That doesn't mean that they're a boy, only that they have a penis. If Chihiro is a trans girl, and thus a girl that was born with a penis, Sakura and Kyoto would only know that. Anime is notoriously bad at dealing with LGBT+ characters, especially trans characters, and if Chihiro was trans then it would make sense for Monokuma to disrespect their memory by saying that they are a boy.
Honestly from what we've been given, I don't think that he's transgender. Chihiro (Via Monokuma) stated that he wanted to be stronger so he can get away from all the bullying he's been through.
