    Excellent advise, thank you doctor! Very good point you make regarding distinguishing throbbing (pounding) pain which is due to swelling which you can quickly jump on and control with ice and elevation!! I had surgery 6 weeks ago that involved cutting and realigning a bone in my mid-foot and two screws placed to secure it in place to allow fusion. I was extremely surprised (shocked really) I didn’t need to take any pain meds following surgery (not even a single Tylenol) with icing near constantly behind the knee in addition to keeping the swelling down by keeping the foot elevated for the first 2 weeks, to gradually icing less often plus as needed and elevating my feet every night and whenever Im sitting at home or work. My doctor said keeping the swelling down was crucial in the healing and successful fusion of the bones, so it was important to pay attention to my body. It’s been 6 weeks and the swelling continues to improve. When you are allowed to begin massaging your foot, I highly recommend doing so to break up scar tissue, decrease soreness, and improve circulation!

    I had theeee most amazing experience ever, no pain, just one drip and and I was awake but slept a bit during the surgery because I was hungry because they told me not to eat before --sigh, and I am lazy, also cause of the effect of the anesthesia...... I am not having any pains, however I tried to walk the following Friday, a day after my surgery And it felt tender, so I try to stay off....... I took my meds and sleep as much(keep in mind, sleeping is my hobby)

      You had a significant injury followed by surgery with a bunch of fixation so swelling will be a factor for 4-8 weeks. Activity and warm temps ( depending on what part of the country you are in) will increase swelling. Elevation and ice will be required and is your best defense. Take a tape measurer and see what the circumference is for your foot and ankle in the am vs pm . You may see a 2-3 cm change. Hang in there. Some people can swell for more than 6 mo after surgery.

      Cervical pathology would not typically produce symptoms in the lower extremity. If you notice the foot pain improving after you SX it may be from decreased activity. It sounds like your foot complaints are low on the problem list. Best wishes for your recovery.

    Hi dr.
    I broke my tib and fib snd had surgery November 1st. I’m healing well and swelling is down as I keep the foot elevated and iced. Only problem is if Im on my crutchs standing and I feel pressure on my surgery ankle like the blood is rushing and causes pain. Is this normal or just takes time to go away.
    Thanks

    • @GoToFootDoc
      @GoToFootDoc  5 років тому +4

      That is an accurate description, When you stand your body is basically a column of water and water will seek the lowest level. Your blood vessels are use to being wide open during your healing and when you fist stand up it is like a tidal wave of fluid heading down to your toes. Compression hose (those funny white socks you were given after surgery) would be helpful. If the discomfort/pressure increases when you first stand, try moving you foot and ankle in circles for 2 minutes prior to standing. This starts the leg muscles pumping fluid back up to the heart and reduces the fluid overload. Good luck with your recovery. Swelling is a top contributor that will slow down recovery.

    I had an operation to repair tendons around one of my sesamoids after a car accident which dislocated my toe and gave me a 1st metatarsal fracture.
    I had an operation on my 1st MTP joint for the resultant Grade 3 Turf Toe around 3 weeks ago now. The wound seems to have healed well and I am on clexane for another week. Was in a cast for 2 weeks post-surgery.
    When should I start testing the toe and trying to increase my range of motion etc.? I have been moving my foot in all directions, but stopping if I feel discomfort/any pain.
    I still have a degree of swelling and discomfort around the ball of my foot. Should I be icing as well as elevating?
    What should I be doing at this stage to best help recovery to hopefully a normal gait etc.? I am in a boot when I have to go anywhere for another 3 weeks.

    • @GoToFootDoc
      @GoToFootDoc  2 роки тому

      Since I am not familiar with the extent of your injury , I cannot advise on your recovery or rehab. It is always beneficial to control swelling following surgery to reduce pain and increase range of motion. You are still early in the recovery for a met fracture/sesamoid injury so elevation and ice is important. Our best wishes for a full recovery.

    May 1, 2022 fractured calcaneus. May 16th surgery, plate and 8 screws. Nerve block immediately in recovery. I've been told I most likely have neuropathy, as pain and occasional weakness occur in the plate area. Plate is on the right side of the right foot.

    • @GoToFootDoc
      @GoToFootDoc  6 місяців тому

      There is a difference between neuropathy and nerve damage from an injury or surgery. The symptoms you are looking for numbness or altered sensation that can extend into the foot. Nerves can get entrapped/damaged that will have symptoms right over the path of the nerve. If the hardware is prominent, that can also produce these problems. Your doctor should be able to sort these out. If not, a physical medicine doc or neurologist may be helpful.

      I am glad you saw value in the video. The Plantar Fascia is a thick structure that attaches to multiple muscles. A release of this band is a significant event for a weight bearing part of the foot. There can be tenderness for 1-2 mo when walking. I had a cheilectomy 6 weeks ago. I have had and still are swelling and pain -the toe operated on is a little stiff but doing PT now, but the ball/pad under the toes and toes # 3 and 4 are being zinged with nerve zaps! It’s pins and needles in that area. When I walk it feels like the “gel” that’s in those hot/cold packs- under my foot! Dr said the nerve pain and pins and needles were “residual” from the nerve block used. I’m getting depressed because I never expected this! And wonder if and when these nerves will heal… and what exactly happened with the peripheral nerve block to have caused this?
    Thank you !

    • @GoToFootDoc
      @GoToFootDoc  9 місяців тому +1

      There is a god chance that a a Posterior Tibial Nerve block was used. This is done at the medial ankle where both plantar nerve branches split from the PT nerve. I use this frequently. This is an common and easy way to block/numb the entire bottom of the foot. If the injection came too close to the nerve there could be some nerve irritation. If you have sensation in the bottom of the foot and toes, the nerve is working, even if it is altered the PT nerve is still working. Try tapping on the inside of the ankle where the injection was given. If this produces tingling in the bottom of the foot - that is the problem. It should improve ...but not fast.
      Plan B: (not the PT nerve) walking differently before and after surgery can overload the lesser toe joints. This would be painful in the bottom of the foot where the 3rd and 4th toes join the foot. Push on this area and see if you grab the chandelier.
      Let me know how my UA-cam medical spider senses did.

At first when trying to walk it feels very strange, but recently i have noticed my foot shifts more to the left going outward. I do have flat feet, but the orthotics do not help. The arch just pushes down flat, they have been adjusted twice. Do i need to have further surgery. I asked my surgeon, but he said no. Should i get a second opinion

    • @GoToFootDoc
      @GoToFootDoc  Рік тому

      It sounds like you had a joint fusion since you were off loaded for 2 mo. An aggressive flat foot will shift outward and probably did tis before the surgery- you may be noticing it now since were were not walking normal for 2 mo.
      If your orthotics were pre surgery, a new pair may be need since you foot is very different now. Orthotics are used to keep a flat foot from going on to surgery.
      Why don't you talk to your surgeon about this before a second opinion.

      Any dramatic changes in swelling or discoloring should be evaluated. An X ray can reveal changes in the fixation (plate and screws). If possible, I would have the original surgeon look at this since they are familiar with his history. I follow any case that has hardware for at least 1 year for this type of change. Best wishes.

    I have a non displaced fracture on my second metatarsal. It did not require surgery. My doctor told me I need to come back at week 6 for an x-ray and a checkup to make sure the foot is healing ok. I don't have insurance and just that office visit with an x ray would cost $400. As long as it feels like it's getting better and I didn't reinjure it, do you think that 4 week checkup is really necessary? or does it just sound like an unnecessary fee for the doctor to make more money?

    • @GoToFootDoc
      @GoToFootDoc  3 роки тому

      Teddy, You are really rolling the dice on this. With the very best of scenarios the bone has become stronger but the question is - is it strong enough to put weight on it? Depending on the displacement of the fx, Vit D levels, your health and age it may not be ready. The x rays at the re eval will give you the answers. Guess wrong and you could spend another 6 weeks in recovery. Second visits are far less expensive. As DIrty Harry would say... Are you feeling lucky.? (Its a Boomer thing)
      I would STRONGLY recommend a re eval. Good luck.

      @@GoToFootDoc Thanks for the response. I don't have much of a choice. I guess I'll just continue to not put any weight on the foot.
      By the way, the reason I'm questioning my Dr's advice is because he's already shown that he's willing to inconvenience me for the sake of making more money. At my initial visit, he wanted to put my foot in a cast instead of a walking boot. When he found out I didn't have insurance, he told me I could just wear the walking boot I already got from the ER because it would be cheaper and wouldn't make a difference in my case anyway.
      So, he was willing to prescribe treatment that's more inconvenient for me so he could bill an insurance company more money. That has to violate some kind of Doctor's oath, right? Understandably, I have to question everything he says now. I'd love to go to a different doctor but he's the only one in my area that accepts self pay patients. The other ~15 or so orthopedic doctors in town only accept insurance, no self pay. The United States is a third world country when it comes to healthcare (except for rich people). :(

      Yeah I had a similar surgery, loved and still love my Stimulator, only wish Medicare hadn't drug their corporate" feet" and made me wait through 90 days of other interventions before saying "Well Okay" , give credit to my Podiatrist over Dr "Osteo" who told me to just suck it up and wait for Mother Nature and Father Time to interact 😀-
      Company told ME they would accept the unit back; wrong! I have another surgery date on other foot and I PLAN to use it there!

If they have asked for partial weight bearing than please do, do not stop, try as much as you can but if anytime it hurts stop, normal pain will be there as there will be stiffness around the ankle . 3. Keep doing bed exercises sitting exercises and try to move you ankle up down sideways rounds as much as you can it will be difficult but do not stop just keep trying it will take time but believe me it will give you results not soon but definitely in some days. 3. Keep leg elevated when there is swelling, swelling will be normal but keep it elevated when not in use icing will also help. How is your range of motion at the moment ? How is the pain ?

    I feel comfortable walking on my foot after severed tendon repair. But it’s only been 4-5 weeks since surgery and my doctor still hasn’t cleared me to walk on it. You are past the inflammatory stage of healing so the site should feel better. That does not mean the tendon is strong enough for weight bearing. You did not mention which tendon that was repaired or the extent of the injury so I cannot comment . If you want to increase your chance to have surgery again- stress the tendon before it is strong enough and it will fail or delay healing..

It does feel like my tendon is attached still and healing good and every time I get X-rays they don’t mention any new damage. I had surgery 6 weeks ago & just got a boot. My toe seemed straight enough when I took the (6 week) splint off. I walked with the boot - maybe too much at first (I live in NYC - it’s not hard to do) and today it looks really crooked again - my Dr said to wait until the swelling goes down to asses the angle. Is this common? I’m not the most diligent “ice-er”, so I’m sure that contributes to the swelling. I liked your calm, logical instructions. Too bad you’re not in NY!

    I had surgery on my ankle on August 28th b/c I broke it. :( It is the end of November now and I still cannot walk properly in a shoe. My Dr says it's ok for the weight bearing and wearing a normal shoe now since it has healed, but that is only the outside, correct? I'm thinking my bones are still attempting to heal b/c I cannot walk on this foot w/o having it swollen by the end of the day. I wear the boot still to work and now I know what my grandma and mother meant when they said they can feel the rain and snow coming! Arthritis here I come! lol Just b/c I was having fun at our state fair Aug 10th, I drank too much, and fell, broke my ankle. :( This fracture will be a permanent problem correct? I am 33 years old. Healing isn't so successful for my age anymore I'm sure. Well thanks for reading!

    • @GoToFootDoc

      Swelling can linger for a year after an ankle injury. The bone strength should bready for full weight bearing after 2-3 months. Future arthritis will depend on the amount of joint damage and how close to the bones/cartilage gets to their original position. Ankle rehab is taxing but extremely important to get the quickest recovery. I hope this helps and stick to the lemon shake ups next year.

I still have pains here and there w/my left foot, but not like with this ankle. I went to physical therapy also, but I'm still having an issue bending my ankle. There can be a rush of fluid that goes to the foot when it has been elevated for a while. This usually happens about a week after surgery. Lower you foot in increments: bring it from heart level to waist level for 2 minutes, then below waist level for 2 minutes, then rest it on the ground for 1 minute. Let me know if this helps
      The dressing may be too tight. Ask your surgeon if they want you to loosen the dressing.

    When we can walk after metatarsal Surgery

    • @GoToFootDoc
      @GoToFootDoc  2 роки тому +1

      This is a broad question. I am going to take a guess that you are referring to a osteotomy or bone cut. This is the same as a fracture. Fixation stability is the key to how soon you can walk again. Locking plates are the most stable, then screws, pins. and K-wires. Other factors are how much you weigh, which metatarsal, and your age. You may be looking at 2-8 weeks before walking is a good idea. If the fracture is not fixated or left untreated, you could triple this time.
