Hello, Victoria. Thank you very much for taking the time to go over a detailed explanation of a moderately complex problem. The majority of my providers send in 99214 or 99215. I'd see a nurse visit once in a while, and you're correct, it's sometimes unclear whether it supports or not. I'd appreciate it if you could go through the nurse visit 99211 when you have a chance. Thank you once more, and have a wonderful holiday season.
This was so helpful. I’ve been seeing increase in Level 4 & Level 5 in my practices this year. I appreciate you listing your resources, I will be utilizing those!
I had a checkup with my primary care physician, as a new patient and also had an EKG. I was billed separately for the EKG, and separately for the exam part which they kept me waiting and kept taking my blood pressure twice. Were they doing this intentionally to bill me as a level 4 by making me wait longer? They took my blood pressure before I saw the doctor. then the nurses did it once more. I want to fight this part of the bill, saying that charging patients $233 just to have their blood pressure taken and for being a new patient is crazy. The ECG bill by itself was only $44. Because the nature of the visit was for a medical clearance for an upcoming surgery , it did not fall under the cheaper "wellness or preventative checkup" category by my insurance carrier.
I have a provider that keeps wanting to bill 99214 with one diagnosis and no prescription management 15 min increments and she is complaining that I’m coding 99213
Im confused. I went into urgent care and all they told me was I had a cold, took a swab of my throat for strep and a covid test? I dont have a chronic illness or did they prescribe me anything.. so why on my bill is it level 4 yk?
Billing can be a bit complex. The level on your bill often reflects more than just the diagnosis. It considers the comprehensive nature of the visit, including the assessments, evaluations, and any procedures done, like swabs and tests. Sometimes there's more documentation and considerations behind the scenes than we may realize as patients. If you're uncertain, it's a good idea to contact the billing department for clarification.
Depends on the MAC. Most will qualify it, but you still need the other pieces and medical necessity. Technically a prescription refill can be done online or over the phone, and doesn't in and of itself warrant brining the patient in for an office visit and charging the insurance company.
Hi KR Abear. Have you gone to the You Tube channel for Contempo Coding? Ms Moll has wonderful playlists on just about every aspect of medical coding! I know she has at least one on how to become a coder!

does reviewing the X-ray or MRI findings w/ pt (i.e. going for the report w/ the patient) qualify for "Review of results form a unique test"?
What is the ratio of L4/L3 that a typical outpatient pediatric practice has in 2023?
