What should have happened is his staff informing the cabinet and national security staff that he in unable to take on the responsibility of the Presidency. It shouldn’t have to come to the point that they only discovered the issue at the last moment.
There would have been a doctor diagnosing the President as unfit for office. There are procedures to ensure that. It's not about comeptency, its more about are you psychically stable and healthy and fit for office (which this person clearly was not).
Strictly following the law would have necessitated them to swear him in, and immediately start the proceedings for declaring a President as Incompetent under the 25th Amendment, and then swearing in SecState. However, they were already "under the gun" timewise, so they simply skipped him, and went directly to SecState. In the situation, it's practical, but afterwards, it would likely be studied and examined, with reports and maybe even Congressional Hearings, so this never happens again.
Unfortunately, the president pro temp position is often granted to the oldest member of the Senate's majority party. It is strictly honorary until the succession act is put into play. I don't know what happened to Air Force One or why they leadership was on one plane. I'll never know because I'll never watch this weekly Hillary Clinton commercial.
