    I’ve watched hundreds of hours about intermittent and time restriction fasting and caloric restriction on UA-cam over many years and I still learned a few things in this video because the information was delivered in such a logical and scientific manner. Thanks Eleanor!

    Personally complete fasting is much easier than FMD. Zero calories is easier to measure and I get the hunger suppressing effects of counter-regulatory hormones like epinephrine, glucagon and the signaling effects of Ketones.

    Thanks. Would be nice of someone could deconstruct the gallstone issue and the cardio issues (skipping breakfast) as neither of the studies are straightforward. They seem to have confounders which could explain the negative outcomes for subclasses of people involved in the studies. Satchin Panda of the Salk is surely aware of these claims by Longo, but doesn't seem to be concerned as he advocates 8 to 10 hr feeding windows and is not overly concerned about skipping breakfast. In my case, I skip breakfast, which is not hard because I'm not hungry in the morning. Dr Jason Fung (nephrologist) has run a fasting clinic for a number of years now(focusing on older diabetic patients) and has told Peter Attia (on the Drive podcast) that gallstone hasn't been an issue with his patients who do various forms of fasting. Part of the reason may be clearing up the diet in tandem with fasting.

    Perhaps it's just my confirmation bias kicking in, but I tend to think that higher mortality associated with skipping breakfast is not cause and effect. I suspect that years of being told about the importance of breakfast means that conscientious people eat breakfast, but also do lots of things which are good for your health. People who aren't conscientious on the other hand have bad habits and are also skipping breakfast.

      Your argument does have merits. If I recall correctly, the study on benefit of eating breakfast was based on all US population, not just people doing IF so there are a number of other factors mixed in. Also not sure if that study was funded by the cereal industry.

    Another nice video!
    I definitely believe the circadian clocks are some of the drivers behind fasting period benefits, and that’s where some key findings will come from. Will need to check out that book you mentioned.
    I don’t see how FMD would garner the circadian benefits though, and FMD seems unlikely to allow any metabolic switch to ketones.

      Thanks, the idea with FMD is low calorie (low carb, low protein) but high fat so that can be used to generate ketone bodies through ketogenesis! 😊

    There's a group of people practicing an extreme form of IF they call "rolling fasts". Basically they do 10 to 30 96h fasts, back to back with a massive refeed 6 hour window. The VLOG is "Finally fasting". Might be worth studying these people.

    Well presented. It's worth mentioning that Valter Longo also says that for those with health issues, fasting once a month for 5-7 days offers benefits.
    I like Dr. Longo's work but his FMD protocol (and prepackaged food program) includes grains, carbs , and dangerous seed oils (although if you're a vegan you'll like his diet).
    1000 cal to begin, decreasing to 800 cal. Grains and seed oils are questionable to many of us, as are some of the other foods recommended and in his pre-packaged foods.
    800-1000 calories is really more calorie restriction, imo, and evidence suggested it's not guaranteed to produce meaningful levels of autophagy and the associated stem cell renewal. His 2015 study fasted the mice 4 days every two weeks. This means out of 22 days they fasted 8 days and refed 14 days, beginning at 6 months of age and throughout their lives. This is much more than anyone would be able to accomplish in their lifetime. Whereas there seems to be evidence that water only fasting is a better - and quicker - avenue to achieve the same results. Even Longo admits this, but he believes people find it fasting too difficult. I guess it depends on how committed one is to maintaining health. Fasting once a quarter for 5 days doesn't seem like a big deal to a lot of folks, and fasting once a month for 5-7 days might be a small price to pay for regained health for others.
    I tried 21 days of water only and I admit it was brutal (my keytones went were high (8+) and blood glucose kept falling too low (25). I had to supplement a small amount of fat and protein every other day which prevented the problem. However, I'm not in good health so I'm sure this played a factor - although the fasting problem was not at all related to malnourishment. In fact I have not seen any evidence that malnourishment is a factor in water fasting unless one is underweight. Dr. Longo is the only one I've seen to bring this up which does support his FMD so he might be exaggerating the issue.
    I've since done 5 day slightly modified fasts with no problems and will next try 7. (I eased into it, first 24 hours, then 48,etc.) I recommend the Buchinger Wilhelmi fasting protocol that allows up to 250 calories of broth and or fresh juices a day (I take a little bone broth and fresh sauerkraut juice) and up to 28g honey only if needed for energy to lightly exercise which increases autophagy. They have a helpful YT channel. They've also been providing prolonged fasts for over 100 years, and conducting research and teaching through their two clinics in Spain and Germany.
    I also recommend listening to Dr. Matt Kaeberlein on YT who is a global expert in the biology and molecular mechanisms of aging. He says it's believed that autophagy starts at around 24 hrs in the liver, but not until 72 hours for the rest of the body. the majority of studies for autophagy have been done on animal models who age faster than humans. Dr. Keaberlein basically extrapolates the autophagic response time in these animals (which might be a day or less) relative to human time. If a mouse reaches autophagy in 18 hours it doesn't mean a human will, so a lot of what you hear regarding autophagy in time restricted eating like 16/8 or 16/6 is not necessarily based on sound evidence. That's not to say the other benefits mentioned here might not make it worth your while.

    intermittent fasting--evening meals--fat deposition--plaque build up--seems fairly obvious to avoid eating from 3 pm on as reduced heart rate and blood pressure while sleeping with corpuscles loaded with food substances including fat would increase deposition of fat and increase plaque comparred to higher heart rate and blood pressure while active.

      fasting is free and accessible by any one around the world. In addition, fasting can be used either alone, or in combination with supplement and/or exercise.

    Great video, as always! I have to say though, that your comment about ketoacidosis is misleading in my opinion, ketoacidosis is very rare, and I don't think it can happen if you aren´t diabetic,and even then I don't think it's related to fasting.
    I have been practicing intermittent fasting or time restricted eating, as well as the occasional prolonged fast. I never consulted a doctor, because doctors in general don't know much about fasting. the science is still pretty much lacking, so the doctors don't have much to go by, they can maybe only tell you not to fast if you are in a particular risk group.

      You and I are of the same opinion on these points. The most important advice I ever got from an MD was, "Stop eating, stop drinking, start walking." All the others just want to feed me pills.

      The science is obviously there, (sure, there’s still lots to be learnt), but the medical schools aren’t teaching this… To be fair, the medical schools, with their limited time available, probably have to concentrate on the structural issues in order to best educate their med. students.

    When mentioning about ketoacidosis when discussing about ketosis, you may be mistaking with a condition occuring in patients with diabetes. Healthy individuals in ketosis state do not get a low pH or acidic blood. On the contrary, ketosis requires the levels of proteins to be lower and thus less occuring gluconeogenesis and also less lactic acid byproduct.
    Physiological ketosis that arises as a result of ketogenic diets is characterized by ketone bodies in blood reaching a maximum level of 8 mmol/l with no change in pH, compared to uncontrolled diabetic ketoacidosis where their level can exceed 20 mmol/l and result in a lower blood pH.

    Based on my readings and understanding of human physiology, I believe that the difference in effect of the intermittent fasting can greatly depend on the type of diet. When a subject is already in ketosis, the fasting will probably only provide benefits. However, when a subject is not metabolic flexible, and solely depends on carbohydrate metabolism, then intermittent fasting can create energy and hormonal disruption which can lead to some adverse effects.

    Periodic for the benefits, intermittent for the lifestyle. If you hate working out and/or you can't/won't restrict what you eat, intermittent is a more sustainable lifestyle. If you're relatively healthy and you want to step up your game even more, periodic is better.

    I've been on a regimented fast/refeed protocol for ten years, haven't really aged at all since I started. I also maintain single digit body fat year-round.

      It varies. General variations of OMAD with significant caloric deficits followed by periods of larger surplus to restabalize hormone panel and reset hunger. Recently begun pushing longer fasts post refeed as the need to eat just really isn't there. Maintaining high levels of insulin sensitivity via fasting and caloric resistance seems to be very beneficial for anti aging effects. I should also add I do routinely implement BCAAs so as to stave off muscle breakdown from more prolonged fasting. I keep chronic low levels of body fat with relatively high levels of muscle mass. Naturally I might add.

      @@chriskanizsai672 so basically since you started, have you essentially visually and physically aged, say, 1 year every 2 years or something? I notice at any time I am always 15 to 20% younger looking than I actually am, since I started this myslef (started at 22 am 29 now)

    I do feel risk analysis is not very good when it comes to medicine: it seems like people miss out on medicine that could help them because of fears it might do harm... but harm can be done by not providing the medicine. A hundred people could die because some medicine wasn't approved in time for them, but if one person dies because of something prescribed then it's seen as disastrous... probably cause it's end of the doctors career? I feel it's certainly something which needs to be discussed more.
    I'm just an outsider looking in though, so maybe there are important things I'm missing.

    Is it a problem that Longo is seemingly the only person on the planet pitching the importance and benefits of re-feeding? Seems like every study I can find that has anything to do with fasting and stem cells or the benefits of re-feeding is a Longo paper, and any discussion of this particular topic is referencing his work exclusively. Has any of this been corroborated by other labs?

    If you have ears to hear check out Dr. Robert Morse and the raw vegan community perspective as well. Everything points towards plant based, lower protein diets being healthy. Modern scientific findings are showing this. Why not take it to another level and make it raw like how all animals in nature eat their food. The foods that can be eaten raw are fruits, veggies, and tubers, and some nuts i think. But be weary when you don’t feel good, detoxification is a must. Generations of eating certain foods that are inharmonious with the human have created changes and adaptations. An unnecessary need for stimulation of glands. The body wants to rebuild itself to become stronger and clear obstructions caused by acidic chemistry in the lymphatic system that derive from meat, dairy, starch. Human anatomy and psychology is close to that of a fruigvore/herbivore. Color vision, small mouths, small hands with no claws, specific ph levels of saliva and stomach, feelings of guilt to kill animals, closely related to chimpanzees and bonobos (frugivores).

      @@bobbobson4030 It seems that fasting decreases ROS+inflammation . But the exercise increases ROS+inflammation temporarily only to activate "ATP production and the mitochondrial antioxidant function".
      But most importantly exercise is good for motor function while fasting can aggravate muscle wasting .

    As encouraging as Valter Longo's research is, we need better research in humans, performed by researchers who don't have a financial interest in the results. Longo's financial interest in Prolon clouds every bit of research he does in this area, unfortunately.

      I’ve heard him ask that question directly and he says he has no financial interest in it. He has signed all of his interest and shares over to a foundation called create cures. He did that specifically so no one would say that he was benefiting from his own research financially.

    I have a general mild criticism of this channel's videos. The verbiage is rapid, but the audience does not have time to ingest (forgive the pun) major points while the narrator carries on at a pace that does not allow the listener to digest (forgive again) or synthetize (yet again) the information. It reminds me of some very unliked lecturers in med school. Maybe a little study in pedology would be in order. Pause at the major points and for graphics that are particularly salient. Is the goal to just compress information into a pill (sorry?) intended for those who already know the information or to actually convey the information to a larger but less specifically specialized audience?

    Not a fan of FMD see ua-cam.com/video/YxiJqmWalMQ/v-deo.html as example. Also it's just not necessary. I think the real solution here would be to first eat a proper diet, so you're not malnourished and then start the prolonged water fasts.

    Given that fasting has implications for longevity, why is it not recommended for people over 65. It seems that people over 65 would benefit the most from fasting.

    Those OMAD split the time difference. Also, were the higher incidences of breakfast skippers with more heart disease on the standard American crappy diet? Makes a world of difference.
    Qwercetain and Fisitin should be taken during fasting to turbo charge autophagy and further blunt MTOR IMHO.
