He tells Ellie that she should not have had to do that because she is just a kid and Ellie responds that it was not her first time. The conversation its left ambiguous (did she just hurt someone? kill someone? a person or infected?) and Joel does not ask any follow up questions, but since Ellie did not have exposure to killing anyone/anything while in training and we can assume that following this mall incident is when Marleen and the Fireflies pick Ellie up, that she was alluding to something that happens in this episode. Sure, maybe someone could argue that she killed an infected and that is what she meant, but Ellie also asks Joel earlier on in the series if it is hard to kill the infected since they know that they were all people at one point. We saw in episode 7 that Ellie really did not feel all that much remorse in the moment for killing the infected that was about to kill her and Riley, but OOF watching your best friend/first love succumb to it and maybe have to kill them out of mercy or self defense? That would certainly leave a person seeking some sort of emotional validation like I think we see Ellie seeking from Joel when she asks about it and Riley would certainly be a 'someone' as opposed to 'I killed an infected'. There is also something to be said about the way she toys with the one infected she finds trapped in the basement. She takes a knife to its forehead and seems to be experimenting - maybe to see if it can feel anything/is human - and then stabs it once the rage fills her. Rage towards infected that is likely rooted in her hatred of them for taking Riley from her. It is also possible that Ellie doesn't kill Riley but its Marleen/the Fireflies who do it when they come to the mall to collect Riley to ship her off as planned. I figure Riley must have turned by then and not in an almost state of turning, but I don't remember the time frame off the time of my head based on the poster we see in episode 1. I assume that the mall is where they (the Fireflies) find/meet Ellie for the first time and discover she is bitten and possible immune and then lock her up to check on her as we first meet them all in episode 1. Riley also gets name dropped there when Marleen asks Ellie if she thought Riley was bad and Ellie says no with a certain subtle facial response (Bella Ramsey is so good at nuances). Part of the reason there are not many infected is because they are in a fedra safe area that has been fairly cleared out of infected for the most part. That's why Ellie is so excited in the first episode when she is outside the walls with Joel and Tess, Her first time really seeing the world.
There’s a lack of infected because in the series the fungus is connected so if one gets alerted all of them do so I guess it’s a bit unrealistic to have them going against millions of infected every time so I just think they making use of them for big scenes like they did for episode 5. Also the actor of Riley is Storm Reid she’s been in multiple movies but I would say she’s mostly known for Euphoria

They've kinda always had an "excuse" as to why theres no infected. -Any Fedra QZ is obviously walled off and secured (which is where this episode takes place). -When they're in the big city Henry says all the infected were forced underground. -When they're looking for Tommy in the Winter, Joel says "there's no infected out here" I assume cause the fungus couldn't survive in the cold. Not to say this is a bad thing. I think all the reasons are pretty believable and it didn't take away from my enjoyment. After all, TLOU is more about the characters then it is a zombie story.
In Mortal Kombat II, Baraka and Mileena are lovers. The fact Ellie and Riley chose to fight as them at the arcade is a coincidence. Whether the creators knew it or not, their gameplay did showcase a grim foreshadowing.
