If it is similar to that of a cleat, then CAD-CUT® Fashion-FILM® [ www.stahls.com/heat-transfer-vinyl-fashion-film ] can be heat applied using a Flexible Application Pad [ www.stahls.com/heat-press-flexible-application-pad ]. As with any unique material, a best practice is to test it out.

As of now, we have not tried Chuck Taylor. However, being a canvas sneaker, application of HTV should work fine.

Use printable transfer. For leather shoes rub the area with nailpolish remover to remove some of the gloss covering or the picture won't stick

No, the heat press secures the material to the shoe. Of course, it can fade over time due to wear and tear.
