I personally have given b-cell depleters without steroids if the infusion has been well tolerated. Generally, the risk of infusion reaction is reduced if it is not the first treatment. The reason for this is reactions are usually due to b-cell depletion itself (causing cytokine release) rather than allergy. Of course, this does not always work out and and people have a reaction and need steroids during the infusion.

Dr BRandon Beaber! This makes me super happy to have another MS master-specialist, as you and Boster my friend , are not normal MS Specialists, commenting, with respect for the other. I would give anything to have you and he on my MS team, but, having Boster at my fingertips, texting, calling, commenting, works great
You can imagine that being introduced to this man in person for the first time 8 years ago, changed my and my family’s world. Let alone after 18 years of nothing slowing my MS down, and being in a wheelchair, doing Lemtrada 7 years ago becasue od Boster, and me having had no new damage whatsoever. Doctors like you guys, are everything to the MS world

Hey Aaron, thanks for the presentation for this weekend's MSVN. My husband and I both listened and, as always, we are grateful for the information you provide in an understandable and respectful fashion.

Hello Aaron, I really appreciate your videos and I often watch them. I have been waiting for 5 months now to be seen by a neurologist here in the UK. The waiting lists are long and - quite frankly - dangerous. I wish I could see you in your clinic in America, but .... such is life. My GP and I have a suspicion that I have MS. I certainly have many of the symptoms and a few other symptoms I am/we are not able to assign to anything yet. I have been having these symptoms since last year's autumn, but I started to recognise something is truly off early summer this year. Also because nothing truly clears up or it takes ages to do so. So .... until a neurologist can see me here, I am keeping myself informed with your excellent videos. Thank you for taking the time to educate people. A very very important thing to do. Also, if you have urges to migrate, I am sure you will find a good position here in the UK as a neurologist (try Scotland, Aberdeen, yes? :) ).

Hello from N.Z.. I missed the live again. After a very long journey I was diagnosed at age 56,with SPMS. Due to "late" diagnosis and stage of progression, I do not qualify for medication. I would be grateful for any advice on how to best help myself for the future. I live a healthy lifestyle and although my cognitive physical ability is declining, I do walk and exercise. I was diagnosed with Focal seizures and suffer from Trigeminal neuralgia and take medication for these. I appreciate your informative videos. Thank you

Hi thank you for your videos. My husband is going to start seeing a neurologist as he was diagnosed with MS 5 yrs ago and has lost all his mobility due to balance issues. He did Ocrevus 4 yrs ago with no change and then we moved and haven’t seen Dr since.

I feel great after receiving my steroids before Ocrevus. No more brain fog, no more tiredness, no more heavy body for 2 or 3 days but the withdrawal is nasti. It comes like an avalanche and I'am out of use for 3 to 5 days.

QUESTION Dr Boster! Is there ever a time when you’d hold off on a DMT when your patient has 9 lesions on brain, none on spine and positive LP markers with 11 ogliclonal bands? Also add in numerous symptoms (tremors, weakness in legs and arms, tinnitus, vertigo, cog fog and a few more).

Great video, as always! 5 year Tysabri user here. I was wondering why, in the absence of new enhancing lesions, I’m still seeing progression of disability? Is it because Tysabri mainly protects the CNS and not so much the peripheral nerves?

      What about Smoldering MS?

    • @lemonpeelangelfish
      @lemonpeelangelfish 7 місяців тому +4

MS doesn’t affect the peripheral nerves. Although DMTs can be extremely effective at stopping relapses and new lesions on MRI they can’t yet stop progression- smouldering MS. Dr Boster has a video called “Why am I worse?” It explains it brilliantly.

Plasmapheresis is done for severe attacks. What does "very severe attacks" entail? Could you give examples?

Hello Aaron ! I'm from France and I really appreciate all the content you do ! You said few years ago that you've worked on different remyelination trials and thought we'll have a molecule to repair in less than 9 years ! I want to candidate to trials and be part of research to help the condition and myself obviously. So : Which study would you recommend I take part in? THANKS A LOT :)

Hey Dr B, I'm about to go in and get another MRI and say goodbye to my job. It's getting harder to walk and I'm getting worse. As a truck driver I've tried to keep going without DMT'S and now I can't anymore.

Hey Dr B how's it going? Long time no talk. I really wish you were the doctor that seen me when I was rushed to the hospital after having my first MS attack. Maybe things would be different right now.
I just had a quick question for you since it's been a long time. I tested JC positive last year, so I couldn't take Tysabri anymore. Is there any chance that in the future I'll test negative and be able to go back on it?

    QUESTION
Good afternoon. I'm reading the book breath. I was interested in the Tibetan method of breathing Tummo, an analog made by Wim Hof, to improve the immune system. Tell me, are there any contraindications for remitting MS?

Hello Dr. Boster, I was diagnosed with fibromyalgia and have repeatedly been told its not progressive. I've gone downhill so much over the years. I get strange things. I burnt my arm recently doing a hot water bottle. I had the arm under cold water, but it turned our the burn was lower down. I get leg pains and itches I have to find too. Why is this?? Thanks.

I would like to know, what exactly are the ms lesions in the brain and spine made up of? I've asked this question on google and I get nothing, more so of what the lesions do, but not their actual physical make up.
They have to be made out of something, it doesn't just appear, it is a physical present thing, so it has to be made out of something.
So my question is as above, what are the lesions in ms made of? Their physical make up, the material etc.

I would like to hear your opinion on symptoms that could be or not be ms. I've been off work for almost a month now, and so far, no one knows what's wrong. I have multiple symptoms that could be "ms", or stroke, etc.

Hello Dr Bostor, not sure if this is the correct place to ask but have you seen any success with dietary interventions on reducing relapses? Im trying cycling ketosis as I noticed I wouldnt get any relapses while doing the keto diet to lose weight. Could be coincidence but im going to experiment with it and I was curious about your experiences

Plasma exchange through neck IV for severe attacks. Never heard of it. Is it filtering out the auto antibodies because they are actively causing damage during the ongoing attack? Or is it just to reduce inflammation like the steroids?
