ADHD and Fatigue

  • When asked to identify the positive features of ADHD, one of the most common answers offered by individuals is “high energy”. Yet fatigue is one of the most common complains of those with ADHD. Can these opposing sentiments be reconciled?
Fatigue is distinct from sleepiness/drowsiness/sedation although the two often co-occur. Fatigue is the feeling of decreased energy so that one can’t perform to the best of one’s ability. Researchers have devised objective ways to measure both peripheral (muscle) fatigue, and central (brain fatigue). Serotonin, tryptophan, and dopamine all play roles in the perception of fatigue.
With many tasks, those with ADHD may be activating a greater number of brain regions than those without ADHD. This may cause depletion of energy resources, contributing to fatigue. Stimulants, and many other ADHD medications can alleviate fatigue. Ongoing studies suggest branched chain amino acids may be another way to alleviate fatigue in those with ADHD.

@@dr.johnkruse6708 I also really appreciate your videos!
You seem to know much more than many other professionals who share their ADHD knowledge, even if they, too, seem to know a lot.
Or maybe you simply have a knack for absorbing new information, whether from other people's research or your own clinical experience, connect it to what you already know, and then share your conclusions in ways that result in many more Aha moments for your audience.
Either way, thank you so much for doing this, and so consistently too. It really does make a difference.

Hey, is it possible some people with ADHD only have a deficit in norepinephrine and have normal dopamine levels. So this is why strattera works in some. It calms down the restless brain and conserving energy. Raises norepinephrine in all areas and maybe provides a bit of that natural wakefulness a drug like modafinil provides from the stimulation of the Alpha 1 A receptor?

    • @dr.johnkruse6708

I have another video on this channel about how we simplify neurochemistry by dividing specific tasks to norepinephrine (NE), dopamine (DA), serotonin or others. ua-cam.com/video/ehrFMfXrFNo/v-deo.html
But we know that NE and DA influence each other, so even a drug that is specific for one type of receptor ends up affecting both systems in the brain. It is certainly that some individuals with ADHD have more deviations from "normal" in their NE systems than in their DA systems.

@@dr.johnkruse6708 oh yes I only saw that you had that video after making this comment. I’ll be here to watch it, if you ever decide to. I also found it interesting you mentioned how people with ADHD are jailed more/ more likely to commit crime. I have gone through an experience that really opened my eyes. So I started taking Guanfacine a month ago. Basically my first adhd medication. All of my impulses and porn adiction are practically gone. I don’t seek sex anymore, before my urges were so strong they overpowered my morals ( I wanted to only sleep with a wife/long term girlfriend but couldn’t stick to it). Anyways, I recently got put on vyvanse and it brought my impulsive tendencies back, there was girl at my work who had a boyfriend and I had the extreame urge to try try my luck by asking for her number. I haven’t even thought about this prior to taking the vyvanse. Also I tried a double dose that day (I’m on 20mg so this day I did 40mg) what I wanted to do to with that girl is somthing against my morals because I belive that “you shouldn’t sleep with another persons girl, because you Woulng like that happening to you”. Now this just shows how the same exact mind can have such different impulses and abilities to stick to morals, just by changing the ammount of dopamine in a mind. I wonder how much of better place the world would be if people were given somthing like Guanfacine to dampen them. There probably would be less cheating, rape, better morals, less gossiping and bullying. Less impulses to do bad that give a short term thrill. I’ve learned so much by just experimenting with drugs. Now I think I will only look for a wife that has signs of low dopamine because they might be more loyal.

i have problems with too much sleep when i take medication. i'm wondering if i have adrenal issues? low dose ritalin allows me to sleep like a baby. when i'm on a normal dose i find i can sleep and sometimes i can't sleep. i seem to cycle in and out of peripheral fatigue!

It's possible, but adrenal problems aren't one of the more common reasons for fatigue. I usually start by trying to ascertain whether or not you are getting a full 7-8 hours, at the same time, each night.

Most of the people with whom I have worked have not had problems with insomnia if they took it early enough in the morning, and at a suitable dose. If the dose of Vyvanse is too high, it ends up lasting substantially longer, rather than peaking at higher blood levels - see my video about Vyvanse.
