    A 35-year-old lady asks a general question about platelet-rich plasma (PRP), presumably for facial rejuvenation. She asks how long the treatment lasts, and if the injections will leave marks on her skin.
    New York oculofacial plastic surgeon Amiya Prasad, M.D. reviews the photo she submitted along with her questions in this video, describing what PRP is, and what it is proven to do:
    1:02 - Dr. Prasad’s experience and use of PRP in several different areas of his practice
    2:02 - Why referring to PRP as a filler is incorrect
    2:17 - How PRP was first used by oral and orthopedic surgeons for its effects on tissue, which lead to it being used for skin rejuvenation in the cosmetic field
    2:53 - How PRP uses growth and healing factors from the blood to stimulate collagen, blood supply, and improves health of fat cells
    3:35 - Examples of PRP treatment for dark circles, fine lines and wrinkles, sun damage
    3:56 - Results of PRP combined with extracellular matrix to stop hair loss and thicken hair
    4:02 - PRP for overall complexion improvement to give the skin a unique glo
    4:43 - Combining PRP with other materials like hyaluronic acid fillers
    4:55 - Skin rejuvenation through skin quality improvement using PRP
    5:12 - The variation of the onset of PRP’s effect
    6:04 - PRP normally takes more than treatment, even though it doesn’t wear off
    7:16 - Why minimal marks and swelling on skin happen, but giving a day to heal is a good strategy

    In my case i am said that prp with micro needling can be done after 21 days again. 1)Is it too early to go for it after 21 days? My skin doesn't have any big issue. I just wanted to have a brighter looking skin and a better texture... So.. I consulted them.
    2) Can a filler help in lifting the drooping eyes? Because When i enquired and wished for a blepharoplasty, I was suggested for filler and avoid surgical way. Is it safe taking a filler for eyelid lift or in getting a double fold on the eyelid?
    3)Is it safe to get fillers injected while MY PRP and micro-needling going on?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  6 років тому

      Hi gerik jini,
      Getting microneedling with PRP done again after 21 days depends on the depth of the microneedling, but it is possible.
      Filler can help with upper eyelid hollowing, which may give the appearance of drooping eyelids. However, if drooping is due to excess eyelid skin, or a weak eyelid muscle, then surgery is needed. Filler cannot give a double fold, and surgery, maybe tape, can provide a double fold.
      You want to have any filler treatment done after recovery from microneedling. Recovery from microneedling depends on the depth. I hope this clarifies things for you.

    Hey sir, thanks for the post. Just a couple of questions if you don't mind... What is the currently thought process of platelet activation (calcium chloride for example) prior to treated, esp in regards to this turning the liquid (PRP), to a more jel-like substance (PRF), and wouldn't the change your treatment methods (as one is liquid and the other not)? Furthermore, what are the differences in PRF/PFRM in relation to just activated PRP (other than the use/non use of anti-coag)? Thank you kindly!

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  5 років тому

      Hi Dynamic Osteopaths & Regenerative Medicine,
      Platelet-rich fibrin matrix is used when a more gradual release of PRP is wanted. In most cases, I do use regular PRP, but PRFM created with PRP and calcium chloride is an option for treatment. I hope this clarifies things for you.

    If after 3-4 session of PRP hair treatment if I stop does my hair quality remains same or I will be change to the previous one

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  6 років тому

      Hi Sonia Sadhu,
      I don’t administer PRP alone for hair loss because it has a limited effect on hair, usually just short-term hair growth. Also, the number of sessions needed does vary, and the longevity of results is unpredictable. Hair Regeneration is a PRP combination treatment where PRP is the activator of the main ingredient extracellular matrix by ACell. Hair Regeneration is a one-time treatment for most patients, and can last up to 5 years and beyond, depending on the aggressiveness of the hair loss pattern. You don't need repeat injections periodically, and many older Hair Regeneration patients don’t need maintenance drugs like finasteride, but finasteride can concurrently help younger male patients with very early and aggressive hair loss. PRP injections for hair loss are needed indefinitely.

    How does PRP compare to getting the stem cells from fat and having that injected? I realize injecting fat creates volume and PRP does not, but in terms of skin rejuventation, is it the same effect?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  5 років тому +1

      Hi GirlyMuscleTV,
      I don't do fat transfers under the eyes as they are highly prone to complications, which you can see in this video - ua-cam.com/video/mNo_EBcDi1k/v-deo.html
      I don't do stem cell extractions from fat. The effects of PRP on skin quality, luster, and glow are not seen from other treatments. I hope this clarifies things for you.

    do you think microneedling alone, with out injections, is still effective w/ prp? I noticed you do injections, but most aestheticians only use a needling pen.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  3 роки тому +1

      Hi Jennifer Cvammen,
      I also do perform microneedling, with PRP then applied to the microchannels created by it. I also perform a milder form of microneedling that also uses hyaluronic acid and PRP. There are different collagen induction treatments, and the appropriate modality can be chosen after physically assessing the issue to be treated. You can refer to this more recent video about it - ua-cam.com/video/UzNvtn0TxMc/v-deo.html

    Hi Dr. Prasad,
    I had Microneedling (1-1.5mm) with PRP. The provider used a dragging technique and i have some marking on my face 7 months later (i.e. the texture of my skin is not as smooth as before, appears to have some scarring lines). My skin and general facial experience also now looks “older” or aged. Prior to, i had completely smooth skin (no breaking out or acne, scars or anything) and now realize i should not have done it/was not a suitable patient for it, but it was pushed to me as a “preventative” procedure although there was no reason for it to be recommended. You can easily notice where it changed the skin texture/caused lines when i shave fully as i have a beard and didn’t do it in the area of my beard.
    My question is, is 1-1.5mm microneedling with PRP possible to cause facial volume loss or is it simply not deep enough? I just had it about 6 months ago, and have noticed quite a bit of fat loss which i believe is unlikely to be from aging only seeing the short amount of time, that i eat a strictly consistent diet, and have had no other procedures.
    Thank you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  Рік тому

      Hi TheMFoxx,
      Microneedling with PRP is unlikely to cause facial volume loss. Facial volume loss occurs with overdone laser treatments that can cook the essential fat layer beneath the skin, but microneedling does not have the same effect as there is no overexposure to heat, or overheating. I would have to perform a proper examination to determine the cause of your issues. I hope this helps you.

      Hi What's inside that heart ,
      PRP injections alone can be very effective in improving skin quality, increase blood supply, softening scars, and to make the skin thicker. It really depends on what you want to treat. As a member of the Vampire Facelift Network of doctors, an examination is needed to determine what the best treatment for your case is. I hope this clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  7 років тому +2

      Hi Droni0201,
      Sorry to hear about your problem. We use PRP for hyperpigmentation when there is too much pigment, but it is not likely to be effective for hypopigmentation where pigment is lacking. Destroying the cells responsible for pigment called melanocytes is a risk in laser treatment. It is best to see the doctor who performed your laser treatment about your options. I hope this helps you

    • @AmiyaPrasadMD

      Hi Jenny Quezada,
      The effects of PRP do last as the improve the skin from within, and treatment builds on results of past treatment. Maintenance treatments may be done one to two times a year, depending on the treatment area, and the person. I hope this clarifies things for you.

      @AmiyaPrasadMD  6 років тому

      Hi Fatty 31,
      I normally administer PRP for my patients with dark circles, which may take several treatments. This can be applied with a hyaluronic acid filler like Restylane for hollowness, and perhaps a laser treatment as well. Each treatment is customized for a person's individual needs and anatomy, so I would need to perform a physical exam before recommending the best treatment or combination of treatments. I hope this clarifies things for you.

    • @Fms1381
      @Fms1381 6 років тому

      thank you for the reply,my age is 23 and since my dark circles are genetic so doctor said that as per my age one treatment would be enough and it would show results in a period of 2 months approx and after seeing the results she will see if i need another treatment or not.
      secondally i was told that fillers wont work for me as i have more of dark circles and less of depression(this word was told by my doctor) and i should go for laser but that machine is not available in my country.
      so m really confused as one says go for prp complete face and other says laser.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  6 років тому

      Hi Fatty 31,
      I hope you understand that I can't make a specific recommendation of one or the other for you because I haven't examined your skin like your two doctors have. Also, I often combine laser and PRP for a synergistic effect, and to also reduce laser exposure without reducing collagen stimulation. It is possible to do both if your doctor has experience with treatments. I hope this helps you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  7 років тому +2

      HI John Peltrau,
      Thank you for your question. I review each patient's medications and supplements before advising any adjustment after treatment. As a general rule, anything that helps healing is beneficial after platelet-rich plasma (PRP) injection. Thus begins with avoidance of smoking and excess sun exposure. I don't usually restrict supplements or caffeine. I hope this helps you.

    • @jcrockoo7
      @jcrockoo7 7 років тому

      +Amiya Prasad, M.D. Thank you. I had one treatment and it seems like the effects have gone away after 3 weeks. So, I thought that maybe the one cup of coffee, two cups of green tea, and the multi-vitamins were having an effect on the blood platelets. I'm glad to hear that I did nothing wrong. I have another treatment in a month, and I'm hoping that the results are better and longer lasting.

      Hi shreegowthami 356,
      PRP is not really used for lightening tanned skin, but it can be used to improve the skin quality of sun-damaged skin. If you are really concerned about getting tanned, I suggest you continue using sunscreen regularly, and cover the areas you don't want tanned with a hat, clothing, or even an umbrella/parasol.

      Hi Aliya Mohd,
      PRP can help improve the appearance of acne scars by improving skin health and skin quality. However, when treating acne scars, PRP is usually not the only treatment used, and other modalities are usually needed. The number of treatment sessions does vary greatly as acne scars usually need an ongoing treatment plan, and severity, depth, and type of acne scars are not the same among people. Hi GotherL,
      I do recommend you stay with the same doctor. As for repeat sessions, find a doctor you trust and who can guide you on how many treatments you need for best results because they do depend on the health of your skin, and any issues you have prior to and after treatment. I hope this clarifies things for you.

      Thanks, I am doing prp+microneedling 3 session. About the trust thing I just get this nagging feeling she only cares about my wallet. She asked if I worked or not and when I said no she seemed disappointed. She pressurized me twice to buy the iS Active Serum which only broke me out even when she is aware my skin is extra sensitive where alcohol would be bad for my skin. And I can almost swear she seems to miss some spots on my face intentionally. I've a lot of blackhead scars on nose and every single time she only does a small part of my nose with the microneedling then applies the prp on my nose. I don't know maybe it doesn't matter if she doesn't do it throughly? (Trying to think positive but I am obviously suspicious of her motives.)

      Hi A P,
      It's not an either/or situation, as the materials have different roles. PRP can help with skin quality improvement, discoloration, pigment, and skin thickness; while filler is used for volume to fill the hollow. I hope this clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  5 років тому +1

      Hi Jaime Gasca,
      Yes, PRP facial injections can be done as they are for improving skin quality, texture, and health, and not a significant treatment for facial volume. PRP is injected at a more superficial level than Structural Volumizing/ The Y Lift. A Vampire Facelift is not PRP alone, but a specific procedure of PRP and one syringe of a hyaluronic acid filler, and is only done by the Vampire Facelift network of doctors, which I am a part of. I hope this clarifies things for you.

Thanks in advance

      Hi nas s,
      While PRP is generally safe to use repeatedly, using it that many times within a month is not necessary. While PRP is useful for skin rejuvenation, I do often combine it with other treatments like laser, Botox, hyaluronic acid fillers, HydraFacial, etc. as for more comprehensive treatment. While generally people over 45 can have PRP maintenance treatments about 3 times a year, there is no set number that applies to everyone. Age is not the only factor determining treatment, as lifestyle factors such as sun exposure, smoking, alcohol intake, diet, etc. also contribute to appearance. I hope this clarifies things for you.

      Hi Biz888,
      PRP is not a filler, so the effects don't necessarily fade like a filler. PRP improves the health and quality of the skin, and subsequent treatments build on those effects for better results. However, PRP does not stop the aging process of the skin. I hope this clarifies things for you.

      PRP definitely improves skin quality, texture, and overall skin health, which I have seen time and again in my clinical practice, and my patients also consistently see its benefits. There are studies that show its efficacy on improving the skin, which are linked below. You need to consider who is funding the studies you refer to as PRP is not produced by any pharmaceutical company, and there is no way it can be patented or sold by a major drug company. Hi QueenBee,
      Injecting PRP and microneedling are for two different indications and treatments. Injecting PRP is for skin quality improvement, collagen generation, increased blood suppy, and for issues like dark circles under the eyes. Microneedling also helps skin quality and generates collagen, but this is mainly done by causing micro injuries to the skin, so the skin has an injury response by generating more collagen. In microneedling, PRP is used topically, and rubbed into the micro holes caused by the needles, but PRP is not absorbed as efficiently when compared to injection. You certainly can have both techniques done for different areas of face, but make sure your practitioner explains clearly why they recommend both. I could give you a more definitive answer and recommendation if I'm able to perform a physical exam of your facial skin. I hope this clarifies things for you.

      Hi Ramani Tennekoon,
      Thank you for watching! While PRP is a lasting treatment as it treats the health of the skin from within, it can't be considered "permanent" as it doesn't stop the skin from normally aging.
      There is no set amount of PRP treatments as it's not clear what you want treated, and you won't know how you respond to PRP treatment without trying it first. PRP treatments also build upon the results of previous treatments. I hope this clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  4 роки тому

      Hi Duchess DelaRue,
      There are risks in injections under the eyes as the skin is very thin, and the under eye area is very delicate. However, the skill and experience of the doctor does reduce the risks. There is less chance of vascular occlusion with PRP than there is for filler as PRP would not block a blood vessel the same way filler would. I hope this clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  5 років тому +1

      Hi Vishal Pandey,
      It isn't possible to treat hollowness under eyes with one procedure that lasts a lifetime. Hyaluronic acid cosmetic fillers are easily accepted by the body, but get metabolized in about a year. Tear trough implants do last longer, but many patients find them uncomfortable, and eventually won't fit as the facial dimensions get smaller due to aging. Permanent and semi-permanent fillers are not safe to use as the linger and migrate in the body, and the body can have a foreign body reaction or infection due to them. I hope this clarifies things for you.

    how does it work on inflamed skin like Rosacea, acne, serreborhic dermatitis, Accutane damage skin? can you just put it topically on the skin and not needle or inject anything? The skin is way to sensitive!
    do you treat other areas with prp? what about fibrotic issues that are mediated by tgf-beta1 which leads to collegen deposition/scar tissue? why would injecting prp with tgf-beta and collegen growth factors be good for fibrosis? thanks!

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  7 років тому +1

      Hi Kyle A,
      Rosacea we do address with a laser treatment. We do use PRP to treat acne scarring. As for Accutane, you do need to be off the medication for a year before we do injectable PRP. PRP can be applied topically after microneedling, but applying it just topically without channels to absorb it like microneedling will not do much - PRP needs to enter the skin for effectiveness.
      We have successfully used PRP to treat hypertrophic scars. Scars need to be evaluated in order to determine if PRP could have potential benefit. Scientific references for scar treatment can be searched in the national library of medicine database if you'd like to learn more about the biochemical pathways. I hope this clarifies things for you.

It won't be painful as long as your doctor uses a topical and/or injectable anesthetic prior to treatment. Hi honey s,
      PRP can be used for hyperpigmentation, but it is not a treatment for overall skin lightening. PRP can be used to treat fine lines, along with laser or radiofrequency treatment, but an exam is needed before heat treatments on darker skin. Other lasers such as Q Switch can be used for dark spots, even on dark skin. There are many treatments for acne scars, so a physical exam is needed to determine what kind of scars you have before determining mode of treatment. Acne scars take more than one treatment session, so it best to stay with one doctor and follow their treatment plan. You can watch these videos about acne scar treatment and dark spot treatment:

      Hi honey s,
      I normally don't recommend dermarollers because depth is hard to control, professional experience is needed in microneedling, and the rolling action stretches microholes unnecessarily. Professional microneedling can be an option, but I would need to do a physical exam on your skin before determining if it's the right treatment for you. I hope this helps you.

      Hi shaz7642,
      Sorry to hear about your problem. I normally combine PRP with a hyaluronic acid filler for hollowness in the under eye area such as Restylane or Juvederm. PRP improves the skin quality, skin thickness, increased blood supply, and pigmentation, and the filler helps with hollowing. Both materials combined have a synergistic effect, for both better results and also for both materials to last longer. I also suggest utmost sterility in you treatments as your problem may not have been the material itself, but external factors and environment. I hope this helps you.

      Sorry, but that's not true in several aspects. PRP may take more than one treatment for best results. PRP or any other treatment cannot stop aging, so no treatment lasts forever. I hope this clarifies things for you.

    Does PRP helps to lift dropy eyes

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  7 років тому +2

      Hi Sim sim Khan,
      Sorry, but droopy eyes are either caused by excess eyelid skin, or a damaged or defective muscle that lifts the eyelid. Droopy eyes is treated through surgery, but it is important to seek an eyelid specialist (oculoplastic surgeon/oculofacial plastic surgeon) if the cause is muscle-related. Also, there are possible neurologicial causes for droopy eyes, so these will need to be ruled out before corrective eyelid surgery. I hope this information helps you.

      Hi Crystal chan,
      At your age, it really depends on what you would use PRP for. If it is used to improve scars, then by all means, but cosmetic treatments at your age are usually not needed or recommended. I hope this helps you.

    how long does the prp effects stay? how often should one repeat it post 40

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  7 років тому +1

      Hi Rubina Guleria,
      PRP does need repeat treatments, but it all depends on person. The actual effects of PRP on improving the skin do not wear off, but building on improvement does take several treatments, depending on what area you want to treat. Combining PRP with fillers for say under eye hollowing actually extends the effective life of the filler. You can expect treatments of PRP alone to be twice a year, but this could be more or less, depending on issue being treated, and the individual reaction of the person. I hope this clarifies things for you.

      Many thanks. Is PRP effective for melasma as well? How important / effective would you rank it in melasma treatment protocol?
      Thanks in advance Amiya

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  7 років тому +1

      Hi Rubina Guleria,
      In our practice we treat melasma with a protocol which includes sunblock, tretinoin, hydroquinone, glycolic acid and Q switch laser. In addition, we often find the same patient also has solar lentigo and non melasma related hyperpigmentation and photoaging. In those areas, there can be a role for PRP. I hope clarifies things for you.

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  6 років тому

      Hi mammukka bhakthan,
      The Vampire Facelift is a very specific procedure to help with volume loss and skin quality from facial aging, but your dark circles and under eye hollows may benefit from a hyaluronic acid filler and platelet-rich plasma (PRP) performed in the under eye area. Hollows may also be due to adjacent puffiness causing the area to look hollow, so you will need a physical exam to determine if this is the best treatment for you. I hope this clarifies things for you.

      Amiya Prasad, M.D. sir iam 19 years old i can doo this prp method for my under eye hallow any promblem doing prpbin this age?

    • @AmiyaPrasadMD
      @AmiyaPrasadMD  6 років тому

      Hi mammukka bhakthan,
      I would have to physically examine your eye area before making a definite recommendation, but PRP and a hyaluronic acid filler are both generally safe. Once you start using filler to treat hollowness under the eyes, it does need to be maintained, but it is much safer, predictable, and cost-effective than fat transfer which I strongly advise against due to its potential complications and unpredictability. I hope this clarifies things for you.

      Hi amyra fc,
      I'm not sure what you are trying to treat at your age as you are still very young, but PRP can be safely done at age 23, but the real question is if you need to do it.

      Hi honey s,
      I'm sorry, I don't administer skin lightening treatments for naturally dark skin. I do perform treatments for hyperpigmentation, but one's natural skin color should be embraced. Skin lightening treatments can be unsafe, so I advise you consider them carefully.

      Hi Brahmin creators,
      It depends on what is assessed during consultation. Frown lines can be helped with filler, Botox, PRP, or a combination, but a proper assessment of your skin is needed first. I hope this helps you.

      Hi ravi gupta,
      PRP improves circulation in the skin, as well as skin thickness and skin quality, which is a lasting change to the skin. However, PRP does not stop the aging process, so treatment may be needed again as the skin ages. I hope this clarifies things for you.
