Through out the stories concerning SCP-049 I've wondered WHAT the Pestilence the Plague doctor has been obsessed with. Since 049 has claimed to see some people with it that seem to have no symptoms and other he says are free of it, it's always been a mystery. For the story, one would have thought they would have taken blood or small biological samples for testing would have been something one might think they would have done first before giving him a supposed cure.
i think the people he sees who are free of it are probably people who die of unnatural causes, like a security breach or d-class testing. someone with 'symptoms' might be someone who is dying of something other than old age but also not destined to die by accident or violence, like a smoker who dies of lung cancer. It's the body's natural reaction to constant carcinogen exposure to get cancer, so 049 might see their progression towards death as a symptom of it.

343 theorized that the Pestilence is the idea of Free Will, and backed it up with some good points. But I think it's actually the thing that causes someone to turn to Free Will. Doubt. I believe that it is the act of questioning whether you're contributing anything worthwhile. Whether what you're working toward is worth the effort. In short, wanting to escape your status quo. And I think that's what happened to the Plague Doctor. He started to doubt he was making any progress on perfecting his cure.
That is kind of a fascinating take on it. Doubt would be 'cured' from those treated by him. But he's also said it manifests in different parts of the body in different patients. I've started to wonder if it's actually mutation. Micro scale mutation happens on the regular, especially as the body's error checking/correcting mechanisms break down with age. Even a benign mutation could be seen as a pestilence, after all it's clearly not part of you (as defined by an unnaturally exacting standard) but it is consuming resources from you.
Bubonic plauge is actually very curable with modern day antibiotics. There are a few dozen cases ever year in the American southwest where it is carried by gophers and prairie dogs.

I do believe the doctor's act of killing someone and turning them into a zombie is that of necessary evil and not just sheer stupidity and ignorance. I believe 049 knows his patience are incurable which is why he kills them. Killing them, by extension also kills the pestilence (and while there are dead bodies on the ground, might as well turn them into mindless slaves because why not lmao)
At 5:40, you mention how SCp-049 wears the outfit of a plague doctor during the black death of the medieval 1300s. However, the outfit actually comes from another case in the 1600s, when the bubonic plague was more limited towards Italy and France.

I remember coming up with a theory on 049 that since during 5000 mentioned people that were being tested by being stabbed, and they didn't feel pain or did showing if they were infected by The Entity or not. That maybe the pestilence is The Entity, maybe 049 was somehow immune to the events of 5000 and still remembers those events, or simply was simply able to sense it. Zombies wouldn't feel pain, so his cure is imperfect, but, yea, I remember at least being one of the people that came up with that idea.

Okay but the Black Plague isn't as deadly as it used to be. From google; "Bubonic plague still occurs throughout the world and in the U.S., with cases in Africa, Asia, South America and the western areas of North America. Antibiotics greatly reduced mortality, and by 1990-2010 overall mortality had decreased to 11%."

It is definitely ethically questionable but my guess is regular prisoners and once the governments of the world say no more jail and prison they will probably use SCP-2000 to make clones of the D-classes they had before basically using the same people as test subjects over and over again after all those clones won’t necessarily know everything there original did as the Foundation can manipulate those memories I know SCP-2000 is only used in emergency K-class scenarios but by that point it might be different
I can kinda see most doctors either being able to diagnose and treat themselves, a sorta all purpose doctor catching the flu, or having to a doctor in a different field of medicine for treatment, a foot doctor suffering pain in their hands. Unfortunately, the plague doctor seems to be mostly a surgeon and it would be hard, painful, and lethal for a surgeon to operate on himself. Anesthesia supposedly not only helps numb the patient and makes them sleep but it also prevents the patient from bleeding to death. I can't see 049-J being able to operate on it's parent alone, especially when it's cure for a sore throat is beating the patient with a shoe. The fainting does sound like possibly a high fever so a lot of tests would have to be done and he should probably be kept cold somehow. With him being an old anomaly though, I can't say if using anesthesia would be wise or effective and with him wearing his skin, which is black, I don't know how well his nervous system works or if he has blood going to his skin at all. I know some people are just dark skinned but again, 049 is an anomaly and dying limbs have a tendency of turning black.
My theory is that the pestilence is not dying of old age but the concept of catching a disease since Dr Hamm was suffering from old age when he interviewed 049 and when he gave his example of the pillar rotting, I believe he was Implying that they should not let his vulnerability to disease get any worse otherwise it would kill him. So the reason scp 049 is sick is because he is gaining a vulnerability to disease. But that just my theory so this may not be the case.

I can kinda see most doctors either being able to diagnose and treat themselves, a sorta all purpose doctor catching the flu, or having to a doctor in a different field of medicine for treatment, a foot doctor suffering pain in their hands. Unfortunately, the plague doctor seems to be mostly a surgeon and it would be hard, painful, and lethal for a surgeon to operate on himself. Anesthesia supposedly not only helps numb the patient and makes them sleep but it also prevents the patient from bleeding to death. I can't see 049-J being able to operate on it's parent alone, especially when it's cure for a sore throat is beating the patient with a shoe. The fainting does sound like possibly a high fever so a lot of tests would have to be done and he should probably be kept cold somehow. With him being an old anomaly though, I can't say if using anesthesia would be wise or effective and with him wearing his skin, which is black, I don't know how well his nervous system works or if he has blood going to his skin at all. I know some people are just dark skinned but again, 049 is an anomaly and dying limbs have a tendency of turning black.

We need an update on 049 in 3008, that was a really cool concept.
This is truly a interesting Situation to think about but it definetly could happen
My theory is that the pestilence is not dying of old age but the concept of catching a disease since Dr Hamm was suffering from old age when he interviewed 049 and when he gave his example of the pillar rotting, I believe he was Implying that they should not let his vulnerability to disease get any worse otherwise it would kill him. So the reason scp 049 is sick is because he is gaining a vulnerability to disease. But that just my theory so this may not be the case.
Tell SCP 343 to cure him, done and simple!
Come to find out, 049 just wanted to be the patient for once.
