Clarence is the ghost of the previous explorer after the abyss absorbed him. He's clearly referencing the broken/stuck mole in the room and calls it his iron coffin.
The game does depict the dirty conditions in coal mines, but some of them really cleaned and addressed the perceived grime. I've been down a few coal mines back in the day when the NCB was still a thing. We made gearboxes for coal mines, even had a few in Australia, deepest pit I went down was around 2300 feet. Controlling breathing is a must.

As a SAR team member who specializes in critical situation rescues I've seen what happens to those who suffer mental breaks, it isn't pretty. They people who survive such incidents, from getting lost/trapped in a cave/mine as well as those trapped by a collapsed structure have a high rate of self deletion after they are rescued.

Some of those places go down past the 600 meter mark and are huge with tunnels connecting to other bunkers. Went down one a few years back to do a SAR for Urban explorers/spelunkers who got lost and trapped when the tunnel they went down collapsed and one of them had to climb a vertical shaft almost a hundred meters straight up with no ladder to get a SAT phone signal to get help. It took over 70 SAR team Fire fighters and medics nearly 48 hours to get to them and get them out and they were damn near catatonic by that point, the shit they mumbled would make your blood run cold especially if you heard saw what was down there.
to be fair, the hole is only 23cm in diameter, which means that cover or not, you cannot fall down the shaft unless you are like... an infant. at most you'll get your leg stuck in there

That thing in the middle seems like a mole, a mole that seems to be covered in dirt or whatever you call the dirt in the void, it literally is the mole where Clarence died in.
Really loving the fact you can see a Mole already present in the hole and taken over by it but always while moving, so you can never actually have it in front of you. If you keep your head mostly down while moving, you can easily miss it. It adds a layer of creepiness. Do you think it was Clarence's mole?
