ADHD is an attention deficit, it isn't made-up, it's not laziness, craziness, or stupidity, and it's not autism, bipolar disorder, depression, PTSD, anxiety, substance abuse, or dementia, even though all of these can resemble, and all of these can occur with ADHD.
Sometimes we need to rule out these other conditions, but very often, it is a "yes, and..." situation where it can be important to identify two or more simultaneous conditions.
Separate videos on my channel go into more detail regarding all of these conditions, but here I present a quick overview highlighting why each of these conditions may be confused with ADHD, and how to differentiate each from ADHD.

    I really appreciate this. Thank you. My practitioner just prescribed this for SAD + ADHD. I am already taking LDN for chronic pain. I have a call in to the pharmacy to make sure I don’t have any contraindications.

    Does the benefits of cymbalta outweigh the risks of cymbalta? There are many stories reguarding people who have taken it and are now have seemingly permanent brain damage. Like severe akathisia. Have you seen anyone have this effect from cymbalta?

    Wow I think you forgot autism is on a spectrum, and every autistic person can have quite different mannerisms, especially if you through 1 or more comorbidities to the mix.

      Nope. I acknowledge that it's on a spectrum. By definition autism still requires symptoms in all three domains I mentioned.

      Thanks for the suggestion. I'd have to think first about what "best" means to me. Many of my talks on specific medicines have done well, but I think that has to do with what people happen to search for, and UA-cam algorithms, and I'm not sure it's ideal to always go along with that beast.

    Knowing that it does come with a few advantages would have helped me when I was first diagnosed. The ones I have found most common are creativity (out-of-the-box thinking), foresight ("seeing around corners"), humor (a sense of the absurd), and the gift (or curse) of hyperfocus. What is your take on the strengths? Do you think it helps to know them?

      I like the neurodivergent framing, because that emphasizes that one is different but not broken, and practically raises the question then of what are you particularly good at and where might your weaknesses be. It makes it hard to lean into your strengths if you're not aware what they are, so yes, I think it is useful to know about their existence and potential. And thank you for bringing up several that I didn't mention in my summary. But I do find it both misleading and somewhat grating when some influencers frame ADHD as only being a "superpower". So often it sounds self justifying and unaware.

      Yes, ideally medication can trigger some positive feedback loops regarding self care, particularly with sleep, exercise, and diet.

    What do you believe is the better treatment option for adult adhd for the long run…stimulants or non-stimulants?

      Both the research and my clinical experience indicate that for many people amphetamine based products reduce ADHD symptoms more than any of the alternatives - but have the potential risk of psychosis, addiction, and cardiac problems that the alternatives don't seem to possess. If an individual has an increased risk for any of those, then a non-stimulant may be a better option. But each person will have their own individual concerns, goals, and reactions to meds to deal with, so as you can see, I don't like to make sweeping generalizations.

    Do you think undiagnosed ADHD, possibly undiagnosed autism and childhood emotional neglect could create maladaptive covert narcissism. Do you think the opposite could be depression and suicidal it y? Wondering if an undiagnosed autistic person could have bad rejection sensitivity due to their undiagnosed ADHD flaws. To protect their sense of self they could become narcissistic? Just a theory

      I have family members with ADHD. I don't think I've met the criteria in the past, but some aspects of my aging brain now seem to align with some elements of ADHD.

      @@Al.531 Not consciously. Some of this is mentioned in my book, but I got started when I was referred a 40 year old man with severe ADHD in 1994 as I finished residency in psychiatry. He had made no progress in 7 years of psychotherapy with a competent Freudian analyst who referred him to the university hospital for an evaluation to see if he had ADHD. They did three days of neuropsych testing and said that if he were an 8 year old boy he definitely had ADHD, but since it didn't exist in adults, they didn't know what he had, and then sent him to me to see if I could figure out what was going on and work with him. In those days there were only glimmerings of recognition that ADHD could exist in adults. I didn't even realize that my mother was somewhere on the ADHD spectrum until she read my book. She masked it well, and also had OCD symptoms that were more noticeable and odd. Knitting always channelled her hyperactivity, but her vision started deteriorating in the last few years. She said that when she waits in doctor's offices now, she will do knee bends and other calisthenics because she can't sit still -and no longer has her knitting to rely on.

    When I first contacted a psychologist for an assessment I was looking at a possible diagnosis for ASD. I was surprised when they said ADHD. However getting an ADHD diagnosis from a psychiatrist seems to be difficult due to the medications being addictive and commonly abused. The one I spoke with at Kaiser flat out said they won't diagnose ADHD. Instead it led to a diagnosis for some of the symptoms, depression and anxiety... both seemingly related to living with ADHD.

    Hi, love this in depth examination of differential diagnoses. I am a psychiatrist that also specialises in adhd and autism, it's refreshing to hear such a nuanced view on this topic. A small point on which we might differ. I believe (and have often clinically seen) that people with autism can also have interests that have deep meaning or are linked to understanding what's behind their narrow interests. I'd actually go as far as to say that passionate interests in some people with adhd have, in my experience, been more associated with superficiality than in people with autism. Perhaps due to it being more flighty/linked to these interests being temporary and induced under specific circumstances. I'd like to hear your thoughts on this?       I think I would differentiate superficiality from brief and intense, and from atypical, non-linear connections. I see intense, sometimes transient interests in those with ADHD, and they often have awareness that they have gone down rabbit holes chasing bits of information, but I wouldn't characterize that as superficial.

    In the Bible, The person with ADHD would be broadly defined as a fool. Although this is not a category one would want to be labeled with, there was more hope for fools than those wise in their own conceit, which can include those with ADHD, and those without it.

    Why is psychiatry so focused on controlling or treating symptoms of disorders and not cures? Is it because enough is not known?

      That's an important part of it. Another piece is that much of mental health seems to stem from how brains are interconnected, and our tools for rewiring are rudimentary (which may be a good thing, given the scary implications of rewiring brains).

      @ that’s a very good point, somtimes it seemed some people can’t get back into homestasis. What I was alluding to was why dosnt pschatry focus on upregulating the receptors they aim to stimulate or downregulate the receptors that they aim to antagonize. Because trying to stimulate a receptors is going to have a contrary effect (tolerance). So why not just hit it with the opposite.

    Did not help and I just stayed on it out of desperation . Same with Straterra despite insane constipation. Methylphenidate and adderall did nothing .

    "Mood swing" &c. are terms of art, on the one hand, and appropriate use thereof enhances the quality of the discourse. I think folk conception of terms also facilitates communication. "Technically it's emotional lability or volatility" is fine there. However, it's definitely true I wish I'd been told "when you're very upset, try to relax, and see how you feel in an hour" decades ago. I worried about "mood swings" and knew about bipolar, and definitely could've benefited from knowing that wasn't even in the realm of what I was dealing with.

    Thank you for this video! as someone suffering a combination of dysthemia combined with reoccuring major depression alongside with ADHD and aspergers its kinda hard to get any sort of understanding for why im just not functional and why im just not doing things others feel are trivial, even if im healthy in body. im still youngish but when I had my frist double deprssion at 18 my life seemed to get derailed more and more, leading to me being unemployed for a few months now after two unsuccesful jobs and a luckily completed apprenticeship in a special institution. Im now actively seeking treatment for both my depressions and adhd so that I may get in a state where day to day life isnt excessively draining.

    I just started straterra i have a history of heart problems in my family and have high blood pressure i asked my doctor specifically for a medication that wont affect my blood pressure and was prescribed Wellbutrin but broke out in hives and had bad headaches after 35 days i got prescribed straterra now what should i expect????

      Strattera does have some potential for mild blood pressure elevation. But each individual has their own reactions to medications, so I don't know how your body will respond.

    Cliff's Notes (for those of us with ADHD who jump to comments too soon): Our ADHD "out of sight out of mind" experience is *not* an object permanence deficit. Calling it an "ADHD object permanence” issue is problematic because it confuses the working memory & executive function issues of ADHD (struggling to keep things organized and accessible in our mind) with an actual object permanence deficit (an inability to *grasp the concept* that something actually exists even if you can't see it). Actual object permanence deficits in adults is *extremely* rare, . Our ADHD does not prevent us from understanding that a ball still exists even if the person holding it puts it behind their back where we can't see it. If you understand that concept, then you don't have an object permanence problem. If you think the ball stops existing when somebody puts it behind their back, then maybe you’re one of those extremely rare adults with an object permanence deficit. IMHO, instead of calling the ADHD “out of sight out of mind” symptom an “object permanence” issue, we probably need more accurate terminology. Maybe it would be better to call this symptom a “*Mental Inventory Deficit*”, meaning objects or tasks quickly fall off our mental radar; or perhaps it’s a “*Functional Object Tracking Deficit*”, meaning items and tasks that aren't visible or immediately relevant may be easily forgotten. Regardless of what we call that symptom, it’s fundamentally distinct from the concept of object permanence, even if it has some striking resemblance to a lack of object permanence. If that doesn't matter to you, then don't get upset with the next non-ADHD person who is dismissive of your diagnosis because they have experieced symptoms that resemble ADHD in isolation but don't really constitute ADHD. Meaning matters.

      My understanding is that they can both elevate heart rate, and if rhodiola does have potent enough MAOI activity, it could prolong the effects of dopamine and norepinephrine that are boosted by wellbutrin.

    I have ADHD and I've tested to be in therapeutic levels of ketosis (2.0mmol/dl) as suggested by Dr. Chris Palmer. If I can stay in these levels for 2-3 days I have the same effect as taking ADHD medication. My n=1 says that this is clearly working. But the problem is staying in 2.0mmol levels is extremely hard to sustain if you want a normal social life. Even 1gr of sugar kicks you out of ketosis at these levels. I'm still trying to find a more effective way to stay at these levels of ketosis (exogenous ketones works temporarily for 1-2 hours then blood levels go down again). I'm using a continuous ketone monitor, so I know how hard it is to sustain this without a real life style change and accept that you'll be an outlier in social settings.

    I have ADHD and are prescribed Elvance (UK). I do not find any pleasurable effect from Elvanse or any Street "speed" and I can't see how or why any person with ADHD would "abuse". For me a similar substance is codeine. Literally zero psychoactive effect even when dose is increased. Only effective pain relief and mild constipation. No idea why anyone would "abuse" it. It baffles me.

      Hey! This is the video editor, thank you for your feedback! A lot of these older videos had quite a few audio issues to attempt to fix in order to make them easier to listen to. But if you can try to describe what you felt was worse quality for this video compared to others, I'll gladly take your response in consideration for future videos! Are there areas in the video you could point to that you think were harder to listen to than others? Thanks!

      @@FlynnaUA-cam maybe it could be that the mic is not as accurately picking up Dr. Kruses voice when he is talking slightly more quetly. But if I‘m honest, I never rly had trouble understanding Doc, whether newer or older vids; so im just guessing what Preacher could‘ve meant since the only thing i‘m picking up on would be said slight variations in sound intensity but not clarity.

      @@garydaslug3025 Yeah, you may be right. Although there are some editing tricks I can do to make audio louder and slightly clearer, I'm unfortunately limited by the way the original sound was recorded. As these older videos get released in updated versions, there are still many of them that will be harder to listen to than others, simply because the original version of that particular video had worse audio. But if anyone has any editing feedback, or know of any tips or tricks that may help make these videos better, I'm definitely all ears.

    Hello Dr. John, I couldn't find relevant research papers on this matter, so I would greatly appreciate your help with a question about the pharmacodynamics of it. I am a definite ADHD patient. Except for being guided by my teachers, family, or doctors for diagnosis and briefly using Concerta for 2-3 months in 2015, I haven’t received any psychological support so far. Now, at the age of 22, due to a decline in my quality of life caused by issues like thinking so fast that it can no longer be contained and so pessimistic which leads anxiety and depression, lack of motivation, inability to complete tasks, cravings, and the impact of depression and paranoia, I started methylphenidate. Against recommendations(beginning with titrating), I directly began with 36 mg, and I am now on my 12th day. Thankfully, I havent experienced any severe side effects, except for a decrease in sleep quality. Astonishingly, this medication resolved all my issues, which I had struggled to manage with countless other methods ends up with failiure, in just one day. My question is this: Although 36 mg feels like the perfect dose for me, should I have started with 18 mg and titrated up to 36 mg, even if 36 mg is the optimal dose for me? If I had followed the standard titration protocol, would it lead to tolerance, withdrawal symptoms, or make me worse than I was before starting the medication once I discontinue it? Do you think I should reduce the dose to 27 mg for a certain period now, while I’m still on day 12? My main concern is that skipping titration might lead to higher tolerance for 36 mg or more over time. My main concern is developing tolerance to this medication with long-term use and, when the time comes to discontinue it, ending up with a worsened dopamine system and ADHD symptoms instead of improvement. I’m thinking that if 27 mg works well for me now, I can keep it as my current dose so that, if needed in the future, I can increase it to the maximum of 36 mg with tolerance built up to 27mg. What would be your suggestions on this, sir? Am I thinking correctly in terms of tolerance? Thanks for your time.
