    Last episode was only 4 hours, and that’s just unacceptable. How are you going to sleep for only 4 hours?
    So, this episode is now LONGER and MORE BORING (YOU’RE WELCOME!) - still not enough for a full night’s sleep, but we’ll get there (need to get a larger storage). Maybe staple this episode with the last for now. I apologize for how brief this episode is.

    "If you make a claim, you should probably show evidence for it". Completely agreed. A bunch of people, especially on reddit, really don't like it, because they'd actually have to do the work to find proof for the claims of their favorite influencer, and these very commonly don't share any evidence whatsoever. People shouldn't be going around making claims without responsibility, and in my opinion, the bigger the claim, the higher the necessity for providing evidence.

    I finally finished this episode. TBH, this format is my favorite!
    I subscribed to your channel after finding your video on the metabolic theory of cancer, and I've been a regular viewer ever since. I'm still waiting for a follow-up, and I really like the idea of a two-part interview with Dr. Seyfried. For me, this topic is the one I most want to find out the answer to: cancer is the second leading cause of death worldwide, so stimulating a major breakthrough in this area could save hundreds of millions of lives.
    Having a scientific background, I've tried to read studies on the topic, but I just don't have enough time. I'm hoping that one day you will cover the topic more thoroughly, one way or another.

    re Urolithin A, I am a 66yo male and since adding pommegranates to my breakfast I have reached a VO2 of 54ml/kg/min which at 99.5% for my peers. I also take Ubiquinol, PQQ, niacin (NA), however I associate the addition of the fruit to the rise in VO2. I am a 5 day/week runner, my RHR is 45 while my max HR is 193.

      Holee! 193 max? Impressive! I am 72 but my internist (on board to figure out occasional inappropriate sinus tachycardia) says my biological age is early 50s. I just can’t begin to imagine having such a low RHR - unless you have brachycardia - let alone a mix of 193. Well done you!

    I'm curious about the following and would be great to get your input. There are multiple valid sources for each.
    - methylene blue, which you did do a short overview of, but nothing since. Considering its benefits, how come it isn't part of your regimen?
    - red light therapy looks freakin amazing. I'm only just researching this now. Originally I heard of it when going down the methylene blue road (used in combination to increase the effects), this has a huge amount of research to boast benefits to mitochondrial health. Can you please provide your input?
    - boron and borax has some phenomenal research to support bone, joint and general health benefits. It also binds with SHBG resulting in higher free testosterone. It is also associated with lower incidence of prostate cancer and lots of other good things. I would've thought this unsung hero would be of interest to you.
    - Just a question on urolithin-a, because it's so damn expensive... can it be produced by fermenting ellegic acid in kefir? I'm curious if there's an easier way to get it into our bodies.

    Nice work as always. I always listen to everything you say around creatine and the kidneys. It would be cool if you could talk about testing for albumin in urine or specificity the ACR. When should people get that test? What does it measure? And what could cause transient elevations?

    One of my seniors in my lab was working on neuronal ramifications of urolithin A. She was able to prove significant anti-epileptogenic effects exhibited by Urolithin A in Drosophila larvae as well as in adults! It was cool.

    Ugh… I had a high creatinine test once that put my eGFR in CKD 3 territory. PCP knew I was muscular and taking creatine but sent me to the nephrologist anyway. Weeks of anxiety and collecting my urine in a jug for 24 hours later and there was absolutely nothing wrong with my kidneys. Wish I had known about these other tests back then so I could have pushed back and saved a lot of money, time, and anxiety. I did get a new doctor after that episode and several other needless CYA referrals.

    Great stuff as always. I have LPa in the 250 range so keep an eye on this stuff. My cardioligist first took that test almost 10 years ago. He and his son both have high LPa as well. About 4 years ago he said he THOUGHT maybe it was just a marker and was unsure if lowering it would change anything. Your discussion makes me think it may be more than a marker. The new LPa lowering tests will be interesting to watch. Oh..Niacin got it down to around 219 so not even sure that's significant. Starting Repatha now..we'll see if that effects it at all

    Hey man! Fyi, when I was coming up in the 90s, there'd been a recent study at that point that'd apparently actually found hyperplasia in the delts of collegiate swimmers... Thought there would've been a body of corroborating research on that, by now - ??! Wtf... 😮

    I understand Atherosclerosis is very complex I am in the middle of trying to somewhat regress it. If its is interesting to you perhaps you could do a deep dive on Reverse Cholesterol Transport, the substances that support it and its effects on regression of plaque?     You said one can not have an epigenetic age that is 20 years younger.than the chronological age, but on the rejuvenation Olympics leader board (absolute leaderboard) i see Dan Sullivan on number one with a chronological age of 78. If my calculation is right his epigenetic age is 78*0.71=55.38. So the difference is 78 - 55.38 = 22.62 years.

    EGFR also goes up with age. As an very active 77 year old who takes creatine, my EGFR is now always too low, even though I am stronger and more fit than 95% of all men. It apparently is assumed, erroneously, that all men my age will have reduced muscle mass. My creatinine levels have come in at 1.2 for about 2 decades, but that level was considered a problem only when my age kept going up [have not found any way to change that!]

    Would you ever consider doing an ep on Nephrocalcinosis? I recently got a diagnosis and there’s VERY little info on dos and don’t with this…ie- can I take creatine? How much oxalate should I take in? Etc…

    At the ~30 min mark (more or less) Nick reads a chat comment from a 78-year-old who mentioned he was on a "HFPB diet". Most likely, the commenter was referring to a 'whole food plant-based' diet but he simply typed the abbreviation using the phonetic version of the first word.

    Glp -1 leaves the intestinal cell after being stimulated by not food in general but sugar. Then glp-1 travels through the interstitial fluid to the lymphatic sensory neurons where it stimulates the glp-1 receptor. The glp-1 receptor stimulates TRPV1 and TRPA1 on the lymphatic sensory neuron that transduces the electrical signal through the peripheral and central nervous systems. You can read this in the 2020 Berkeley study.

    Many claim UA-cam algorithms are manipulated, this makes sense. Some may quit if not making enough money. Every now and then they get a HUGE view rate (artificial?).
    Then, popular channels will get a dismal view rate. It kinda eqauls out over time.

    Feldman's initial motivation was to deal with his prediabetic state. HbA1c of 6.1. that can be done by diet without resorting to high fat keto and avoid the risk associated with very high LDL. He exchanged one risk for another.

    Great video. I do have a question on the "5 factors that Extend life" section...did the data indicate which of the factors had the biggest effect?
    Sure, they're all bad, I'm just curious as to which of the factors has the strongest negative effect. Is a physically active smoker better off than a sedentary nonsmoker?

    On the obesity/autophagy - I have wondered whether this explains why a large study showed decreased risk with AZ or dementia with mild obesity. I seem to remember there is an IGF1 pathway which triggers autophagy which I thought may have explained it.

    Something that I need clarification on is what heart disease actually is.
    Is it the calcification of the valves? Is it the weakening of the muscle walls? Seems there is a generality covering this area that causes concern, so your clarification would be welcomed.
    Last question I have - if all heart attacks are caused by blood clots, how does cholestrol /LDL/VLDL etc play into creating blood clots?

    About the Reversing Atherosclerosis section, with the extended discussion of the need for multivariate correlation analysis... that seems to involve what the famous Dr. Ioannidis, in "Why Most Published Research Findings Are False", derisively called a "hypothesis-generating experiment." That is, the power of the test, which you Physionic did properly bring up, is the probability of avoiding a false negative finding (a finding of no association between an investigated factor and the outcome when in fact there really is one). But there is also the probability of producing a false positive (a type I error). And if you have an analysis that casts about widely in search of relevant factors, the probability of a type I error is increased--- greatly, generally in proportion to the number of factors under consideration (especially if the factors are independent of one another), to the extent that even a seemingly large number of subjects in each factor-defined category would not suffice. If you have a large bag of coins and are searching through them for one that is unfair, that comes up heads more often then tails when tossed, and if to test you toss the coin only ten times, then one out of every eighty-something coins that you toss will produce nine or ten heads *even if all of the coins are fair*... which would be a gross type I error. So yes, if instead of ten tosses of each you do 100 each, that will greatly reduce the probability of error, but... if you sift through a big enough bag of coins (a large enough number of categories into which subjects are sorted) you will still be able to find a fair coin that comes up heads 90+% of the time. See "The Control of the False Discovery Rate in Multiple Testing Under Dependency" by Yoav Benjamini and Daniel Yekutieli for a procedure that reduces the probability of a type I error in these circumstances (which has the unfortunate side effect of reducing the power of the test).

    I also appreciate your section on muscle building at age 80. I'm 62 trying to help someone who is 83. I'm sad because he started out strong but is slowing down on his efforts to workout. I can see him slowing down both physically and mentally.
    Also we're both homeless, and i can see people taking advantage of him because he's going down hill. People are using him because of his monthly income. He gets mad or brushes me off when I point it out.
    At least i have your videos to try to help me explain things to him, when he does listen. I do get him on the treadmill at least once or twice a week. ✌️😊
