That TSR was not particularly well run (something of an understatement) has been pretty well documented. Ben Riggs' book "Slaying the Dragon" gets into the nitty gritty. EDIT: And while we may never know what really happened, I think those caricatures are probably a pretty good hint.
Oh, I know the issues at TSR have been chronicled by both those internally and externally. I just find it fun to read through the tea leaves to see some of the issues in the work they released. I'd love to do a video at some point about how things really collapsed in the mid-80s, leading to the Gygax ouster and whatnot.
