      I want to add that I can't have HRT.
      Ive made a conscious decision to get my health sorted now but while I was great fasting in 2018/2019, it went west during the pandemic plus menopause didn't help. I'm post now and it's harder than ever plus I've aches in shoulder and neck now that's an additional challenge.

    Longer your waistline, lower your testosterone levels,Shorter your waistline,higher your testosterone levels.
    Physical activity increases testosterone production as it is the brain with its pituitary gland which is the ultimate originator of Testosterone via LH.

      There are many factors which will affect TT levels. Leptin is one thing, insulin is another. A high-fructose diet will lower TT (of course if the nature wants to prepare your body for the winter it will supress the reproduction and cause you to gain weight).

      @@btudrus Yes Dear but the thing is any activity say walking, running sensetizes hormones like insulin and even if you are taking great amont of carbs the metabolically active body will prefer to burn it as fuel.Then their is genetics at cellular level and as one ages of course TT will diminish.But I believe a 70 years old man born in 1800
      Would certainly be having more Tt than Todays young man !!!!!

    Worth noting in relationship to how often to test your level: I'm 72 and started meat based keto diet from WFPB in January of this year. In 3 months my testosterone went from 450 to 730. Diet was the only change.

      If you get your liver in shape and don't have a tumor your levels will be good, and nothing is better for that than fasting + low carb!
      Some of the many benefits of fasting and a lower carb whole food diet on the immune system, blood pressure, blood sugar and anti-aging, all backed up by clinical data:
      Thymus stem cells are regenerated, which suppresses aging and renews the immune system.
      Fasting stimulates phagocytosis, the ingestion of bacteria and viruses by the immune system.
      Blood pressure is quickly and dramatically lowered, which is very important for a good outcome with the current pandemic.
      Fasting increases nitric oxide, which has manifold postive effects like reducing arterial plaque.
      Weight loss from fasting only loses10% lean tissue and 90% fat compared to the typical 25% lean tissue and 75% fat lost when calorically restricting for long periods.
      Blood clotting is reduced and blood clots and arterial plaque are reabsorbed into the body.

      The hormone Leptin is an immunomodulator that keeps the body from attacking itself and obesity causes leptin resistance. Fasting very quickly reduces leptin resistance and leptin levels and one day of fasting can cut your leptin levels in half and gets your immune system working properly again!
      When you move out of MTOR your body shuts down the building blocks of the cell which are used to produce organelles and proteins. This means the mechanisms needed by viruses to replicate are by and large unavailable when you are in a deeply fasted state.
      It stimulates the AMPK complex and activates autophagy. Autophagy (literally self eating) will cause cells to recycle foreign matter such as viruses and kill cancerous and senescent cells. AMPK does many helpful things in the body including activating the body's antioxidant defenses.

      Your body releases interferon which is also triggered by the body during viral attacks to shut down the replication ability of the surrounding cells and stop infections.
      After 72 hours or more fasted, your body actually recycles large numbers of immune bodies and creates new ones, rejuvenating your entire system.
      When you fast, this stimulates apoptosis in senescent or genetically damaged cells. This kills these cells off completely. Senescent cells are responsible for the effects of aging and are the root cause of the development of cancer. If it were possible to destroy them all it would completely stop aging and cancer. That is not possible but fasting can help limit these effects by killing off many of the affected cells and limiting the future effects of aging.
      Fasting also releases BDNF and NGF in the blood which stimulates new nerve and brain cell growth, helping a great deal with diseases like MS, peripheral neuropathy and Alzheimers.

      Dieters losing weight through intermittent fasting show greater reduction in waist size and increase in insulin sensitivity for the same weight lost and alternate day fasters were shown to lose fat while gaining muscle at the same time over a six month period.
      Fasting has been shown to increase bone marrow volume by 10% and levels of carnosine in the body in as little as a few weeks.
      Fasts of several days will not affect short term female fertility and may increase long term fertility especially in women with PCOS.
      Fasting also increases telomere length, negating some of the effects of aging at a cellular level.
      Eating one meal a day can bring most of these benefits to a lesser degree, and adding a few entire days off from eating per week will have even more effect especially when starting to feel poorly.
      Exogenous ketones can aid with fasting, making it easier in healthy people and allowing some people with specific issues to fast in spite of them without worrying as much about hypoglycemia.
      Children, pregnant or nursing women should not fast for periods longer than 16 hours. People with pancreatic tumors or certain forms of hypoglycemia generally cannot fast at all. Type 1 diabetics can also fast but it is more complicated and should be approached with caution as it could lead to ketoacidosis. Those with Addison's disease may also be unable to fast without liberal use of exogenous ketones, depending on severity. If you experience extreme symptoms of some kind, especially dizziness then simply break the fast and seek advice.

      @@drivingteacher786 ​ I was eating a "clean keto" meat based diet. 1 to 1/4 lbs beef, 6-8 packed cups mixed greens, added fat (tallow or olive oil) to get about 80% calories from fat. For me that's about 1 gram protein/1 pound body weight. ( I did not need to lose weight)

      That's wonderful! I often wish that someone had mentioned Dr. Fung to me sooner. My endocrinologist actually suggested gastric bypass surgery, despite the fact that I wasn't morbidly obese! Never discussed dietary interventions. And he was the best doctor I had seen in 20 years of T2D diagnosis.

    I'm going to assume that this vid doesn't apply to metabolic syndrome, correct? As a female I had PCOS, and had such high testosterone it was devastating to me as a young girl. At puberty I gained 50 lbs in 6 mos without doing anything different. I was obese my entire life.
    Now, IF and a ketovore, at 64, I'm in the best health and condition I'd ever been.

    TRT changed my life, i used to be skinny fat and couldn't get normal muscle mass/strength no matter how hard i tried. Heck i couldn't even properly do a single pushup when i had the testosterone level of a 90 year old. Now i can work out like an average person and actually increase strenght/muscle. Getting a healthy testosterone level allowed me to reduce my depression and binge eating too. Interesting fact, ssri's make low tosterone much worse. Having healthy testosterone is required for wanting to be alive. You will be absolutely miserable with low testosterone.

      @@thedoomslayer5863 I went to a doctor and got my levels checked. I take blood tests every few months to check my levels. Never take Testosterone blindly.

      @@Bertuzz84 odd. My hospital is utterly worthless in that regard. I always ask to photos or printouts of my levels but the best I get is maybe a phone call where a nurse who clearly doesn't want to be at work that day very quickly skims my numbers for me out loud
      I'm looking into kits to test it at home

      Same story here and started at age 60. Although I was just fat even though I was keto and doing cardio 6 days a week. I researched it for 3 years and when I became clinically anemic and my doctor still wouldn’t do anything I went to a trt clinic. My test was low normal and my estrogen was high normal. I decided to try it. Wow! What a difference. Not just my body but my mental outlook. I wasn’t a grouchy old man anymore.
      I finally found a urologist to treat me too. I only take 90 mg per week and it got my test up to mid to high normal. Weight fell off but more importantly my anemia is gone and my mental outlook is back to my old self.

      I need to find me a place where they'll actually accurately measure my test and give me the results and then a good trt clinic if I decide to go down that road
      My doctor is worthless in their regard
      They just eyeball me and go "nah you're fine" and I basically have to do a blood sacrifice to get a nurse who clearly doesn't care to quickly tell me my numbers over a phone call

    I had a drop in testosterone but powerlifting and ketones over the last couple of years seem to have increased my levels greatly. Before, I considered therapy but not anymore because now my energy levels are very high, workouts are very productive, building muscle and at 55 my wife is extremely happy with how I look and you can figure out the rest, 🙂.       @@playerone1431 I’m guessing yes. I will eat some fruit but have nothing to do with any grain products, processed foods or any sugary snacks. I do eat big meals consisting of fatty meats and slightly starchy veggies. Two cups of coffee, and a few mixed nuts is all I will eat pre workouts and I have no energy issues.

      Some of the many benefits of fasting and a lower carb whole food diet on the immune system, blood pressure, blood sugar and anti-aging, all backed up by clinical data:
      Thymus stem cells are regenerated, which suppresses aging and renews the immune system.
      Fasting stimulates phagocytosis, the ingestion of bacteria and viruses by the immune system.

    As an advocate of fasting, Dr. Fung, what is your opinion on how fasting effects testosterone levels? Do repeated extended fasts - fasts lasting more than a day - adversely effect testosterone production when done regularly.

      The levels don't really matter, the results do. I have done literally dozens of fasts over 72 h and I can bench 150 lb dumbbells with no PED use ever. If it did sap your virility this would be completely impossible as almost no one can manage that in the first place.

    Last year, my male endocrinologist said that my testosterone level was a bit low(279,50) besides being within the reference levels provided by the lab. My thyroid-related hormones were a bit high. He said we were going to fix the T4 dosage and then go from there to see if any treatment would be needed since it could naturally rise. It could mean a major improvement in quality of life, he added.
    Well, my testosterone level is about the same despite my thyroid hormone being within normal range now. But the doctor has moved to another city. Now, when I've mentioned this possibility of increasing my testosterone level to my current endocrinologist, an old lady, she looked at me as if I were saying that the moon is made of cheese or something.
    This is utterly confusing.

      279 is a really low total test. 50 is free Test im guessing which is super low as well. Plenty of telemed clinics that are very affordable.

    I'm female and recent blood test show that my testosterone is too high at T 0.7. I have an appointment in June to do a more specific blood test at the laboratory because my healthcare isn't the best so I have to wait forever. I have Adipositas. I'm doing prolonged fasting now until my next appointment and see if it gets better.

    Dr. Fung, When I do a 4/5 day fast, i struggle with Acidity issues. I burp, have hiccups and also have loose motions, bordering incontinence. Can u pls advice how to overcome these issues ?
    Things I have already tried, but didn't work:
    Lime juice. Apple cider vinegar.
    Things that helped, but not much:
    1. Not working out during fasting coz it increases my acidity
    2. Avoid tea/coffee
    3. Drinking a lot of water, then vomiting it out to clean my tummy. This process is called Kunjal Kriya in Yoga.
    Pls advice why acidity and loose motions happen and how to overcome this.

    When I started bodybuilding, I went from 135 lbs. to 160-180 lbs. I used to eat between 4 and 6 meals a day, and sometimes I would get up at night to have a snack or protein shake. Oh boy, that was a mistake. Yes, I gained a lot of mass; however, I had a lot of fat around my mid-section, which I hated. When it was time to cut, I was never satisfied with my physique. I would get down to between 145 and 150 lbs. and would never be able to lose most of the fat around my waist. I had everything, except for the mid-section. I went from eating God knows how many calories down to 900-1100 calories. I was miserable, sluggish, always hungry, had brain fog, lost muscle mass, and couldn't perform in the gym. I started to look into Keto. I wanted to understand more about hormones like insulin and cortisol. I wanted to understand why a calorie is not a calorie. Today, I fluctuate between 145 and 150 lbs. My calorie intake increased a lot. I eat only 2-3 times a day. I'm ripped, have more muscle than before, and I perform a lot better in the gym. I lift heavier now than I was around 160 to 180 lbs. I don't take any kind of testosterone medication, but ever since I tried Keto, my physique has changed for the better, and my energy levels are through the roof.

      Building muscle will displace a lot of fat, but you will always have a little. Trying to get really skinny and muscular at the same time is really only possible with PEDs

      Keto and Intermittent fasting will both send your SHBG way up, which in turn will crush your free bioavailable Testosterone, irrespective of having normal total Testosterone levels.

    Why are women with PCOS who have levels of testosterone as high as men always obese? The inability to loose weight is also one of the symptoms of PCOS as well as elevated testosterone levels.

    Fasting may work for myself and others, but it doesn’t work for a lot of people. Some people binge and restrict!
    There is no one size fits all approach. I know several people who never fast and eat multiple times a day and they’re very thin and healthy!

    Hi Jason. Love your videos, they have been eye opening and have helped me a lot to achieve my first 100lb of weight loss in my journey so thank you !! Would you PLEASE do a video to clear up the confusion about calorie deficit during a keto/IF lifestyle? While I’m keto my body should “use” calories from my stored fat when it needs them and lowering my calorie intake / fasting should not harm my metabolism? Lots of videos explain that reducing calories lowers your BMR is there anything you can say to clear this up?

    Eighteen months ago I initiated an intermittent fasting and occasional extended fasting program in an attempt to limit the progression of a prostate cancer recurrence, 5 years post robotic prostatectomy. On your intermittent fasting program I lost 27% of my bodyweight and became much healthier but my PSA score continued to rise above the treatment threshold. I am currently taking a combination of androgen (testosterone) depravation and radiation therapies and I'm wondering if there is any evidence to suggest a safe and significant improvement to treatment outcomes incorporating OMAD and/or occasional 72 hour fasts?     Dr. Fung my testosterone, I'm a woman, was always 300 to 350, until I started menopause at 42 and it dropped to 87. I turned to flab, gained 10 lbs instantly, went on biodentical testosterone for a year, didn't make a difference. I'm 53, 7 years postmenopausal and testosterone 20. Is there anyway to get into the 300s like I was. I'm taking saw palmetto, which does make me feel better but as a woman how can I get to the 300s like I used to be?

    A problem arises with what is considered a 'normal' testosterone level. Normal levels have been declining for decades, for men of all ages. So what exactly is really normal?

      @@PistolPete3322 id guess that was due to your age. Mine was at 6% with a total at 425ish. Also to op normal level is 800. Not too low and not too high. Once you get to 900-1000 you can get sides if that isn't your normal level your body can produce.

      What is the levels of total and free testosterone that you'll see that would make you say "hey you may need TRT" for a male in his 20s?
      And if the male has a "normal" level of T but still feels symptoms of low T would you still let him do TRT?

    There is a good deal of new evidence to suggest errors in the old studies relating testosterone supplementation vs cardiovascular disease. In addition, arms of and follow up to these studies actually demonstrate decreases in the incidence of cardiovascular disease with TRT.

      Studies now show that TRT not only does not cause cancer, nor does it cause it to spread, TRT can cure some forms of prostate cancer that has metastasized... go figure!

    Losing body fat does increase testosterone without doubt as higher fat in the body increases estrogen and water retention. It's best to be cautious when using TRT despite it's shiny apparel. There are test products that aromatase and others that don't. Plus, they cause a lot of liver issues that are unwarranted for despite being injected. Some say the oral forms cause liver issues but the injectables also cause this to happen despite not going through first pass metabolism in the liver.

      Yeah problem is the world as time went now got more and more and more filled with nasty nasty chemicals that poison our soil our water and our food and our air
      As well as our soil losing it's quality.
      This is why men from the 50s have way more testosterone then men now.
      Problem is you can't escape these things even in the strictest diet
      So if we were doomed by our environment artificially lowering our testosterone we might need to fight fire with fire and artificially raise our testosterone back up

      Mick, everything effects the liver. Aspirin is more toxic then any test injectable’s, and ALL test compounds goes through the liver. The difference are some compounds do not convert to estrogen as others..

      I have a similar situation with my 9 year old. High, high carb and sugar diet. I don't want to do a cold turkey strategy. I think that's too extreme. It will take long but I will start cutting back on carbs and replace with more protein and get more physical activity in.

    I had osteoporosis and they found out I didn't produce enough testosterone.
    I ended up having 1 injection every 10 weeks and barely had any testosterone left in my body by then.
    Then I stated doing OMAD and now I need an injection every 7 months! I hope that it will improve even more over time but I'm very happy.

    Hi Dr Jason,
    Thank you for all the help that you provide by sharing your wisdom about the body and how it works in your videos. It has helped a ton of people as can be seen from the comments section of your channel. Kudos!
    Most if not all of your vids assume that T2 diabetics have the ailment because of high insulin in their blood or insulin resistance. I am a Type 2 Diabetic and tried IF (16/8) for 6 months expecting to decrease my blood sugar levels and lose the medication (Evopride 1/500 twice a day). However, the results have been shocking with my insulin going from 8.0uU/ml to 1.3U/ml in about 1 year and A1c spiking from 7.2% to 10.1% with an unintended consequence of a 5kg weight loss. I weighed 63kg and now down to 58kg. (there are some people who do not want to lose weight you see). I actually need to gain weight and strength. Any help on that would be awesome!
    Could you please make a video regarding the effect of intermittent fasting on Insulin for people who do not suffer from Insulinemia and how to improve the insulin levels other than eating more carbs?

    Dr. Fung. You should look up Dr. Rashid Buttar's presentation on Testosterone replacement therapy leading to cancer.
    I would rather stick with natural ways via nutrition. (Animal fat, minerals, etc) *or even ND-01 from Dr. Buttar himself.

      Mostly yes, if this was the cause for a low TT.
      (But an organic defect can be another cause and then you will not be able to rise your TT levels).
      So you need to know what the real cause is, i.e. you need a proper diagnostics...

    i d like to ask a question about diabetes, dr Fung... from what i understand, when we say someone has reversed diabetes, we mean that he is at all times and for the rest of his life on a low carb diet. Does nt that mean that if he goes back to normal inclusive of carbs diet, he ll be a diabetic again? so diabetes was never cured in the first place, just restricted because of limited intake of food.please, explain cause i m confused. All i what to know is , will a diabetic person ever be able to eat like a normal person or is he condemned to a ristrictive diet for the rest of his life? does he ever stand a chance to have a normal well functioning metabolism some time along in his life or not even dare to dream about it? is compromised health what diabetics have to settle for for the rest of theirs lives?Thank you

      Hey! I'm not Mr. Fung, but I'd gladly point you to Bart Kay and his elaboration of the Randle cycle which tries to explain Diabetes T2 as symptom of long-term abuse and inflammation in your body on a bio-mechanical level. What you refer to as "back to normal" is literally "back to eating habits that abuse my body" (the standard american diet).
      I get it - if you're new to this topic it's a lot of mind-bending at first, but once you get all the available information and try to paint a big picture you'll see that this form of diet isn't "restrictive" at all.

    So if I’m 30 years old at around 25% body fat and my total Test level is around 370, it COULD be beneficial to start taking TRT to help with my fat loss journey?
    On the other hand, I’ve heard that taking hormones like testosterone when you’re overweight/obese can be dangerous. What’s the deal with these two perspectives? Is it the dose that matters?

      TRT, or HRT, is not a joke, synthetic steroids like that could have an adverse effect, so consider trying a fasting regiment for a couple months before you jump on TRT at only 30. Workout consistently as well, build muscle. Because at your age, we as men have no problem with natural testosterone production.

    I am 61 i am also 61 inches tall. I weigh 190 pounds and have had diabeties 2 for two years. My tsh was a 5.99.
    I quit eating refuned sugar and all forms of bread and crackers except organic tortiallla chips with organic salsa. No sugar in either.
    My sugars are mostly in the high 100s and 200s. I don't have to eat a thousand calories to gain weight. I feel i am trying very hard since i gave up ice cream, candy, pop, chocholae milk, breads, donuts, rolls, well, you get the picture. My doctor had me on metformin, but I didn't like it, so i just got off it. Ive been watching these videos. I do snack. Maybe i will try the intermittent fasting.

      Unfortunately those chips will still turn into sugar in the body. Fast and try to eat the chips like once a week or month and it should start coming down

    This begs the question: Is low testosterone causing obesity or is obesity causing low testosterone. Common sense would tell us that too many highly refined carbs are causing a spike in insulin which forces the body to store fat. Excess stores of fat cause the body to suppress testosterone. Its far less likely that everyone who is obese got that way because they had low T.

    I’m frustrated with losing weight. I have metastatic prostate cancer have been medically castrated and take medication to mop up any other testosterone production. So the takeaway from this talk is I have no hope of getting rid of excess fat. Also the medication limits the ability to exercise for any meaningful weight loss.

      Just wishing you good luck with your treatment. Ask your doctor if you can have energy boosting supplements such as b-complex and BCAAs/luecine to help build or preserve muscle , I think they will help.
