  • In this video, Maryke explains why you need to transition gradually from walking non weight bearing to full weight bearing. She also shares tips on how you can overcome the fear of having to suddenly start trusting the injured limb again. It includes an exercise demo of various ways in which you can get yourself used to carrying full weight on your leg and not end up limping as you walk.

    Nasty tibia plateau fracture. Went to physical therapy today and I was terrified. I expressed my fears to the therapist. Best thing I did. My knee is pretty locked up but I trusted her enough to move my leg down. Now that I'm past the fear, I feel like I'll progress better. Great video.

My next appointment is coming up on 1/29/23, i will have my X-ray and hopefully I can finally begin partial weight bearing. The break is still not fully healed & it been 7 months since injury. It’s only been a month and a half since I was placed w brace. I have been and out physically therapy because my dr of physical therapy has had to cancel my follow ups and their aids cannot work w me until the dr sees me. My mind sees me walking my body starts to walk then it’s like nope pain. Needless to say I am frustrated & scared to walk or not to get back to walking without assistance. I will be using your video on top of my at home work papers from my physical therapy team. Thank you.

      Hello, I had the exact same fracture. My OCF surgery happened on October 3rd 2023. It has been five months since, and from Mid January I began using the arm crutch and from Mid Feb, I was told to take it off and walk. But the pain is unbearable. So I still use the crutch, more for moral support and also because I know pain and fear combined can cause more injuries. So just want to say it;s getting better each day, I am trying to overcome the fear one at a time each day, and walking with my arm crutch for 25 minutes 4 times a week. Plus daily self-physio. Hang in there. It will get better I promise.

    Hi Maryke,
    I twisted my ankle on November 16 while playing football. I thought it was a ligament sprain and rested wearing a bandage for 2-3 weeks gradually bearing weight . I was able to walk and do everything normally with slight pain in my ligaments after 4 weeks from injury but the swelling was still there so i took an x-ray where i got to know i had a hairline fracture in my lower 1/3rd of fibula. I wore a POP cast on Dec 12th and i removed it after 3 weeks , i was able to walk with little pain and discomfort . I took the x-ray again , found out that the fracture is not yet healed and the doctor asked me to wear the cast again for 3 weeks .
    Last week (Jan 22nd) I removed the cast and the doctor said the fracture is healed but i was experiencing severe pain just to land my foot , he said its because of the stiffness due to immobilozation and it will reduce over a week .I did small exercises for the past one week but now also I am not able to bear weight. It's like the small improvement i feel while doing exercises one day is completely gone when i wake up the next day.
    Can you please share your thoughts and recommendations?

    Age 55. Trimaleolar fracture dec 17, surgery and hardware installed dec 19, cast removed Jan 5 and staples (36) removed. Waited a week+ for sutures and staple holes to heel and started swim/pool work myself Jan 15. Consisting of a mix of water running, water polo drills, swimming (50m), building various exercises like calf/foot rolls, squats, walking in water starting at shoulder depth and progressing to waist depth after 10 sessions (on top of exercises in desk chair at work and home). 6 weeks post surgery was Jan 30 and official 6 week check up for x-rays and evaluation is Feb 2 for official go-ahead for weight bearing (I have started with air cast over a week ago and can walk well in the air cast but only do so with knee scooter close by getting to pool from car is on knee scooter down to bleacher, swim suit under sweat pants and swim parka, so just take off air cast and sweat pants on bleacher and crawl on my but the 10 feet to the deep end).

      Feb 14 update, can walk easily with air cast going on 3 weeks, if it is icy will use a cane for stability. Can walk without air cast at home but only do so when doing physio. Plantar flexion is almost as good as the right ankle, dorsal flexion still working on, I feel this when lunging my right foot forward (I only do this in chest deep water at this point). Cannot turn my left foot in as far as my right yet. I can do breast stroke kick though with a bit of force, could keep up with freestylers doing my breaststroke for a set of 100s.

      March 5 - 10 weeks following my trimaleolar fracture surgery. Since my last update I had been only using the Air Cast/walking cast at work. At home I had not been using it. This week I have been walking around with regular hiking boots. Walking is good. I can support weight and balance on the damaged left ankle (even raising my right leg), of course balance is not as good as the right unbroken ankle. The difference is most noticeable when doing lunges, specifically when I lunge with the right foot. Anyway, I have another month for my 3 month check-up so I will continue doing my workouts and hopefully the Dr will give me good news and allow some jogging. With my fracture my fibula was completely broken at an angle, on that I have multiple plates with 5 screws holding them in. On the tibia, most trimaleolar fractures I have seen on UA-cam have 1 or 2 long screws holding the tibial fracture, I had multiple so I have plates with 6 screws holding them, and the rear of my tibia was also fractured so I have another screw there holding the back.

      @@madelynkelly5777 Good luck - a little longer in the surgery cast but different surgeons have different approaches. I know with mine the 2 weeks post surgery I had the swelling go down in my ankle and my calf had atrophied quite a bit so my cast was very loose - I probably could have slid out of it like a boot. The best piece of equipment I used was a knee scooter. In terms of therapy if you have a facility that offers water therapy try that. I went to the water very quickly but I am a swimmer/water polo/triathlete and I have a great deal of personal aquatic therapy experience from decades ago that is another story. In terms of the air cast - get the taller one. That will have more support. I did get a second ankle one, mainly for my bike on my bike trainer, but I found the ankle air cast was not as good as the taller one.

      @@madelynkelly5777 Good luck on your recovery. In terms of the tingling, I also had tingling toes, ankle foot. That is normal with the swelling, as the swelling goes down it gets better. It seems that most of the videos I have seen with people that have this injury the dorsal flexión seems to be the hardest to get back. And that is needed to go down stairs, walk etc. My surgeon did say he had to remove damaged cartilage, which has been my biggest concern since I woke up after the surgery.

Wasn't prepared for all the new aches and pains in various parts of my ankle, foot and leg! Everything is still so tight.

    I had my ACL replaced with autograft by artroscopy 5 weeks ago and i was non weight bearing for 4 weeks. Now i have started to put some weight on it but i was so frightened. I walked for a few days with 1 crutch only but i have tender muscles on the opposite calf because i don't have the courage or feeling of walking with that midpoint correctly. This video is helping me because i feel the transition from crutches to no crutches is really rushed and they look at me like i am a scared kid but i simply cannot yet go without the crutches. I notice i find the midpoint if i use lightly both crutches, just touching then to the ground and trying to keep the weight mostly on the legs. I am going to exercise some more. I don't want to lose the crutches only to walk inclined to one side and in pain.

    What should I do if when I attempt to stand normally I feel pain on the backside of my calf ? I have normal range of motion in my leg and foot but can’t stand without discomfort and I’ve lost a good portion of strength to walk. 😢 I didn’t break anything , I had skin cancer surgery on my foot that caused intense swelling / pain that kept me off my foot for a couple weeks and now I’m trying to get back to normal with some difficulty. I didn’t expect it to affect me the way it did. I thought I would be able to bounce back once my excision site was healed enough.

      As long as you do the following 3 things, you are doing partial weightbearing:
      1. Use crutches - you can't do partial without crutches
      2. Place some weight through the crutches as you step with the injured leg
      3. It should be pain free - so if it hurts, place more weight through the crutches and less on your foot.

My mom has been forcing me to train to walk again, but I always feel tingling feeling when i put my feet on the floor, and it starts feeling pain when i start putting weight on it. It's also still visibly swollen. Should i start walking again, or should i wait until my next appointment (5 April)?

    • @SportsInjuryPhysio
      @SportsInjuryPhysio  Місяць тому +1

      It is absolutely normal for it to still be swollen. The tingling often also happens because a little nerve is irritated and it is usually fine to walk on it (although it can be super uncomfortable at the start). You will find that the more you start to gently use your leg in a normal way, the tingling often also then starts to go away. So perhaps just start by standing normally on it and then you can build to walking over several days.

    Is it normal to get calf pain due to immobilization?
    Lastly, can you show us how on earth to get up & down from a floor mat?

    • @SportsInjuryPhysio
      @SportsInjuryPhysio  3 місяці тому +1

      If you're having trouble getting down to the floor, why not do the exercises on your bed? It works for most exercises.
      Fractures in the foot can take 6 to 12 weeks before you can walk on them - follow your doctor's advice with regards to that because otherwise you can interfere with the healing and they will adjust the advice depending on your specific case.
      Yes, your calf can hurt but if it may also be useful to discuss with your doctor to make sure it is not a blood clot.
