Have just watched this video from 3 YEARS AGO...thanks...makes a lot of sense. But I wonder whether you have already or could in the future do some helpful videos for those of us who have had years of 'no therapy' experience and are wanting to get self started again after years of poor self management and after dozens of bad habits have taken shape. Lack of funds/ lack of access to a decent physio close by is often the reason for this + of course absence of motivation/ poor self esteem. >And then...just occasionally one might feel spurred to 'give it another go';;;when spring is in the air and you get a whisper of a notion that it might be worth trying to wake up those lostnlimbs again/ Never, never underestimate how important you are and how important it is to weave hope and belief into your excellent programmes. Thank you for adding to "my bag" of tools to use every day. I now construct my daily exercise routine, after I return home from my morning walk. My leg has recovered down to my ankle and my foot, toes, and ankle are all mildly spastic and negatively impact my confidence when placing my affected foot on the ground. That's right- I knew a man that always ran in a circle around a track at the YWCA p, thought he was doing good. Years later he had a hip replacement on the left side. HE always ran counterclockwise. Live and learn.
Great advice: Quality vs. Quantity. "Walking with focus on Quality" putting more body weight on my affected leg, not swinging it out over compensating. No more lock-downs, With first Vaccine shot done, Light at the end of the tunnel during these trying times. Warmer weather in these parts "Keep moving with Quality". Thanks, Dr. T.
I liked. Walking seeing the distance everyday. And listening to walk without hypertension my left knee. In the past week with the warmer weather I’ve gotten over 15 miles in steps.
I do multiple sets every day something I call sit downs and stand ups which help your abs I keep my phone in my pocket in case something happens. I brace the back of my computer chair again the wall for stability and do 50 of more a day.
The biggest mistake caregivers make after a survior make after being released from the rehab hospital is treating the survivor like an invalid. When they should be sllowed to learn there limits
Inpatient therapy taught me to ride the bike correctly. My foot is still strapped to pedal after 5 months but it is getting better in outpatient therapy. Thank you for expanding on my therapy.
Good lady. Thank you for this series. I fell in 2016 resulting in an undetectable brain aneurism. I began having seizures and convulsions followed by a stroke. I was hopeless at first as my legs were too weak for me to walk but I willed myself into doing research like this video and devised my own plan. I have most of my mobility back but I walk with a gait that I am very self conscious about. My left hand looks like a claw: Advice please? This has been horrible for my self esteem. I wear 2 braces interchangeably and they relax my hand and I definitely have feeling there. Any advice is appreciated. There is a whole sub set of people who don’t have access to therapy at a facility so thank you kindly for this series.😃😄😁 ~CJMP
I was so frustrated at PT they would only have me walk, no guidance on correct movement and led to pain in my hips and back. I wanted them to coach me on how to move correctly and give me exercises to correct my movement and allow me to expedite the correct movement progress. Thank you for these videos, I've made more progress here than in a months of 3 full days a week of therapy. So much time was spent sitting and waiting to work with someone
As far as I have experience the bike is not very good for learning to walk, You explained very wellI. I think it amplifies the abnormal muscle synergies, shown in the previous few videos. At the beginning, it is best to walk slowly and, over time, accelerate as long and slowly as the patient's psycho-physical condition allows. I mostly use the exercise bike as a stable support for daily stretching and other exercises when I need support. Thanks for the good ideas for new exercises.
My most recent PT was 50% on bike. Then I was one of 3 ppl the Physical Therapists were simultaneously working with. My previous MS specified PT had walking, then Yoga and then also Pilates, with some balance board on ballet bar at first two months.. Im now relying on UA-cam FB PT therapists videos.
Thanks for this video. I’m nearly 4 years out from my stroke. I tend to get a lot of steps. During the pandemic, I bought a recumbent exercise bike. While I need to work on my form, I have noticed significant improvement in walking on downhill slopes. That said, I take your warning to heart about focusing on quality movement to avoid problems down the road (I tend to swing my leg), especially since I’ve always struggled with back/hip/neck issues/aches, anyway. I'm 7yrs post stroke.. left hemiplegia,I walk for 8. To 10kms everyday from 4:30am to 6:30 without rest. N I am good with this Is this good / not? . N I have circumduction too
Thanks a lot Dr Tara Tobias for another informative video. As you correctly mentioned, it was not helpful for me doing bike exercise and walking alone. I done it for 6+ month rigorously but couldn't find any improvement. Later I change my exercise to 2hrs Physio mainly for arm twisting and leg stability taking instructions from your videos and 1 hr physical walking in outdoor ( initially with the help of wheel chair walker ) Now I am able to do walking with out any assistance. As you mentioned initially it was loading my working leg but after analysing and doing corrections slowly able to load my affected leg ( still continuing my exercise). I know that I'm lucky in my out patient therapy center. I get occupational and physical therapy and we always start with stretching then go to weight bearing and movement. The time we do spend on walking involves slow walking and really focus on each movement to build good habits in proper long term walking. Same goes for stairs. They are more focused on these aspects of my therapy than putting me on a stationary bike or treadmill. Thanks for the video, It makes me feel even better on the treatment I am receive.

Had my stroke over a year ago. I'm now back at work litterpicking the streets and do anything from 7000 to 11000 plus steps a day and walking way better.
I had this issue of spending 15 minutes of my one hour session on the “bike” 🙄 Since I have a stationary bike at home, I asked if I could do my 15 mins at home first and then come in and have my full hour... he said NO!🙄
The best PT my wife had was an OT that took the time with her to do things correctly, and we loved her for it. Unfortunately, she was pulled after 9 months because BCBS of and Medicare rules. Lynn# was great.

Hi Edwin. Yes, I think the "balance" point as far as walking AND quality movement AND cardio fitness will be different for everyone. I am glad you have found your balancing point 😊
Thanks Tara, I experienced that in one of the "best places" for my loved one Survivor. Now, we are in a different place and they are more about the quality move. And you are correct, those experiences other than just walking are paying off. I am a witness myself.
I feel like I've plateaued and have developed bad habits when walking (particularly hyperextension) - i have issues with glute, Quad weakness)..and I can feel how I compensate with my good side ALL the time...im fairly independent and I have no idea how to cut out or stop these habits. .I dont know how to change this pattern without stopping walking altogether (which is impossible in my life) and just exercise- work on isolated movements, muscle re-education and all that. How am I ever going to retrain myself if I have to walk every single day repeating the bad habits over and over again. I'm so confused.
i had a stroke april 2020 at the same time as covid19 kicked in and spent two months in hospital. what a waste of time no family couldnt. see me and my family have given up.BUT since i got home and got back to my computer, i watched your video;s and can go shopping etc.the rehab nurses turn up late every day. bloody useless, i didnt walk out of the hospital but i will walk back in.still not seen my children or the rest of family.im learning to walk again and cook my own food, housework,etc. thanks for videos
I am 12 years post stroke and just found your extremely useful video's. How I wish I found you years ago. The left side of my body was greatly affected and I had to learn to walk again. Sadly, though, I was left with drop-foot in my left foot. Because of the drop foot, walking is somewhat difficult. I can walk without an aid, but am not confident. If I have to walk any distance, I use walking poles to avoid falling when the toes on my left foot do not fully lift. My left arm does not fully raise either. However, I started ballroom dance 5 years ago and it has been tremendously beneficial. My entire body and my balance is greatly improved. Taking forward steps is a bit scary, but people often say they had no idea I had left side weakness. In my experience, ballroom dance for stroke rehabilitation has provided the very best rehab. Do you have any recommendations for improving - "eliminating" - my drop foot?
I am sorry to hear about your hishaps and I am currently going through the same situation as you are on my left side of my body and I want to really try to get rid of the walker and just be back to normal. This has been a very long and hard journey for me,I pray that all gets better for you and if you can provide me with any advice that I can benefit from I would really appreciate you.

My wife and I do decent amount of walking but we always focus and good quality steps and positioning cause that's what your brain will learn. Minimising the lasting effects of having bad form and later causing using "cheat" ways to just get it done

My problem is that I had a CVST & Subarachnoid haemorrhage (5 years ago, due to an underlying blood disorder), involving both sides of my brain equally so I don't have one side better than the other, they're both bad haha. My mobility is really poor and I use a mobility scooter/ 2 walking sticks for stability and to keep going (otherwise I just stop or collapse)/ bed bound & I suffer badly with fatigue (to the point of sleeping/ body shutting down, even after a full night's sleep) & immobility after I try to exercise. The more I walk, the less I'm able to walk. My body, particularly my legs, just seem to cut out all of a sudden & I can't do any more (within minutes of exercising). Can you do a video specifically with these adapted exercises? (my core, legs and arms are getting weaker because everything I do is making me more fatigued & I'm ending up not exercising at all which is making me weaker), with many thanks 🙏 After watching your video on core & improving walking (legs), I'm thinking maybe 5 or 6 targeted exercises of 30 seconds (like HIIT) with 30 seconds off after each exercise, so that's 3 minutes of exercise & 3 minutes rest, total 6 minutes. That would be more than enough for me to start with. I can only dream of 15 minutes walking haha. But gotta start somewhere right?
I can ride a bike, no problem. Therapy...lifting weights...can do. Walking.... I think my spine has shifted in some way. Spasms or spasticity won't allow me to stop walking without a limp let alone drag the affected leg. One side of my hip is offset... higher on one side.

Very good topic that's exactly what I did in physical therapy for almost two years problem is I can't bend my knee to squat and I can't lift my knee to walk it was all about how far I could go how many steps I can take how much farther than I did last night nothing about the quality or how I was doing it even though I was walking wrong

Too many interests, too much time spent on those fun cerebral activities that I live for, and not enough on exercising and mobility... calisthenics, walking, riding a trike, etc. It was an easy habit to fall back on. I woke up at 4 am falling head first out of bed! (Sleeping too close to the edge). I landed on the left side of my head, neck and shoulder (two weeks ago). Couldn't get up off the floor. Took me several hours and finally succeeded but it was a wake up call to get back to walking, biking and weight training. Thankfully I have good therapists; you, Dr Tara, and my other resource, a weight lifter/dietitian/physical therapist. My left kidney was screaming at me but it made no sense. I'm not fat, drink black coffee and distilled water, no alcohol, and consume the usual vitamins and fruits with yogurt. I have been on a strict intermittent fasting diet (less than a thousand calories) w limited carbs, lots of veggies, very little red meat, for a long time. He said it sounded to him like a slipped disc. He advised that I try standing at 90 degrees to a wall 3 ft away and leaning in and stretching the left side. Pain went away immediately. Definitely a spinal pinched nerve issue. Now I have to get the strength back in my legs and center core. You're right, Dr. T. It never ends for us. It's a fact of life. Stroked out @ 62, given a week to live according to what the doctors told my sisters. When I made it out of rehab and back to living in an apartment, I was chatting with a bus driver one morning and mentioned I'd made it my plan to go on surviving for five more years. He laughed, "Survive?" then told me his mother just had her 94th birthday, and her stroke had occurred 30 yrs ago. I'll be 78 in a month, 16 yrs so far and now I'm hoping to make it 22 more, a full century, but hope alone won't get it done.
Walking with good gait at even slow speed is better than fast speed with bad gait. That's my experience. If you want to increase heart rate. Just do sit stand by wall. Btw, I left a comment on your website for online consult. Did you get it?
my stroke was 18 days ago, i keep having deepdreams of playing basketball, i just ordered pne from amazon, so i csn try bouncing it at least. passing from one hand to the otheron hard floor.
Dr Tara. I was on a bike both with PT and using ahraltg club. Holding with the affected hand was problematic. I started HIIT IN MY SUBDIVISION. USING TELEPHONE POLES I’d walk a distance briskly, the next was leisurely. Using a quad cane. Felt like a walking stick would be better (eagle scout). So now I use Nordic poles on my path. Should increase cardio and work out my neglect. Enjoy your videos. At some point I’ll work out each challenge. (Reading a book on contracts. 😂😂
My physio concentrates on movement quality and giving me rehab exercises then I do these exercises and walk daily at home. I do seated exercise classes at home for cardio. Currently I’m enjoying seated Xumba.
First 10 minutes on a bike, yes, and walking laps for next 5 minutes. But I practice walking on my own everyday. My biggest problems are weakness in my chin. That's why I want to remove the AFO for strengthening.
I've had an Apple Watch for the last couple of years, which is helpful with counting steps. And I've found by trial and error that 6,000/day is challenging but reasonable for me. I'm not competing nor getting paid by the step. so I see no compelling reason to do more. I do most walking on my own, but do a 10 minute "warmup" at my PT sessions and am glad to get in some steps at PT. Most of the older PT patients I see (during my sessions) have joint problems of some sort, so I definitely don't want to end up with rehab induced joint issues. As odd as it might sound, even as a stroke survivor, I feel less disabled then patients with more acute joint issues/pain. I feel like my gait is pretty good, but would still like to improve it. I would like more out of my PT sessions, but the place I go is very upbeat and motivating, as well as nearby. Overall, I am much better off with it than without it.

But as I was practicing my walkingI was also trying to correct my foot posture, my foot subpoenaed and I fell and fractured my wrist.. that will not stop me from correcting My walking performance I know the pain Of my fractured wrist will go away but If I don’t correct my walking now I will have more pain later, thanks again 🙏🏼
I would go for 40min. sessions and usually 10 of those minutes would be a "bike" for legs or hands, then walking in a harness. Mostly I would benefit from learning about my condition. Hardly anyone knows or has answers. I had a cerebellar stroke.
Dr T, leg gets stiff and muscles tighten up in my hip, thigh and outer calf just walking a block, now when I am holding on to say like a buggy at the store, it's less painful... will that ever improve, 2 years post stroke
hello Dr. Tara tobias. my name is Rohit (36 yrs oldpresently) from India i see your videos very oftenly. i had a brain stroke on 06062020. underwent a surgery called carotidendorectomy. my left limbs had paralysed. i had hired a local PT for some time later had to quit due to financial hiddles. but after watching your videos i did lot of progressions for legs now im able to walk. some times i will have curly fingers of involved foot. i have a question for you. DOES only WALKING WILL IMPROVE HAND recovery as well? looking forward for your response.
Dr. if the temperature is 9 or 10 degrees and dry wheater, i walk with my wife or my brother 15 or 30 minutes ( something like 400 meters). I do every day 2 hrs in the morning from your excersices, which i think is best for me. Right now the weather in holland is not so good, that i can walk, but i hope to do that in the spring, if the weather is oke. I did also the treadmill, but you know, that is not good, because you have to hold strongly the bars of the treadmill. That i'm walking outside, i know that i'm compensating to mine left leg. You know that. I hope the weather change soon. I know that the path to succes is not easy, but stay strong, to do that. That is the only way in the world.
Thank you for this video my question my husband has had a stroke with left side impairment how do I know which exercise is good for him? He also has right shoulder pain and he cannot extend his arms that far up either arm. There is no grasping his fingers on the left or the right side. He’s also been told he has congestive heart failure at the end stage, so what kind of exercises would be good for him if PT is to continue with home health what would be the best things? Thanks for some good ideas Pre stroke I had some shoulder issues my doc sent me to a sports Pt for. 15 hand bike. He said for warm up. Most of what he he did wasn’t very helpful Therapists post stroke 2-5-2012 massive right MCA were fantastic. During my 1 month hospital Thdn 2 month residential rehab and to a degree in some of my 5 mthOp therapy. The walking part of therapy was mostly practical and progressive Sitting on edge of bed. I was bad and stood up on right leg which got me 2 of the best things for me one my favorite to this day saying. rule number one is do not fall. This was in response to the collective gasp of two nurses snd 3 med techs. But I was so dam tired of being picked up and strapped into a wheelchair. It also resulted in a therapists talking to me 15 minutes later asking if I wanted to start therapy. H$m&ll yes. The walking part was Progressive use of assistance equipment. When I got to where I could shuffle for a bit on my own things like scanning for obstacles how to get through doorway without banging my arm, walking in crowds, navigating real world situations. At the end and for a long afterward. I used hiking poles. I still keep one in my car and by my door that I can grab if I think I’m going to be-on my feet for a long time I had arthritis pre stroke as well as a Bunyan. On my affected side which is still an issue. Using my right side too much is due to that, having s buzzing low level pain which hasn’t stopped for one minute since 2-5-12, some weakness and not 100% confident in my balance. I can walk fairly well, if I’m tired and having a more painful day I may have a bit of a limp/not proper full rom. I’m not good on curbs and I need a handrail on steps. My distance is about 3/4 mile stroll with my dog on a good day. Things like shopping can wipe me out. I will now after watching this work on Using both legs more evenly. Also Watch more of your previous videos. I just recently discovered your channel Do you know of any channels as good as yours focused on cognitive therapy? Do you have video on curbs and stairs ? Fighting fatigue. ? Getting rid of the dam painful buzzing in my leg and arm. Mostly my hand?

My wife is a left hemiplegic. She is walking well but there is some glitch in walking ( like a baby walk). Not circumduction gait but slight hip raise. How to correct it madam?. She could lift her hand but finds it hard to lift beyond her shoulder height( above head).she is also having a slight facial muscle slag under left cheek. I quit physical therapy because I never saw improvement in my walking technique. I’m still walking incorrectly without improving the way I walk. After walking 10 steps my left hip hurts
Well my unaffected side is not my strong side as I had an cae accident and that is rje weakside. My affecred side rhe lefr side was the strong sidw so i making sure do exercise for borh aide. So bicycle tjen steps on stairs then strength training

I would search body weight supported treadmill training and/or spinal cord injury outpatient programs
I have been watching your videos for years ,to strengthen the involved leg. The VA got me a functional electrical stimulation device and I can walk 1.5 miles. It makes walking so much easier. But I am getting arthritis in the effected foot. Do you have any suggestions on use of FES device. Thank you for what you have given to all of us!
How about some lying exercising..I like tightening back, leg and gluts to attain some limited ROM while side lyn with in affected leg down. Limited reps helps loosen tight back ..while tryn to get back to sleep. This is semi 'sitting s. lying position. There r lots of rom musc tightening positions also while lyn'.
Standing frames are absolutely critical when and if you are unable to stand and walk. And most insurance companies will pay for it with a well written letter of medical necessity.

Interesting video thank you. I had been wondering about this. I don't have therapy sessions any more but when I did they didnt have me walking very much. I have always loved walking around and exploring the local area so on good days I like to go out for a walk but have noticed more recently that if I walk more than about 4000 steps in a day I tend to get injury or pain afterwards that takes a few days to heal. Problems more noticeable on the affected side. A few years back I was walking over 10000 steps while on holiday but had a lot of pain the following day! I've started to think that the problems are caused due to my having weird gait and so I'm now focussing on improving my gait and keeping the distance down. I've been using some of your videos for ideas. My therapist told me I hike my hip (but didn't suggest how to stop!) and I think I got into vaulting gait as well. Don't circumduct any more so that is one positive! I'm using your video for fixing vaulting gait at the moment - any other suggestions? Thanks
Thanks for watching and for the comment! Yes, generally speaking, circumduction, hip hiking and vaulting can all fall into a similar bucket (so you may see some of the same exercises in all of the videos I have on these three movement problems). Keep up the great work!
I’ve started counting my steps again. But it’s so difficult. I know I’m compensating on right side. It makes my hip hurt a lot so I’m barely going to see my neurologist next month. I have a bike but I use it at home. I only did PT for 2 months prior to the pandemic, then it closed down. I did bike 10 mins. The tens machine. And we did practice walking in the mirror and he would throw a ball for me to catch to help my arms.

That's right- I knew a man that always ran in a circle around a track at the YWCA p, thought he was doing good. Years later he had a hip replacement on the left side. HE always ran counterclockwise. Live and learn.

Great advice: Quality vs. Quantity. "Walking with focus on Quality" putting more body weight on my affected leg, not swinging it out over compensating. No more lock-downs, With first Vaccine shot done, Light at the end of the tunnel during these trying times. Warmer weather in these parts "Keep moving with Quality". Thanks, Dr. T.

I liked. Walking seeing the distance everyday. And listening to walk without hypertension my left knee. In the past week with the warmer weather I’ve gotten over 15 miles in steps.
I do multiple sets every day something I call sit downs and stand ups which help your abs I keep my phone in my pocket in case something happens. I brace the back of my computer chair again the wall for stability and do 50 of more a day.
The biggest mistake caregivers make after a survior make after being released from the rehab hospital is treating the survivor like an invalid. When they should be sllowed to learn there limits
Inpatient therapy taught me to ride the bike correctly. My foot is still strapped to pedal after 5 months but it is getting better in outpatient therapy. Thank you for expanding on my therapy.
Good lady. Thank you for this series. I fell in 2016 resulting in an undetectable brain aneurism. I began having seizures and convulsions followed by a stroke. I was hopeless at first as my legs were too weak for me to walk but I willed myself into doing research like this video and devised my own plan. I have most of my mobility back but I walk with a gait that I am very self conscious about. My left hand looks like a claw: Advice please? This has been horrible for my self esteem. I wear 2 braces interchangeably and they relax my hand and I definitely have feeling there. Any advice is appreciated. There is a whole sub set of people who don’t have access to therapy at a facility so thank you kindly for this series.😃😄😁

I was so frustrated at PT they would only have me walk, no guidance on correct movement and led to pain in my hips and back. I wanted them to coach me on how to move correctly and give me exercises to correct my movement and allow me to expedite the correct movement progress. Thank you for these videos, I've made more progress here than in a months of 3 full days a week of therapy. So much time was spent sitting and waiting to work with someone

As far as I have experience the bike is not very good for learning to walk, You explained very wellI. I think it amplifies the abnormal muscle synergies, shown in the previous few videos. At the beginning, it is best to walk slowly and, over time, accelerate as long and slowly as the patient's psycho-physical condition allows. I mostly use the exercise bike as a stable support for daily stretching and other exercises when I need support. Thanks for the good ideas for new exercises.
My most recent PT was 50% on bike. Then I was one of 3 ppl the Physical Therapists were simultaneously working with. My previous MS specified PT had walking, then Yoga and then also Pilates, with some balance board on ballet bar at first two months.. Im now relying on UA-cam FB PT therapists videos.
Thanks for this video. I’m nearly 4 years out from my stroke. I tend to get a lot of steps. During the pandemic, I bought a recumbent exercise bike. While I need to work on my form, I have noticed significant improvement in walking on downhill slopes.
That said, I take your warning to heart about focusing on quality movement to avoid problems down the road (I tend to swing my leg), especially since I’ve always struggled with back/hip/neck issues/aches, anyway. Thanks!
I'm 7yrs post stroke.. left hemiplegia,I walk for 8. To 10kms everyday from 4:30am to 6:30 without rest. N I am good with this Is this good / not? . N I have circumduction too
Thanks a lot Dr Tara Tobias for another informative video. As you correctly mentioned, it was not helpful for me doing bike exercise and walking alone. I done it for 6+ month rigorously but couldn't find any improvement. Later I change my exercise to 2hrs Physio mainly for arm twisting and leg stability taking instructions from your videos and 1 hr physical walking in outdoor ( initially with the help of wheel chair walker ) Now I am able to do walking with out any assistance. As you mentioned initially it was loading my working leg but after analysing and doing corrections slowly able to load my affected leg ( still continuing my exercise).

I know that I'm lucky in my out patient therapy center. I get occupational and physical therapy and we always start with stretching then go to weight bearing and movement. The time we do spend on walking involves slow walking and really focus on each movement to build good habits in proper long term walking. Same goes for stairs. They are more focused on these aspects of my therapy than putting me on a stationary bike or treadmill. Thanks for the video, It makes me feel even better on the treatment I am receive.

Had my stroke over a year ago. I'm now back at work litterpicking the streets and do anything from 7000 to 11000 plus steps a day and walking way better.
I had this issue of spending 15 minutes of my one hour session on the “bike” 🙄 Since I have a stationary bike at home, I asked if I could do my 15 mins at home first and then come in and have my full hour... he said NO!🙄
The best PT my wife had was an OT that took the time with her to do things correctly, and we loved her for it. Unfortunately, she was pulled after 9 months because BCBS of and Medicare rules. Lynn# was great.

Thanks Tara, I experienced that in one of the "best places" for my loved one Survivor. Now, we are in a different place and they are more about the quality move. And you are correct, those experiences other than just walking are paying off. I am a witness myself.

I feel like I've plateaued and have developed bad habits when walking (particularly hyperextension) - i have issues with glute, Quad weakness)..and I can feel how I compensate with my good side ALL the time...im fairly independent and I have no idea how to cut out or stop these habits. .I dont know how to change this pattern without stopping walking altogether (which is impossible in my life) and just exercise- work on isolated movements, muscle re-education and all that. How am I ever going to retrain myself if I have to walk every single day repeating the bad habits over and over again. I'm so confused.
I told mý thearpìst not using treadmill sìnce I can do it at the gym. I can have a better use of my therapy tìme. I am glad I made a good decision.

i had a stroke april 2020 at the same time as covid19 kicked in and spent two months in hospital. what a waste of time no family couldnt. see me and my family have given up.BUT since i got home and got back to my computer, i watched your video;s and can go shopping etc.the rehab nurses turn up late every day. bloody useless, i didnt walk out of the hospital but i will walk back in.still not seen my children or the rest of family.im learning to walk again and cook my own food, housework,etc. thanks for videos

I am 12 years post stroke and just found your extremely useful video's. How I wish I found you years ago. The left side of my body was greatly affected and I had to learn to walk again. Sadly, though, I was left with drop-foot in my left foot. Because of the drop foot, walking is somewhat difficult. I can walk without an aid, but am not confident. If I have to walk any distance, I use walking poles to avoid falling when the toes on my left foot do not fully lift. My left arm does not fully raise either. However, I started ballroom dance 5 years ago and it has been tremendously beneficial. My entire body and my balance is greatly improved. Taking forward steps is a bit scary, but people often say they had no idea I had left side weakness. In my experience, ballroom dance for stroke rehabilitation has provided the very best rehab. Do you have any recommendations for improving - "eliminating" - my drop foot?
I am sorry to hear about your hishaps and I am currently going through the same situation as you are on my left side of my body and I want to really try to get rid of the walker and just be back to normal. This has been a very long and hard journey for me,I pray that all gets better for you and if you can provide me with any advice that I can benefit from I would really appreciate you.
Yes that is what's going on at my session with my PT, and I think a supervision should be present during therapy therapy's.

My wife and I do decent amount of walking but we always focus and good quality steps and positioning cause that's what your brain will learn. Minimising the lasting effects of having bad form and later causing using "cheat" ways to just get it done
I did a lot of walking in pt, sometimes 2 hours a day or more hours a day plus some biking and stairs but my therapist was beside me the whole time

Great suggestions, with thanks! My problem is that I had a CVST & Subarachnoid haemorrhage (5 years ago, due to an underlying blood disorder), involving both sides of my brain equally so I don't have one side better than the other, they're both bad haha. My mobility is really poor and I use a mobility scooter/ 2 walking sticks for stability and to keep going (otherwise I just stop or collapse)/ bed bound & I suffer badly with fatigue (to the point of sleeping/ body shutting down, even after a full night's sleep) & immobility after I try to exercise. The more I walk, the less I'm able to walk. My body, particularly my legs, just seem to cut out all of a sudden & I can't do any more (within minutes of exercising). Can you do a video specifically with these adapted exercises? (my core, legs and arms are getting weaker because everything I do is making me more fatigued & I'm ending up not exercising at all which is making me weaker), with many thanks 🙏
After watching your video on core & improving walking (legs), I'm thinking maybe 5 or 6 targeted exercises of 30 seconds (like HIIT) with 30 seconds off after each exercise, so that's 3 minutes of exercise & 3 minutes rest, total 6 minutes. That would be more than enough for me to start with. I can only dream of 15 minutes walking haha. But gotta start somewhere right?
True just keep on pushing yourself and don't give up.
My PT instructor put me on the bike for 10 minutes before I start my exercises for blood circulation and warm up and I like it. 😊
With having a stroke and a temporary treatment needed how much walking is necessary for the healing process
I can ride a bike, no problem. Therapy...lifting weights...can do. Walking.... I think my spine has shifted in some way. Spasms or spasticity won't allow me to stop walking without a limp let alone drag the affected leg. One side of my hip is offset... higher on one side.
I am experiencing heaviness affecting my walking, and which is not due to excess weight. Can you advise?

Very good topic that's exactly what I did in physical therapy for almost two years problem is I can't bend my knee to squat and I can't lift my knee to walk it was all about how far I could go how many steps I can take how much farther than I did last night nothing about the quality or how I was doing it even though I was walking wrong

Too many interests, too much time spent on those fun cerebral activities that I live for, and not enough on exercising and mobility... calisthenics, walking, riding a trike, etc. It was an easy habit to fall back on. I woke up at 4 am falling head first out of bed! (Sleeping too close to the edge).
I landed on the left side of my head, neck and shoulder (two weeks ago). Couldn't get up off the floor. Took me several hours and finally succeeded but it was a wake up call to get back to walking, biking and weight training. Thankfully I have good therapists; you, Dr Tara, and my other resource, a weight lifter/dietitian/physical therapist.
My left kidney was screaming at me but it made no sense. I'm not fat, drink black coffee and distilled water, no alcohol, and consume the usual vitamins and fruits with yogurt. I have been on a strict intermittent fasting diet (less than a thousand calories) w limited carbs, lots of veggies, very little red meat, for a long time. He said it sounded to him like a slipped disc. He advised that I try standing at 90 degrees to a wall 3 ft away and leaning in and stretching the left side. Pain went away immediately. Definitely a spinal pinched nerve issue. Now I have to get the strength back in my legs and center core.
You're right, Dr. T. It never ends for us. It's a fact of life. Stroked out @ 62, given a week to live according to what the doctors told my sisters. When I made it out of rehab and back to living in an apartment, I was chatting with a bus driver one morning and mentioned I'd made it my plan to go on surviving for five more years. He laughed, "Survive?" then told me his mother just had her 94th birthday, and her stroke had occurred 30 yrs ago. I'll be 78 in a month, 16 yrs so far and now I'm hoping to make it 22 more, a full century, but hope alone won't get it done.
Walking with good gait at even slow speed is better than fast speed with bad gait. That's my experience. If you want to increase heart rate. Just do sit stand by wall. Btw, I left a comment on your website for online consult. Did you get it?
my stroke was 18 days ago, i keep having deepdreams of playing basketball, i just ordered pne from amazon, so i csn try bouncing it at least. passing from one hand to the otheron hard floor.
Dr Tara. I was on a bike both with PT and using ahraltg club. Holding with the affected hand was problematic. I started HIIT IN MY SUBDIVISION. USING TELEPHONE POLES I’d walk a distance briskly, the next was leisurely. Using a quad cane. Felt like a walking stick would be better (eagle scout). So now I use Nordic poles on my path. Should increase cardio and work out my neglect. Enjoy your videos. At some point I’ll work out each challenge. (Reading a book on contracts. 😂😂
My physio concentrates on movement quality and giving me rehab exercises then I do these exercises and walk daily at home. I do seated exercise classes at home for cardio. Currently I’m enjoying seated Xumba.
The bike and stretching with a therapist are funny. I do not think many of them are smart enough to think of this. Except you. You are very good. 😳

First 10 minutes on a bike, yes, and walking laps for next 5 minutes. But I practice walking on my own everyday. My biggest problems are weakness in my chin. That's why I want to remove the AFO for strengthening.

I've had an Apple Watch for the last couple of years, which is helpful with counting steps. And I've found by trial and error that 6,000/day is challenging but reasonable for me. I'm not competing nor getting paid by the step. so I see no compelling reason to do more. I do most walking on my own, but do a 10 minute "warmup" at my PT sessions and am glad to get in some steps at PT.
Most of the older PT patients I see (during my sessions) have joint problems of some sort, so I definitely don't want to end up with rehab induced joint issues. As odd as it might sound, even as a stroke survivor, I feel less disabled then patients with more acute joint issues/pain.
I feel like my gait is pretty good, but would still like to improve it.
I would like more out of my PT sessions, but the place I go is very upbeat and motivating, as well as nearby. Overall, I am much better off with it than without it.

I would go for 40min. sessions and usually 10 of those minutes would be a "bike" for legs or hands, then walking in a harness. Mostly I would benefit from learning about my condition. Hardly anyone knows or has answers. I had a cerebellar stroke.

hello Dr. Tara tobias. my name is Rohit (36 yrs oldpresently) from India i see your videos very oftenly. i had a brain stroke on 06062020. underwent a surgery called carotidendorectomy. my left limbs had paralysed. i had hired a local PT for some time later had to quit due to financial hiddles. but after watching your videos i did lot of progressions for legs now im able to walk. some times i will have curly fingers of involved foot. i have a question for you. DOES only WALKING WILL IMPROVE HAND recovery as well? looking forward for your response.
Dr. if the temperature is 9 or 10 degrees and dry wheater, i walk with my wife or my brother 15 or 30 minutes ( something like 400 meters). I do every day 2 hrs in the morning from your excersices, which i think is best for me. Right now the weather in holland is not so good, that i can walk, but i hope to do that in the spring, if the weather is oke. I did also the treadmill, but you know, that is not good, because you have to hold strongly the bars of the treadmill. That i'm walking outside, i know that i'm compensating to mine left leg. You know that. I hope the weather change soon. I know that the path to succes is not easy, but stay strong, to do that. That is the only way in the world.

Thank you for this video my question my husband has had a stroke with left side impairment how do I know which exercise is good for him? He also has right shoulder pain and he cannot extend his arms that far up either arm. There is no grasping his fingers on the left or the right side. He’s also been told he has congestive heart failure at the end stage, so what kind of exercises would be good for him if PT is to continue with home health what would be the best things? I hope that you get this message and somebody could leave a comment 2 to 4 hour reply.

Pre stroke I had some shoulder issues my doc sent me to a sports Pt for. 15 hand bike. He said for warm up. Most of what he he did wasn’t very helpful
Therapists post stroke 2-5-2012 massive right MCA were fantastic. During my 1 month hospital Thdn 2 month residential rehab and to a degree in some of my 5 mthOp therapy. The walking part of therapy was mostly practical and progressive Sitting on edge of bed. I was bad and stood up on right leg which got me 2 of the best things for me one my favorite to this day saying. rule number one is do not fall. This was in response to the collective gasp of two nurses snd 3 med techs. But I was so dam tired of being picked up and strapped into a wheelchair. It also resulted in a therapists talking to me 15 minutes later asking if I wanted to start therapy. H$m&ll yes. The walking part was Progressive use of assistance equipment. When I got to where I could shuffle for a bit on my own things like scanning for obstacles how to get through doorway without banging my arm, walking in crowds, navigating real world situations. At the end and for a long afterward. I used hiking poles. I still keep one in my car and by my door that I can grab if I think I’m going to be-on my feet for a long time I had arthritis pre stroke as well as a Bunyan. On my affected side which is still an issue. Using my right side too much is due to that, having s buzzing low level pain which hasn’t stopped for one minute since 2-5-12, some weakness and not 100% confident in my balance. I can walk fairly well, if I’m tired and having a more painful day I may have a bit of a limp/not proper full rom. I’m not good on curbs and I need a handrail on steps. My distance is about 3/4 mile stroll with my dog on a good day. Things like shopping can wipe me out.

My wife is a left hemiplegic. She is walking well but there is some glitch in walking ( like a baby walk). Not circumduction gait but slight hip raise. How to correct it madam?. She could lift her hand but finds it hard to lift beyond her shoulder height( above head).she is also having a slight facial muscle slag under left cheek. I think Tampa general might have one. Miami for sure. Also the Shepard center in Atlanta and I think brooks rehab in Jacksonville. Check out the Hocoma website and they may have a list of the facilities that have purchased their product. I would search body weight supported treadmill training and/or spinal cord injury outpatient programs

I have been watching your videos for years ,to strengthen the involved leg. The VA got me a functional electrical stimulation device and I can walk 1.5 miles. It makes walking so much easier. But I am getting arthritis in the effected foot. Do you have any suggestions on use of FES device. Thank you for what you have given to all of us!

How about some lying exercising..I like tightening back, leg and gluts to attain some limited ROM while side lyn with in affected leg down. Limited reps helps loosen tight back ..while tryn to get back to sleep. This is semi 'sitting s. lying position. There r lots of rom musc tightening positions also while lyn'.

Standing frames are absolutely critical when and if you are unable to stand and walk. And most insurance companies will pay for it with a well written letter of medical necessity.

Hi Dr. Five years after stroke my toe still gets hurt by getting rubbed on the floor when I walk.Im diabetic.How can I avoid my thumb by getting hurt.
Interesting video thank you. I had been wondering about this. I don't have therapy sessions any more but when I did they didnt have me walking very much. I have always loved walking around and exploring the local area so on good days I like to go out for a walk but have noticed more recently that if I walk more than about 4000 steps in a day I tend to get injury or pain afterwards that takes a few days to heal. Problems more noticeable on the affected side. A few years back I was walking over 10000 steps while on holiday but had a lot of pain the following day! I've started to think that the problems are caused due to my having weird gait and so I'm now focussing on improving my gait and keeping the distance down. I've been using some of your videos for ideas. My therapist told me I hike my hip (but didn't suggest how to stop!) and I think I got into vaulting gait as well. Don't circumduct any more so that is one positive! I'm using your video for fixing vaulting gait at the moment - any other suggestions? Thanks
Thanks for watching and for the comment! Yes, generally speaking, circumduction, hip hiking and vaulting can all fall into a similar bucket (so you may see some of the same exercises in all of the videos I have on these three movement problems). Keep up the great work!

I’ve started counting my steps again. But it’s so difficult. I know I’m compensating on right side. It makes my hip hurt a lot so I’m barely going to see my neurologist next month. I have a bike but I use it at home. I only did PT for 2 months prior to the pandemic, then it closed down. I did bike 10 mins. The tens machine. And we did practice walking in the mirror and he would throw a ball for me to catch to help my arms.
