For me one of the hardest difficulties is bad audio mixing.
Not because actors or creators speak with accents or quirks. I speek more with other L2 speakers than natives, so "non standard" english is far from a problem.
But when I can't even hear what people are saying because of background noise and overwhelming music, that makes it really hard.
And when in doubt, just drop the subject.
"getting hungry, gonna grapbsomething. want some?"
