  • Circadian rhythm sleep disorders can account for many sleep problems. There are many people who have an irregular body clock and they just don’t know it. Circadian rhythm sleep disorders occur when there is a mismatch between the sleep-wake cycle required by your environment and your own internal sleep wake cycle.

Why don't doctors ever mention or ask for further tests to see whether one suffers from one of these disorders? My whole life, I've felt guilty for being lazy even though I am not a lazy person, for not being able to get up early in the morning. At many jobs, I'd call in sick because I felt sick, especially on a Thursday morning (true hell, after 3 days of dragging one's body through the social clock) Doctors always dismissed it, until I saved up enough money to go to a private sleep clinic, and finally at age 38 I was diagnosed with 2 different and serious sleep disorders. 38 years of exhaustion, self-hate, loss of social life, career, relationships ...

I feel you. 28 and recently self diagnosed myself with adhd and sleep disorders after reading several books and monitoring my sleep and energy. Mornings are hell for me and I gain clarity only by afternoon 3.00 pm. My mind would be buzzing with energy by 12.00 am even if I am physically exhausted. My mind would quiet down by about 4 am and I go into deep sleep around 6 or 7 am. Lost so much in life because of this. Was always groggy and lethargic. Thought I was lazy. Trying out different strategies to regulate my sleep. Hopeful that I would be able to lead a normal life one day.

I didn't even know this was a recognised medical disorder. I told my Doctor over 7 years ago that i must have been born on mars because my body clock was set to 26 hours, his reply...? Drink less coffee and set an alarm :(
I manage to 'sync' every 10 days but the cycle starts again. I have pretty much cut myself off from society as it is impossible to operate on their 'normal' clock. Work, family, shopping for groceries, eating, talking on the phone..it all goes out the window.

Thank you!!! I’m in the worst part of the cycle. Where there is no off button in my brain right now. I’ve had about five hours of sleep in the last four days. I’m totally out of sync with society. A friend had to come help me get myself together for my day.
What makes it worse is the sleep I’ve had is not restful in this phase. I had two sleepwalking episodes, woke up walking into my closet door. And I fell out of bed. Ugh.
I have found non traditional jobs work best for me. I am so grateful for 24 hour businesses.
Belsomra has worked best for me but is so expensive. Things are so bad at this point that I will probably have to pay the 470$ for the rx.

I’m certain I have delayed phase sleep disorder. Even when I force myself to get up at 6am I still can’t sleep till 4am. I tried to get a diagnosis and was told I had to force myself to wake up early every day for two weeks so I’ll be tired at night. It didn’t work and I made myself extremely ill trying. I’ve been in bed since 9pm trying to sleep and have only just now (at almost 4am) picked up my phone. That’s 7 hours of laying there doing nothing but get pissed that I can’t sleep!

My psychiatrist diagnosed me with delayed sleep phase disorder last year.
I can remember back to age 5-6 not being able to sleep when other kids were, and from that age it only got worse for me. Twenty years later it has impacted my life like crazy. I usually go to sleep around 3 am and wake up at 11am or noon, but I’d happily sleep in until 3-4pm given the opportunity. During the covid lockdown in my city I just let myself fall asleep whenever I felt like it and I was nearly nocturnal after doing that for a couple weeks. Ive used light therapy, sleep deprivation, etc. but nothing is super effective for me. I have a few other severe mental illnesses and it’s kind of a circle of one making the other worse.
It was hugely helpful to be diagnosed though, because I was so sick of being guilted by morning people like I’m a lazy pos when I am actually very driven, creative, and productive… just not at the same hours as most people.
Living with delayed sleep phase has been more shameful to me my entire life than my depression, anxiety, and even my periods of psychosis.

Same. For me it is because of chronic lyme. My sleep disorder started at the same time all my lyme symptoms (all kind of pain in my body) started, and since they somehow ease up at the evening and night, I am up and feeling more alive at night. But it is tough because I hardly see sunlight and my body needs a lot of vitamin D.

I have for the longest time suspected that I have non 24 hour sleep disorder. In highschool it was a big problem, I had trouble falling asleep, sometimes I slept right after school, sometimes I slept normal. I was almost always tired and would sleep basically when I could and was sleepy enough. Now I have been two years doing nothing with my life, on "early retirement". Now I can sleep, I'm not tired all the time, but my sleep goes forward all the time, usually by 2 - 3 hours a day. It feels so much better to be able to sleep, but it makes getting back to studying very challenging

So glad to have come across other sufferers. I am having a problem with my GP giving this disorder any consideration. My gp would like to treat me for depression as opposed to listen to the explanation that the sheer exhaustion & stress induced from trying to maintain an ‘unnatural’ schedule for so many years is a huge factor in said depression & anxiety. After years of what I labelled insomnia I started to do my research and it’s lead me to DSPD. It is my life on a page. My natural rhythm is 4am - 12pm but owing to work responsibilities & school etc I have to wake at 7am most days. I am beyond exhausted and have felt this way for as long as I can recall. I’ve always had problems keeping jobs and maintaining friendships due to my erratic sleep pattern. The problem has been catapulted ten fold since having a child and losing out on even more sleep than what I used to. Does anybody have any information on sleep clinics/doctors in the UK who will accept self referrals? Thank you in advance!

Sighted individuals with non-24 are often mentally ill. And it isn't difficult to believe that they started out not as severely mentally ill but this disorder completely unraveled their functional abilities. It can be deadly. I treated one sighted client with non-24 disorder. He had not received enough treatment and had spiraled into a non-functional state. Unable to work, chronically physically ill, needed aides to come to his home. One step away from being sent to a nursing home. Severe mental illness and now severe physical illnesses (multiple).

I have delayed sleep phase disorder with occasional noncircadian days. I do believe I've had this since birth, so most likely hereditary for me. Apparently I was a bit of a curiosity in the maternity ward as a baby, because when the other babies were sleeping I was wide awake and fully engaged with my environment (and I slept very little). Right from early childhood I was fatigued and ready to sleep during the day, but come nighttime I was wide awake and had all the energy in the world. Christmas time was interesting, most children are up at the crack of dawn to open their presents, I was still asleep past lunchtime because I hadn't actually gotten to sleep until around 2 in the morning. I very rarely sleep before 2-3am, sometimes later, and regardless of how little sleep I've had the day before once it hits around 10/11 pm I am awake, focused and energised (and still not sleeping 'til at least 2 am or later). I've tried light therapy, a controlled forward shifting of sleep time by 1-2 hours a day, various types of sleep therapy (including CBT-Sleep), sleep hygiene, sleeping pills, and so on - and nothing has worked. These days I just let my sleep/wake cycle work how it works and I structure my day/life/work around it. Took me a while to stop fighting the body clock I was born with, but I find I'm much less stressed and actually have better sleep quality (albeit during the day time mostly) than I did when I was trying to force myself into a 'normal' sleep pattern.

I can see fine with glasses but I am completely convinced I have Non-24-hour sleep-wake disorder. My doctor foolishly told me I have insomnia. I showed her a sleep journal, which looked IDENTICAL to all the ones you see when you look the disorder up on google. It's not that I have trouble sleeping, it's that I get tired and tired later and later each day no matter when I wake up. I have had this issue since I was 13-15 years old and nobody has ever diagnosed me. It is completely debilitating. I missed SO MUCH college because of it, and no matter what I do, I can't seem to fix it. I did have some success at slowing it down quite a bit, to where over 3 weeks back in September, it only moved ahead 3 hours (whereas normally I would've already gone around the clock at least once), but it still wasn't enough.

I'm a registered nurse work full time shift work. I am missing too much work because I just cannot sleep when I need to and I'm switching my circadian rhythm so quickly- every 2-3 days. I'm feeling so frustrated because I'm trying everything to produce healthy sleep hygiene. Exercising, exposing myself to sunlight early in the day, eating healthy, not eating late, not using electronics 2-3 hours before bed.. . I keep going to my doctor every 5 months or so for benzodiazrpines, I know they are addictive so I take them very sparingly. One 30 day prescription lasts me a few months, hence visiting my doctor every 5 months or so. I'm so frustrated, I feel guilty for calling in but I don't feel safe working on my feet for 12 hours, rarely taking a break when I don't sleep for 24 hours before work.

Great explanation. Further discussion on treatment and effects would be appreciated. My daughter was just diagnosed with N24 and is sighted. Two months of a to gigaton y confirmed it. She recently turned 18 but has struggled with it since age 11. She has been literally unable to attend school since 5th grade. Depression due to isolation and occasional suicidal ideation as a result. No one took her seriously as she and we described this shifting sleep cycle. We just got endless lectures on sleep hygiene. She eventually got her GED and started college online. Once she turned 18, finally doctors gave her a referral to a sleep specialist and she got her diagnosis. From what we under, treatment options are limited and not always very successful. If they do succeed, they require significant dedication and work to maintain or you’re back to square one. And it is lifelong. So many aspects of life are affected by this. I would appreciate a deeper dive. Thank you.

    • @Cathy-xi8cb
      @Cathy-xi8cb Рік тому

Nope; you got all the info on non-24 there is out there. The drugs are limited, and the effort is lifelong even with the drugs. She has a chronic brain disorder. No cure, few treatments. Left untreated, she will have no life at all and she is at risk for severe physical illness. There is no deeper dive unless you want more information about how badly her health will become if she doesn't adhere to the treatment. There is a lot of information about that, but none of it is good.

I am so tired … pun intended.. of people telling me to set alarms, go to bed one hour earlier each day, etc.

I work myself into anxiety over this quite often... as I write this, I am feeling the anxiety in my upper stomach. It's 3:54am, and I am kinda sleepy, but not at point where I can lay down and go to sleep. I am at the 8pm ish for day walkers... best way to put it. If I go lay in the bed I will experience, mild form of anxiety, I will have hot flashes, I will toss and turn, constantly waking my wife, because I am trying to force the sleep to happen when I am not fully sleepy. (because I don't wanna be up at this time and I know things I have to done tomorrow!, its outta my hands) She is a engineer and her job is quite mentally exhausting and it gives me anxiety even more to know I am waking her up but I can't help it. I took melatonin past couple night, it made me so tired but still I couldn't sleep until 5-6am. Next week I will be staying up until 9am, then couple weeks after that I will be passing out at 2pm, it's FOREVER rotating. My wife understands because we live together, my family does not understand nor do friends, they think I am just lazy and want to sleep all day.
I still get up and do house chores, tend my children. I wake up before they get home from school, right now, but soon I will be passing out after they get home. I will force myself to stay up when that time comes until my son is off the bus in the house and I make him a snack until my wife gets home. I try my best work my sleep cycles around my 2 kids life, luckily my daughter is 16 and can fend for herself, my son is now 11. My son tells people ya my dad he sleeps all day! Its so embarrassing he is getting better now at understanding but I honestly can not help what my body is doing. I feel lucky to be married to successful woman whose beautiful and willing to put up with my retarded sleep patterns. I have tried everything, I have forced myself to stay up until I am so tired I pass out... just sit like a zombie waiting for evening to finally sleep to try to get my sleep on track. Sure its on track for about 2 weeks (sometimes less) then it starts rotating! I can't sleep until I am sleepy, and nothing I can do about it! I wanna be sleepy at 10pm, wake up at 6am and go about the same and even when I get to that time frame from my rotating sleep I can't keep it there... my body just won't, sadly its completely out of my hands.
I am 35 and I have battled this since I was 15 years old... and I beat myself up every day over it. I want to get my CDL's and drive semi, I love traveling, and being able to work alone but its impossible because of my wonky sleep pattern... I have had jobs and it didn't change anything! I was staying up getting 3-4 hours of sleep and going to work, working jobs where I am walking poles and beams 100ft in the air (apart of the job) so exhaust I am a zombie... dangerous in that line of work. I tried some night shift jobs, but I can't do em because my sleep ends up rotating back to daytime. Even working exhausting jobs I couldn't keep my sleep stable, as in work hard as hell all day then go to bed at night, doesn't work with me. I always work myself into a bit of a panic too when it rotates... am I ever going to be sleepy again? this is it, I am going to finally die from lack of sleep. Its freaking terrible, at times you just need someone to tell you its normal and it will be just fine. However I know I am not normal but I long for a group where people are just like I and we can discuss this topic. Even doctors do not understand, they just say have to turn off TV, computers etc. and get ready for BED at same time every night. Nah that doesn't work, and computer don't keep me up any longer when my sleep hits it hits, its out of my hands its out of the computers hand. I am victim of my own biological clock right now, and probably will be forever.

I am remodeling a room in my house, that's on hold because of my current sleep pattern but once it rotates back to me being up in daylight hours I can get to work finishing it. Yea this is my life, and people do not understand it... I don't even understand it.

Hi John. I be interested to see how you do using bright light therapy.. You would use it within the first 30 minutes to an hour of waking up each morning. You have to set your alarm clock to wake up then use the light. That's how people use it to advance your sleep. (move it forward).

Okay great. BTW - Not sure if you saw my video on strategies for bipolar disorder. I discussed dark therapy. I plan to do another video on dark therapy because there's a growing body of research that supports its use in regulating sleep in anyone not just people with bipolar disorder. I've been experimenting by wearing the glasses in the evenings even if I'm not using a mobile device. The recommendation is to use them at least two hours before your desired bedtime. Here's the video where I first mention it ua-cam.com/video/awPP5YrVGyY/v-deo.html So far my one person result is if I start wearing the glasses around 7pm, by 9pm I'm super tired. Not sure yet if it's coincidence, but I like it so far.
All this to say for you, maybe a combination of dark and light therapy will be what you need. Good luck.

I've had Delay phase sleep disorder since I was a kid and I wasn't one of the lucky ones that grew out of it lol.
It wasn't until I went to college and had an afternoon class schedule that I learned I could be a motivated and normal functioning human being because when I was a kid/teenager I didn't give a &#$@ about school. I was definitely pleasantly surprised that it was that simple.
The only time DPSD was helpful was when I was in the military. It was like having a plus 5 alertness skill. I also didn't have a problem with staying awake for long periods of time and only getting a few hours of sleep. I definitely do now.

Random question, but do you consider melatonin paired with light therapy effective for these types of disorders? I had to shift to days for medical training, and I'm having insomnia. Didn't sleep really at all last night; and the day prior to that I maybe got 3-4 hours. I've just begun melatonin and light therapy, and I've decided to start going to bed now at the same time every night.

      @DrTraceyMarks  6 років тому +1

Hmmm - without watching this video again I have 2 possibilities. I shot this video in 2012 when we were using the DSM4. The nomenclature was different then, in fact I believe in DSM5 it's under the general category of sleep wake disorders along with a lot of other sleep disorders. The other possibility is that even using the DSM4 I may have left one out to keep it simple, but I think it's more likely that DSM5 has reorganized how we label the disorders.

Does the circadian rhythm timing change according to your sleep pattern, or is it fixed...regardless of when you sleep or wake up?
Some hormones follow your biological clock (circadian rhythm) - being high in the morning and low in the evening...but I'm wondering if they'd change to being high in the evening and low in the morning if you're sleeping in the morning and waking up in the evening.

If it’s a disorder you don’t get the right signals at the right time, you’re not staying up because you want too, you’re staying up because even if you feel tired and you want to sleep, your body will not let you rest because those signals are hours late

Hello Dr Tracey. I have a really weird question... I was coming out off a short period of little sleep. I had really much stress and anxiety because of all the sleep loss. I did everything to sleep better like fasting, sleeping pills, sleep music, meditation, exercises, daylight. The only thing that now works a bit is daylight to fall asleep faster.. So now i sleep a lot better and longer, and i sleep really deeply now. I know sleep 10 minutes shorter than the average person (7 hours) I know that sleep loss is wortheless, but it it irritating me. They say how better the sleep quality the less sleep you need. But thats not what i want. So my weird question is why can't i sleep longer and how do i sleep longer? It’s annoying because I want a job, a life but I can never seem to keep a regular sleep pattern.

I have delayed sleep phase disorder. I was diagnosed very young (around 9 or10). It runs in my family. We’re all vampires. I’ve managed to work my career around my sleep schedule, but its increasingly become a problem in my relationship over the years. My partner and I have been together for almost a decade, but recently he’s becoming more and more frustrated that I’m not awake to have a day hike with him or brunch with friends. I feel like I’m failing him in that regard, like he deserves someone who can do those things with him. We are happy in every other way in our relationship, but I feel like maybe he wants someone to enjoy those things with. Does anyone have any advice?

Hi Dr. Marks. My problem is that I "fight" sleep. I don't want to go to sleep, I do anything and everything on planet Earth trying to avoid sleep. At 10 pm my eyes pop open, I'm wide awake, and actually, I'm looking forward to the night (or the rest of the night/early morning) because I consider that "me time" - when nobody bothers me, all that time is just for myself. When the evening and bedtime approaches, I become "uneasy" about the thought of having to go to bed. What is this condition? I've been googling it but haven't really come across anything that could help me. (I'm writing this at 6:13 am, I haven't gone to sleep yet, must get up in 2.5 hours and I'll have a meeting at 10:30 where I have to speak. Not looking good.) Thank you!

But I never feel fully rested cause I wake up every hour... throughout the day I’m very tired and just want to go back to sleep... this has caused me so much anxiety cuz I always fear I’ll have the same problem the night.. it’s rlly made me depressed and I don’t know how to fix it and sleep like a normal person

You could try some relaxation activities before bed. You also have to take a look at what your day is like. Because what you do during the day affects how you sleep at night. there are lots of things that affect your sleep and make it broken like anxiety, stimulating foods, needing to urinate, being too hot, hormonal changes etc. There’s lots I could go on...
all this to say if you want more specific help for overcoming this you may need to speak with a doctor or a therapist to get help on sleeping without interruption.

I think I have one of these because I have trouble falling asleep; I've had it for about a few years, i didn't think much of it, but it started off as 10 minutes to fall asleep, then 25, 40, 1 hour, and now it's hours. Sometimes I don't want to fall asleep, and sometimes it's where my mind won't shut off or be tired. I also realized I have trouble staying awake. But it's not insomnia because it's not thoughts that keep me awake. I've tried counting sheep, thinking about just nothing, all lights off, that kind of stuff.

If you have non-24 sleep disorder, wouldn't the ideal option be to live according to YOUR length of day? Sleep for however long you need, then stay awake until you feel tired. Of course, I get that it would be unrealistic with many work schedules. Since I was a child, I always wanted to stay up late and had extreme trouble with maintaining a consistent schedule. There are so many times when I've had to completely reset my sleep time to exist in society and I end up feeling sleep deprived most of the time. Over the past couple of years as I've worked from home my sleep/wake times have literally circled the clock as I go to bed later and later each day. Recently tried polyphasic sleep to see if it would help, but the periods of sleep in those schedules seem to be way too short for me as I need a fairly long block of uninterrupted sleep and hate taking naps. Trying to figure out what to do next🤔

Chronotherapy is a behavioral treatment for sleep where you move your bedtime back an hour or 2 each night until you get back to falling asleep at night. It's pretty hard to do, especially when you're sleeping during the day. But the idea is that it's easier to stay awake than to make yourself fall alseep earlier. Fortunately bright light therapy can help pull your sleep forward or help keep you regular.

The first time it would be establish a regular sleep schedule. When your sleep shifts around it has a negative affect on your body clock. Then you can get wacky sleep and have trouble sleeping when you want to. A disrupted circadian rhythm can also result in you Feeling tired even when you do sleep.

Early morning bright light therapy can help pull your sleep forward. You could start doing it was in the first 30 minutes of waking up for 15 minutes to 30 minutes

I'm 99.9% sure that I have Dsps (delayed phase sleep disorder) I literally can't fall asleep until 3:00am - 4:00am and then I wake up at 2:00 in the afternoon, my mom says I'm just being dramatic and trying to stay up but it's the complete opposite. She actually gets mad at me for "self diagnosing" but I'm just saying

That's anxiety related, when you are concerned about falling asleep and how tomorrow will be a crappy day as a result of not sleeping.
I was a night person but fell asleep easily at any moment, recently i got hit with an episode like yours - the key is to let the mind go and reset the clock.
Cannabis oil is amazing, tart cherry supplement is supposed to be good and of course Melatonin.
Resetting the clock and keeping a strict schedule is key.

Wake up 30 minutes earlier each day until ultimately wake up at the time you want. Takes about 10 days. But it is tough. The hardest part is not falling back to old habits. Always wake up the same time
