I actually killed the big demon before Defender's Heart. Basically you need Sendri and any other mercenary spellcaster shooting Magic Missiles while the rest act as tanks.

After reaching the Defender's Heart (the overall goal of the main quest), that fellow at 2:52 shows up and immediately teleports the entire party to the following scene (I only edited out the loading screen). He is explicitly part of Sendri's backstory (the entirety of which I included at the start). From extrapolating between what he and the unknown fellow said through him, the "Ice Flower" crystal that Sendri was searching for was in fact found and is perhaps now replacing his left eye (the previously bandaged one).
This crystal seems to be connected to a lesser/demoted demon lord by the name of Sithhud who is associated with frost and undeath.
TL:DR; Sendri may have jammed an artifact belonging to a demon lord (or a piece of the demon lord itself) into himself and thus gained powers from it and now that demon lord's minion is after him and thus by extension the party.

Any romance to be had in this side-story?

I'm here because I just finished the DLC and have been trying to find out what's going on. Not going to lie - Feel pretty salty about a "to be continued" ending... That and the sudden incredible difficulty spike regarding one battle (that, as far as I can tell, is impossible).

Was it easy to save everyone/did you save everyone? Or do some of the city's survivors inevitably die no matter what choices you make (I don't care about spoilers).

Of the named NPCs that follow you, the only one I didn't take to the end was Bemir due to his true backstory. So long as you think about the tasks that you're assigning to each of them (the gravedigger is strong but isn't shaken by corpses, so I sent him into a burning house to try to save some civilians) they'll retain their strength and if they don't you can always give them potions/scrolls of restoration when you find them. Of those who weren't following the party, there's only one individual who I can recall that died that couldn't be saved and that's because he was already dead before I arrived (and he wasn't a helpless civilian anyway).
You can spend rope/tools to get around combat, but using them in that way prevents you from using them to save people/prevent injury to your followers, so it's a trade-off. In addition, you can find supplies locked behind strictly optional fights if you need them.

@@Chrimp1724 If you don't mind telling me, what's Bemir's true backstory? Is he, like, a secret cultist of Baphomet or what?

      @@Deserit Bemir Spoilers:
He is actually a well known serial killer who strangles young women. The inquisitor officer at the cathedral northeast at the end of the Rich Quarter area will check your hands to see if any of you have 2 fingers bitten off and then inform you about him. You can confront him back at the survivor camp (he flees in a cutscene), kick him out, kill him on the spot (without warning so he doesn't flee, you get his normal hide armor) or trick him into bringing him to justice when you move to the next zone with the inquisitors (idk what you get for turning him in, maybe XP?).
Even if you cover him at the cathedral when you get to the tavern Irabeth tells you she is pretty sure the miller is Bemir the strangler and will jail him when things settle a bit.
It is best to keep him though as you don't get anything if he flees or you kill or kick him out and he has some instant sucesses during some bookstory checks just after. It makes easier to save everyone. You only need him for a short storybook section after the rich quarter and then with a almost unwinnable CR 11 boss fight (which btw you should avoid by going through the rooftops with a 3-check storybook section, for your own sanity) and then you reach the tavern.

Unless he flees he gets his just deserts at the end of the DLC?

unless you ask for their help non stop in map checks or use them on unrelated checks they stay pretty healthy, specially Bemir who hardly loses any health when asked to push something in the enviroment. Some checks can be bypassed with tools or a rope or are just optional (like some rope uses and Bemir's checks). The only problem is Langart, the bard (first one you get at the very start) bc he has terrible health and anything tires him. Just feed him a restoration potion every time he is in very bad health. He will drop constantly to near death so check him often, if the is stumbling or liying down you need to feed him a potion or use a scroll to stabilize him before resting. The grumpy healer Aina who you get in the church at the graveyard (2nd map) can make restoration potions with 1 rainbow quartz and 1 benzoate. You get those items fairly often in each map (like 2-3 potions per map) and you can buy 1 or 2 at some merchants (don't, they cost 400g and money is super scarce)
It is best not to let anyone die and not kicking out Bemir when you find out he is not really a miller nat the end of the RIch Quarter map.
No one has to die if you work together and refuse to sacrifice someone when asked to a few times near the very end before getting to the tavern. After that you are in the clear but at that point you are 95% done with the DLC.
