Bob and the Angel seem to be separate entities sharing a body, the Angel essentially let Bob’s consciousness possess its body to gain the doctors trust and get his location. At least that’s my understanding of it, based on how Bob refers to it as “the Angel” and not as himself at any point.
People say the angels are just messing with him but he literally says his consciousness is reanimated. That means he is still thinking like how he always was, but with the control of the angel. Bob was always honest even when asked if he was scared, so his consciousness was why he was honest about the situation even without the angel's approval because even when you have control over a voice, the consciousness wll always have their own mind even in your control. Think about it, why would the angels risk anything? If they had ALL control they would not have given hints to anything, they know what the doctor is capable of, they are smarter than to play around with him, they want him dead as soon as possible, unfortunately for them, things didn't go their way because they controlled an honest man's consciousness, that's why, before his death, his honesty was repeated over and over again in the beginning. They mentioned his honesty a few times and tested his honesty a few times before he died because the author wanted the audience to know that his honesty plays a big role in some way. Like foreshadowing.

What truly scares me is only one angel in that whole place was full power, the others were starving and nearly dead so they couldn't use the time power they have naturally until they fed on a proper meal

I‘d say that the angel sent its own ideas to his brain, had him interpret them into humanly understandable concepts and then express them the way he would, saying them using his vocal cords.

Theirs this episode of hxh where the villain was trying to figure out how to use "nen" a form of an ability, so he used a human and opend his brain up (who could use "nen") and using two antenas he made the human say what "nen" was and how to use it, thats how i imagine they communicate
