- Crash, fortunately without injuries, of a helicopter of the Guardia di Finanza on Saturday morning at the airport of Bolzano. The accident occurred during a training session when the aircraft, an Agusta Westland AW169, was still on the ground with the rotors in motion. Suddenly it tipped over on its right side. The crew of two was uninjured. Investigations have been initiated to establish the exact dynamics of the accident.

he managed to crash an aircraft from a stationary position. That is a remarkable achievement. You can't even do that in a flight simulator.

@@blessedheavyelements8544 hard to say without more information. The best I heard was a failure to release the front nose wheel while they were taxiing. That’s is my summation after reading all the comments.
All of my aircraft I flew were skid helicopters. So, I don’t want to speak as an expert on something I have never flown. Keep reading the comments. There are some smarter people here on this topic than me.

It's a complicated machine which doesn't forgive a tiny mistake. If a pilot is a noob he will total this thing just by sitting there.

A large number of helicopter accidents happen on the ground as it's the most dangerous place for a helicopter.
Actually he didn't "crash" the heli, it just turn over on spot.

This event reminds me of my last day in Vietnam. I got into a Huey for my flight to Cam Ranh Bay, and as the Huey pilot was revving up the Huey engine, our pitch change link broke. Immediately the Huey rotor began to oscillate wildly. After a few seconds the pilot began to shut down the engine and the main rotor blades began to smooth out. Then the pilot ordered all 8 passengers to evacuate the aircraft. He didn't have to make that command twice. Later that day I caught another Huey ride to Cam Ranh Bay, and caught my DC-8 jet (Freedom Bird) flight home to my family and friends. I got home to Pennsylvania one day before the Woodstock Festival in August of 1969.

Incident report says :
A Leonardo AW169, operated by the Guardia di Finanza (coded 504) started to taxi, paused, veered to the left, lost directional control and fell on one side, no injuries are reported. Substantial damage is reported on the helicopter. In base of the investigations and simulations made by Leonardo the accident occurred because the pilot didnt unlock the forward wheel and tried to began taxi, then he understood that the front wheel was blocked and he unlocked It, that caused the helicopter to veer to the left because of the thrust applied by the pilot and crash on the taxiway.
Very interesting. The pilot should concentrate upon finance matters and forget about being entrusted another helicopter.
That report doesn't make sense... but, why didn't pilot apply full right pedal or why didn't he cut the power (throttle) or something?

From initial information, nose wheel lock was left on (it can only go straight). As the FP applied left pedal to taxi left, the aircraft didn't turn as much as expected. They stopped and likely realised that the wheel lock was still on and released it. Unfortunately the left pedal was still applied and the helo started spinning on the ground. Expensive error.

Too expensive! That is why we normally train pilots using Cicare SVH4 first... who in his right mind will use a U$ 10 million helo as training equipment!!!

0:45 The pilot uses "a lot of power" trying to turn the helicopter, but he forgot to disengage the front wheel "lock".
0:47 The pilot disengages front wheel lock, now helicopter starts turning a lot.
Once he was in the spin, was there a way out without crashing?

For all you keyboard critics out there, the flightline safety manual clearly states "If you are within 40 feet of an out of control helicopter, gently mosey away from the situation."

Bravery has nothing to do with "ignorant" or "exaggeration" that would be the concepts of "stupidity". A man facing a beast to protect someone, even though he knows he will not(or can't, or have fewer changes to) succeed, is bravery and not "stupidity". I would say he is stupid trying to stop the machine by hand. But that wasn't what happened. He probably couldn't run and was worry about the human being in that machine. The opposite of the other civil man.
One thing is for sure: the quality of the helipad is superb. Not a single scratch even after hammered by the rotor blade. Who is the contractor of this helipad?

I'm no expert in rotorcraft but do you think it could have also happened because of the possibility of tail rotor failure that ultimately led to non-existent counteraction of the torque effect ultimately causing the whole thing to spin out of control?
Expensive loss of an aircraft indeed but it is incredibly fortunate that neither crew member were injured.

@@vinaymulukutla358tail rotor failure would generally cause the machine to spin in the direction opposite to what you see here, as a rule if viewed from above , rotors that spin anticlockwise have a torque reaction that would cause the fuselage to rotate clockwise, Newton's third law of motion, for every action there's an equal and opposite reaction. In this case the machine is on the ground and there's not a lot of torque at the main rotor subsequently, however you give it a bootfull of left pedal that's still a significant counteracting force for torque that's not at the level it would be when the helicopter is in flight

A similiar thing happened in 2018 on an airfield near my home. A military helicopter landed to fuel up. Got to close to the tower, where the gas station is, and touched it's wall. Pieces of brick hit an employee of the airfield and killed him.

Can't believe how casually that guy just walked away while looking back, the second it started turning you could tell something was seriously wrong, he should have been running as fast as possible to get around the corner of the building, he's very lucky he wasn't killed. And then there is the other guy who was behind him, just popping in and out of the building and when the chopper breaks apart he looks like he's just worrying about a bunch of luggage, doesn't even run to see if the people in the chopper are ok.. wow..

This accident made me remember a similar crash on ground, when pilot made his 8 year old son sit on co pilot seat just for adventure and his son suddenly pushed the cyclic to extreme left..

So many things can go wrong when operating a helicopter. That looked like he lost tail rotor authority by the way the aircraft spun like that. The main rotor started to tilt and I didn't see any extreme angle in the tail rotor blades. Perhaps there was an issue with the tail drive shafts, or IGB, or TGB. Who knows? Either way, that was definitely a bad day.

They had nose wheel lock engaged. While pilot had peddles all the way to try to turn, copilot released lock.

@We The People I think pilots hold cyclic stick as holding rack in withstanding centrifuge force.
Its not by intention. But wheel lock might be the cause of accident.

You can see the nose wheel instantly turn 90 degree when the lock is disengaged but is one of the main wheels still locked? ... You can clearly see black skid marks getting left behind as if one of them has its brake fully applied.

Happened in March of 21. Failure to unlock nose gear before taxiing (taxi checklist item), proceeded to input left pedal, nose gear was then unlocked with left pedal input still applied causing an uncontrollable spin.

When we visited Disneyland as a kid, this is what my dad would do on the tea cups ride. We’d start spinning and he’d just keep spinning us faster until my head was pinned back and all I could see was sky. Thank God my dad wasn’t a helicopter pilot.

When’s wheel is misaligned, the helicopter tilts toward that direction, causing the heli to spin and then as momentum increases as traction is present, it is prone to topple over and just shred them wings, but at least the pilot is still intact throughout the ride.
When* the* wheel is misaligned ... and a helicopter has rotor blades, not wings.
The Olympic sport of heli spinning is just too expensive for me. None the less I give him a 8 out 10 for his noble effort and pirouete style.
