I went on Keto diet and OMAD intermittent fasting. All my neuropathy pain went a way, prior to that I took r-lipoic acid as the same brand you recommended. I took 600-1200, it didn’t work as well as changing my diets. X-out all the processing food, refined carbs and sugars and omega- 6 oils. Straight Whole Foods and intermittent fasting! It also removed my osteoarthritis. It took effective within 21 days. I will continue with this diets and fasting the rest of my life. I love it. Let’s food be Thy medicine!
I am using HMS Nutrition LLC for R-Lipoic Acid. As for Keto diet, I am not a vegan Keto. I eat animal products , vegetables, nuts and seeds and combine with intermittent fasting. I eat twice per day with 18:6 Intermittent fasting for 5 days, then I eat one meal per day (OMAD IF) for the last two days of the week. I am on this Keto diet and IF, my body needs a lot of vitamin B complex and electrolytes. I drink two tsps of moringa powder daily to remove any oxidized LDL and provides 92 essential nutrients. I also take raw vitamin B complex, B12 has to be form methylcobalamine, not cyanocabanomin ( manmade). I drink 1 scoop of electrolytes powder per day. The 4 main electrolytes are potassium, sodium, magnesium and calcium. Everyone needs 4,700mg per day and sodium is half of that to get the cells to be hydrated and functions properly. It’s hard to get potassium. Eat bananas, it has a lot of starch and sugars, it’s not a keto friendly. Avocado has 76% fat, 16% carbs and 5% proteins. I eat two avocados per day. I also eat 1-2 tablespoons of northern white beans, it has the richest source of potassium, but it’s high in carbs. My advice is anyone decides to get on any special diets, needs to do a lot of research and understand the body needs. I recommended to watch the videos from Dr. Sten Ekberg, Dr. Eric Berg and Rayan Taylor. These ppl are very knowledgeable about nutritions, diseases and cancers. If you do the intermittent fasting, it’s a must in any diets you choose. Keto diet, you trained your body to use fat for fuels. Sugars damaged the liver, glucose damaged the brain cells. Use MCT oil while you are fasting, it feeds your brain, you feel the fullness and no cravings. Eat all your calories in one hour, then your next meal is at least 6-8 hrs window for beginners. Also, take 1-3 tsps of organic raw unfiltered apple cider vinegar in a cup of water 20-30 min before each meal. I enjoyed eating like this very much. My body prefers fat over sugars and carbs. I have a lot of energies, clarity, focus, well sleep and happy.
I did the same thing. Then went to an idiotic neurologist who tested me for neuropathy, and she said, "you don't have a neuropathy. Your nerves are healthy. But we'll do an mri just to make sure" I say idiotic because she was convinced neuropathic symptoms can't resolve with diet changes. The symptoms resolved when I changed my diet to mostly non-startchy vegetables and meat. I add in bits of yogurt and quinoa here and there, and a splash of berries and statchy veggies. Bits because these are quick to raise glucose levels and the primary key to fix neuropathy is eliminating surgar. Salt is low with this so I've got LMT electrolyte powder on the way, along with drinking chicken broth in the morning here and there. Keto will elevate your LDL, and make you risk gallstones, so I would recommend a slow switch to keto and not a drastic one. You fix one problem and it starts another. Prevent the gallstones by drinking 2.5 litters of water. I wouldn't do keto without research or with the guide of a doctor who can test your electrolytes cause you can mess yourself up pretty easily.
@@bleeblah3583 Yes to all of that... I would just add that from my research, I suspect ACTUAL KETO is very good for some specific problems and can be used as SHORT term treatment. But I have come to believe that most of the LONG TERM benefits attributed to Keto are actually derived from the "LOW-CARB" part of the diet and IF. Striving for CONSTANT DAILY KETOSIS may turn out to be a LESS optimal as a lifestyle.. All The Best, Stay Well : )
I was taking the cheap, s type crap, for several months -- zip, nada, nothing. I heard about the R type, got it the same day. Cost more!! Here's the thing, through my own personal experience, I have come to find that taking the Ayurvedic approach of counter-balancing, I utilize 1000-2500 mg Bitter Melon before each meal, way cheaper than buying fresh veg. Further, I've found that if I eat like, pumpkin seeds in the evening for their magnesium, my feet burn, so I can either take the bitter melon before I eat, or take Chromium Picolinate to neutralize the pain. Berberine (600 mg.) is also great before eating. Experimenting with each of these gives one a lot of options that actually work to eliminate the pain ASAP!!!!
R-ALA has helped me hugely with bad neuropathic widespred pain. I have/had tingling, numbness, deep pain, deep internal and external itching, shocks, severe nerve pain, and water trickling sensations, sensitive "sunburn feeling" skin on my back,shooting pains, stinging pain everywhere. My right upper outer thigh bringing me the very worst agonizing pain the longest..it is now numb, as are other areas on my body. Way less heat sensations felt ( not sure about cold sensations), on my legs, chest, back. Horrible heavy legs feelings like there is cement inside them. Originally (not knowing) I had the good R-ALA which was helping, then when i went to purchase more, they werent carrying that one anymore, so I bought a different one, which turned out to be RS-ALA and i noticed my pains returning. By the time i had almost finished the bottle, i discovered a medical video much like this one, explaining the differences of Alpha Lipoic Acid, I checked my bottle and sure enough, i had bought the lousy kind the second time. I ended up ordering the R-ALA on-line and have been back on the good one a few weeks now, again HUGE IMPROVEMENT I am feeling...its working! Thank you!!! ❤
I take alpha lipoic sustain with biotin for vegan Jarrow brand. I’ve been taking for about 4 years. Took for neuropathy but to my pleasant surprise I no longer have hypoglycemia symptoms which I had my whole life for me alpha lipoic is my miracle 🌈🌞
I’m excited for you and I’m hoping it helps me as much. Just bought this brand 2 weeks ago. Jarrow is the only brand I’ll buy because their products are legit. So fare I’ve noticed a huge energy boost. I feel more calm, not having anymore heart palpitations and the tingling I get in my legs and hands has almost disappeared. I am pre-diabetic. Can’t wait to get blood work done to see what my numbers are after taking these.

@@boomerang0101 Jarrow Formulas Alpha Lipoic Sustain with Biotin. It says you can take 1 300mg pill 1 to 2 times a day. I take two a day equaling 600 mg a day. I do one in the morning and evening. So far I’ve noticed huge burst in energy, no tingling through hands and feet, more calmness and I feel like it’s made my stomach feel way better. It’s still early so there may be more.
@@boomerang0101 oh also I am out of my add meds and can’t get a refill because the pharmacy is out of stock. This is helping md make it. Without my add meds my focus is off and I get very fatigued. It’s hard to get through the day. This gets me through the day easily and I do feel like I have been focused on it. Starting to wonder if I need my add meds anymore.
@@boomerang0101 one night I felt like it did, but I think because I took them to close together. I took the first one later morning. I was going to go down to one a day after that but something told me to keep taking two a day but try to take the first one as early as possibly.
Make sure to get the stabilized form of R-lipoic acid. The Cyclodextrin kind is the best. I've been taking it for a month and it's helped relieve the nerve pain I had in my back as well as relieve the numbing tingling I had in my thigh.
Congrats! You are the second doctor to mention the two types of ALA. I just found out what I've been taking probably has the type S in it. They are also mixing the two kinds to save money. Some call those a third type but it's really just the two. They should have to stop selling the type S. Type R is the natural kind.
I think the process for manufacturing synthetic ALA produces equal amounts of R (the naturally occurring one that works) and L (doesn't exist naturally, and won't work) which are not easy to separate, and that's why both together are cheaper.

@@red-baitingswine8816 They can and do. It is unethical. I used one for at least two years before I found out. There are other videos that specify the difference and that S blocks the R type. S does nothing so you pay for a worthless product. These companies only care about profits. They don't care if their product works or not, as long as you will buy them. Most people just think ALA is ALA and don't know about the differences. That is how those companies get away with it. I would rather pay a bit more for something that works as opposed to paying less for something that doesn't. If it doesn't work, then you might as well save your money and take nothing. The kind I buy now says on the label that it contains no type S and only contains type R. I've only been using it recently and can already tell an improvement in my neuropathy. Especially in my left hand which was the worst. Don't count on professionals knowing the differences. My doctor was the that suggested my trying ALA and he never mentioned the different types. They don't concern themselves with details about supplements. They concentrate on drugs. Through reading, I learned that because I take Metformin, I should be taking Ubiquinol which is active CoQ10 because Metformin depletes it. So I ask my doctor about it and he said yes I should be taking it. Yet he had not told me to take it before. I had to do my own research. So-called professionals know little about supplements.
The other doctor said there is a third kind. It is a mixture of the R and S type, which you may think is ok but still the same pro the S type cancels are benefit of the R Tyra.
You can take this compound at much higher levels than what is recommended on the supplement label from jarrow or any other company. My opinion is that time release works better because it only sticks around in the blood about an hour and then a little more but not very long. You can read up on this. If you're not getting results, it is safe to take much more. I take one to 2,000 mg a day sometimes when I'm really pushing my boundaries with overtime work. You have to spread your doses out throughout the whole day when you take much larger amounts. I've done this for years. It keeps me from getting sick and keeps me looking young.
Oh thats why I feel nervous tension with this .. I was told vit depleted B1, a calming vitamin... And I am deficient in B1. I take Benfotiamine to calm my nerves

the radical scavenging and chellating properties of ALA dont matter no matter if its the R or S form but the other effects such as the metabolic effects for blood sugar do matter. If you are taking it for diabetes then you need only the R form but if you are taking it for its radical scavenging properties then it doesnt matter.
I was having neuropathy. And have no reason. All test was normal non dibaetic. I tried every medicine nothing works. Finally i tried alpha loplic acid. 600mg was giving me vertigo. I go to 400 mg now months and my neuropathy is very much reduced. I feel like i was not having it
You must have brain MRI. Unfortunately if brain have lesions it leads to MS with such symptoms though all blood test and cervical X - Rays are ok.Same is with my case

You need twice as much of the racemic rs-ALA as you would r-ALA, but it's effective and economical and also more stable than r-ALA on its own. Eating sweets is not my favorite, but if i take a large portion of them, my blood sugar level rises high. I think it's the high concentration of the glucose flowing into the bloodstream all at once. Thanks for the great response Sandra! Som reports state that there is a 40% better absorption on an empty stomach. It is also important not to cook the vegetables as it does ruin the alpha lipoic acid. I do believe most stable formulations are sodium as it is a negatively charged acid otherwise. @@scottharm3932 It's actually both water AND fat soluble, which is one of the things making ALA so excellent. That said, it's absorbed with or without food, but seems to be better absorbed on an empty stomach and you're right about that.
Is the DL- Alpha Lipoic Acid the same as S-ALA? ( I tried DL to start because it was readily available) and all it did was cause an unpleasant urine smell, suggesting to me that it is not being absorbed at all, but just being flushed out as fast as it goes in. Then I switched to R-ALA (pricier) and developed a crazy itch all over my body even at only 200 mg/day. What gives??? I was taking it for neuropathic pain- not diabetic.

Lots of confusion in these comments, so let me try to clarify. The synthetic racemic mix of RS-ALA is not S canceling out R activity/bioavailability. Taking R-ALA is certainly active and bioavailable, but the RS-ALA mixture provides 50% of the active R-form and the S-form mixed in may in fact prevent the R-form from polymerizing (i.e., making it inactive). However, the relative effectiveness of pure R-form vs the RS-racemic mixture has not been quantified yet (but that doesn't mean the S-form is cancelling out the R-form activity). Here is the quote from the article (see also link below): "These results suggest that the R-enantiomer would be the most appropriate form to provide as oral supplements; however, S-LA in the racemic mixture may prevent the polymerization of R-LA and thereby enhance overall bioavailability. Hello!! It’s my first time taking alpha lipoic acid and I realized that none r or a or s is written, just THIOTACID, on the label just ALA. In my country no other alternative. Is this the right one I am taking!! I am suffering from leg numbness, lack of toes’ mobility & unbearable pain under foot - just right leg. The neurosurgeon recommended this…… thank you

Just knowing the chips will give me a horrible stomach ache from the grease cures my craving (bad oils w/ Omega 6 not 3, high temps break it down to toxic). Poor nutrition also causes cravings. Keto has helped me so much with all that.

Most of the research showing ALA works has been done using the racemic 50% mixture that you have. I think the doctor is wrong about r-ALA being much better, although it is twice as strong per mg, that much is true. I would, at least, use the supplement you have until a new r-ALA supplement arrives at your door if you do decide to switch and pay the higher cost.
@@christophdollis1955 - Thank you for this information, this neuropathy came about from the sciatica attack I had in Jan...I would like to repair this numbness as quick as possible, it's an uncomfortable feeling from my knee on the outside down to my foot. I will continue and finish what I have, will try one bottle of the r-ala, and then switch back..I want to at least try it and see....not a comfortable un-feeling if you know what I mean...Happy New Year to you and yours...may it be a healthy 2023...
@@Twins0302 You're welcome. Incidentally, benfotiamine (a synthetic vitamin B1 supplement) is a prescription drug for sciatica in Germany. It, or sulbutiamine or TTFD, two other fat-soluble forms, legit may help. Benfotiamine most reliably gets to the peripheral nerves: sulbutiamine and TTFD get to them peripheral nerves, but they pass the blood-brain barrier moreso than benfotiamine (which offers certain benefits, especially for some people's depression, energy, etc.) and so probably a bit less of them get to the peripheral nerves. I like all of these forms, and mostly use sulbutiamine personally.

They are the best for diabetic neuropathy. It works for me and I have been able to carry a cup of coffee. Neuropathy come to me because of Metformin because it kills vitamin Bs. I contacted a supplement manufacturing company with this question about R vs. S alpha-lipoic acid, and they said all the studies that indicate it is effective in treating neuropathy have been done with a racemic combination of R and S. They stand behind that research. Hard to tell who to believe. My own oncologist said it didn't matter.
It is difficult to 100% lock this down in all honesty, but this is a proposed theory. At the very least even in that case you are getting 50% of what you are expecting to be getting. Let me know if you can find anything extra 😊
I thought R α-lipoic acid was unstable alone and needed to be combined with S α-lipoic acid to be functional as a supplement, so unless the product specifically states R α-lipoic acid, your getting a 50/50 blend according to a nootropic experts
I have NOW ALA and contacted them - since they put nothing on label. They said it was a combo of both - so you are saying that is the best way to take it vs taking R+ exclusively?

I read comments on Amazon. There are various ALA bottles not labeled as R, just ALA. People are reporting good results from these supplements. So just assuming they don't work may not be a good assumption.

I make green smoothies with pineapple, ginger, lemon, spinach, water and a little pineapple juice. It is a good way to quickly get the greens. I don't have a fancy expensive blender, I use a food processor and then an emulsion mixer. I was skeptical but it actually tastes good.

There are no fully natural ALA supps available in South Africa - the best I can find is a combination of R-ALA and S-ALA with "50 percent more R-ALA" whatever that means. (50% more than what?) After writing to the supplier I was told they need to combine the two isomers because R-ALA on its own is too unstable and the S isomer is inactive but stabilizes the R form. So basically they are saying that the R form will still work even in combination. Is this true? Or does the S-ALA antagonise and effectively neutralize the R form?

There are 3 forms of Alpha Lipoic Acid on the market: R-Alpha Lipoic Acid. S-Alpha Lipoic Acid RS-Alpha Lipoic Acid. The R-Alpha Lipoic Acid is the only pure form and costs a lot more than the synthetic ones, which are a waste of money! It is vital that YOU high-light this fact to your viewers!

Jarrow has an r-alpha lipoic acid, but their other ALA supplements are 50% r, 50% s. This is called a "racemic mixture" and most of the studies showing ALA works have been done using THIS form. I don't think there is any good reason to pay extra for r-ALA (although you need twice the dosage of racemic ALA to equal the same dosage in r-ALA). That said, there is research showing that instant-release ALA is better absorbed than continuous-release ALA. Some of Jarrow's ALA (their "Sustain") are a combination of instant-release and continuous-release in a bilayer tablet. I think you're paying extra for this for no good reason to get a supplement that is less effective. nutricost has ALA you may want to consider: great value for the money, but any reputable supplement company's ALA should do.

You forgot to mention that some brands are labeled RS alpha lipoic acid just to confuse people into buying that brand. I love potato chips but they got too many carbs.
I'd add something - everybody should check - R-ALA should be stabilized - than is multiple times more effective! If is not written on the package - probably isn't! Also - I'd like to ask you - is it same thing with pyridoxine - HCl and pyridoxine - 5 - phosphate. Are they compet too as the S- ALA and R- ALA ? Is it safe to take them both? I use 50 mg of P-5-P and 10-12 mg of P-HCL 'cause P-HCL is, almost,in every suplement... I enjoy to watch your videos - some of them many times - it's very helpful. Greetings from Serbia!❤

However, due to theoretical interaction of ALC and K2 others recommend taking one in the morning while the other in the evening. @@JasonBuckman Jason is right, Jeanne: 50% S 50% R. Isolating the useless S isomer would be a waste of money and costly and no company would bother. I personally think RS-alpha lipoic acid is fine. Just double the dose you would plan to take if you were taking straight R-alpha lipoic acid. MAYBE the S competes a bit with the R, but since most of the research showing that alpha-lipoic acid works was done using RS-alpha lipoic acid (like you have), I can't imagine that the S form blocks the R form to a very great degree. But you can get straight R-alpha lipoic acid if you disagree and feel it would be more effective.

It is a Super R-ALA and is 200 mg once daily and advises not to take more. I am on a second bottle and have not noticed any difference in the tinnitus. If I'm not mistaken the S type doesn't act against the R type - it just doesn't do anything. The chemical process used to manufacture synthetic ALA produces equal amounts of R and S, which are not easy to separate. . Anyone who claims that the S type acts against the R type needs to cite a scientific study with those results. . Of course anyone is free to spend the extra money to get the pure R type.
I started with pain in the pancreas area, and after weeks, the pain spread practically to my entire back, mainly under the shoulder blades, the time came when it was unbearable and I couldn't sleep, I heard about R Alpha Lipoic Acid, and I investigated the correct dose for my neuropathy, I started taking 600 mg per day, and after a few weeks, the back pain practically disappeared, but I still had pain in the stomach area, so I increased the dose to 900 mg, and the pain on the right side The stomach area also disappeared, now, I'm just waiting to no longer have pain, and this happened in only two months, so it probably won't work for everyone, but in my case it has been a miracle, and I didn't have than spending money on doctors or hospitals, I have only spent a little more than $40.00 Dls, and I am extremely happy, so I do not agree with this Doctor's opinions, and like me, there are thousands of people with satisfactory results.

I went on Keto diet and OMAD intermittent fasting. All my neuropathy pain went a way, prior to that I took r-lipoic acid as the same brand you recommended. I took 600-1200, it didn’t work as well as changing my diets. X-out all the processing food, refined carbs and sugars and omega- 6 oils. Straight Whole Foods and intermittent fasting! It also removed my osteoarthritis. It took effective within 21 days. I will continue with this diets and fasting the rest of my life. I love it. Let’s food be Thy medicine!

I am using HMS Nutrition LLC for R-Lipoic Acid.
As for Keto diet, I am not a vegan Keto. I eat animal products , vegetables, nuts and seeds and combine with intermittent fasting. I eat twice per day with 18:6
Intermittent fasting for 5 days, then I eat one meal per day (OMAD IF) for the last two days of the week. I am on this Keto diet and IF, my body needs a lot of vitamin B complex and electrolytes. I drink two tsps of moringa powder daily to remove any oxidized LDL and provides 92 essential nutrients. I also take raw vitamin B complex, B12 has to be form methylcobalamine, not cyanocabanomin ( manmade). I drink 1 scoop of electrolytes powder per day. The 4 main electrolytes are potassium, sodium, magnesium and calcium. Everyone needs 4,700mg per day and sodium is half of that to get the cells to be hydrated and functions properly. It’s hard to get potassium. Eat bananas, it has a lot of starch and sugars, it’s not a keto friendly. Avocado has 76% fat, 16% carbs and 5% proteins. I eat two avocados per day. I also eat 1-2 tablespoons of northern white beans, it has the richest source of potassium, but it’s high in carbs.
My advice is anyone decides to get on any special diets, needs to do a lot of research and understand the body needs.
I recommended to watch the videos from Dr. Sten Ekberg, Dr. Eric Berg and Rayan Taylor. These ppl are very knowledgeable about nutritions, diseases and cancers.
If you do the intermittent fasting, it’s a must in any diets you choose. Keto diet, you trained your body to use fat for fuels. Sugars damaged the liver, glucose damaged the brain cells. Use MCT oil while you are fasting, it feeds your brain, you feel the fullness and no cravings. Eat all your calories in one hour, then your next meal is at least 6-8 hrs window for beginners. Also, take 1-3 tsps of organic raw unfiltered apple cider vinegar in a cup of water 20-30 min before each meal.
I enjoyed eating like this very much. My body prefers fat over sugars and carbs. I have a lot of energies, clarity, focus, well sleep and happy.
I did the same thing. Then went to an idiotic neurologist who tested me for neuropathy, and she said, "you don't have a neuropathy. Your nerves are healthy. But we'll do an mri just to make sure" I say idiotic because she was convinced neuropathic symptoms can't resolve with diet changes. The symptoms resolved when I changed my diet to mostly non-startchy vegetables and meat. I add in bits of yogurt and quinoa here and there, and a splash of berries and statchy veggies. Bits because these are quick to raise glucose levels and the primary key to fix neuropathy is eliminating surgar. Salt is low with this so I've got LMT electrolyte powder on the way, along with drinking chicken broth in the morning here and there. Keto will elevate your LDL, and make you risk gallstones, so I would recommend a slow switch to keto and not a drastic one. You fix one problem and it starts another. Prevent the gallstones by drinking 2.5 litters of water. I wouldn't do keto without research or with the guide of a doctor who can test your electrolytes cause you can mess yourself up pretty easily.
@@bleeblah3583 Yes to all of that... I would just add that from my research, I suspect ACTUAL KETO is very good for some specific problems and can be used as SHORT term treatment. But I have come to believe that most of the LONG TERM benefits attributed to Keto are actually derived from the "LOW-CARB" part of the diet and IF. Striving for CONSTANT DAILY KETOSIS may turn out to be a LESS optimal as a lifestyle.. All The Best, Stay Well : )

Very good motivational inspiration!!! Here's the thing, through my own personal experience, I have come to find that taking the Ayurvedic approach of counter-balancing, I utilize 1000-2500 mg Bitter Melon before each meal, way cheaper than buying fresh veg. Further, I've found that if I eat like, pumpkin seeds in the evening for their magnesium, my feet burn, so I can either take the bitter melon before I eat, or take Chromium Picolinate to neutralize the pain. Berberine (600 mg.) is also great before eating. Experimenting with each of these gives one a lot of options that actually work to eliminate the pain ASAP!!!!
R-ALA has helped me hugely with bad neuropathic widespred pain. I have/had tingling, numbness, deep pain, deep internal and external itching, shocks, severe nerve pain, and water trickling sensations, sensitive "sunburn feeling" skin on my back,shooting pains, stinging pain everywhere. My right upper outer thigh bringing me the very worst agonizing pain the longest..it is now numb, as are other areas on my body. Way less heat sensations felt ( not sure about cold sensations), on my legs, chest, back. Horrible heavy legs feelings like there is cement inside them. Originally (not knowing) I had the good R-ALA which was helping, then when i went to purchase more, they werent carrying that one anymore, so I bought a different one, which turned out to be RS-ALA and i noticed my pains returning. By the time i had almost finished the bottle, i discovered a medical video much like this one, explaining the differences of Alpha Lipoic Acid, I checked my bottle and sure enough, i had bought the lousy kind the second time. I take two a day equaling 600 mg a day. I do one in the morning and evening. So far I’ve noticed huge burst in energy, no tingling through hands and feet, more calmness and I feel like it’s made my stomach feel way better. It’s still early so there may be more.
@@boomerang0101 oh also I am out of my add meds and can’t get a refill because the pharmacy is out of stock. This is helping md make it. Without my add meds my focus is off and I get very fatigued. It’s hard to get through the day. This gets me through the day easily and I do feel like I have been focused on it. Starting to wonder if I need my add meds anymore.

Congrats! You are the second doctor to mention the two types of ALA. I just found out what I've been taking probably has the type S in it. They are also mixing the two kinds to save money. Some call those a third type but it's really just the two. They should have to stop selling the type S. Type R is the natural kind.
I think the process for manufacturing synthetic ALA produces equal amounts of R (the naturally occurring one that works) and L (doesn't exist naturally, and won't work) which are not easy to separate, and that's why both together are cheaper.

The video gave no evidence that the S blocks the R - no studies, let alone controlled experiments. I suspect it's common knowledge among professionals that the R/S mixture works just fine.
@@red-baitingswine8816 They can and do. It is unethical. I used one for at least two years before I found out. There are other videos that specify the difference and that S blocks the R type. S does nothing so you pay for a worthless product. These companies only care about profits. They don't care if their product works or not, as long as you will buy them. Most people just think ALA is ALA and don't know about the differences. That is how those companies get away with it. I would rather pay a bit more for something that works as opposed to paying less for something that doesn't. If it doesn't work, then you might as well save your money and take nothing. The kind I buy now says on the label that it contains no type S and only contains type R. I've only been using it recently and can already tell an improvement in my neuropathy. Especially in my left hand which was the worst. Don't count on professionals knowing the differences. My doctor was the that suggested my trying ALA and he never mentioned the different types. They don't concern themselves with details about supplements. They concentrate on drugs. Through reading, I learned that because I take Metformin, I should be taking Ubiquinol which is active CoQ10 because Metformin depletes it. So I ask my doctor about it and he said yes I should be taking it. Yet he had not told me to take it before. I had to do my own research. So-called professionals know little about supplements.
The other doctor said there is a third kind. It is a mixture of the R and S type, which you may think is ok but still the same pro the S type cancels are benefit of the R Tyra.
It did help a lot for my sciatica for less than a month, to the point that I stopped taking it, for awhile already, but pain is kinda coming back!
You can take this compound at much higher levels than what is recommended on the supplement label from jarrow or any other company. My opinion is that time release works better because it only sticks around in the blood about an hour and then a little more but not very long. You can read up on this. If you're not getting results, it is safe to take much more. I take one to 2,000 mg a day sometimes when I'm really pushing my boundaries with overtime work. You have to spread your doses out throughout the whole day when you take much larger amounts. I've done this for years. It keeps me from getting sick and keeps me looking young.

the radical scavenging and chellating properties of ALA dont matter no matter if its the R or S form but the other effects such as the metabolic effects for blood sugar do matter.
If you are taking it for diabetes then you need only the R form but if you are taking it for its radical scavenging properties then it doesnt matter.

You need twice as much of the racemic rs-ALA as you would r-ALA, but it's effective and economical and also more stable than r-ALA on its own. Instant release also seems to be MORE effect than continuous release.

Som reports state that there is a 40% better absorption on an empty stomach. It is also important not to cook the vegetables as it does ruin the alpha lipoic acid. I do believe most stable formulations are sodium as it is a negatively charged acid otherwise.

@@scottharm3932 It's actually both water AND fat soluble, which is one of the things making ALA so excellent. That said, it's absorbed with or without food, but seems to be better absorbed on an empty stomach and you're right about that.
Is the DL- Alpha Lipoic Acid the same as S-ALA? ( I tried DL to start because it was readily available) and all it did was cause an unpleasant urine smell, suggesting to me that it is not being absorbed at all, but just being flushed out as fast as it goes in. Then I switched to R-ALA (pricier) and developed a crazy itch all over my body even at only 200 mg/day. What gives??? I was taking it for neuropathic pain- not diabetic.
I have nephropathy in both of my feet after hip surgery. ANA has controlled the pain, at 600 mg x2 daily. If I miss a doses I feel it.

Lots of confusion in these comments, so let me try to clarify. The synthetic racemic mix of RS-ALA is not S canceling out R activity/bioavailability. Taking R-ALA is certainly active and bioavailable, but the RS-ALA mixture provides 50% of the active R-form and the S-form mixed in may in fact prevent the R-form from polymerizing (i.e., making it inactive). However, the relative effectiveness of pure R-form vs the RS-racemic mixture has not been quantified yet (but that doesn't mean the S-form is cancelling out the R-form activity). Here is the quote from the article (see also link below): "These results suggest that the R-enantiomer would be the most appropriate form to provide as oral supplements; however, S-LA in the racemic mixture may prevent the polymerization of R-LA and thereby enhance overall bioavailability. Just knowing the chips will give me a horrible stomach ache from the grease cures my craving (bad oils w/ Omega 6 not 3, high temps break it down to toxic). Poor nutrition also causes cravings. Keto has helped me so much with all that.

Most of the research showing ALA works has been done using the racemic 50% mixture that you have. I think the doctor is wrong about r-ALA being much better, although it is twice as strong per mg, that much is true. I would, at least, use the supplement you have until a new r-ALA supplement arrives at your door if you do decide to switch and pay the higher cost.

Incidentally, benfotiamine (a synthetic vitamin B1 supplement) is a prescription drug for sciatica in Germany. It, or sulbutiamine or TTFD, two other fat-soluble forms, legit may help.
Benfotiamine most reliably gets to the peripheral nerves: sulbutiamine and TTFD get to them peripheral nerves, but they pass the blood-brain barrier moreso than benfotiamine (which offers certain benefits, especially for some people's depression, energy, etc.) and so probably a bit less of them get to the peripheral nerves.
I like all of these forms, and mostly use sulbutiamine personally.

Amazingly...it MIGHT be lightly helping me. I've inconsistently used about 300-600 mg./day of stabilized R-ALA for 20 days or so and it SEEMS to help a touch with foot numbness.

I contacted a supplement manufacturing company with this question about R vs. S alpha-lipoic acid, and they said all the studies that indicate it is effective in treating neuropathy have been done with a racemic combination of R and S. They stand behind that research. Hard to tell who to believe. My own oncologist said it didn't matter.
I'm a chemist interesting analysis, never thought about the S-form being an antagonist.
It is difficult to 100% lock this down in all honesty, but this is a proposed theory. At the very least even in that case you are getting 50% of what you are expecting to be getting. Let me know if you can find anything extra 😊
I did spend a decent about of time trying to confirm some of these claims and the evidence is not overwhelming one way or the other.

I read comments on Amazon. There are various ALA bottles not labeled as R, just ALA. People are reporting good results from these supplements. So just assuming they don't work may not be a good assumption.

I make green smoothies with pineapple, ginger, lemon, spinach, water and a little pineapple juice. It is a good way to quickly get the greens. I don't have a fancy expensive blender, I use a food processor and then an emulsion mixer. I was skeptical but it actually tastes good.

There are no fully natural ALA supps available in South Africa - the best I can find is a combination of R-ALA and S-ALA with "50 percent more R-ALA" whatever that means. (50% more than what?) After writing to the supplier I was told they need to combine the two isomers because R-ALA on its own is too unstable and the S isomer is inactive but stabilizes the R form. So basically they are saying that the R form will still work even in combination. Is this true? Or does the S-ALA antagonise and effectively neutralize the R form?

I got hand and finger tremors from taking alpha lipoic acid (S+R combo). Did anyone have this issues?
There are 3 forms of Alpha Lipoic Acid on the market:
R-Alpha Lipoic Acid.
S-Alpha Lipoic Acid
RS-Alpha Lipoic Acid.
The R-Alpha Lipoic Acid is the only pure form and costs a lot more than the synthetic ones, which are a waste of money!

Alpha Lipoic acid has been tested on mice and apart from the benefits, tests showed that the lifespan of the mice who were give ALA was decreased by around a third.
Does anyone know about [so-called] bio enhanced or bio actaved ALA? Or is straight R-ALA 600 mg. all one needs for macular & cataract eye health & prevention?
I bought R-ALA and then read it can take mercury from the amalgam fillings and possibly move through the blood brain barrier and get deposited into the brain. So I've been afraid to try it. As I am a type 2 diabetic. Anyone else heard of this?

I’ve been taking the alpha lipoic acid for about 2 months now and it hasn’t done anything..so now i ordered the R-alpha I will give that a shot …I’m so miserable
I’m surprised you recommended one with both R & S in them since the S cancels out the R!
Tried R-ALA (only 100mg) and it gives me a terrible case of heartburn. Don't know what to do, bc I really want to get off gabapenten.
My experience is to take ALA with a full meal. Not a snack.

Amazon is probably the best bet!
Jarrow has an r-alpha lipoic acid, but their other ALA supplements are 50% r, 50% s. This is called a "racemic mixture" and most of the studies showing ALA works have been done using THIS form. I don't think there is any good reason to pay extra for r-ALA (although you need twice the dosage of racemic ALA to equal the same dosage in r-ALA).
That said, there is research showing that instant-release ALA is better absorbed than continuous-release ALA. Some of Jarrow's ALA (their "Sustain") are a combination of instant-release and continuous-release in a bilayer tablet. I think you're paying extra for this for no good reason to get a supplement that is less effective.
nutricost has ALA you may want to consider: great value for the money, but any reputable supplement company's ALA should do.

It would say if it was R before ALA on the bottle.
You forgot to mention that some brands are labeled RS alpha lipoic acid just to confuse people into buying that brand.

I'd add something - everybody should check - R-ALA should be stabilized - than is multiple times more effective! If is not written on the package - probably isn't! Also - I'd like to ask you - is it same thing with pyridoxine - HCl and pyridoxine - 5 - phosphate. Are they compet too as the S- ALA and R- ALA ? Is it safe to take them both? I use 50 mg of P-5-P and 10-12 mg of P-HCL 'cause P-HCL is, almost,in every suplement... I enjoy to watch your videos - some of them many times - it's very helpful. Greetings from Serbia!❤

There are two schools in this topic. Some believe that due to lack of any empirical adverse effect ALC and K2 may be taken at the same time. However, due to theoretical interaction of ALC and K2 others recommend taking one in the morning while the other in the evening. Both methods are in use and there are practical and theoretical arguments supporting both way of taking ALC and K2.

My supplement doesn't have the R or S on it. How can I tell? Do I assume it's an S if there's no R? I'm switching to your recommendation. Neuropathy pain is hard to live with. I don't have diabetes tho thank goodness. Thanx for the info!

Isolating the useless S isomer would be a waste of money and costly and no company would bother.
I personally think RS-alpha lipoic acid is fine. Just double the dose you would plan to take if you were taking straight R-alpha lipoic acid.
MAYBE the S competes a bit with the R, but since most of the research showing that alpha-lipoic acid works was done using RS-alpha lipoic acid (like you have), I can't imagine that the S form blocks the R form to a very great degree.
But you can get straight R-alpha lipoic acid if you disagree and feel it would be more effective.

@@DrTomBiernacki Great video and info, thank you. I started R-Lipoic Acid after learning it was suppose to also be good for anti aging and tinnitus. It is a Super R-ALA and is 200 mg once daily and advises not to take more. I am on a second bottle and have not noticed any difference in the tinnitus. But did notice I still get older by the day lol. Perhaps I will try one you have listed. Thx again!
If I'm not mistaken the S type doesn't act against the R type - it just doesn't do anything. The chemical process used to manufacture synthetic ALA produces equal amounts of R and S, which are not easy to separate.

Anyone who claims that the S type acts against the R type needs to cite a scientific study with those results.

I started with pain in the pancreas area, and after weeks, the pain spread practically to my entire back, mainly under the shoulder blades, the time came when it was unbearable and I couldn't sleep, I heard about R Alpha Lipoic Acid, and I investigated the correct dose for my neuropathy, I started taking 600 mg per day, and after a few weeks, the back pain practically disappeared, but I still had pain in the stomach area, so I increased the dose to 900 mg, and the pain on the right side The stomach area also disappeared, now, I'm just waiting to no longer have pain, and this happened in only two months, so it probably won't work for everyone, but in my case it has been a miracle, and I didn't have than spending money on doctors or hospitals, I have only spent a little more than $40.00 Dls, and I am extremely happy, so I do not agree with this Doctor's opinions, and like me, there are thousands of people with satisfactory results.
