This story reminds me a lot of Edgar Allen Poe's Masque of the Red Death. A wealthy, powerful man revels in his home with his rich friends, convinced nothing can harm him. Then a mysterious uninvited guest in a mask arrives, bringing death in his wake. Even the failed attempt to unmask him is in both stories.
It really does strike me just how much 049 is, well he's a genuinely good person, he's just completely delusional. If you take away the anomalous aspect, he's a doctor who's given up everything so he can wander the globe and personally attempt to eradicate a infectious disease.
Or it could be the cognito hazard scp that states that humans when they die, become more of a paralyzed conciousness inhabiting every atom of their original body and experiencing eternity as a conciousness that has direct feeling of what happens to every atom they inhabited. So that would make sense. OR maybe its a bout the psychospace entity mentioned in scp 5000, maybe 049 is trying to disconnect humans from that entity, giving humans actual freedom from the entitiys steering of fate? OR, just stay with me, OR 049 is just an anomalous individual who is actually deluded, there is no pestilence, as evidenced by the way he responds to being confronted for killing humans, anyone that is against him or his narrative has this pestilence, and simply being nice and cordial and agreeable around him means you automatically do not have this "pestilence".
If the pestilence is fear, I think would be more specifically just fear of 049. If it was just fear in general, every D-Class in a containment cell would be dead.

The invisible disease is for me, as i see and thought, would be only ourselves' unkindness. I'll explain, when the billionaire "accepted" to work with him he gladly accept his offer, but when he refused he said that he was sick, so basically it means the unkindness people have.
I think the pestilence might be pain and suffering. Could be connected to that other story where the Foundation made some experiment and found a thing (only described as 'it') that fed on human pain. More than that, they found out that humans initially were unable to feel pain at all and that thing made them feel pain and made them think this was normal, just so it could feed on their suffering. Even after death, it would prevent their consciousness from fading away, so they'd still experience the pain and desperation of their body rotting, their molecules eventually spreading out across the entire planet, and still they'd be forced to remain conscious and aware of it all, just so that it could continue to feed on their pain. In the end the Foundation chose to infect as many people as possible with a meme that would make them unable to feel pain and then they'd kill everyone else to rebuild the world afterwards. They were almost successful but then the thing managed to manipulate some dude into turning back time to before it was discovered. I think the doctor might be aware of the thing and his pestilence is the ability to feel pain and suffering. So in order to get rid of it, he turns them undead and they won't be able to feel pain anymore.

I'm wondering if this story is a reference to "The Masque of the Red Death" by Edgar Allen Poe. Some elements are similar-- it focuses on a masquerade thrown by a wealthy man during a plague that is killing everyone, and the masquerade is thrown to distract the wealthy people from all the death even though they know exactly what's going on. It's a pretty interesting story, there's 7 rooms all decorated in one color with the last being all red but no one wants to go in there and every time the clock rings out the hour everyone stops until it stops. It ends with a strange plague doctor (or they might be dressed as a plague victim, can't remember, been a while since i read it) entering and proceeding through each room until he reaches the wealthy man who confronts him and attacks him, but immediately falls over dead from the disease. From there, everyone else tries to confront the interloper and rip off his mask but he vanishes like he was never there. If this is intended to be a reference, I wonder if perhaps, by looking at it through this view, we could guess that maybe the pestilence is selfishness and a callousness to others? I know that it's a little loose, but still interesting, at least to me.

Could pestilences be deceit? Many people in the party are just using a facade to cover what they really are. Maybe that’s why when SCP-049 arrived at Site-19 he said that they did pretty well with the pestilence, as the Foundation tries to hold truth to their ideals and scientific approach to anomalies contained.

Thr doctor leaves Foundation members alone for a while then the guy who is going to retire(and is thinking about retirement) suddenly gets killed. Or the guy who had other plans for 0-49 but was going against the chain of command gets killed while scp 049 still didnt breach containment after that opening. The only time he seems to kill a lot is when he is out the Foundation doors and in containment breaches (where he could run away but instead gets more kills) this is a time of chaos, where nobody knows the best outcome and everybody has to follow their survival instinct instead of, well a site director or a manager.
Foundation Tales are always so interesting to read and watch. We need to have the tales of the Shark Punching Center. Day 201 of asking the O5 to use SCP-1968 to create a universe in which 682 doesn't exist.

From everything about 049 collected here is what we know about the Pestilence - only exists in humans , 049 never targeted any other animal and has stated it only exists in humans - not all humans are infected - in the story with Shock, there is a community of "pure" humans that do not have the Pestilence and wishes to leave them alone, and in the post-apocalypse he has finished his work. but because there's no strict canon and SCP exists as a multiverse - the Pestilence could be a different thing in each different SCP universe and each version of 049 in the multiverse has a different Pestilence they are tring to cure.

here is what we know about the Pestilence
- only exists in humans , 049 never targeted any other animal and has stated it only exists in humans
- not all humans are infected
- in the story with Shock, there is a community of "pure" humans that do not have the Pestilence and wishes to leave them alone, and in the post-apocalypse he has finished his work.
but because there's no strict canon and SCP exists as a multiverse - the Pestilence could be a different thing in each different SCP universe
and each version of 049 in the multiverse has a different Pestilence they are tring to cure.
I wonder what would happen if someone says "I am a healthy carrier of the pestilence, I don't wish for treatment."
