    Causes of Hip Pain
    Pain can arise from the structures that are within the hip joint or from the structures surrounding the hip joint. The most important thing is to ask the patient to locate the site of pain. ask the patient to point at the site of pain. When the patient states that their hip hurts, it doesn’t mean that the pain is coming from the hip joint itself, so ask the patient to point at the site of the pain. The pain can arise from structures that are within the hip joint or from structures surrounding the hip. The hip joint is a weight bearing joint. The joint consists of two main parts: femoral head (ball) and acetabulum (socket). The hip pain can be anterior hip pain (deep groin pain). The pain can be lateral hip pain. the pain can be posterior hip pain. The pain can be far posterior hip pain, coming from the sacroiliac joint and the lower spine. Anterior hip pain is usually deep within the groin, and it can result due to arthritis of the hip. Conservative treatment is physical therapy, anti-inflammatory medication, possible injections, and surgery is done in late cases, usually by total hip replacement. It is usually diagnosed by clinical examination with a provocative test of flexion, adduction, and internal rotation. The diagnosis is confirmed by an MRI arthrogram. Conservative treatment is therapy, anti-inflammatory medication, and injections. Surgical treatment provides good result and is usually done by arthroscopic debridement or repair of the tear. Stress fracture is usually diagnosed by an MRI. The x-ray may be normal. Early diagnosis is important before the fracture displaces and gives a bad result. Treatment of avascular necrosis is usually surgical fixation of the fracture. Fixation of the fracture is usually performed utilizing screws. Femoral head replacement is done in rare, late cases. Avascular necrosis means death of a segment of the bone. When the blood supply of the femoral head is interrupted, a segment of the bone dies and becomes necrotic (femoral head will collapse). Treatment for early stages of AVN without collapse of the femoral head include decompression by drilling of this segment in the femoral head to bring a new blood supply to the area. Vascularized fibular graft may be used also. In severe cases with collapse of the femoral head (usually diagnosed by an x-ray), the treatment is usually total hp replacement. Treatment of an inflamed bursa is usually conservative treatment of physical therapy, anti-inflammatory medication, and injection. Surgical treatment by excision of the bursa is rarely done. In case of chronic, resilient trochanteric bursitis, try to get an MRI to exclude a tear of the abductor muscles of the hip (gluteus medius and gluteus minimus muscle tear). Posterior hip pain is usually due to piriformis syndrome. The sciatic nerve can be irritated from the piriformis syndrome. Treatment is usually physical therapy, stretching, anti-inflammatory medications, and injections. Surgical treatment is usually rare. It is the last resort. It includes release of the piriformis tendon and exploration of the sciatic nerve. It is done in cases that fail to improve with conservative treatment. Far posterior pain may come from the sacroiliac joint or from the lower spine conditions. Sacroiliac joint (SI) problems is a challenging diagnostic and treatment entity. There are a lot of clinical diagnostic examinations that can be used to diagnose sacroiliac joint (SI) problems such as the Faber test and others. However, injection of the SI joint is probably the method to diagnose pain originating from the SI joint. If there is improvement of the condition of the patient after injection of the SI Joint, then we will probably consider that the problem is in the SI joint. The SI joint problems are usually under estimated and are unappreciated. Lower spine conditions can cause referred pain to the buttock and hip area. In fact, symptoms of hip and lower spine conditions can overlap or both of them can coexist in the same patient. You have to separate pain from the hip from pain that comes from the spine.

    Compared To Most,, I Guess I'm The Exception..I Had Both Hips Replaced & Have Been In Severe Pain 24/7 For 11+ Years Now..After MANY Tests,, Surgeons,, Specialists,, Etc.,, Their Conclusion Is That Not Only Can Nothing Be Done For Me,, But That I Will Only Continue To Get Worse & I Have..At This Very Moment I Am Lying Here In Absolute Agony..The Past 4 Days I Have Been Dramatically Worse,, So Much So That It's Too Painful To Stand,, Let Alone To Try & Walk..At This Point I Can't Even Take Care Of Myself..I TRULY PRAY That ABSOLUTELY NO ONE Else EVER Has To Experience What I Am.

      I am very sorry you are suffering so bad. Have you been told that it is possible for another surgery to replace your Hip or both?

      I'm so very sorry to hear that you are experiencing so much pain . I have hip pain runs down my thigh and stenosis and numbness and nerve pain, burning from stomach all the way to feet. So I understand. Some years back I had such severe sciatica running down both legs that I could hardly get to bed and took me 10 minutes at least to get in the bed. The pain can get so severe to make one cry out. I didn't know about ice then so I took a prednisone pack Doctor ordered. Got me on my feet pretty quick but no one told me to take it with good meal. I've had a burning stomach ever since. Please try ice. 20 minutes at a time every where you have pain. For sciatica I use ice on lower spine. Ice really helps. Find a good natural cream to rub where you hurt. I like Topricin cream. Odorless all natural. Really helps.

    This video does not mention one of the most common cause of anterior hip pain, especially in younger, more active individuals: hip flexor tightness or injury. The Iliopsoas muscles, the Iliacus attached to interior of the iliac bone (big pelvic bone) and the psoas muscle attached to the lower spine. These muscles run together anterior to the hip joint, they wrap around the femoral neck and attach posteriorly in the lesser trochanter. Anatomy jargon aside, if a person sits a lot, or suddenly starts working their legs out, they may get tightness and pain in the anterior hip area. The pain my be very localized to one spot, and it might be alleviated by pushing on the painful spot. Treatment includes rest, NSAIDS, stretching or standing more if you sit a lot at your job.
    I hate to sound pedantic, but if you're going to have a video called "Everything" You Need To Know, and then leave out one of the most common causes of hip pain, describing it only once as "surrounding structures," I thought I might mention it.

      can you define hip flexor tightness? also the video seems to be referring to more chronic conditions rather than "hip flexor tightness" and acute injuries, which I believe you are referring to muscle strain. Basically both have no place in this video

      @@blaschito1 Well, if you sit at work everyday, or continue to work out through pain, these conditions can become chronic, or even permanent, and if you don't know the cause, you can't get rid of it, can you?       Hello, I hope you can shed some light on my very painful knee. It started over 3 years ago and manifested a few days after a total hip replacement. I have done a lot of ballet in my life and it has been suggested to me that the hip was replaced "at the incorrect angle". It is literally excruciating to walk using that leg. Referring to a doctor, he maintained that the only solution was to have yet another full hip replacement!
      I have been doing physical therapy exercises the entire time and walking as much as possible but the pain in my knee remains as bad as ever and I am becoming desperate that I will never be pain-free again.
      Any positive suggestions are welcome...thank you

    Thank you, Doctor for posting this video. Today, I was walking and all of a sudden I got this excruciating pain in my groin area. I had no idea what it was but I figured it was probably my hip as I’ve had several back surgeries and fusions. I figure that’s like putting a Vice grip in your lower back. That’s got to cause problems down the road, and here we are, and another surgery, I’m sure.
    You have helped me because instead of sitting here worrying what it might be, I have a couple of things that I can figure are possibilities.

      I have inflammation etc in the si joints and have had good (temp) relief from corticosteroid injections over the past 30+ years. They usually last 3-4 months x

    I had a hip pain for a month, i was exercising and prying. One morning I woke up like pain has never been there. I told it to everyone I met that day. I still don’t know how that terrible pain disappeared, like it has never been there…

      Mine comes and goes too. I had an xray and there is deterioration in both hips. You should still see your physician to get a proper diagnosis.

      What kind of exercises? I have pain near my hip. I sometimes feel it at the side of my knee, and side calf

      @@dinop7488 I didn’t have a specific exercise, but I would turn repetitive in different directions in the area of the painful spot, like 100 times until pain is gone , especially when I am in the bed. You will have at least temporary relief. Mine was an inflammation and didn’t let me to fall sleep. Good Luck!

    A few years ago I started to suffer a lot of pain deep in my hip joint. My family doctor could not diagnose it and sent me to a specialist. The specialist found nothing. I was seeing a chiropractor at the time for some minor back issues. I told him about the deep hip pain, he flexed my hip, watched me walk and asked a few questions. The chiropractor then suggested I had partial claudication in the femoral artery that "starved" my hip for blood, even though the rest of my leg was doing "OK". I passed this to my GP who set an appointment with a specialist who determined the location of the claudication. I had an outpatient procedure to have a stent inserted in the femoral artery. Even as I walked out of the hospital that day, I could feel the difference. I am now pain free and no limitation to using my leg.
    UNFORTUNATELY, THIS DOCTOR DID NOT COVER CLAUDICAION WHERE IT ONLY REALLY AFFECTED THE HIP.

    Thank you for the very explanatory video. I’ve been suffering from severe hip pain for a month finally will be seen in 3 more days by a orthopedic doctor. Another 3 weeks to see physical therapy. I only mild pain medication. I’m so disappointed in my health care. I hope it is not serious.😢

      Hope your problem can be and is fixed, sad to say but I’m in pain in the hips all the time, unfortunately I can’t get an answer from multiple GPS, at least I know I’m alive, that’s how I deal with it.

    That was SO helpful, thank you, Doctor. For years my degen. disc disease and Con. Tissue Disease were thought to be the cause of my hip pain, but I persued it and eventually tests confirmed problems in the hip itself for 20+ years! New pain is awaiting exam.

      They’ve discovered I have a congenital malformation of the hip socket. It’s the wrong shape and is rubbing against the neck of femur when I move. The ligaments are too flexible and there is too much ‘play’ in the joint itself. No bony disease, but outer trochanteric bursitis is a devil!! 😫 a steroid injection now and again is all they can do. Xx

    I presented with deep groin pain. I have been diagnosed with bilateral hip displasia but that is not mentioned at all here. Please consider including it in further updates, it is so commonly missed and takes and expended period of persistent asking for testing and referrals from patients along with many incorrect diagnosis to get to this.

    This video makes a whole lot of sense my right hip has been kicking my ass all day and night, the pain came out of nowhere and hasn't let up. I tried muscle rubs of all kinds and no luck. Never thought that I would be dealing with something like this. Hip replacement runs in my family.

      Ice really helps, I do 20 - 30 minutes. If its very cold no more than 20 minutes. every day use ice packs. My hip has been painful lately and runs down thigh to knee . Over exertion. I also use topricin cream. Company may still send free samples . they use to . Its very good all natural and has no smell. Can feel the difference in 20- 30 minutes.
