ALL these things are supposed to be -- and are -- handled at the local and state level. All the Federal government does by involving itself in these local activities is add another layer of bureaucrats to be paid.
Because what is paid into Medicare is not enough to cover the costs. How about the growing roles of medicaid. Those recipients get medical coverage for being above room temperature. And if you qualify for that, you qualify for free food and other handouts too. A nation of lazy freeloaders.

The problem with Social Security is that it didn't change as the life expectancy got longer. In 1935, when Social Security started, the life expectancy was 61, but Social Security didn't kick in until age 65. Most people were on Social Security for a short time. If Social Security continued to kick in 4 years later than the life expectancy, then it would kick in around age 82 in 2018. How long would seniors be using Social Security? Not too long. I have long believed we need to slowly move the Social Security ages later and later. Perhaps every 5 years move it 1 year higher. It will take many years, but eventually we'll restore it to its original intention, keep it solvent, and bring the debt level down.
