Thanks Matt, great insights, as always. At age 30 after experiencing hip and low back pain I was diagnosed with hip joint dysplasia, Coxa Valga, where 'ball and socket' connect at a less than perfect angle, causing one-sided wear and tear on bone surfaces within the joint. Doc said I had the hip degeneration of an over-seventy-year-old. Now I AM an over-seventy-year-old! Thanks to very moderate exercise over 40 yrs (a bit of yoga, a bit of pilates, regular walking, lately a few 'upright health' stretches too ;) I now have MUCH LESS discomfort than at age 30.

I recently found out that I have the same condition that the woman that they talk about in the video has, Avascular Necrosis (AVN), where the bone collapsed in the ball part of the joint (the femoral head) because it lost its blood supply and died.
I also found out that I have hip dysplasia like you. The surgeon said that that caused the AVN, with those excessive forces at bad angles pressing on the femoral head, from the shallow hip socket pressing like that on the femoral head. That somehow that damaged the blood vessels in the femoral head, causing part of the bone in the femoral head to die and crumble.
So I'm definitely getting hip replacement surgery in the next few months. The joint's just got to be restored back to normal. But with my case as bad as it is, the only way is to replace it. But I do have a lot of pain as well.

Hi Matt, wrote to you before on your "Asian Squat" video. I am 68 years old and am the wife of Sander Rosenberg, who co-authored the 1978 water exercise book for arthritics and chronic pain sufferers called, "Pain-Free Arthritis". (Dvera Berson and Sander Roy). Sander wrote in 1978 that surgery for any chronic pain should be THE LAST RESORT!! He also was one of the first, if not THE FIRST to write about how x-rays can look "bad" and there could be no pain and x-rays could look not so bad and someone could have a lot of pain. My husband wrote that it was MUSCLES that were the culprits of chronic pain. (Unless a tumor or something else was pressing on a nerve.) Like you, I have experienced many aches and pains as I became a valet at age 42 and worked for over 20 years. And every ache I had was relieved with stretching. I find that crossing your legs when touching your toes, relieved my hip pain that I used to have. We call it muscle imbalances, muscle weakness and muscle tightness. Before every water exercise in our book, Sander states, "to relax, stretch and strengthen, the tense, tight and weak muscles surrounding your shoulder joint, etc. BTW, I have been working on the squat. Years ago getting into it, by holding on to a bar by a pool, helped me overcome an ankle problem that I had for two years!! I still cannot get down without holding on to something. It is hard for me to hold the position for longer than three minutes, that is why I still cannot do it without holding something. BTW, our book was recommended in a medical journal in Great Britain called, Rheumatology And Rehabilitation. Even though it was recommended by many others the status quo never changed as arthritis and chronic pain are still multi billion dollar businesses for the drug companies and orthopedic surgeons.

I have had one hip replacement already. Now the other hip is causing a problem. I can't walk much at all without a walker. Same as last time. My doctor thinks it is my back. Went today for a MRI on my back. My back does ache bad. I do not have a lot of pain in the hip but can not function...walk, put on socks and shoes 10:17 have trouble getting in and out of car, sleeping is a problem but not being able to walk is really depressing.

That blows my mind. I’m 65 now and a life long athlete. Always had some hip pain on the left side and attributed that to running hurdles in high school.
After a 20 year soccer career and my last knee surgery ( where my doctor refused to fix my ACL) his advise to me was to quit running.
Cycling became my new main fitness activity.
That was all fine until my daughter asked me to run a 5K with her earlier this year. We are running 3 days a week now and loving it.
We’ve been building up slowly and your videos have been a staple.
My left hip pain was back with a vengeance though but with your ATM as my first line of treatment and training it is getting better every week
Those flexors that didn’t get the proper attention on a bike are steadily getting back into shape and I’m continuing to build strength and increase mobility.
Thank you so much for your help.
Looking forward to many more years of activity with kids and grandkids.
ps.. using Asian squats while gardening and every chance I get as part of the wellness routine as well. 😉👍

Hi Matt, my son had avascular necrosis in both of his hips at age 33. It took the Dr.s over a year to find out what the problem was, because he kept complaining about his back. Finally, a Dr. at UVA told him it was probably not his back, but his hips. Sure enough, that's what it was. He's had both hips replaced, and is doing much better. He was almost wheelchair bound, before. I'm grateful that he is better, and able to function almost normally, but still worry that these artificial ones will wear out long before he becomes a senior citizen.

I've been doing lots of stretches and strengthening for my stage 3 arthritis of both hips and bad back for many years now. Lately, supine pigeon stretch helped me loosen my hip joints noticeably and I even stopped limping on my left leg. Lie on the floor, grab your foot with your hands or wrap a belt around your foot and pull the foot toward your belly slowly several times. Rest and repeat. Do it with other stretches. You can add resistance later to strengthen hip muscles. Keep your obliques strong as well. Lose weight. It's a work, yes. But much better that having a surgery. Be well.

I've living with this and arthritis for the past 3 years ( currently in stage 4 ) and have only been given the option for surgery when I am no longer mobile. I am 53 and going through treatment for AVN and it came out of nowhere. I also have arthritis in my lower lumbar region. The constant interplay between the hip joint and the back is frustrating. After a year of NSAID's and Fosamax, I have shown bone growth at the femoral head, and I feel much less pain, but it is far from recovered. I can ride my mountain bike aggressively, but standing for durations, running, and walks over 1/2 mile are tough. My psoas fires up when the hip gets fatigued. The joint was not in collapse, so I am glad I was in pain and went to get it checked out. I have really worked on my core strength and lost some weight. I really hoping to keep trending positive, but feel the Orthopod is going to say this is as good as it gets. I can't stay on the meds forever, and I was told I could have another AVN event when off of them. I have been able to cut the anti-inflammatory dose to once per day, so that is positive.

I really enjoyed your info. I was told I needed hip replacement in 2013. I have done everything I can to avoid it as I deem surgery as an unnecessary trauma on the body. I worked with a naturopath doc who put me on a big cleanse where I have uncovered some digestive issues. I have changed my diet to include high levels of collagen found in Ham hocks. Sounds weird but my body health has improved. During the time of my healing I didn’t move much and that has affected me. I knew I need to exercise but dint know what to do. I have registered with your office and will be in touch soon. I think the lack of exercise affected my fascia and Bowen therapy is really helping a lot. My leg is shorter on the side I was told I needed the hip operation. I was also a vegan and a vegetarian for quite a few years and I think this affected my bones.I feel very good now and just want to get back to full body motion so I can ride my horses.

Wow. That's exactly what I had! My hip didn't hurt either but I did have terrible leg pain, foot pain and thigh pain including awful foot cramps. My hip would slip and didn't hold my weight. No back pain. The hip was a pulpy necrotic mess. The replacement gave me pain-free full range of motion....once I reconditioned the muscles.

I'm with you in taking a practical approach. The surgeons that only repair the hip when therre was pain seemed to more likely be covering their behinds rather than addressing the base problem. If the hip is severely damaged then the possiblity of future pain and problems will always exist. Where as If the hip is replaced by a highly skilled surgeon, although there is a small risk of problems related to surgery, fixing the hip problem addresses all future problems. And if someone is of an advanced age where surgery itself involves risk as years progress then sooner the better.

I injured myself slowly over the years by doing a Romanian deadlift. My adductors are tight, particularly on my right leg which translated into a tight right hip. I noticed the other day that I really struggle (more like can't do it) to do a reverse clamshell so I lay on the floor and rest my right foot on the couch so that my heel is above my knee and I feel the stretch.
I mentioned this because I was told that I have osteoarthritis in the right hip and the doctor mentioned in passing the idea of getting hip-surgery. I CAN walk it's just that after a 1/4 of a mile my lower right back gets stiff but I am doing a series of stretches in the hope that it gets better.
After watching this video I have my doubts about getting surgery and will continue to do the stretches to see if I will get any positive results (reduction to elimination of lower back stiffness).

Thank you for a thoughtful video. I have been blessed with a surgeon who replaced both my hips, nearly ten years between successes, and who was concerned about both pain and function. I think the decision should be up to the well informed patient if the bone damage and loss of function is as severe as you describe, even without pain. We should be able to choose surgery to improve our quality of life as well as to end pain.

Matt, I like this issue of hip problem as it mirrors mine.
I was diagnosed with bone on bone problem which showed on my MRI. Although described as such, the term they used is lumbar radiculopathy. Am not quite aware of these terms but it gives me unbearable pains that I don't want to move out from my computer chair anymore. Before I saw your videos, I used to follow chair exercises of the legs and arms just so I can be limber.
My hip pain is really excruciating especially when I do my bathroom necessities plus bathing.
When I stand in from of the sink to brush my teeth, I feel my left knee buckling down if my stance is awkward.
Am afraid of hip replacement as I've encountered 2 close friends who did that but had terrible complaints after the surgery i.e., pain is still there and another's was not properly set.
    Oh God, that's terrible!
Right now am getting therapy where the PT applies electric stimulation plus deep tissue massage afterwards.
But it doesn't alleviate my painful situation. I've been prescribed Gabapentin 100 MG 3x/day, though am not epileptic (but I think the meds calm the nerves) but not in my case. The pain is unbearable while standing or walking, even for a short distance.
What exercises can you advise other than surgery?
I'm really miserable I don't want to go out of the house anymore.
Even climbing the bed is a hassle for me.
    SOS please!

Told me to continue PT, massage and chiropractor. All I have been doing for two years straight. It is so distressing to not have function! Hi all and Matt. Tin man here. Well I have way too much to say in this issue and others related to FAI. so I will use this setting as a prologue. Don’t know if it’s the health care system in this sweet country where I come from or the specialist I chose - yes we can choose our puente doctors - but after, not before discovering upright health but after I had total hip replacement surgery. The reality was I had requested the surgery many months in advance of getting it. And though I met upright health and have seen each applicable video several times and bought the FAI fix program, I got the surgery. The results have been spectacular such that my surgical hip now outperforms my non surgical hip. No complications no nothing. So here’s my challenge: do I follow upright health and work to vastly improve my non surgical hip, or do I throw in the towel and get that one done too. I’d love to fix it myself with your help but the journey is steep and this morning my hip hurts bad. How do you trust a program when you see little or no results and your hip gets worse. And... you just don’t have the money to parachute or fly in or participate in forums or round tables. I’ll keep trying cause I’m a true believer but... and as the tin man I’m so far away from pigeon anything. I do like that you and your colleagues do address clients like me who couldn’t even reach some of your positions in the womb. I remain hopeful but...

I had an appt with ortho that recommended hip replacement because of pain and arthritis in my right hip. However, my left hip with mild arthritis also hurts. To me, it all feels muscular, not what I would think as joint pain. They did tell me that they wouldn't do surgery if I didn't have pain. I found that interesting and confusing because my structure would be the same regardless of my pain level. I'm still dealing with the pain and it has severely limited and changed my life. It is extremely painful but it’s getting better I have lower back knee groin and hip pain due to alignment and I crack my back many times everyday just by lying on the floor straightening out my legs I can crack my lower back several times per day! I do not use crutches unless I overextend myself and I can’t walk. This happens a few times a week because I am an extremely hard worker using my body to push pull and lift all day long! So by stretching and I have made up many of my own stretches I am able to walk without crutches and a not as bad limp until my third hour of working then the limp gets bad so hopefully in another year or two I hope to be 1000 percent in my left leg 🦵

Hi, this is really facinating for me. I have had hip and lower pain problems since my mid twenties. This seems to be from a range of issues; one shorter leg, tight hamstrings and quite a curved lower spin (from back to front). I would get pressure on my lower spine and be careful to aviod disc bulging and pain around my sacrum joint. Eventually in my early 40s I was diagnosed with piriformis disorder. And I still think I have some stiffness in my SI joint. Now in my early 60s I have some hip arthritis in my longer leg side and hip spurs that may be on the outer edge of the joint. I am only get pain when I have walked too far or have been standing too long. To be honest its not as bad as the muscle pains I have had in the past when I havent been looking after my piriformis properly e.g good posture and the right kind of exercise. I am not sure about having a hip operation as I dont want it to make my other issues worse again and not be able to excercise and stretch in the way that helps all my hip muscles. I now swim regularly and this is helping everything feel better. What do you think about the risks of a hip operation making my other issues worse? Many Thanks Van

Hi I just wanted to know if I should get a hip replacement done. I have a collapsed hip along with avascular necrosis. I at times don't feel pain in my hip but it does make me limp do to the knee hurting. Sometimes I could walk without that much of a limp and sometimes it's very noticeable and painful. I don't know if I should go forward with the procedure or not what is your opinion.

Update: I went to the dr. I could not walk, get into a chair, or stand on one foot on my bad leg.
I wear BFR bands when exercising. But my exercise is less than a third of what it was before the injury. I can’t stand on that leg because the ball and socket don’t align. I saw the X-rays before and compared them in to the ones I saw recently, it is night and day. Could see a space between the joints a few months ago. Now it is horrible. This happened when I re-injured my hip. In a few months it changed. So, I will need a hip replacement. My plan: to lose weight 33 lbs., strengthen my muscles. We found the glutes ok, but the gluteus medius and minimus are shot. I will get them up to par. Also, had a cortisone shot yesterday so after a while I will start my scorched earth plan of exercise. I will wait a few months then asses my hip for surgery. I want to do the 4K at Thanksgiving. Wish me luck!

    Hi,
My case is quite similar ... & I am struggling with this issue since quite a few years but past two years it is effecting my daily life & I don’t have real pain around the hip but more of referred symptoms lower back & I don’t properly sit on the hip which has problem as a result constant lower back pain. As I don’t have real pain in the hip drs are confused to go ahead with surgery and it even confuses me . I am generally very active person as had been doing advanced yoga since years and know my body well, now I am at a point where it’s effecting my work as I can’t sit at one place for long I mostly slouch and sit partially sitting on my lower back & walking where in used to walk ten kilometres easily now I fear going to lot of places ... is there anywhere I could share my xrays with you . Would be really thank ful and share more in details my symptoms .. I am 50 year old .. not only drs get confused I myself get confused in hope to get better life I may not lose what I have , as per say all drs I don’t have direct symptoms of hip replacement . But coz my body has been compensating for years my knees are getting bad , pelvic gets unaligned ..
ps I would be really thankful if anywhere I could reach out and share my X-ray and symptoms in detail point by point recap ..
    Thanks

Thank you so much this video has given me new inspiration to carry on strengthening my hip muscles - after arthroscopy surgery they told me I had level for arthritis and would probably need a new hip -It’s very hard to stay motivated and not give up when you have this kind of advice - so thank you so much for your positive outlook as always - I know you probably won’t read this because you get the haters - but now there’s an awful lot of people like me who you help to still look forward to a healthy life- So I’m blocking the system and losing weight even though I’ve started the menopause and I’m getting fitter and stronger - pain can actually be a good inspiration to turn your body around- It might actually have been the best thing that’s ever happened to me because I’m fitter thinner than I’ve ever been in my life - so say thank you to hip pain LOL

Imo surgeons are selling surgeries. I was told that I needed hip surgery several years ago and almost fell for it. My pain from stage 3 arthritis was terrible, but found an amazing Dr who treated me for ilopsoas tendonitis and amazingly all the pain went. Of course the stretches and exercises have to be done on a regular basis or the pain returns. I highly recommend trying this. Best wishes for all

    Hi Matt,
    My hip pain
    1
Effect my sleep if i dont do exercise. If i dont exercises for the hip the night tightness will be worse. Pain is 2-3/10 but tightness of the muscles. Even if I do exercise (recomended by physio) the hip pain is also there but less. Applying heat oil is also good.
    2
Not much range of motion problem if i warm up or do daily exercises.
    3
Cannot play golf or badminton or run much. Pain of hip if i do. Mainly sleep problem. Level of pain is as describe above.
    4
I brisk walk 3 times a week. But not too much the hip dont like it, so I do your hip joint exercises. So I walk only about 3km instead of 5km.
Do I need a hip replacement?

This was unfortunately before I discovered Upright Health . I have purchased your healthy hip’s program and things are going well. Do you advise any additional exercises or exercises to avoid on the replaced hip. Are there any exercises to prolong the life of the replacement. I think the way you advise people is tremendous by putting items on UA-cam . Because of your advise and exercises which are common sense I would definitely advise anyone else going down the surgical path. Thank you.

I am in my 30s and have join loss since I was 13 years old. I suffered an accident that placed my femur head in a position where my leg cannot move normally, so I naturally started making muscle compensations. As a consequence, I have severe muscle atrophy in multiple muscles (gluteus, femoral, quadriceps). Surgeons have always given me this diagnosis: As long as the pain you experience is bearable, as long as the pills still have an effect, don't have surgery. They recommended that I stay at a low weight and do low-impact activities. And here I am, so limited in movement today that I can't ride a bike, walk for more than 10 minutes, stand for prolonged periods... Putting on shoes, putting on socks, cutting my toenails are already impossible tasks. And the pain is still bearable for me... I have a son, and I feel like I'm losing my life with him. Carrying him means suffering for me. Playing with him is difficult, and there are things that are impossible today.
To tell the truth, no one knows how long we will be in this life, and I scheduled the surgery without the doctor's recommendation.
I'm having the surgery in 10 days.

My experience, most strain and pain falls onto the lumbar facets & SI joints with "end stage osteoarthritis, 0 rotation, hip. Facet joint peripheral nerve oblations ended nagging and debilitating pain. Replacement would appear in order, to fix leg lengths, and increase ROM, but hospital group BMI restrictions with surgeons delay 2020 date along with pandemic, so counter productive.
Right hip has a acute tendinitis, more painful that "shot" hip. Go figure.

I've got a labral tear (anterior), Bertolloti's syndrome, plantar fasciitiis, and L4/L5 bulging disc against sciatic nerve. I'm doing a lot of hip strengthening exercises with resistance bands, using OOFOS shoes around the house, and superfeet inserts in my shoes. It's been almost 9 months, but after doing some of the above for the last month I feel like I'm getting stronger in my hips (I wasn't able to jump for the first 6-7 months without pain in my left side). I also have seen a chiropractor for traction (to move the disc back in place).
My groin is still pretty tight just standing/walking around the house (labral tear). I'm watching some of your exercises.
What I'm finding is I need to move, I can't just sit here. I put on my oculus quest 2 VR headset and started playing games. Something so simple as trying to putt in a golf game hurts my hip. I hit it with a Theragun, because I'm getting tired of pain, and it seems to be improving (the SI joint area). I'm starting to think the labral tear caused some internal scarring in my hip/back maybe and the theragun is breaking up internal scar tissue - is that possible? Just leaning to the right usually hurts in my back left hip area, some of your exercises seem to be helping too. I miss not thinking about how to move and just doing turnaround jumpers and yelling Kobe. I'm trying very hard to stop muscle guarding.

I do advise you to try the MELT Method. It will be highly beneficial for your entire body, no matter how you proceed eventually with your hip issues. MELT and specific stretches (start gently while inflammation is still active) have largely contributed to making me pain -free. I have end stage hip osteoarthritis. MELT hydrates your fascia, fascilitating nutrition to those contracted muscles. Sue Hitzman, the creator of MELT, has two great books out. I use her on demand channel. There are some videos here on UA-cam. You'll need a SOFT roller. Either her's (expensive) or, if in Europe, the Blue Pilates Roller by Sissel is wonderful. All the best!

Great video with really important points to think about. I do think that there's a time and a place for surgery, but I think that addressing hip function has to come first. In this case, since function was so limited, I absolutely think that she needed to get the hip fixed and I'm glad she was so proactive about it even with the surgeons balking, because that lack of function would just lead to more and more damage, both of the hip and the rest of the body. However, most people will be able to improve function without surgery, and that needs to come first. You need the correct function of the muscles to restore the hip. I personally would argue that some people may need to address the structure of the hip as well through surgery (even arthroscopy, if it's with the right surgeon), but fixing the function is enough for so many that it has to start there. Besides, even if someone was a case of needing to address the structure, they will only get the optimal results if they ALSO address the function.

We were playing pickleball yesterday and she got a very sharp pain in her hip and couldn’t really walk. Tremendous pain when she put any weight on it. I’m sure hip replacement surgery will come up in the discussion with her doctor, but I’m hoping she can avoid it. I think any surgery should be avoided if possible because it is invasive and always carries dangers. People I know who have had hip problems speak of indescribable pain where you can't sit, lie down or get relief in any position. In these cases surgery is amazing and really does transform people's lives. Hip surgery seems to be one of the more successful types. If there's a non invasive solution evidently that is wholly preferable, otherwise thank goodness for the hip replacement option.

My daughter 7 got septic arthritis and osteomylitis in February. She was in unbearable pain. Doctors prescribed her antibiotics and she took them for 3 and a half months. Last month she got an xray done in which her left femoral head was collapsed completely but she hasnt been complaining of any pain. She is also walking with a slight limp and I feel she has got half inch leg discripancy. The orthopedic surgeon told us that she is asymptomatic and we cannot do anything for now till she turns 14 and the bone will remodel in that time. Then they would decide what to do. The doctor has advised us to get her into swimming and no extreme sports or other sports which involves giving pressure on her weight bearing bones. I am still confused what to do and how to go about it. A friend of mine also advised me to get her acupuncture treatment which can be helpful. Kindly advice me what should I do. Fascinating video. I do FAI Fix - and will be attending the Healthy Hips Clinic this month. I am excited and nervous about it. Been diagnosed with arthritis, FAI, cyst. Saw two surgeons to get the xray and the second to get another opinion. The first one said I was a candidate for surgery but I should wait until I am in excruciating pain. The second opinion confirmed the first diagnosis and said I should get surgery. I have put off surgery, and I have seen improvement with the exercises and sweat equity. I felt like I was making progress, and starting hiking again. I would be sore after hiking 5-6 miles --I live in Tucson-- and the mountain trails are rough and uneven. However, I would recover pretty quickly which to me is a good sign. But then I had the big set back - I slipped on some wet rocks while hiking, landed on my bad hip and now I have the sharp pain in groin again and sore adductors, and knee pain. I started thinking about surgery, but then Matt I saw your video today and its something to chew on. Not sure what to think, especially since it seems like every other person I meet seems to have had hip surgery and have said they regretted waiting so long to get it. When I tell folks I am using non-surgical options to deal with my hip, they look at me with pity. There is a band-wagon effect that should not be discounted especially when you live in an area where a significant percentage of folk are baby boomers!

One problem i want to stop taking is pain meds, and inflammatory meds. i really like your information. If anyone have any ideals that have worked for them please let me know for walking without a limp and pain thank you. Yes the doctors do tell you if you have no pain they don't recommend surgery my luck i have pain not the worst, but not the best either at times. Also try foot insert's it helps to reline the body i went to the good feet store. My doctor told me get surgery but when i told her about the foot store she said "Well you will be using those forever" i don't care its better then surgery. i saw what it did to my mom and brother knees.

cause of hip pain can be a most important asset When we know what can cause a messed up furmer , like the one showed and I have that messes up bone as well...I think its acid that eats away at the bones....I was on a paleo diet ,and that was meat twice a day..and another meat diet...My acid from food is very bad..40 mg of a pill for the acid removal..I don,t do much meat these days, tho I am left with taking a pill each day.a study on cause for eaten away bones.so this surgery can stop!!!

I think the surgeons mentioned in this video are probably in the minority (not wanting to perform replacement surgery due to lack of pain).

I'm surprised you were told not to move your hip joint because you had arthritis. My understanding is that exercising the arthritic joint is good for osteoarthritis.

I am 74 years old and in constant pain. Hips and knees are bone on bone. Severe arthritis. I can barely walk any more. Dr said I am not a good candidate for surgery. Can’t take nsaids because of kidney disease, no cortisone because of diabetes. Life sucks.

It hurts to move but moving makes it hurt less. I got an Alinker, it has never been out side but I walk in my living room for 30 min at a time. I am moving more, able to do exercises and am having pain free moments. It seems crazy that you have to hurt to be pain free.

I have the same problem, and my doctor gave the same reasons. And I've had the avn since 2008, they couldn't diagnose it well during that time until 2014. Even then they told me no surgery and adviced I should get leg shoe sole raises to try and correct the leg length.
