Please clarify the topic of GI bleeding coding guideline along with scenarios.
If md queried for sepsis and agreed than diagnosis dropped off and not in DC summary do you query again
There is no coding guideline that requires a diagnosis be included in the discharge summary. Unless your facility has an internal guideline requiring that the diagnosis be included in the DCS, you can code the sepsis that the physician verified via query.

Hii mam , I have one doubt patient came to hospital ( ED)for weakness and admit in to hospital for further evaluation, final pathology reports mentioned CLL....? , in Discharge summary mentioned Differential prophylaxis CLL and patient follow up treatment in outpatient visit...... Here CLL can we coded or not ....? ( In lab results WBC very low )
The official guidelines for inpatient coding allow us to code an uncertain malignancy diagnosis documented at discharge. Some facilities have internal guidelines against it, though. Depending on your facilities' internal guidelines, you may want to query to confirm the pathology result.

Would you be able to explain more when to use the E11.8 DM2 with complication? What would be the complication this code is referring to? I understand (I think) through this video that the E11.69 would be a complication of the DM2 that the physician would link. Can you give an example of this?
E11.8 would have very limited use. It would be assigned if the physician stated that the patient had diabetes with complications, but there was no documentation of what the complications are and no documentation of conditions that have a presumed causal relationship to diabetes.

I have found one article that the Coder said assigning code A41.9 unspecified organism does not provide any additional information that is not already included in code T81.44 therefore, no additional code is assigned for a unspecified sepsis. This is noted on page 25 of the first quarter 2017 issue of AHA coding clinic, which states that an unspecified code should not be assigned as an additional code when it does not provide any additional information. In order to maintain the major MCC status, the provider must specify the organism associated with the sepsis so that a code from A40-A41 can be assigned that specifies the organism. What are your thoughts on this guidance? I could not find the coding clinic mentioned to verify.
That is so interesting! I have never thought of that before. I am not sure what the correct answer is. I think in theory, I agree with what you are saying. It might be redundant to add the unspecified sepsis code, but in practice, I think that it is the norm to do this and I have never heard of a denial because of this issue. The coding clinic that you referenced is about coding unspecified asthma with COPD and the answer was the code also type of asthma note under COPD does not apply for unspecified asthma because "Unspecified" isn't a type of asthma. So not exactly the same topic but the concept could be transferable. Although I think with the sepsis coding one could argue that A41.9 Sepsis, unspecified organism does add the additional information that the organism causing the infection is not known. They can't always determine this and the patient would have to be on broad spectrum antibiotics rather than targeting the specific organism. I will have to pick some brains on this one and maybe submit to coding clinic. If I hear anything I will let you know. Thanks for bringing this up!

I passed my CPC late last year, I finished Practicode earlier this year. I'm still looking for a job. I love to watch these videos because I know this career is one where you never stop learning. I feel these videos will help because I don't feel as confident as I should & am worried once I do get a job that I'll do horribly. :( I'm looking to possibly work towards my CCS, but I'm not 100% sure on that pathway yet. May I ask what does qualify as substance induced disorder?
Hi! Those disorders that have specific codes in chapter 5 are considered substance induced disorders. For example there is a chapter 5 code for alcohol induced sleep disorder, so if the physician specifies that the patient has insomnia due to alcohol abuse/dependence than you can assign that code. I hope that helps!

Sure, I can add that to my list. Briefly though, you would use E11.59 when the physician has specifically linked a condition that is classified to the circulatory system to diabetes and that condition does not specifically have its own entry linked to diabetes by the word with. For example, hypertension is not one of the conditions listed under diabetes. So there is no automatic link. But if the physician were to specifically document hypertension due to diabetes, you would assign E11.59, because I10 hypertension is a circulatory system disease. Hope that helps!

Hi! I would recommend coding as currently documented. If there is any doubt whether or not the patient currently has combined diastolic/systolic, query is recommended.

when it comes to removal of an intradural spinal tumor, is this excision of extirpation? For some reason I get hung up on the thought that they aren't removing the spinal cord, but rather solid material in, on or surrounding the cord. Am I overthinking here?
Hi! The location of the tumor being of the spinal cord would not impact root operation selection. The method used to remove the tumor would be the determining factor. If the method to remove the spinal tumor meets the the definition of root operation excision then yes, it would be coded to excision, body part spinal cord. Think of it as you would any debulking of a tumor. The objective is to remove the tumor from the body part that it is attached to and therefore is coded to that body part. I hope that helps!

If the provider documents ‘dm with ckd stage 2 & dm with microalbinminuria’ would we code E11.22, N18.2, E11.29, R80.9 or because the microalbinminuria is integral to nephropathy which codes to E11.21 would it be accurate to only code the dm with ckd? Hi! You would only code the diabetic ckd. As you said, microalbuminuria is inherent to diabetic nephropathy, and the ckd is the specific type of nephropathy, so no additional code is needed.

Heel ulcers may be automatically linked to diabetes since the heel is part of the foot, but ulcers of the ankle are classified to the lower limb, not specifically the foot so ankle ulcers do not have the same automatic link to diabetes.
Ok what about pt admitted with gi bleed and the egd shows gastritis (or duodenitis, gastric ulcer, esophagitis). Is there a casual link with those and the bleed? (Same with colonoscopy and diverticulosis..so on)? I see the CC 1992 saying yes but 2005, 2007, 2018 CC have put a question for me on this.

Hi Carla, Yes, all of those conditions are automatically linked to bleeding because of the alphabetic index linking each condition to bleeding by the word with. Be careful when reviewing older coding clinics because they were written based on the guidelines, conventions, and instructional notes at the time of publication and may no longer be accurate due to the ongoing yearly updates.
