To the first question, i would think certain mental illnesses and personality disorders would add a genetic predisposition to narcissism and what triggers the disorder itself might be parentification or infantilization which might be a common form of parenting style among people who are already damaged in some way. One would think of themselves as better if they were forced to grow up early because of a demanding environment in their youth. Meanwhile infantilization just makes one unwilling to change, content with what they have, schizoid. These two are genetically predisposed the same way but underwent through different dysfunctional parenting styles. I think there's also a hybrid form of parenting featuring both parentification in certain aspects and infantilization in others.
I know one diagnosed narcissist who was forced into treatment due to be incarcerated. His mother and grandmother are exactly like the grandiose narcissist Prof Vaknin describes. My brother and sister are both not diagnosed but would be if they actually went to a diagnostician or someone who can diagnose them. They are HIGHLY TOXIC living in delusional fantasies with identical traits as the one diagnosed narcissist I know. That’s five extremely narcissistic people I know in my immediate family but I have a very large family.

I would answer both yes and no to your statement above, yes they do have preferences, i.e. they prefer those who provide all of the Ss, i.e. sex, narcissistic supply and also services, and will engage in attempting to mind control you to provide whatever they can get out of you. At the same time they have 2 strategies, 1 to have a main source of supply that provides the main 'mother replacement' source, as well as the generic multiple replacement sources, which are generic, general and all pervasively generic, i.e. they will engage in narcissistic attention seeking behaviour wherever they go, towards anyone, anything, and everything, mostly with a pulse, but also potentially without a pulse, if it is an AI robot for example instead, and generically tending towards 2 dimensional, for example in the form of pornography, which is the easiest most common denominator form of supply they can get. A pole might also act as a form of supply for example for narcissistic female pole dancers as well, since it will easily replace any male. If you are a parental mother replacement figure however, rather than just using you as a prop or tool or object to gratify themselves with, they will instead attempt to debase you, minimise you, degrade you, in all shapes or form, and if you tolerate them to an excess degree, and mistakenly enable them by thinking that being nice to them will cause them to be nice to you in turn, then their abuse might potentially escalate to a lethal scale. (Speaking from experience, after dealing with an ex narc who was spoilt by his mother who he both excessively loved being far too attached to her, at the same time as also hated due to her having divorced his father, when he was 4, and then he projected his 4 year old anger onto me, trying to murder me, when I attempted to leave him after discovering his multiple affairs etc. Literally, I speak from experience, not text books).

Or is a true narcissist always a narcissist? Does this switch always happen? Can there be a narcissist simultaneously with multiple personality disorder?

The Narcissist I was involved with when out in a crowd would be the life of the party, laughing, joking and seemed to be extremely happy with all of the attention. What does a narcissist feel when they get high tier narcissistic supply? Or if they manage to accomplish or achieve something that proves they are godlike? My understanding is that they then feel euphoric or elated. Isn't this a positive emotion? If so, how does this differ from a healthy person feeling elated when getting attention from a loved one for example?
Wasn't his point in saying that the narcissist doesn't have access to positive emotions, that they don't have access to things like love? When you love someone the positive emotion you get is trough empathy. I think his point was that narcissists can only get the most shallow part of positive emotions, which is how it affects them and their story in their head.
Eight so I still have a way to go. I have another question about narcissists issues with their mother. Is it possible all her adult children could develop narcissistic disorder? I ask because they all exhibit similar traits. The oldest was verbally and physically abused by the mother and diagnosed with BPD, the middle was ignored and diagnosed with NPD. The youngest is the favorite and adored by the mother yet still seems to act out and strongly dislikes his mother and most women.
