hello jhon, I am Marouane Misse physical therapist and I am one of your biggest fans and my English’s not very good . I recently received a patient with an IT band Syndrome and he has a « clic » at the distal insertion of the tfl when passing from the full extension to 30 degrees of flexion. what can I do to limit this « clic ». I thought that the method with muscle energy is to contract the antagonistic muscles of the muscles you're treating. In this case, treating the patient's left tensor fascia lata by making them contract its antagonistic muscles (muscles used for adduction) thereby causing the tensor fascia lata to relax. So shouldn't you abduct the patient's leg to their feather edge of the restrictive barrier and have them contract against you (trying to adduct the leg)? Please correct me if I'm wrong.
There are two main methods of MET, one is using the Agonist muscles and is called PIR (post isometric relaxation) and the other is RI where you use the antagonistic muscles (Reciprocal inhibition)...maybe read my book i wrote on Muscle Energy Techniques but thanks for the comments. regards JG
Hey John, diagnosed with Patella maltracking-Scans look okay apart from Patella doesnt have that much space at the top-Just came about the last 2/3 years and I went to the Mayo Clinic here in Florida, USA and was told I was actually born with it? weird since Ive been surfing over 30 years and was competitive and also read electric meters for 7 years and walked 20-30 miles a week easily? I do need to lose 20-30 lbs for sure but anything else u could suggest would be greatly appreciated!
Thanks for the message and I tend to say a problem with the knee is typically as a result of an issue with the hip joint or ankle / foot so rather than always focussing on the knee perhaps focus on the two areas I mention....that tends to help a lot...I also say strong Glutes is a stable knee...regards JG
