My personal favorite crazy item is the mizzium apperatus from Guildmasters guide to Ravnica. The fact that its uncommon is bonkers. Side note, it allows you to cokelock at level 3. Despite that though, the flavor and mechanics are amazing.

No DM should ever run Curse of Strahd without first watching all the videos on it from this channel. It will turn a great adventure into something epic that your players will live and talk about for the rest of their lives.

I was going to mention Dahlver Nar in curse of strahd but you already knew.

Running CoS for almost a year now. Our cleric just accepted a pact with Dahlver-Nar! I didn't know that much about him. I'll have to do more research! So cool!

    "What were the WotC thinking?"
    That's a question I ask constantly!

The Gnomengarde Grenade from Divine Contention is bonkers. On D&D Beyond, because of its "varies" rarity, it shows up in the Common magic items of the inventory section. 2x 8d6 damage + 3 rolls on the Wand of Wonders table. Crazy.

    Nice items
My craziest is one I made years ago. It’s a ring that is intelligent and identifies as a ring of 3wishes. Problem is it talks, usually when the person wearing it is trying to be quiet or trying to persuade someone else and the ring will tell that person something the wearer doesn’t want them to know.

My party managed to get the Ruinstone after successfully defending Leilon, with the help of my character’s long lost sister. The next day she left, her ally vanished, and our parents were no longer killed by bandits years ago. Oh boy

I was about to mention the Teeth of Dahlver-Nar if you didn't. You can focus an entire campaign around them if you wish. Another crazy one in Tasha's is the Mighty Servant of Leuk-O. The closest thing D&D has to a mech and, if you happen to be playing the right module or have a DM who just wanted to use the Machine of Lum the Mad, can cause the single most damaging effect WotC has printed in 5e, at least that I'm aware of. Its self destruct ability already does 75d6 damage if you manage to get it to function without the machine turning the pilots against themselves in the process, but if you just get it to strike the Machine of Lum the Mad, it triples the damage and the radius. Yup, 225d6 damage over a 300' radius sphere, with triple that to objects and structures. A great last ditch effort to take out something obscene... like maybe the portal created by the Crook of Rao.
The last one I'd mention also from Tasha's (I wish they'd release more items like these, as they're great campaign inspiration) is Luba's Tarokka of Souls. Allowing you to twist the fate of a creature for an hour, positively or negatively, it also holds the souls of a number of undead. If the card holding the soul of one of the undead is pulled, the soul is released somewhere within 100 miles of you, gains the boon effect from the twist of fate, and both you the card puller and the original target of the Twist of Fate effect get the bane effect, lasting until the released creature is destroyed.

You should check out Lost Laboratory of Kwalish and Infernal Machine Rebuild. Both adventures revolve around the same area, and for the most part the same story. It's very easy to integrate both stories into the same adventure. They really are pretty unique and cool. Highly recommended. But they also revolve around a mech like armor you can obtain, that a party member can obtain that while powerful as all get-out, could get them into as much trouble.

      Thanks!
It's actually the same microphone with a bit more processing. I upload my audio to podcast.adobe.com/enhance, which does some major cleanup. Then I drop the pitch back down to my normal range, since their tool raises it for some reason.
