I'm 54 and went on HRT six months ago after two awful years of suffering with menopause (I had terrible symptoms). Life is now 100% better. Don't suffer, go on HRT.

I suffered for 4 years trying every supplement and natural way to stop the hot flashes that came every 30 minutes. Every time I try and go off of HRT the hot flashes come back. HRT is the only way I can have any quality of life.

You could also try improving your diet and exercising. The body is not malfunctioning when it goes through menopause. Hormone levels change over time for a reason.

@@Himmiefan, my diet and exercise were excellent even before menopause, and I still had issues. I made it through menopause on diet and supplements alone, but I still hadn’t slept well for 13 years. I could no longer function on a daily basis. I was afraid to even drive in fear I’d have an accident. At the age of 58 when I was in post menopause, I went on plant based hormones, and it was a lifesaver. I sleep so well now, and plus my blood pressure even came back down to where it used to be. If they give me a better quality of life, I’m all for being on them.

@@Himmiefan not at all a solution for many. For some yes diet and exercise alone works great but many others , not at all . I’m one of those women.. I’ve been strength training for years and an avid hiker , yoga , meditation, just to name a few out of many more activities. I’ve been on Mediterranean, Paleo , Carnivore , whole30 . I’ve even resorted to water fasting and IF.
Suffering with horrid and very alarming and extremely dangerous insomnia and brain fog , I’d be a fool to not utilize HRTs.
I’ve finally come to a place that I’m deserving and worthy to benefit from a much better quality of life and I’m striving for exactly that. HRTs aren’t the complete answer for me but they are most certainly a huge help. I’m very grateful for the benefit that HRTs give me 💝

@@crh251 reading your comment gave me immediate empathy for the insomnia you experienced. I know first hand those horrid horrid what I call dark nights of the soul with crippling insomnia. Living alone , working in healthcare and my average night was 2 hours of sleep. It was an experience that I have no idea how I survived and remained sane. However those years of insomnia cost me and my nervous system a great deal that I’m still affected by. I’m beginning to restore and renew but it’s been a lengthy process.
It’s cruel and a disgrace that disgust me when a treating doctor shrugs and says “it’s a natural part of life , eat better , lower stress “ without a moments seeking to understand what a person is going through. Those 15 min appointments are a disgrace. Our healthcare system is extremely faulty/ I shudder at what you and I and many many women are or have gone through .
I’m extremely grateful you are ok and living so much better/healthier.

Thanks for this enlightening episode. I’m 70, had a total hysterectomy at age 50 and did not go on hormone therapy after the surgery because it was when all of those articles you referred to came out. I changed doctors this year and have started back on identical hormone replacement therapy. While it’s taken some time to get dosages adjusted, I’m tickled with the benefits I’m getting from …everything from nicer skin, to increased sex drive which had been dormant for a long time…for many other reasons, too. Losing 80 pounds and having my hubby lose over 100 pounds on keto/carnivore has rejuvenated us both! ❤

She highly recommended it also and wrote many books and on TV. The Womans Health study never included hysterectomy patients in the study. Progesterone was the culprit!!!! I am a Registered Dental Hygienist with BS to teach it also from Ohio State. My RN mother and MD dad insisted on Premarin too. I had a hysterectomy at age 43 and decided to go on HRT, it was a hard decision because of all of the scary things about it. I asked my doctor about HRT when I had my hysterectomy. I told him I would like to try that. He said it caused cancer. I asked him, "If I am replacing what my body was making, how is that dangerous?" He looked at me with a puzzled look and said, "I don't know." In our lives we have to make our own choices and hope they are the right ones. I also found it interesting that obecity can be linked to 13 different kinds of cancer but you don't hear the fear mongering about that. Why? I thought that at the age of 43 it was young to be without HRT. I have no idea what rewards I have from it, I am now 65. I had worried about my bones, I am small boned and was told that I was a candidate for osteporsis, my DEXA scan showed my bones were good. I hope the rest of me is as well underneath it all. Best wishes on making the best decisions for your own health and well being.

This is just my story and anecdotal, but not many people talk about the suicidal ideation that can occur because of HRT, or the fact that women on HRT have a significantly higher rate of suicide. I certainly had never heard of it when I started it, and honestly, even if I had heard it, I never would have thought it would be one of my side effects. And my symptoms were so severe I was end of the end of my rope anyway.
About 14 years ago at around 50, I was where you are now, when I started BHRT. I suffered many of the same symptoms as you and many more, since I was 38 years old. I had watched an Oprah episode on menopause, and did know most of my symptoms were related to the hormonal changes that can start in a woman's 30's. Knowing that didn't really help.
I had researched HRT and knew of the risks and really didn't want to take it. I had white-knuckled it through a couple of very bad years until the hot flashes and migraines got so severe I couldn't' stand it anymore. I decided BHRT was a better way to go, but honestly I'm not sure how much better it really is, but the idea of it was safer, and made me feel better than taking HRT.
What worked in the beginning for hot flashes and migraines, was no longer working 6 months in, and I was feeling like crap once again.
And to make matters worse, along with the return of hot flashes & migraines, I had a severe episode of suicidal ideation. It started an hour after I took my pill (and I mention this because after researching, I learned my BHRT reached its peak in the bloodstream about an hour after ingestion). It was about that time (midnight) that every thought I had spiraled straight to hell. Luckily, as a trained psychologist, I realized it for what is was. I woke up my husband and told him he had to stay up with me because my thoughts were spinning out of control. In about 2 hours it went away. Went away almost as suddenly as it started. I can tell you, the drug induced episode is not something one can control or easily escape. It was the worst experience of my life. I used to cry just talking about it. And I know that it wasn't from "depression" because it was a "sudden onset" (as in instant on/off) and a complete 180 from my reality.
After researching the issue I discovered that women on HRT have a much higher rate of suicide. Which was a real shocker since I had NEVER heard of that from any DR, nor during my own research. After the second episode I decided I had to get off of it. Because the drug induced episode is so sudden & severe, I wasn't 100% certain I would be able to get through it if happened again. After all, if it significantly increases the actual rate of suicide, who am I to ascertain if I can survive it? I had already experienced the side effect twice now, so it could easily happen again.
I tried to ween off he BHRT but I quickly got a migraine that lasted 3 days, and I started taking the pills again.
That's when I decided to investigate alternatives that I knew about but hadn't tried. I knew women who told me they eliminated their symptoms through diet and lifestyle changes, and admittedly it was the last thing I tried.
Lifestyle changes worked much better than the BHRT for me and I wish I had started when I first experienced symptoms at the age of 38.
After much research I chose the Paleo lifestyle, and I say lifestyle because it's not just a diet. There are so many changes that go into it that it's far beyond a diet. You have to change your entire mindset. You have to understand that you can actually control your health with the food you eat. Hormones are extremely influenced by what we eat.
Paleo stopped all of my aches and pains and I easily dropped 50lbs in 3 months. My hot flashes and migraines that had returned even while on BHRT, started subsiding again, even while I slowly weened off of my BHRT over the first 30 days on Paleo.
I also added Mindfullness Based Meditation that completely eliminated my anxiety attacks and most all of my generalized anxiety, while also improving my overall mindset as well. (even if the diet change is too difficult, I definitely recommend mediation)
Paleo and intermittent fasting, (eating between noon and 7pm) cured my insomnia. I don't think Paleo alone did it, because it was when I started the intermittent fasting that my sleep just fixed itself. I think it could be about the blood sugar levels that occur at night if I eat too late. I was actually having bouts of sleeplessness that would last 36 hours!
But since all of us are different what worked for me may not work for you. But, if you're considering BHRT why not try strict Paleo for two weeks first? If you can stick to KETO you could certainly do Paleo.
I noticed changes in my health within 1 week. Then at about 4 months was a great change in mood and mindset. That being said, I have found it difficult to stick to Paleo. But for every day I manage to maintain a healthier diet I know it makes a difference, even if I don't feel it.
I hope you find what works for you. I highly suggest doing a lot of your own research. What bothers me most about downplaying the extensive research on HRT is that DRs normally tout random, double blind studies, until they contradict what makes them money. HRT is a multi-billion dollar business annually. And it's certainly not without its risks. And since it stopped working for me anyway, I found I do better with lifestyle changes, I just wish I could maintain it all 24/7 😂

Great lecture Dr Boz, but I didn’t catch your making the point that there’s a big difference between bio identical progesterone and progestins (as were used in the WHI) Progesterone protects against cancer, while the progestins can promote cancer. Since bio identical estrogen is not much more expensive than horse estrogen, why take Premarin. I don’t know if it’s true, but I was told that oral estrogen (passing through the liver) changed the level of matrix metaloprotinase which broke loose plaques that had built up since menopause 10+ years prior. If there’s a long period of menopause, then it’s far preferable to give the estrogen topically or
vaginally. I would appreciate your opinions, where I am right and where incorrect.

I'm 13 years post menopausal. During this time, I've had two Dr's reluctantly prescribe oral estradiol/progesterone. I had breakthrough vaginal spotting which was concerning for my Dr. and me, so I quit twice. I recently started the estrogen patch and micronized progesterone. The progesterone stabilized my mood. Much more than the previous form I took. Edited to add that early in my menopause journey, I experienced a bout vertigo and bilateral shoulder impingement, which can lead to frozen shoulder. Hot flashes stopped quickly with diet, dexascan was unchanged at the end of 5 years.

I've heard stories of ladies going through this stage of life and can't get any real help so my heart goes out to them. I'm a 64 Year old man and know I have low T but my doctor when I said something about it brushed it off and said lift weights will help you. I was in my head like I know that and that's one of the reasons I do lift but not like i did in my 30s and 40s. I've been fighting Crohn's and that's the only health problum known to me and went carnivore about 10 months ago. Seems to be helping but trouble getting my Ketones up over .5? I may go to OMAD but I do only eat in a 6 hour window. I met a obgyn who said I am clinically in perimenopause stage. I knew nothing about HRT then. When I asked her for options, she said HRT is but she shut it down in the same sentence that it comes with a risk of breast cancer. I am trying to educate myself. I have no family history of cancer. I have three important questions: 1. What is the criteria for knowing if I am the right candidate for HRT? 2. When is the best time to start HRT? 3. If the HRT is to be started later in menopause, is there something I should do now to prevent further decline of estrogen?

I started getting migraines during perimenopause, it also was a very stressful time, and I can still get them if pressure drops before a storm rolling in or accidentally consuming MSG, Soy, Nutrisweet in food while traveling.
I have been scared to go on both estrogen and progesterone due to the migraines, as my Dr. went with progestin only.
This video showed me if it stabilizes both hormones then that would be neuro protective and stop the wild fluxes?
My Mom had obesity, and was a former smoker, but no cancer, however, she had 3 strokes and heart disease is what took her from me at 84. She had a hysterectomy and I don’t believe any HRT (My poor Mom), besides her other issues I can not help but wonder if that plummet in estrogen set her up for the compulsive eating and also let down the neuro protection and slowed down the regeneration of brain cells after that quick drop into menopause. I am going to be 54 not a smoker but overweight 5’2” 155, I just started estrodiol cream due to umm, due to the reasons women usually ask for that cream and, for years I had been on progesterone only (progestin) pill. This makes me wonder if I could switch to estrogen/progesterone pill and get my brain and health back a bit. Fibromyalgia hit since peri started, mood fluxes are awful, I struggle with fasting as I go hypoglycemic, insulin resistant, and have insomnia, also the loss of muscle has been terrible. Sadly, we are not well enough financially for me to come up from Palm Beach County. But I am so sad and tired of not being me. I have no idea where I am even in my journey due to being a caregiver of my Mom, mourning her and had been also caring for & homeschooling our adopted special needs son, and our biological daughter who had some special needs diagnosis but, has overcome much of it. Time for me is limited and getting to a my appointments is difficult as my son is always with me. Thank you for this video it gave me hope I might be able to work back to better health.

I am a huge proponent of HRT. I started hardcore perimenopause in my 40's was on oral HRT for over a year (my skin couldn't handle the patches) and it was fabulous, then I developed the very rare side effect of blood clots. Pulmonary embolisms in both lungs in March of 2023. Through lower carbs and no added sugar foods i've lost 20 lbs. I also take DHEA and that seems to help with some of the hormone balance. HRT is great but there can be complications.

Very rare according to whom. Blood clots from the steroids in HRT are not all that rare, nor is breast cancer.

My understanding is oral estrogen, being processed through the liver via the portal vein, can contribute to DVT. The patch doesn't pose the same risk.

Yes it’s the oral estrogen. Transfermal, patch, pellet or cream is the answer

The first 30 minutes were very helpful, and extremely important and impactful information. I will now go on HRT, and have a lot of gratitude for this explanation. Now at 36 minutes, I’ve wasted 6 minutes so far listening to something completely irrelevant about a conference and tuning out. It irritates me when people waste other people’s time with self-promotion

I haven’t taken HRT bc my Dr didn’t recommend and now would like to consider and wondered if I still could safety if I’m 61? I’m right at that 10 yr mark.

Wish I could’ve been on the live and asked this question; I am 66 and post menopausal. I never suffered any hot flashes or other symptoms since. I had two small stage 1 breast cancer tumors in 2022 with lumpectomies and radiation. The recommendation was to take Anastrazole for 7 years. I am doing this begrudgingly but feel it isn’t in my best interest to do you have an opinion about Anastrazole?

sooo, we should advocate for ourselves to get on HRT?
I think even at 75, after being forced to be without HRT during the pandemic, I should go back on it. I am healthy, not insulin resistant (based on Dr Boz ratios), less than 20 g carb, mod protein, hi fat for 2 years, and graduated to carnivore for the past 8 months. IF every day minimum 19 hours, usually 23 hrs, weekly 36 - 42 hour fasts.

Why are there no negative comments? HRT was a nightmare for me. Hair fell out in clumps, bad acne, weight gain, aggressive behaviors. I know I am not alone . Skeptical of something that is only positive for everyone.

I tried bioidentical hormones and they couldn't get right so I suffered and symptoms got horrible and I felt like a lab rat. I'm a Christian and I prayed and I've since been using natural methods and recently started rebounding and feeling much better. I get blood work done and find out what my body's lacking and address the problem with changes in lifestyle diet and vitamins. I know exactly you feel and I pray for you to get through this change in life with dignity and Grace.

Dr Boz. I’ve seen the interview you mentioned with Huberman and Attia and i recently discovered Mary Claire Haver. Hearing you say the same things has completely convinced me. However, I just read this on medline. “In a large study, women who took estrogens (a group of medications that includes estradiol) by mouth with progestins had a higher risk of heart attacks, strokes, blood clots in the lungs or legs, breast cancer, and dementia (loss of ability to think, learn, and understand).” This is in an alert box on the page when you look up estradiol patches. Can you explain? It sounds like the same risks for me anyway if I don’t take it.

Older women (10 years post menopause) who started taking hormones in the WHI had higher rates of cancer, dementia, and heart disease. A few other studies have reported similar issues, but those studies, as far as I know, all used older versions of the hormones and not bioidentical ones.

@ Thank for the reply. I had forgotten that I posted that. I found out that my own obgyn office does prescribe hormones. Im now on the combipatch and I take testosterone drops due to very low testosterone.

Bravo! Thank you for laying out the facts. Women need educated because the standard approach to medicine is ignoring data. Women should switch doctors until they find one willing to actually consider HRT.

Why is no one talking about Pregnonolone, the hormone precursor? I'm 73, never took HRT & have no known risk factors. Since I started Pregnononolone 3 wks ago, I now have energy & concentration...& my brain fog is gone. In AM, I started with 10 mg, slowly upped it to 20 mg, then 30 mg & dropped it back to 20 mg because I felt better at 20 mg. I'm currently doing 3 wks on, 1 wk off...but will consider 2 days on, 1 day off in the future. And I sleep like a baby now that I added bio-identical Progesterone cream 1 hr before bed. Thots?

I experimented with Pregnenalone myself. The problem with it is it’s a gamble. It can go down many different pathways so it’s hard to know for sure. If it’s working for you, then I’d continue!

Geeez, you had me until the end, your totally not in agreement with many good MD's who say BHRT is not the same as HRT, I'm 59, I've been on BHRT for 8 years, finally got it balanced for the most part, and also would you please expound on why you think collagen raises insulin, I've never heard that before??
