    A friend of mine from grade school lost his parents and two little sisters in this tragic crash. They were flying in from Bangkok for his high school graduation. I remember hearing he was at the airport waiting for their arrival. ‘I have some just awful news’ I remember my dad telling me. So sad, so unnecessary. It’s been almost 50 years and still I think about him often.

    JAL had a terrible reputation. One of their DC-8s landed on a small runway of a local flying club in Mumbai when the crew misidentified the airfield. It was a miracle that pilot was able to bring the aircraft to a halt just before breaching the perimeter wall and running across a busy city road. In this case it was complete negligence when they ignored the instruments and flew below MDA when clearly visibility was bad. Even if the GS was malfunctioning the DME along with the altimeter would have given them an indication of their position. As usual a wonderful rendition by TFC.

      I have a great childhood memory of another pilot error “crash” in 1968. I was living in the South San Francisco Bay Area. I got my mother to drive me out to Coyote Point to look at the “Shiga” which landed in SF Bay. There was a similar lack of situational awareness. The co pilot was American btw. Fortunately, no one was hurt. In fact, since it was low tide, no one even got wet!

    How can the Japan authorities claim an incorrect flight path signal? What evidence did they have of that? Did other planes landing report problems with the ILS? It's pure speculation and claiming a "possible" explanation as the cause. The pilots flew the plane directly into the ground without ever seeing the runway...plain and simple. Great simulation once again!

Two that stick out in my mind: Egypt’s denial that pilot suicide brought down Egyptair 990, and the report issued by the Netherlands’ regarding the Tenerife runway disaster (let’s just say they had “alternative facts”). On the latter, Mini Air Crash Investigations made a really interesting “from the Dutch perspective” video as a companion to his main video about the Tenerife disaster. Shows how differing interests can cause a single set of circumstances to lead to wildly divergent conclusions-unlike the actual aircraft in that incident (pardon my gallows humor).

    I used to forecast the weather for several airports in India for various airline clients. The air is frequently smoggy with dust, smoke, and haze. Often the visibility is one mile or less. Such conditions would have made it imperative to continue an ILS approach. The MDA should not have been breeched because sighting the airport landing lights would have been impossible to discern except at a fairly close distance. It sounded as if the ATC wasn't helpful either.

    There are so many systems and procedures today that might have saved this flight. GPS approaches with vertical guidance, Enhanced ground proximity warnings, electronic map displays and terrain display and warnings among others. Non precision approaches are much more dangerous than ILS, GPS with VNAV or LPV or RNAV-RNP approaches which is why they are very rarely used at today’s airlines.

    The pilots had to be ignoring a lot for this to happen. The ILS, the glide slope, the baro altimeter, the radar altimeter. It does not mention here if the crash was short of the runway but aligned with it, that matters if the aircraft was on localizer but below the glideslope (landed short of runway). Also, an ILS does not have a MDA or minimum decent altitude, but a DA or "decision altitude". Whats the difference? The MDA is an altitude you don't go below, but you can stay at the altitude until you identify the runway, or reach the MAP or missed approach point, the point at which you are supposed to abort and go on a missed approach. A DA means you go missed. Like NOW.

    • @lindawakiyama1603
      @lindawakiyama1603 3 роки тому +1

      Thank you for your explanation.

    • @scottfranco1962
      @scottfranco1962 3 роки тому +1

      One small addition to the above would be WHY (why MDA/DA). Even a lot of pilots are confused by the difference between these two. And MDA based approach results in a ramp down to the MDA followed by a straight line until the runway threshold, but a DA results in more of a V shape, down and back up again. A localizer just tells you if are aligned with the runway, vs an ILS gives you an exact ramp down to the runway. The MDA means "stop descending and wait to see the runway, then you can go down". Since the localizer does not tell you have reached the runway (the most common MAP) you have to use another means to identify that point, usually a timer. Its got built in safety because the runway is a nice flat area without obstacles on it, so if you don't time it exactly, you are ok. On an ILS, the "ramp" reaches to the runway in THEORY, but realistically it does not. The ILS Glideslope antenna is there at the approach end of the runway, and stops giving you a good signal when you are too close to it. This is why it has a DA. Its telling you "this is as good as it gets... if it is not good enough, go back to the sky".

      As Larry Be said, that is correct, I am in an airline family ( my grand father ,dad, uncle and my son are/were all airline pilots) now with simulators , all pilots must pass certain standards built around take offs landings and emergencies, however in order to gain "experience" one must take the controls, being monitored by the PNF, ( pilot not flying) imagine going through sim training for driving, and watching and following through with and experienced driver but NEVER being allowed to drive because you dont have "experience" . many other factors played into this crash, accidents like these changed crew training and resource management. As it is said " the aviation regulations are written in blood".

    It was interesting to observe the internal view of the instruments and if they were correct as the Captain had a different inbound course in his course selector window than the correct inbound course for the approach. The first officer did but there were several anomalies such as the no glide slope references as you would have with the ILS dialed in as a back up for glide slope reference. The runway would be visible to the right of the cockpit. Just a whole bunch of apparent errors and lack of situational awareness. So sad.
