  • Learning points:
    The distribution of metastases is determined by the locations of stem-cell niches and BM stromal cells including regulated fat cells
    The balance between osteoblastic and osteolytic mechanisms determines the nature of the metastases observed on x-ray methods
    Combining appearances on MRI with CT enables an expanded classification of bone metastases according to their underlying cellularity & matrix appearance
    The BM cell signalling status of bone metastases can be inferred from MRI/CT/bone scan appearances

    One Question: 🤔
    If cancer cells first manipulate the osteoclasts and the progenitors of fibroblasts are the same as osteoblasts, is it correct to say that the early phase of marrow involvement for metastatic prostate cancer would be like this:
    PSMA-PET: +
    Bone scan: -
    FDG-PET: + (osteoclastic phase, if agressive phenotype and high GG)
    FAPI-PET: - (my main question)
    WB-MRI: -/+ (not hypercellular yet, similar to osteoclastic-only activity in the fat-predominant phenotype)
    CT: mildly hypodense, yet not lytic

    If a ct scan reveals osteolytic and osteoblastic lesions along vertebral areas, is this a indicating mets?

      Yes. If there's a history of Cancer
