How Orthodontic Extractions can change Facial Profiles

Today I'm talking about my patient Christina. Christina presented to me with some really severe crowding. When we have a look at her facial photos, you can start to see some of her concerns. I mean, the crowding is quite severe and it's even apparent that she has a crossbite between some of her front teeth. But what might not be so obvious and a little more subtle is that her lower jaw is actually very large and very prominent. You can see the lower jaw sticks out a lot further than the rest of her face. When we start to have a look inside her mouth, we can see just how severe that crowding is. She has a cross bite here, a cross bite there as well and one there too. So really there just wasn't enough room to fit all of her teeth in the proper alignment. So with Christina, what we had to do was remove some teeth. Now that wasn't to create space. But more so actually to deal with her facial profile. We did orthodontic extractions of premolars and we had Damon braces for 18 months together with bite turbos and elastics. And this is the end result, which we were able to achieve. A much wider, fuller smile in perfect alignment. Looking inside her mouth. We can see just how severe that crowding was. And here, she's already had those teeth extracted. And here they are all nicely aligned. So this is the before and after result following orthodontic extractions. So, Christine was in the unique position of having a very forward a very prominent lower jaw and following the extractions we can see that her lower jaw has been brought back more in line with the rest of her face. And in this picture, we can see some of those subtle differences that has made. We can see her lower jaw is less prominent and not quite as full as it was before. And its also made her jaw a little bit slimmer. So she is very happy with that result.

I had a similar problem for years, my Lower teeth were perfectly aligned, but my lower jaw was too big compared with my upper jaw, it started to move my upper teeth forward, my TMJ muscles gave me terrible pain because the bad alignment in my bite. the procedure enhanced my features and relieved the pain.
But definitely some people that did it looked worst compared with their before, this girl looks amazing.
Or have you stopped to consider that it's your upper jaw that wasn't expanded proportionally? You just stunted your potential for a broader, better nasal airway and forward midface growth. Far healthier and aesthetic. Your dentists screwed you over.
To much jaw protrusion can look bad too, the distance between my upper lip and nose was alright.
For someone with very small lips, having the upper jawline expanded might actually look better.
@An13 I'm going to tell you as it is. Your lower jaw IS NOT the problem. Your upper jaw IS proportionally underdeveloped compared to your lower jaw. Smaller jaws is a dysfunction. Ask your self. We're you breast fed for at least 2 years, did you have breathing related allergies growing up aka were you a mouth breather, and did you chew enough times and tough foods? That should reveal to you why your upper jaw is underdeveloped. Use it or loose it. And your orthodontist just mutilated your lower jaw. You should be angry about that. Go read up/ look into jaw hacks and Dr. Mike Mew. Better help you navigate your situation.
She looked better in the before treatment. There was absolutely nothing wrong with her lower jaw in the before photo, and the prominence of her lower jaw was harmonious to the rest of her face. She had fuller, forward facing lips and overall facial features in the before photo than in the after photo.
Orthodontists extracting teeth like it's nothing will be an absolutely ridiculous practice in several years.
I agree with you and you are saying exactly what I was thinking. I thought that the before and after pictures were switched because she looks better before and worse after. In the before picture her Face is more symmetrical, her complexion is even toned, and she looks more youthful. I’m the after picture she has on the thick glasses, and she looks much older and tired.

I agree. I'm so f--king bummed that my teeth were extracted, but the knowledge we had at the time of my extraction was the knowledge we had at the time of my extraction. Wish orthotropics was a thing we all knew about 10 years ago. I look similar to my mom except that my mom's face is fuller and she has aligned teeth even thought they're SpongeBob teeth like mine, probably because she grew up in Laos, whereas I grew up eating American food. My face is narrow in comparison to my mom's and I have less chin in my side profile lol Oh well, I'll just wear the face that I have now proudly for the remainder of my life. It's an argument among orthodontists that sometimes patients inherit large teeth from one parent and a small mouth from the other parent but both of my parents grew up in Laos and have perfectly straight teeth, and all of my siblings (5 of us) have overcrowded or cooked teeth there must be more to this than genetics at play.

@@ruthezenweani4152 it really depends on your profile right now please don’t worry so much changes can be subtle and some severe but that’s normally because they’ve already had flat faces and generally no lips, you facial structure will change regardless of any procedure if you like it or not really depends on your personal aesthetic preference but truly don’t worry just hope for the best and remember not everyone is the same

I went to 2 orthodontists and they both said I needed 2 teeth extracted my Mom was like that's not happening and we went to a 3rd one and he said I didn't need extractions at all. So I got braces there. They don't all make you get extractions for money they do it because that's how they were taught in school.

Update: so I still have brackets on, but I'm almost done! I was 18 when I got them removed, now I'm going to turn 21 and so my face has changed, but that being said I like my face even more now. I still have to get my wisdom teeth removed, been delaying that but will do in a few days. Wish me luck! Will keep updating. I am hoping to be done with my treatment by the end of this year (2023)
I did research before but when I told my dentists about it they said that nothing bad was going to happen but I just can't stop thinking about it
I had my teeth extracted almost 4 years ago. Got braces and everything, and honestly I regret wholeheartedly. But I know people who’ve gone through the same procedure and loved their results. And either way, you’ve already gotten them extracted, so there’s nothing that you can really do now but hope for the best❤️sending love

She's lucky she had such a perfect jawline, so after treatment, she ended up looking kinda "normal". Can't imagine what this would do to her face if her jaws were not so forward...

Extractions have to be done in certain cases. In this case, she had a more large lower jaw which isn't normal.

What a load of crap! Her lower jaw was fantastic, but you screwed it up. Her mid face area is the problem area and needed to be expanded proportionally. It is criminal to extract and retract jaws. That is the degradation of our health.

The only thing that I don't like about it, these procedures tend to enhance nasio-labial folds, in my case I didn't had a lot of tissue around my cheeks, but it can make look slightly older.
Of course they do b/c they are retracting and flattening your face, which shouldn't happen.

@@ruthezenweani4152 Her face wasn't too bad after b/c her mandible was already so far forward (which imo is ideal b/c she has a super open airway). Most people will have flattened, lengthened, narrowed faces and eyes (no prominent cheek bones) and a recessed mandible (no definite jaw line) which will cause one to look older than one should as they age.
@@ruthezenweani4152 People of color usually have better facial growth, so these procedures can look better, caucasians usually have flat and small mouth areas so it looks sunken a lot of the times.
@@RS54321 my face look broader from one side...bcz other side doesn't have wisdom teeth. Left side looking much much broader..n wisdom teets are causing difficulty in eating. Will removing these will make my face look thinner
she was better before she had full lips without any fillers, her jaw was nice without fillers, better if she chose veneers or crowns

@@ruthie8973 Extractions lead to recessed mandibles which leads to a narrowed airway...it's a domino effect that will cause health problems as one gets older (sleep apnea, undereye circles, nonrestorative sleep, heart problems, issues with memory and concentration, gastrointestinal issues....the list goes on and on). Please look into mewing and the discussions around how braces and extractions change faces.

I majorly regret having it done (4 wisdom and 4 premolars), but I was 12, so I had no idea what would happen. Decades later and I'm experiencing the side effects of this barbaric practice (undereye circles that never go away no matter how much sleep I've had, tiredness, recessed mandible, a double chin even though I'm thin, poor side profile, premature ageing in face, not enough teeth to chew with so eating takes way longer...the list goes on and on).

You don't have to have your premolars removed...look into palate expanders and save your teeth and face!

@@haileymock1879 If they are doing a palate expander, why do they want to pull your premolars? I would definitely get a second opinion!! The expander should widen your palate enough that all your teeth will fit properly.
If you do get the premolars extracted, the teeth anterior to them (your incisors) will be pulled back to fill in the space that the incisors took up. This will give you a flatter profile, thinner lips and possibly recessed jaws. Not a pretty picture.

Her lower jaw was perfect before...she just needed her arches widened a bit to allow room for her teeth to straighten. Thankfully her protruded jaw still looks alright even though it's been retracted with the braces. However, now there is less room for her tongue, and you extracted 4 perfectly healthy teeth. This barbaric practice needs to end.
True. You are absolutely right. I got 4 teeth extracted when it wasn't really needed. Now, I have a sunken gaunt face and the orthodontist just wouldn't listen. It's a mess. I paid so much and they made it worse. I miss my old face.

Your jawline will almost always look better after getting braces (especially if you have an overbite). Don't just blindly believe in what people here are saying. She clearly had a facial imbalance, and she looks way better in her second foto.
The face is better before. It is somehow younger, vibrant and fuller, I think in the after she has lost some midface, it has been set back. The bit just under the eyes. I would be interested to see her without the glasses. And how this is for her in about 15 years time. Better to have slight crowding and a nice face then the dental arch made smaller.

@@PushpaRani-ru3lq The teeth can be made straight with good tongue posture. The tongue should always be fully on the roof of the mouth, while the person breathes through the nose, all day. The tongue will widen the upper dental arch and the lower dental arch will become wider as well. So, the teeth will be straightened slowly. This is especially important for children, as they are young enough to change their facial shape more effectively than adults.
