Thank you for this - super helpful! As an aside, this is a much more effective format than "mythbusting". I work as a behavioural scientist in communications and we strongly advise against mythbusting as it increases the exposure to myths by repeating them. Often people don't remember if the myth they saw during a mythbusting session/in an article ended up being dispelled or not. They just remember the myth itself! So the format of this video is likely to be a lot more effective :)

Omg that's hilarious 😅 we remember the myth (and more likely to do it) but not whether it was true or false??! 🤣 It reminds of a placebo, even if you KNOW you're taking a placebo the statistics show you're just as likely to have improvement?! We know so little about ourselves!!🧐

I wanted to respond to two points.
My daughter has a relatively rare condition and has been seen by 3 different dermatologists (and many Derm residents as on multiple occasions, the doctor has asked if a resident could see it because it isn’t common) Each time she’s seen the doctor for the first time, they immediately and correctly identify and without hesitation even though, to a layperson, it looks like it could be a number of things. This didn’t make me feel ripped off in the least- in fact my confidence in their diagnosis was bolstered by their certainty and prompt identification. There’s a comfort in your doctor knowing exactly what they’re dealing with.
Second point is, I am glad to see you sharing info about keeping a wound wet while healing. My mother had a degloving injury in an auto collision and this was explained to us by the plastic surgeon that ultimately saved her leg when it had become infected. First thing that surprised me was that the doctor (my mom had a whole team of doctors with different disciplines) in charge of wound care at the trauma hospital is a plastic surgeon and the second was that she was training nursing staff in wound care (I expected that wound care is basic and that they’ve been doing it for literally hundreds of years, why do they need to be taught, surely it was rote learning in nursing - but it was not clear. She was training staff to ensure they dressed the wound to keep it wet and how to clean it- it was quite different from previous practice. And this was gross and surprising, plain soap (no disinfectant, no antibiotic soap) and water with hard scrubbing to remove dead tissue (as it will cause decay of healthy tissue).
I learned much from my mom’s traumatic accident.
    Thanks for educating your viewers

Yes, would love a video for pre-teen and teenage acne. Something that I would likely watch myself, but would definitely also share with my daughter. I also wanted to mention that I understand what you are saying about a link between kids being "dirty" and having acne, but in my personal experience, and also walking through this with my daughter, I know that acne isn't necessarily BECAUSE of that, but it is definitely MUCH easier and more well-controlled when a regular face-washing (or body if there is body acne present) routine is being kept in place. I would also love if you would cover the body acne treatment/management in the pre-teen/teen video as I see this a lot in that age group and really struggled with it myself when I was young!!! Thank you so much for all the great information you give us!

Healing acne and many other skin conditions means healing the gut! The skin is the largest organ in the body and skin issues often are a warning sign for what is going on the gut. Healing a leaky gut, the microbiome and eliminating things that damage the gut will mean a healthy body….skin, brain, lungs, eyes, heart….the entire the body!

Just one thing, though.....My mum had melanoma. When I developed a spot on my lower back which resembled almost exactly in color distribution and configuration Mum's early melanoma before it turned black and blue, I went to my dermatologist and asked her to remove/biopsy it. She was sure it was a post-inflammatory hyperpigmentation. I pressed her. I said it looked just like mum's. Finally, she acquiesced, and took a scrape of it. Sure enough; it came back "moderately dysplastic." Not mild - moderate. Was I surprised? Nope. Was she? A couple months later, I received a letter from her office. She was retiring from her practice, her patients being referred elsewhere. ;) Please consider that those of less common skin type (i.e. "olive" skinned, for example) will not present as commonly as thousands of other patients. Thanks!

My thoughts exactly! I am from
Argentina, if anyone knows about us, we are of European ancestry. My dermatologist basically dismissed me because of my “ethnicity” when I went to have a check up. My skin is so fair, not even olive! Yes, I do have a moderate accent when I speak but that doesn’t make me “different race”. I never went back to him and I tell everybody about his lack of knowledge!

We too have a history of bad acne in on both sides of our family. My best advice I would give is the sooner the better, before the psychological and physical scarring happen. Our children are so happy to finally have clear skin and will do it again if needed in the future. There is no need to suffer and my heart breaks for those who do. I felt like the dermatologist was reluctant to use this treatment but I pushed and would have switched if need be. We haven't needed to go back after treatment and maybe that's why

I must address the in and out or quick diagnosis. Be open and honest with a patient, don't rush people or act annoyed with questions. A person who has sat in the waiting room for over an hour and then 15 minutes in a room without being seen is going to be angry or anxious when the provider is in and out in 2 minutes. Front desk and the nursing staff will hear about it. Arrogance is never appreciated.

The 3 basics everyone/teens should have in their routine cleasner, moisturizer, and susnscreen.
For treating the acne you can get a cleanser with either SA or BP. Apply and leave on for a few minutes before rinsing off. So the active has time to penetrate into the face and work. I personally like to apply cleasner first then brush my teeth as it saves time for me that way.
For BP/benzoyl peroxide don't get the 10%. 2.5% or 4% works just as well without being as harsh. BP can also bleach fabrics. Although I like to combat this by doing my nighttime routine 40mins before I sleep. Then apply moisturizer on damp skin after cleansing without using a towel to dry. I find I don't bleach any fabrics this way.
Start with a few times a week then she can move on to nightly use. Can also be used 2x a day if your daughter's skin can tolerate it.
A good sunscreen that dosen't break you out, sting eyes, or irritate the skin can be hard to find. Chemical or Mineral.
If after a few months you don't see improvement I would see a dermatologist.
Sorry for it being long lol

I don’t think it’s usually the time with a Dr. It’s the bedside manner, as you said. If a Dr in any field barely hits the spinning barstool in a room to address your concerns, they have a thin shelled obligatory smile, throw out a prescription, then say see you back in 3 months. It’s dehumanizing and the medical field is ever increasingly heading in that direction. In fact the ONLY time I’ve felt “human” in an office, I was almost brought to tears. I thanked him so much for taking time to hear me. He smiled and said, well I work salary here, so I make the same amount wether I see 10 patients today or 200. So while I still appreciated him, it did shed a lot of light on what’s going on in the medical community.
I’m not inexperienced in the medical community either. My mothers side alone, she has 10 siblings. Of those 10, 4 are OBGYN, & 1 Pediatrician.
In that whole family alone (aunts, uncles, cousins) we have 13 Drs all in specific fields, a physician assistant, a pharmaceutical sales rep, and tons of nurses of varying degrees. I went into rad tech and then nursing myself and my sister is a nurse. We worked at an urgent care together for a long time. She now works at a chemical refinery plant as one of two nurses and I decided to be a stay at home mom. So I have some experience personally because of family.
HOWEVER, nothing prepared me for the 12 yr struggle with what MAY BE Lupus or Ankylosing spondylitis and chiari malformation and CFS issues with mixed headaches. The lack of emotional care provided by Drs during a visit is appalling, wide spread, and just expected. I’ve not had the nerve to try and offend any family over there, but I often wonder if ANYthing is taught on bedside manners other than to not use “terms of endearment”? I certainly didn’t get much on that in 5 years even as a nurse how’s supposed to be more on the patient care side….
So to gloss over that part from your perspective as the Dr. And make excuses for it, really made me ill honestly. If a Dr cannot instill a certain lvl of personal connection, that’s probably ok and part of a disconnect issue they have. If a Dr sends you away feeling more sick on the inside than when you walked in their door, that is not ok. Personally, I’ve noticed the second usually happens at a rate that is alarming.
Plz don’t do this to your patients EVEN IF you know what it is immediately. This is NOT OK. Its NOT OK to be treated like a revolving door. And the fact that usually we have no choice in who we see is making the tension grow between the medical community and the public that rely on them.
Don’t make excuses for this. It’s not ok.

    • @DrSamEllis
      @DrSamEllis  2 роки тому +2

I definitely see your point and empathize with everything that you have been through regarding your health and personal experience with the medical field. I completely agree that you should feel comfortable with your doctor and satisfied with your visit. Really the point I was trying to make is that a doctor may be able to make a fast diagnosis and that doesn’t necessarily mean that they haven’t thought it through.

Thanks so much for the wound care information. Amazon has some great wide brim hats you should link to. I just ordered another panama hat and people do not realize these flatten and pop right back up, so you can pack in a suit case for your vacation. Also they have different colored straps on Amazon you can attach to the hats to keep those wide brimmed things from flying off on a windy day. That was always a pet annoyance of mine. I found one that comes with a scarf around that ties. They have unisex styles and some that are really floppy looking which are silly. I pop mine on every time I leave the house as my personal fashion statement. I am not sure why more people do not wear hats because they are great fun. Especially men who used to wear them constantly. I got my sister's husband one before he went to Hawaii because he shaves his head. For gardening I found an Aussie wide brim canvas hat with mesh around the sides on top to allow air and a leather brim inside so you do not sweat into your eyes in the heat. Best gardening hat ever.

Are they actually preventable? I also find it hard to apply sunscreen to my hands with all the handwashing that goes on

Find a sunscreen lotion you like. That way after you wash your hands just apply lotion with the added benefit of sun protection versus the thick sunscreen you’d use at the beach

Omg I developed an allergy neosporin , I had a chemical burn and kept using it and it got 100 times worse , went to a doc and he just said air it out lol finally went to a dermatologist and they told me I might be allergic to neosporin, well I immediately stopped using it and put a steroid cream on it and 2 days later it was back to normal. For weeks i kept putting neosporin and was wondering why it was getting worse. Now I stay away from it .

THANKS! about 25 yrs ago I told mom I wasn't using Neosporin anymore cause it made whatever I had feel worse, and she was like WHAT!?! Said it was all in my head. but I tossed it & use Vase for most things-no problem. Poor Mom, if she was still alive I'd send her this. Also my dermatologist saved my life. I was having something removed from my back. and she said you've got a mole on your leg I'd like to take off. I said ok; I didn't even know it was there. It was melanoma. It all went well; that was 30 yrs ago. Now I love sunscreen and fake tans!

Hi Dr Ellis, can you please provide some advice on how to treat teenage skin with acne at home - what are the best products, how often do they need to wash their face, and the best pimple treatment. My 15 year old son doesn't believe in a skin care routine and I'm worried he is causing long term damage by doing nothing. Love your videos!!! you are my favourite. thanks so much, hope bubba is doing well.

I loved your video. Bravo! I agree that all of those are 10 things that we should know. The manner that you explained each one reminded me of the Spanish expression, “al grano” , which means, “to the point”. My 2 favorites of the 10 was that we should keep a wound covered and not open to air and your explanation of the reason why. The other favorite was to avoid the use of Neosporin and use vaseline instead. I am in charge of medical supplies where I work and triple antibiotic ointment (generic for Neosporin) is kept in stock. It is ordered for patients from time to time and staff members, nurses included, request a little pkt of it once and a while for a small wound they have. Every time a request is made for it, I think about why it shouldn’t be used as I learned a long time ago about the sensitivity that many people have had to the neomycin in the ointment. I don’t say anything because I feel it’s not my my place to do so. Thank you so much for all 10 of the things that dermatologists want us to know.

I had skin cancer on my face a few years back but only recently started wearing sunscreen on my face daily, thanks to your videos! I’m hoping you can make a recommendation for a daily body lotion with SPF as I often have my arms or shoulders exposed. Anything you can recommend that feels like regular lotion? Preferably something I don’t have to wash off before climbing in bed. Thank you for your work - oh! and my very fair skinned 10 year old now wears a daily mineral sunscreen on her face too - hoping to help her avoid the years of sun damage Ive accumulated.

I know I’m not the Dr, but I really like Skin Aqua SPF 50+ in the white bottle and gold pump! It’s the best body sunscreen I found that’s also relatively affordable, and I also like it on my face. However you have to wash all sunscreen off, because all sunscreens form a film on your skin that can't be absorbed (that's how they're able to protect!)

I tan easily and seem to keep my tan for months after exposure. I wear sunscreen and reapply. She said I wasn’t using enough and she had just came back from FL and had no tan lines. I get a big ol' 32 oz of Equate Sport SPF 50 sunscreen for my legs from Walmart. Because it is reasonably priced - around $12 the last time I bought it - I put lots on. Thankfully, it also makes my legs look well moisturized. I use it for my arms when I go swimming, too. I use a rash guard for my body when I swim because putting sunscreen on my torso is an endless endeavor. I can't use it for the neck and face region, but it's fantastic for my limbs.

Enjoyed this! Is the being able to diagnose things quickly ability also related to the fact that derms’ eyes are so keenly trained (since this is what you primarily use to make diagnoses)? I imagine that helps too. It’s interesting what you say about being able to do something quickly after experience. I am a teacher and am the same with students. A lot of the time, after a brief interaction, I can instantly see what they are struggling with, whereas someone like a parent might think I’m not investigating properly, but it’s exactly like you’re describing: when you do something so much, you don’t need a lot of time Bc you just know.

I just discovered your channel and I love this video, thank you for all the great advice! When it comes to sun protection and tanning, I have to disagree with the un-nuanced sentiment that there is no such thing as a safe or healthy tan. I wrote details below if you are interested, and I would love to know your response to this if you haven't made a video about it already. It would certainly help me!
I have medium olive skin (Mediterranean ancestry) and will tan in the sun from just being outside, even if I am in the sun for only 15 minutes with 70 SPF freshly applied (true story). I tan from just existing, and it would be impossible to not tan in Summer unless I was house-bound. I wear sunscreen and reapply when needed and rarely burn, especially when I am consistent with sunscreen application, but the tan always comes along with that, no exceptions. I don't typically go in the sun around solar noon because my work prevents me from doing so, and I don't lounge in the sun for more than an hour at a time and still tan enough to need a new shade of face makeup every time. I feel that I practice pretty good sun habits, and there is no history of cancer in my family to increase my risk. Therefore, it feels misguided to state such an absolute about tanning always equaling damage, because it makes me feel like I am not being safe, that my skin is not resilient enough to be in the sun at all, that I should stay inside for the rest of my life, etc. This is a scenario I think of to illustrate my experience; if I had an identical twin with a fair/porcelain skin tone and does not tan at all, and we put the same amount of sun protection on, and we go outside for the same amount of time (say 30 minutes), and neither of us burn but only I develop a tan, that means that I am somehow more prone to skin cancer and am more damaged than the fair twin, who is supposedly more vulnerable than I am... How can this be true? It makes me feel like protecting myself is impossible and hopeless because I am naturally bronze, or that existing outside is inherently dangerous because of my tanning abilities despite practicing safe sun habits! Where is the gap here? Any thoughts? If you read this whole thing, thank you!!

The anti Neosporin skin care is not new to me since I watch other derms on youtube. I tried explaining this to my PCP and at first she thought I was crazy. Ha ha! And she's pretty knowledgeable and experienced. What matters is I know better and if I have a cut or scrape I just wash it with soap and water and then cover it with petrolatum and a bandaid if necessary.
