primal fears are more complex with their abilities than regular devils, like the darkness devil separating people from their limbs without a drop of blood or the fall devil making people want to test the fall damage, it's what makes them so powerful and cool so if the death devil just causes people to die thats gonna be pretty boring
I got a silly theory with little proof but with pochita being the primal fear of being forgotten or extinction. The "chainsaw man" that killed yuko after her contract with the justice devil was probably the true chainsaw devil and it'll be a plot point when Denji learns of him and realizes him and pochita are more than a chainsaw devil. We've only seen two primal fear devils so far and it's too early to say but it's looking like a reoccurring design for primal fears is for them to have multiple limbs and pochita's true form in part 1 having four forearms gives me primal devil vibes after thinking about it.
