      Would this help with a bilevel machine with easybreathe or a similar feature enabled? These machines ramp up the onset of the inhalation pressure per breath already. So would V-COM help here or would it slow the inhalation flow too much?

    I got different results, but as you said everyone is unique. I use the V-Com for comfort and leak reduction. Airsense 11, Wellue O2 ring, P10 mask, 11-18 pressure (w/V-Com), no EPR. I bumped up my min pressure from 10 to 11 when I started using the V-Com to compensate for the resistance and I am not seeing any 02 drops or reductions in volume over what I had without it. My mouth leaks are almost completely gone tho, and it feels easier to breathe. I think that last guy bumped his pressure up too much (or possibly left his EPR on which fights the V-Com), but I'm no expert. Wonderful, thorough study - I appreciate that you are willing to dig into everything in such detail. Thank you for your informative videos!

I didn't get to mention it in the vid because the recording was corrupted but there was a very significant drop in central apnea and also total time in apnea also.     When I first read about this non active device my thoughts were that it cannot really have a positive effect because it is restricting airflow, your data shows an oxygen reduction although AHI looks "better" in some cases. This could be due to the Resmed's ability to detect these events, which you have previously shown to be not the best, being further compromised by the device. One of the detection techniques that the Resmed uses is Forced Oscillation Technique (FOT) to determine the state of the patient’s airway during an apnea. When an apnea has been detected, small oscillations in pressure [1 cm H2O (1 hPa) peak-to-peak at 4 Hz] is send, this works like echo sensing and I foresee that the device will severely compromise these results making them "look better"!

      My concerns are that the pressures and other stats (including AHI and leak) cannot be accurate as the VCOM is introducing a “blockage” between the person and the CPAP device. I definitely agree that people could be finding it easier to use CPAP devices with a VCOM as they are not experiencing the expected pressures. Has anyone tested the pressures in the mask after a VCOM is applied and then compare it to what the CPAP device is saying?

    Loved that you did this. What would be great is for you to repeat the study where you have users use their CPAP alone without EPR, CPAP with EPR, and then CPAP with Vcom. Would be interesting to compare the endpoints you looked at between EPR and Vcom to see if further reinforces that reduction in pressure during inspiration makes sense. Remember flow limitation/airway obstruction occurs during inspiration.

    Excellent, excellent analysis and great video. I like going into the data and am using the V-com with a Bleep Eclipse at 13 cm water. I love it but will try it more without the V-com now to dive into the data changes. Thanks mate!

    Thank you, uncle Niko. The video convinced me I do not have a problem that needs fixing with VCOM. I use Airsense 11, O2 ring magic upload my AHI (POS) averages 1.3. My ODI averages 0.3 and my oxygen averages 97%. Yes, I can have occasional bouts of CA.’s but they appear to be of no consequence and have zero effect on my oxygen. Leak rate averages 0-9 with occasional excursions to 20.

    I was 4-6 ahi and then increased my pressure, added vcom and epr 2 (with the help of chatgpt) and now < 3 ahi so pretty happy with results 👍

    I got one for free when I bought my first cpap (Airsense 11) from Lofta after an at home study. So I have nothing to compare it to as I never tried without however I adjusted to CPAP very well and I assume this played a part in it. Thanks to your videos and help I was able to dial in my pressure settings and have had great results. Thanks!

    Uncle Nicko> I would be very interested in a new week study, where the participants went from using the Resmed "pillow" setting to "nasal" and "full face" setting, using a nasal mask like the Resmed n20.
    As the only setting changing.
    Curious if this would give close to same effects as the v-com! 😉
    Btw Nick, have you ever looked into the raised concearns, that the v-com may lead the CPAP machine's algorithm to generate inaccurate data?
    Check the comments section of Jason's latest video, where he tries to show how v-com affect breathing.
    Someone mentioned the v-com sqewed the recorded data, as the airflow "manipulation" falsely/artificially made the machine do things like set fever hypopnea flags, because the v-com "tricks" the thresshold limitations set by the Resmed algorithm.
    Would be nice if you could make a video covering these concearns as well! 😉

      i commented in that thread after the one guy mentioned that. i'd be curious to see if that were true. i've been using v-com more or less straight for a year now. my flow limits and leak rates are much better when i use it, vs epr, etc.
      i use CPAP only, no APAP.
      i'm not a big proponent of epr as a "comfort" setting. v-com seems more natural to me to be (perceived) working harder to exhale than inhale, as opposed to the easier exhaling of epr.
      but that's me.
      But if v-com is skewing the flow limit data and certain flags, that would be good information to have. one person commenting in a video isn't exactly scientific.
      My machine is the AS10.

    I’d like to see sleep study data from the devices they use to determine if you need cpap therapy. When not using the vcom and compared to using the vcom. Putting this device in line is going to impact the data the machine is recording. I’d put far more significance on data from outside the cpap machine that isn’t being impacted by the vcom device.

      I originally thought the same thing, according to the white paper. The CPAP device does not detect the V-Com™. The V-Com™ introduces a known amount of non-compensated resistance into the PAP circuit, which the CPAP device cannot compensate for. This means that the PAP device continues to operate as usual, but the presence of the V-Com™ results in a reduced inspiratory positive airway pressure (IPAP) and preserved expiratory positive airway pressure (EPAP).
      Specifically, the V-Com™ takes advantage of the pressure-flow characteristics associated with turbulent flow. During inspiration, the high flow through the V-Com™ causes a significant pressure drop, which reduces IPAP. During expiration, the flow is minimal, so the pressure drop is also minimal, preserving EPAP. This selective pressure drop helps improve comfort without significantly affecting the overall therapy delivered by the CPAP device.

Testing with outside equipment would go far to help with people skeptical about what this device is doing. its a very simple device and this type of airflow restrictor is used in many other places. you're in a good position to complete a test like this and get the results out to the community! thank you for all the hard work and efforts you go to for all CPAP users!

    I have had one for six or so months. Oscar data looks a bit smoother and there are less flagged events. Resmed shows my AHI under 1 , for whatever that is worth. I don't feel better (or worse). The machine no longer shuts itself off when I remove the mask.

    I'm a sleep doctor for 22 years. So that, I'd like to know how does this device work intrinsecally? These data does not explain how is it possible. My apologize for my english.

It's bascially a flow resister that softens the airflow velocity and reduces ipap by 1-2cm without reducing epap. Their ideology is ipap is less important for therapy efficacy and causes more harm than good. In many ways, they are right. Cheers

    I find that I don’t trust the data from the machine with the V-Com in place.
    I do find that it does help with aerophagia and leaks but the rest of the data looks like it’s masked by it. Don’t really know if it’s working or hurting.
    I would have sent in my data but the O2 ring gave up after day 3. New one was ordered.

    Was Phil using it in auto adjusting mode or fixed pressure? Would it have been the same change if it was auto adjusting? Thanks for the info I just ordered one to try. Also I have EPR on should I turn it off when using the V-com or leave it on?

      Phil was on fixed pressure my friend. If he was on Automatic i would anticipate his pressure delivery would be higher due to the increase in upper airway resistance. The EPR question is a good one and probably needs a little more investigation. I'd leave the settings the way you have them now to start and see how you go. Cheers

      @@CPAPReviews tried it last night. It was noisy and raised my pressure so much I felt it in my ears. like I was at a depth right before the pain would start if I went any deeper. Definitely NOT for me, I'll be sending it back for $30. I messed around with EPR and when it kicked on it was too abrupt, so my advice to those using it would be to turn the EPR off, but it's really evident when using it. Thanks but no thanks for me. FWIW this thing should only cost $5.00 not $30.00 it's just plastic no moving parts like I expected.

    So I switched from full mask to nose pillow…and had a cold! No nose breathing…why can we not have both nose and mouth masks..switching out as needed

    This was a really fantastic video. It really helped answer a lot of the questions I was wondering about but I still have a couple: 1. Is there any relationship between the effectiveness of V-Com and the type of mask used? and 2. How do you know that the v-com isn't distorting the data? Also, as far as I can tell, the research on this product by the vendor has not been peer review? Is that right? I guess I'm nervous to put something in between me and a medical device that has not been fully vetted. Am I crazy? Lastly, I did buy one but I haven't been able to use it because I have an n30i mask and Sleeplay doesn't sell the small dick attachment. I only like to buy from Sleeplay because I trust them.

    Thank you for such great info. I have one point to make. You made a big deal out of statistical significance of much of the data but, statistical significance and physiologic significance are not the same thing. Many of the people did have a drop on blood O2 but only of maybe 2-3% or less. Going from 98% to 96% especially when sleeping is not likely physiologically significant. I wonder also about the other perameters you discussed. You did convinve me to get an O2 ring and to strt using sleep HQ. Thank You.

    In my experience, VCom works best when my pressure is 8 CM or higher.10 CM with epr of 2 works for me. I use nasal pillows.

    Why can't CPAP machines just use less pressure, so devices like this wouldn't even be needed?

    Reduction in Minute Ventilation is a great thing. Ventilation rate (X) Tidal Volume = Min Ventilation. So a reduction in Ventilation rate OR Tidal volume will reduce minute ventilation as long as a reduction in one does not cause a larger jump in the other. This device reduces both in the average patient and decreases the minute ventilation in ALL subjects. Some may ask, "Why would you want to reduce the air you breathe at night? The more oxygen the better, right?" No, overbreathing leads to excess oxygen which increases oxidative stress, reduces oxygen uptake, and leads to many problems. Shoot for Min vent under 5 LPM. There is an informative book called "The Oxygen Advantage" worth reading if you want to learn more about overbreathing.

      It really comes down to what the cause of that reduction is. When the cause of the drop is an increase in upper airway restriction during inhalation and it results in a drop in O2 then it's not a good thing. Cheers

      Hey buddy, thanks for the support. I just checked, and it looks like there are no videos about it in the SleepHQ academy yet, so I'll have to put one together. I still personally prefer to go off the SpO2 data as I'm not sure how accurate the CPAP calculations are, but the data is certainly useful as a guide. More often than not, a reduction in tidal volume and minute ventilation will result in lower SpO2 data (not always, though).

    I don't trust AHI hardly at all. I have normally been between 1-3 AHI average. Then for like 2 weeks I got between 0.2 - 0.8 AHI. Don't know what changed. I didn't mess with anything besides I've been doing neck exercises for posture. So that's all I can possibly tie it to. Still don't feel amazing sleeping tho so I've given thought to getting one of these VCOM devices to see any changes. I already tried 2 different masks that haven't worked, so this $30 item wouldn't be much difficulty. But I'm supposed to go in for an in-lab sleep study for a BiPAP since most of my events are labeled as Central Apnea. The issue is my sleep doctor said I never had one Central apnea on his study. So who's wrong? Dunno. My numbers are still good according to the machine's data, sleep seems okay--but not phenomenal.

    The only patient I've tried with V-COM preferred therapy without the device.

      It can be hit an miss for some and often you just need to play around with the pressure levels a little bit. Ie, you might need to increase the ipap a bit (sounds counterproductive i know) but his device not only drops the pressure, it really drops the velocity which is why there's the big improvement in leak rates etc. I'd encourage you to keep handing it out to patients especially beginners.       @@mews1950 I felt the same and that’s the main reason I upped the pressure to 15.6. This seems to be the highest pressure I can use on my vitera full face mask before the leaks start while using the vcom. Without the vcom it was only 13. I’ll soon have a Lowenstein Lena mask which is designed for really high pressures. Should be more than I’ll need to try to recover the flow limitations. I really want to try to make the vcom work because having less leaks makes for a better nights sleep, for me at least.

    Is there a minimum pressure where VComm would not be affective/beneficial below that minimum pressure (assuming fixed pressure).

      no i don't think so, however the lower the original pressure and flow, the lower the drop in the inhalation pressure caused by V-Com. For example, a ResMed device will provide a flow of 48 L/min for a pressure of 16 cm H2O with a full face mask, resulting in a decreased pressure of ~1.6 cm H2O using the V-Com™ device. The higher the pressure, the higher the flow, the greater the drop in inhalation pressure from V-Com but i don't think it can drop the ipap any lower than 2cm (don't quote me on that though!)

    I can't buy in without an in depth explanation. I don't see how this works. Having done other testing with vents and lung simulators this device confuses me. Admittedly not hard to do.

    Unfortunately, with only 18 people in the study there's zero statistical significance because your sampling size is too small.

      Statistical significance can still be achieved with a small sample size if the effect size is large and the variability is low

      @@CPAPReviewsThe size of the effect is independent of how statistically significant the effect is. I learned this in a memory lab at the University of Pennsylvania. However, you’re right that you can achieve statistical significance even with a small number of people, depending on the details. Thanks for your videos!

      This Gent giving us great CPAP information and has been doing so for some time …18 Guinea pigs coming back with positive feed back would warrant one to possibly use this new device ….why go into king of the fraternity dummy throwing out of the pram geek mode thus raining on this great guys parade ….after the effort and research he’s done on this product Im going to try to source one ….give the guy some credit .

    It looks like it simply restricts pressure so changes are not as abrupt, maybe just change the cpap settings to soft and maybe a slightly lower min pressure. The reduction in air and oxygen is of concern. Agree measuring o2 is a key to truely understanding how effective the treatment is.

      Well, I found the answer myself. There is a 3d print file with multiple levels of restrictions. Anyone happen to know the diameter of the restrictor holes?

      The inhalation pressure drop is much less at lower pressures some members have reported a feeling like there's not enough airflow. Cheers

    SOOOOOO PLEASE READ AUSIE MAN! XOXO. I wanted to bring up and important point, in the way you explain your Vt Tidal Volume and the way you explain the Minute Ventilation...... You keep explaining it as in the device gives you 350 mL of air which is significantly less than that of the previous 400mL without VCOM..... And sounding like your bashing the fact the device GIVES less.. WHEN its actually WE demand less.... If our bodies wanted more Vt or MV it would get it, our work of breathing would just increase. You SHOULD be wording it. Your Bodies demand is 50 mL less, which means the amount of Minute Ventilation you breathed Went down by 12.97% change.... BECAUSE if we wanted more, we would get more, and we would breathe more.... The device is NOT going to change that, WE will just increase our Work of breathing to get whatever Minute Ventilation we demand. So INN my Opinion, due to the leaking being 55% less, due to the pressures being easier tolerate, due to increased comfort, and the other Positive things you mentioned using VCOM, your body has to work less, due to less restrictions, requiring less air.. which is better, thats just my two cents.... You could have changed the whole viewpoint of alot of important key factors..... ALso I am a RRT.

      I appreciate the insights. The notion that the body will simply "take what it needs" negates the fundamental issue at the heart of sleep apnea. The entire point of using CPAP therapy is to address the body's inability to get adequate airflow due to upper airway restrictions, particularly during inhalation. Despite how hard or fast one might breathe, the airflow will diminish significantly if the upper airway is restricted. This is why APAP devices, increase the pressure in response to airflow limitation and other forms of airflow resistance. You can't fill a bucket if your standing on the hose.
      Additionally, regardless of tidal volume and minute ventilation, our study observed significant changes in airflow limitation. Some participants also noticed notable changes in SpO2 levels. These findings underscore that lowering the inspiratory pressure might result in negative outcomes for some. Lucky we developed the platform and tools so CPAP users can try it and see the results for themselves. Cheers

      @@CPAPReviews TO add to your point, unfortunately I do NOT agree with the point. The reason is that, you are comparing With and WITHOUT CPAP. When there is a physical block in the airway.... THIS is the SOLE reason we are getting LESS MV and our Sp02 Drops... , BUT without question we are working WAYYYY harder... Now when your comparing VCOM to without VCOM, the airways with and with out are both open... The subject is now able to breathe without having apneas.... so in reality if a person is breathing LESS on VCOM its due to their bodies demanding less since their airways are open in BOTH situations... So still feel the way I was wording it is more correct.
