If you have the Exclude Home option set for lock on walk away, then the geofence radius for this option is very large, at least 500m. For me, it won't lock at time local shops due to this issue. Also, intermittent Bluetooth problems with your phone or car may cause this.
As I'm walking away, I listen for the beep. Once, I didn't hear the beep and returned to find my phone still in the car. That scared me. So I've enabled PIN to drive.

this happened to me a few times on my 2019 M3. I'm now checking the app regularly if it locked. If there's 2 of us there must be more....
