regarding switching accents: the entire show is performed only by the 6 Queens. So if there are other characters, such as the Holbein staff, it's still the Queens actresses. For Holbein, they have sunglasses and neck ruffs to differentiate from their Queen role. The random bursts of British are more like side notes, being interjected from a 3rd POV. Also, high heels did exist! They were pretty different, more blocky. They actually originated as men's fashion, intended to make it easier to keep feet in stirrups while riding horses.
