom from dependency keeps me going. All the best to anyone needing to quit for their health and wellbeing. As for artificial sweeteners, I'm trying to keep them out of my life because they stall my weight loss for some reason..

I'm another long term smoker who used vaping to stop smoking cigarettes. (I had a nicotine-free vape.) I vaped for about 18 months after my last cigarette and then the vaping tailed off. The last cigarette was nearly 6 years ago, and I haven't had any relapses.
      .
I think it varies between people but for me, the most addictive thing about smoking was the ritual of lighting up and smoking a cigarette and doing things with my hands, and the nicotine just backed that up. Other people may be the other way around.
      .
Unlike quite a lot of other ex-smokers, I don't mind being around when people are smoking and I don't feel any desire to put on a big song and dance about it.

Stevia (erythritol), monk fruit, Sweet & low (phenylalanine) are money if you can't cut out sweet entirely. Splenda and equal will trigger an insulin response. No idea why equal does it. Splenda, I assume is because being made from sugar, your body can't tell the difference and responds the same. No response from the first 3 listed. My dad's diabetes was horrifically uncontrolled. I put him on a strict, regulated 10 carbs a day meal plan, but after a week his sugars were still in the mid 150-170's. Then I read sucralose (Splenda) is the sweetener in his flavored water bottles. Removed those, and his fasting/resting sugar came down to under 100 within 48 hours. Splenda is a tricky one, gang. If you see sucralose, drop it. :(

I was always baffled about people who promote the keto/carnivore lifestyle now touting snacks and desserts. Shouldn't they be talking about IF and 2MAD and OMAD as great options?
But it boils down to followers.
I chose to be a ketovore to save mylife, and the best side effect was the elimination of whole food groups from my life. Black and white. Protein, Yes. Fats, Yes. Carbs, No.
Unfortunately I'm now fighting my mental hunger. It's much harder and way more insidious. But I'm not going back.

Dr. Cywes. I feel like I agree with you 100% regarding artificial sweeteners as a transition tool. My only question is this. As we know elevated sugar levels are harmful to the body, we also know that elevated insulin levels are harmful. So, where you may be correct that the artificial sweeteners do not raise glucose levels, do they not however trigger an insulin response, which, is not a desired thing? Seems we would want to discourage things that elevate both.

My taste buds are super adaptive. Twenty years ago, I decided to stop drinking cola and began drinking diet cola and it took me about a month to acquire the taste. A few months ago, I stopped buying my most favoritest, bestest, drink ever; Diet Pepsi. I loved that tish and used to drink six or more cans a day. I picked up a can of Diet Pepsi on a hot day recently, thinking nothing of it until the liquid passed my lips at which point I recoiled. It tasted Nasty with a capital N. I much prefer a refreshing glass of water now and I'll never touch Aspartame or Ace-K again.

However, throughout the past five years I’ve started to abuse artificial sweeteners and tricked myself into thinking that they were OK to eat constantly because they were “keto “. I am now dealing with a lot of health issues due to aspartame I believe and I’m having to kick all sweeteners cold turkey. After really looking into aspartame, I can’t believe it’s allowed to be sold. Not all artificial sweeteners are a like! I have to stay away from the artificial ones and stick to stevia, monk fruit or more natural in extreme moderation.

The most valuable thing that I ever did on my diet was to start using a blood glucose monitor to see what is increasing MYblood sugar, your mileage may vary. The best no sugar margarita is made with pickle juice and club soda. The sin was the marketing vaping like cigarettes. Like quitting mountain dew by tapering/diluting with seltzer water vaping was the only thing that helped me quit smoking. I made my own vape juice with quality products then over two years reduced the amount of nicotine until the juice no longer had nicotine. Had vaping been marketed as a stop smoking aid instead of a cigarette alternative then we wouldn’t be having this conversation.

If you aren’t willing to tell people straight out that artificial sweeteners are not the answer to optimal health , and that they are not the harmless answer to getting away from sweets and carbs.
Then I can’t take you seriously.
And you can’t say that artificial sweeteners are the lesser of two evils AND say they are harmless both.
You say you tell it like it is , so tell it like it is.
The issue is not sugar , it’s insulin spikes , and there are some studies that show artificial sweeteners causing insulin spikes and in some cases even more than sugar.
So as you predicted you’ve gotten your flack.

My 3 year old brother was diagnosed in the 1970s with diabetes, after almost dying because he’d lost so much weight and was literally almost comatose and his pediatrician kept saying it was viral. Anyway, sweet n low was the only non-sugar sweetener back in the day and our whole family converted. Everyone said we would die. My parents are thin and healthy in their mid 70s. I’m the obese one. But I’ve felt guilty for using it while trying to do no sugar and for me it is a bridge. I don’t fix keto snacks with artificial sweetener but I do like it in my tea or coffee. That’s all. Now I can embrace my bridge!!! Lol!!! Thank you and most of all for your statement about, “no one has died from artificial sweetener”... love that! You are my go to doctor for keto as well!!! Thanks, but on the topic of artificial sweeteners every doctor there is say there's this lymbic reaction to sweet taste (of even artifical sweeteners) that immediately triggers insulin. I know your blood levels keep stable, but how about your insulin level?
In any case the most important take away for me is:
    1) artificial sweeteners are actually okay to some extent,
    2) artificial sweeteners don't actually raise blood sugar levels.

Artificial sweeteners trigger hunger, and therefore Ghrelin, the hunger hormone.Insulin has NOTHING to do with hunger so is not involved at this early stage as no food has been eaten yet
Insulin is only secreted AFTER food has been eaten, digested, absorbed into the bloodstream. It's job is to send nutrients used by the human body (Glucose NOT artificial sweeteners) out of the blood and into cells that need those nutrients. Any compound foreign to the body (e.g. sucralose, aspartame, splenda, sorbitol, xylitol etc..) will not be taken up by cells (as they don't use ANY of these chemicals), so Insulin is not required. They will be delivered to the Liver, via the bloodstream, for breakdown (if possible) or disposal
There are many hormones, each with very specific roles. Ghrelin triggers eating after a neurological (brain) signal driven by the the 'idea' of eating something yummy and sweet (very specific to the individual and their unique flavour preferences etc..). This has NOTHING to do with Insulin as we are talking about hunger
If the person eats the food, Insulin will only rise IF there are nutrients used by the body (glucose or amino acids)

I try not to consume artificial sweeteners as part of my daily diet, but an occasional Diet Coke or such really saves my sanity, when I‘m out and about and have to watch other people eat sweets or drink alcohol. Sugarfree drinks also help me stick to rather eating nothing at all, than something with carbs, when nothing keto approved is available in a given situation.
I definitely can’t handle ‚keto desserts‘ and such, though. I‘ll overeat on them and they are simply too energy dense for that. Healthy or not, simply from a weight management standpoint, they’re a big no-no for me. They totally trigger my addictive behavior with food.

2 weeks in and any sugar added to my caffeine tasted sickly sweet..
It's crazy how our bodies can adjust.
I felt fantastic after 10 days. Well worth it.
Please come off all those fake sugars, none.of them do you any good.
The best thing you can do is perhaps add cinnamon as this helps lower blood sugar.
      Or ground up dates..
Best option is to go cold turkey and switch to something like Greed tea for a more gradual & extended caffeine lift..
