Would this work for medium duty truck radiators?
I've seen this at the store for 130 and the Mac 1 for 200
I'm debating wich one to get

Yes it would work on a medium duty truck and I really like the Maddox tester but if you can get a mac for a little more I would probably go with Mac. Let me know what you decide to get and how it works for you.

Is that the correct way to screw the rubber adapter part on to the universal adapter?

No he screwed it on upside down. The brass washer on the plug should face away from the adapter.
