      brother i want to just say this video has helped me to much, especially the first 2 exercises alone, I notice a clear increase my rotation range on the problem shoulder.
      i had a fall on my left shoulder, and something popped out (maybe a sublaxation not sure).... the arm was upright (like holding your hand up pointing to the ski), and it smashed on the ground in this position when I fell....then the arm was locked. I managed to somehow pop it back in at the time of the accident.
      It then locked again when my arm was overhead during a shower (I think I was raising my arm too much, too fast and too high, and with too much aggression).
      have you ever seen this where people raise an arm up, and the shoulder suddenly moves, and locks, and then preventing people from lowering their arm until it pops back in....any feedback is appreciated.

      What about full tears? I still have use of my arms, have some arm issues with left though. I also am having neck/ribcage issues now from all this dysfunction. I want to strengthen everything. Would this be a good program for me. Thank you

      My PT diagnosed me with teres minor tendinitis. I can not externally rotate my arm without assistance (No other movement issues). The first exercise of this video really irritates my teres minor! Do you have any exercises / advice specific to the teres minor? Thank you! jen

      Stop wasting time saying x or y person says to do this thing or that thing. Get to the dam point and stop wasting time. Just say clearly what I should do or not do and be done with it. Stop trying to argue anaginst a health care professional who you have created as an example.

      Every time I’ve gone to physio for rotator cuff problems they’ve always said to never shrug during the exercises. And it’s always worked for me. But now I see this video and you’re saying the complete opposite

    THIS GUY HEALED ME FREE ON YOU TUBE!! I had a bad rotator cuff tear and the doctor wanted to operate. I wanted to try non evasive therapy first however money is short and I can’t afford insurance. I found this guy and his exercises not only gave me immediate relief to get through my work day as a baggage handler but in a few short months I regained complete range of motion with little to no pain at all!

    I have been told to get a shoulder replacement on my left shoulder due to a full thickness tear (MRI results). I decided to follow your 5 exercises and the shrugging exercises have actually relieved my terrible pains I've been having. I will continue to follow your videos, but wanted to thank you for giving me 30 mins of no horrific pain. You are a very smart person. Thank you so much. You are an angel

      You will still need surgery to fix the problem! I have the same injury. Just saw the sports doctor for a major league baseball team. He says I need a TSA (Total Shoulder Arthroplasty). Another orthopaedic surgeon confirmed that finding.

      If that person is not having pain and strengthening the surrounding muscles etc. why do the surgery? Full tears don’t heal I know but if you still have full use of shoulders why would you do the surgery

      Why would they recommend shoulder replacement instead of rotator cuff repair surgery? Shoulder replacement is a much more serious procedure.

      A full thickness tear doesn't mean that the whole muscle/tendon has separated, it just means that at part of that cuff, there's a tear that goes from top to bottom e.g hypothetically say a muscle and tendon was 10cm Long x5cm Wide x 3 cm depth, a full thickness tear could be 1cm across but go through the whole depth. So you'd still have 2cm width of full thickness at that site. They won't necessarily heal back to what they were but they can stabilise. I don't get the rationale for a TSA for a cuff tear? Surgeons gonna surgeon...

      Start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase both the range of motion and intensity. It’s best to do these exercises every day throughout the day. Aim to tire out the muscle without losing form or control. Consistency is key. Always avoid working through pain; soreness and mild discomfort are normal, but pain is not.

    Excellent video. I am 71 yrs. old, had rotator cuff repair surgery 10 yrs. ago. I am very active, work out, and even the Dr. was impressed w my strength. My shoulder has been hurting lately and it's reminiscent if my old rotator cuff tear pain. After watching several of Dr. David's videos and following his suggested exercises , I actually could feel some relief right away. I'm soooo glad I came across his videos. He definitely knows his stuff !!! I am VERY grateful. I didn't want to have to go through another surgery and MONTHS of recovery. Thank you, Dr.David.

    I just got diagnosed with RCT and it's so painful 😖😖 I cry all day when I saw this video I started doing phase 1 , it was hard at first but I keep doing the exercises and I feel like I'm doing alot better while doing the rotation. I haven't had surgery as yet but I'm hoping I don't have to get it so I'm going to continue doing the exercises and pray I don't need the surgery. Everything still hurts a whole lot but I thank you so much for your videos 😢😢. I don't wish this pain on no one 😭😭😭😭😭

    Excellent advice and thanks so much .I could hardly move my right arm do to the rotator cuff pain,after two days of doing the exercises I am 50% better! A huge improvement.

    I received my pulley from Amazon Italy today, so I started to exercise five out of five from this video. I can tell you since October 13th that I found this video my shoulder movement has been improved significantly I’m sure it is going to get better with the pulley exercise.

    I have acute tendonitis of the right shoulder, wrist and elbow, and have had 3 separate surgeries. I still get soreness and pain, especially in my rotator cuff. This exercise that you just shared has significantly helped the soreness and pain! And it’s so easy to do! Thank you so very much! 😊👍

    You are such an excellent teacher! I have been getting PT for a rotator cuff (undiagnosed term) - Dr said “some aspect of your rotator cuff.” After @2 months of therapy, he cautioned me that I was getting a “frozen shoulder” and needed more aggressive therapy. That consisted of “stretching” my arm and twisting it in all sorts of positions that I would not ordinarily do in everyday life. They insisted that we needed to “loosen up” the shoulder. Problem is, they were painful sessions and I found it outright torture. Now my pain is much worse in parts of my shoulder that were not hurting so overall I am much worse than I was to begin with. THANK YOU for your videos and suggestions. I am anxious to try these to see if I can get some relief and improvement. THANK YOU!

    Doc I have no idea why you aren't getting quadruple the number of views you have. I'm almost 67, very physically active, and had left shoulder surgery 4 years ago. I was preparing for right shoulder surgery when I saw this video. My years of experience with rotator cuff injury tells me your videos are going to be helpful, and will help me avoid or at least delay surgery. I'm going to try the exercises you prescribed before I submit to going under the knife. I will do the surgery if necessary, but I'm psyched to try your exercise regimen - I firmly believe it's going to benefit me.

    On that "Wall exercise," I couldn't do it every hour, at the beginning of
    my rehab-----It would make the shoulder sore and tired; I had to do it
    every 2, or even, 3 hours, after the first couple of sets. But gradually,
    I increased, not only the amount of times a day I performed this tech-
    nique.... but the numerical count. I went from counting to SIX, to
    counting to TEN. It just takes patience, and repetition.
    I am now working the, "Speed bag," like my Orthopedic Surgeon
    Said I would.
    NOTE;
    I'm not going Full Bore on the, "Bag," but rather, medium speed and
    reduced power, until I can put more 'Charge' into it.

    Tried the weighted the overhead press using a gym machine that allows a neutral grip. Released a trapped nerve in my traps/neck that apparently caused the shoulder pain. Thank you so much!

    Fantastic! I’ve been doing these every day multiple times. I can feel and see how stiff my shoulder is. Ugh. Today I applied heat to my shoulder for about 20 minutes then did the pulley and the cat/cow stretch. Wow! Big difference after the heat. Joint felt more open and lubricated. My shoulder flexion was so much higher. Lateral and medial rotation are nil. Will keep working slowly on that.

    Thank u for the great excercises..they are helping..I didnt have the over the door pulley BUT made one myself..had an over the door hook from Dollar Tree and used a jump rope with handles AND it worked just the same!! And I got more range of motion when I extended my arm out in front of me , shrugged then lifted and there was LESS pain..

    I'm new to RCI (v. painful), this helps a lot. It also confirms what I am already doing in my weight training (for fitness, not body building), i.e. including shrugs in several exercises. Particularly, the shoulder rotation is most helpful as my RCI is compounded by advancing arthritis - I make a point of doing as much exercise as I can to help keep it at bay. Thank you for this, very helpful.

    If these exercises will get me back on the competition stage for bodybuilding, I'll be singing your praises. Partial thickness cuff tear here for nearly 10 years and reignited my pain after trying to get back into the gym. Almost giving up hope and do NOT want surgery. I think I have impingement, tendonitis and a tear, so unhappy triad. Hope your approach helps!!

    My right shoulder joint had been dislocated anteriorly few months before and manually re-located successfully
    After doing such exercises, i really feel good
    Please suggest me everything regarding how to gain approximately the same strength of the shoulder as it was before the dislocation
    U r doing a good job

    Hey Bro! You know your stuff. My situation was getting down a bit. I was trying a lot of different exercises from the net. One I was lying on my back and noticed one goes to the floor while the other is way off, same as your first exercise and a day later you turn up with the same exercise. So Did all your exercises and it has basically gone healed. I am going to continue with the weight and shrugging. Still trying to get my arm to the floor in the first exercise getting closer. THANK YOU!

      Hey, thanks for the info. I’ve had a partially torn rotator for years, I have full range of motion but still pick the scab with even my light gym training while avoiding benches or chest work. Would you recommend I have good strength on my cuff would you recommend I just immediately shoot the the last exercise, can I also do it with a one arm variation on the affected arm because my left side doesn’t like shrugs while I’m rehabbing a neck issue on the other side

    Dr David I am doing 1st exercise 2nd angry cat rocking 3rd shrug. 4th weight shrug hold I0 seconds,,, Then hangs off a bar. Started 3 times a week and found the pain lessen and big improvement

    Thanks for posting this video. My left shoulder is the issue. I've been seeing a physical therapist for this for a few weeks. He suggested to see an orthopedic surgeon for further review. Anything overhead kills me, sleeping at night is really tough since I turn quite frequently. I have been doing the bands since the aggravation. My left shoulder was hit by a .50 cal butt plate while serving. I have subscribed to your channel and hope that with your guidance things will get better.

    I had severe shoulder pain after gym injury during shoulder press , not able to drive especially making a u-turn and other daily activities is affected , that pain is quite unbearable, I have been practicing all the exercise here consistently, only for 3 days , my pain have reduced up to 90%, really amazing, I'm currently on the shoulder shrugs on the walls, going start with the weights soon. Super amazing exercise Doc!
    I have a question doc , I did research online that we have to strengthen infraspinatus muscle, how true and how it much could it help doc?

    This is great guidance! I had recently dislocated my shoulder a second time within a span of 10 years from the first incident. I did not get surgery on the first dislocation - both dislocations caused by a traumatic fall with my arm out. I will definitely use some of these exercises and modify what exercises were recommended by the physio. I am now 3.5 in since the dislocation that took place, but I know for a fact these exercises are SO IMPORTANT!! Thanks Dr.David!

    Thanks so much from a shoulder sufferer in Las Cruces. Thrilled to hear that surgery is NOT required for rotator cuff tear. Have had open surgery for right rotator cuff tear but still have limited mobility and pain 1 1/2 years later. Now left rotator cuff is torn. Have had shoulder pain, osteoarthritis for years! I am now doing physical therapy and home exercises. Can't take NSAIDS, and age 74, kidneys, liver limit acetaminophen use also. Use home TENs, cold, heat, topicals.

I started arm wrestling about a year ago and over the last few months seemed to have hurt my right shoulder(I was doing heavy singles on my right only, regular excersizes on both arms). Im going to be using all the information you have given to heal up as well as try to get an MRI to make sure. Ill track my progression to verify if this information helps. I can already tell the shrugs are key to proper recovery excersize already.

    El Paso is so beautiful. Thanks for this video, I hurt my left shoulder 3 weeks ago and still can't lift my arm. Last week I hurt some ribs on my right side, so now I can barely get in and out of bed and work is torture. I just hit the start button on these first 4.

    Been a nagging right shoulder for the past year then I did something at the gym this weekend that finally was the last straw. My shoulder is killing me. Going to ortho in morning and hopefully will get an answer, but I’m starting these asap. I do not want surgery. Thanks for the info/advice, it’s very much appreciated.

    Thanks doc, i seriously wish i had access to this video back in 2016 when the wait for the physio led me to overwork the shoulder and ultimately requiring 18months to fully recover. This time round i have a lesser injury, your weighted exercises obviously help pick the one that best fits for the rate of recovery/mobility. Hat's off to you sir ...outstanding advice

    Dr. David, Thanks for the great information and series of videos. It appears that I recently toar my rotator cuff. The pain is not horrible, and is tolerable, but my motion is somewhat limited. I'm assuming that these 5 exercises should be done sequentially, progressing to the overhead presses. . . . .and not all 5 done to begin with. Is there a prescribed schedule to move to the next exercise? Thanks!!!

    I just wanted to leave a comment and say thank you for these workouts. I've been to a physio twice with little to no help other than a diagnoses of a rotator cuff impingement. These workouts took away the constsnt tension and stiffness I've been dealing with for the longest time.

    - At the moment I am taking a vacation in Italy, and I found this video by accident randomly
    - When I was in Seattle, Washington last August I visited a sport medicine doctor, when she looked at my shoulder MRI scan, She said my rotator cuff tear is almost 4 inches apart, You need to see an orthopedic surgeon.
    -
    - I was about to contact Kaiser California to make an appointment with an orthopedic surgeon but after watching this video, I don’t think I’m going to do that.
    - I have been following only 4 exercises out of five in lasts few days that Dr. Dave suggesting on his video. I can see a significant improvement on my shoulder movement.
    - Thank you Dr. Dave, I appreciate that very very much. I’ve already sent this video to several of my friend in US.

    Wow, I'm so grateful for these exercises and your explanations on how/when to perform them. I've lost days of sleep from shoulder pain, and my impingement felt like one of those injuries that would take weeks if not months to heal. I've only been doing these exercises (several times a day) for two days, and I've already experienced massive decrease in pain, increased range of motion, and a much keener sense of how to hold my shoulder and shoulder blades when exercising. A massive thank you! I'm genuinely hopeful now that another week and I'll be back to training normally. THANK YOU!!!

      It sounds like i have exactly similar symptoms to yours, i have been ignoring my shoulder pain for 2 months, but id didnt go away, im wondering if you’re back to normal now? How long did it take and if any advice you have I’ll appreciate it.

      @@qpvvqp Not fully recovered. The exercises got me very quickly over the acute phase and I won pain relief and range of motion back very quickly, but still had to spend some months slowly and religiously doing my rotator cuff strengtheners with light bands. Still not 100%, but I'm happy to say I'm back to benching and rowing again, which feels amazing. Give it time and patience my friend, and don't push too hard too soon. Best of luck.

    Thanks, the degree of progression between the exercises was really useful in giving a way to feel through the injury as healing came together. I’ve a feeling I only had a partial tear as, once the initial inflammation settled recovery was faster than I expected, yet I felt reasonably safe in the process.

    Had my collarbone surgery and already at my 11month post ops.. couldn't figure out what was happening that i couldn't raise my arms fully on its side all the way up for weeks.     Awesome video. I had a MT of my Supraspinitas 3 yrs ago and going through PT saved me from having to have reverse shoulder replacement. I have 90% recovery and can do most things I need to do. 2 days ago, I stumbled and fell and now have the same type of pain/symptoms in my opposite shoulder. Most likely a RCT but nowhere near as bad. My question is how long should I wait to start these excercises? Thanks for your help!

    Thank you, good tips! I had surgery last year on my left shoulder. Just had an MRI on the right one and DR says I have a tear, and of course wants to operate. Your tips are different then what i have been taught, I will do them for sure, thanks!!

    Hello @epmanualphysicaltherapy You are the only one who has helped my rotator cuff problem improve after suffering with it for 23 years. NONE of the other doctors I saw or the books I read from Amazon helped me in the slightest. Their exercises with bands and weights actually made everything worse, every time. Here's my question: I'm very confused about exercise #5. First you tell us to lift the 10 lb weight WITHOUT pushing upward with our legs, then you tell us to make sure that we push up with our legs to give us momentum to get that weight over our head, otherwise we are going to cause problems. Please please charify which is the correct way to do it. Thanks, in advance, for your reply. And thanks again for giving me my shoulder / life back! I've done your exercises for about 5 weeks now and am about 85% cured. Have regained all range of motion with left arm. A Minor Miracle! Thanks for being bold and innovative in your approach!!!

    So, when doctors say rotator cuff does not heal by itself without surgery, I think these are fibers, so why not get healed if the person has good cells to produce naturally? Especially the surgery somehow doesn't give 100% results. Could you do a clip on this topic, please?

    I went to therapy for my left shoulder, they had me doing the exercises you did in the beginning of the video. It didn't help at all, I have been doing them twice daily at home on my own, it doesn't help!! I am Soo glad I found this video! I am going to try this and see if it helps, hoping it does!     Thank you for sharing your expertise! I'm concerned about doing the exercises that involve reaching my arms up high as I have a pacemaker and the underlying fear of reaching too far and pulling out a lead wire into my heart... The majority of my pain and restriction of movement is from moving my arm down and back (as in reaching for the seat belt or cleaning my bottom). I can't find much info on that situation. Also a lot of pain down the back of my upper arm. Weird.

      that sounds a lot like my RCI (if that's what it is). i got mine from stepping into a stairwell at night not know the door was open and falling face-forward down the steps, stopping my fall with the arm that is now causing me trouble.

    Thank you so much Sir for showing us the perfect exercises for rotator cuff tear. Most of the physiotherapist are doing it the wrong way, with all the pulling of resistance band which actually worsened my tear. Now I have started your exercises and I can feel the difference. It’s all about understanding the concept of how the rotator cuff muscle works, which I think you have figured out perfectly. I have 50% partial tear of the Supraspinatus on the articular side. I am a Badminton player and had an injury due to overuse and poor condition of the muscles (muscle loss due to covid). Few doctors suggested for surgery but I am quite skeptical about it. It has been 8 months since the injury. I would try your suggested method and I hope to recover and start playing again. Thank you once again for showing the right way of treating the rotator cuff tear.

    I like your first two exercises, but I am curious... why do you feel it is a good thing to encourage a scapular substitution? sure, it may take some pressure off of the rotator cuff itself, but wouldn't it be potentially encouraging a new problem? This scapular substitution normally occurs when the rotator cuff isn't doing it's job properly, so don't you usually see people with rotator cuff pathologies are already using too much upper trap to upwardly rotate the scapula when trying to use their effected arm? It seems to me the excessive upper trap facilitation recommended by these exercises will create a new issue that will need to be fixed later. I would like to hear your rationale behind this.

    O heck ! I learned a ton as I have just went through the physical therapy and I was told too hold shoulders back and down! I love the theory of shrug position because, that is the only way I could raise arm after injury. additionally, I am completely familiar with the kneeling position with extra movents for my neck and spine. The shrug pressure is very interesting and is somewhat the same pressure the therapist would apply with her body while doing a very relaxing ROM, the magic the therapist had done put my neck completely relieved and rotators felt unusually strong.I was taught the wall stretch without shrug and on my own thought of adding a pully to use as warm up since I don't have that arm bicycle at home but,it ends up as a stretch exercise .I feel so lucky getting better after 4+mos.and super lucky knowing about shrug! Is this working the suprasinatus ? I will spare you the details of the other reason I bought the pully eventhough it was not part of exercise routines.

      Hey Amy, thank you for your question! A major underlying cause of a lot of tears and injuries comes from a muscle imbalance. Most people live with weak upper trapezius, deep neck flexors and develop too much strength in other areas that cause uneven pressures to the rotator cuff tendon.
      Improving your ability to elevate, protract and retract your shouder blade can alleviate these pressures and allow your shoulder to be in a healing environment.

    I injured my rotator cuff in March, and the pain comes and goes. But the worst part is when I try to practice the violin and I bring the bow arm up to the strings ...ouch! I really don't think surgery is necessary, since most of the time I'm okay, but that particular series of motions I need is super specific. I tried some of your exercises already and I can tell a difference! Thank you.

    Thought my injury was healing but woke in the night to new pain. I've been in agony for 2 days, pain killers have made no difference at all. Found this video and a couple of others here and my shoulder feels so much better just after a couple of circuits of these exercises. Hopefully this is a good sign!!

    I tore mine, excruciating pain! I had two steroid injections, as against the surgery that the hospital said I needed. I also ceased with heavy weight training and cut back on my cycling mileage, I was doing. Anyway at age 66, I’m now okay and I do light weights three times a week and about forty miles on the bike, plus daily walks of 10-12 miles a day, keeps me fit and pain free.

      @@epmanualphysicaltherapy my friend who is a theatre nurse advised me not have the knife, though had it not settled down, I think I would have opted for surgery, such was the pain. I also ceased pull ups and press ups, too.

    I was doing bands with the rotation exercises in the beginning and getting no where. Thanks for the correction. Focus on shrug without flaring up the rotator cuff itself. Got it. thanks

    My rotator cuff has been gradually hurting more over 1-2 months during my workouts until the point that it became too much; eventually almost every exercise hurt and even bending over to tie a shoe would cause pain. In particular, pullups (at the top) and bending over to pick up weights or pull in a rowing motion hurt. After tapering exercises off, and finally not lifting at all this past week, it feels a lot better but it's still not right. I've watched like 10 of your videos and I'm pretty confident it's not a tear based on your tests. The tendon was sensitive to touch before, and certain movements you listed were painful last week, but not now. So I think it's either an impingement issue or tendinitis. Most body weight movements feel okay now besides when I retract my shoulder far back when doing a rowing motion. It seems to get caught because the pain isn't consistent. I can do shoulder press motions with no pain, and then if I protract my shoulder forward really far it seems to get off position and I can feel it painfully catching when I reach the low point of a shoulder press motion...then after doing your exercises, it can stop catching with no pain.
    All that to say, you mentioned exercises to do for recovering from and preventing future impingement in another video but this is the closest thing I could find (but it specifically says RTC). Are there any other exercises you would recommend beyond progressively increasing shoulder presses at the end of this process? I want to progress back to doing all my heavy lifts and movements in the gym pain free.
