Today I learned that I don't have many whiteheads/blackheads at all, I have SEBACEOUS FILAMENTS. Small revolution in my thinking and approach. Thank you for the ongoing skincare education!
My dad had tons of sebaceous filaments and the oiliest of skin with no history of acne just tons of oil. And so do I. He passed away at 95 and didn’t look a day over 80. The oil kept him young. I am 40 and people tell me I look 30 at the most. I credit his genes, oily skin and Retin A! I will take sebaceous filaments over wrinkles and sagging any day!
My Grandmother a very wise and beautiful woman who had a big knowledge of natural cosmetic treatments used always to tell : wrinkles need oil and protection from the sun! ❤️ She died with 97 and looked like 60 without makeup after giving birth to 8 children and a super turbulent life!!! ❤️ At this point a big thanks to this wise beautyful girl 😉✌️ named Dr dray❣️
Moisture and oil are 2 different things. Skin needs moisture for smoothing , but overproducing oil doesn’t typically keep the skin looking any younger.
Well, could be just a coincidence, but I'm a male in my thirties with oily skin and no one believes me at first when I tell them my age. They think I'm 25 yo at max.
@@amygalvin1799 Dryness exacerbates tightness in skin, which leads to stretching and wrinkles over time - think of the tightness you might feel after using an overly stripping cleanser. That dryness and tightness in your skin means that every time you move your face it's pulling at your skin. Now think of the feeling of putting on lotion after this tight feeling - your skin is no longer being stretched every time you make an expression. Oilyness is not the same as hydration, you're right, but being oily is much better for wrinkle prevention than being dry.
@@amygalvin1799 I took part in a skincare trial a few years ago where I couldnt use moisteriser for a few weeks. My skin was tested under a machine once a week where hydration and oil levels were checked. I was told I had the highest levels out of the participants. And that my oil created a barrier which "sealed" in the hydration. I do have very few wrinkles. I'm 58 years old.

I recently realized I was eating foods with little to no fiber and it was effecting my gut health, I now try and eat a good amount of fiber and I feel so much better!! this vid was so helpful

I am especially grateful that you recommend more than just retinol and BHAs: those of us going through fertility treatments and pregnancy/breastfeeding cannot safely use those products. While the reat of my face benefits from oatmeal, and other gentle ingredients, I still get the sebaceous filaments and they drive me mad. Thanks for some alternatives to try.
Unfortunately the Adapelene is not buyable in Germany. I tried the Effaclar Serum- and it's really really good!! The first product in 40 years that helped reducing the sebum production. Thanks for your advise!!
I love the explanation of why salicylic acid works well to treat sebaceous filaments. As a geologist, I want to know why something works, not just the name of a product that works.

Don't squeeze these! I bought a 15x mirror and was horrified and started squeezing these, and now I have those little broken capillaries that don't go away but the filaments come right back.

Have you tried a retinoid? I've had some real results with over the counter. It's helped the sf around my nose and also fine lines and smoothing of the skin.
I have sebaceous filaments and dry skin at the same time, sucks!!! The best cleanser to get rid of filaments (temporarily of course) is the hada labo oil cleanser.
@@chelsy2134 it’s called Hadalabo Gokujun Oil Cleansing. The trick is to use it before wetting your face, circular motion to get rid of the sebaceous filaments. Then rinse it off with water.
Omg! I wish I knew what it was in high school, because I'm prone to hyperpigmentation, mine looked like I had a goatee on my chin😭 It lighten over time but you can still tell. I'm so happy I've found your video!
I'm so frustrated at this point because I've been using Retin-A for years now with no success in reducing my sebaceous filaments and the tiny clogged pores along my cheeks. They cause texture and make my skin look congested. I started incorporating the Paula's Choice BHA into my routine last week and I've not been seeing any difference as yet either... I will give it more time, but I'm hopeless at this point.
I find that using a high-quality pore strip every 5 to 7 days and regularly using 2% BHA keeps these at bay. The quality of the pore strip does matter, because the cheaper ones can dry our too fast and tear off the surface skin, without even getting the filaments out properly. Also, when peeling the strip, start from outer edges from both sides and slowly lift the edges toward the center of your nose and apply an even pull. I use the Nivea strips; they work really well after a shower. Squeezing and tweezing is really not a good idea 😬
Dr Gray, I'm sure you've seen the hundreds of day spa videos about. Exactly what you said don't do, they do. And I think they're left with more scars than to start with

Now I have scars from picking the top and sides of my nose. It’s horrible. So the hyperpigmentation on my face is a lot. To wear makeup I have to cover up a lot.
Honestly im on prescription tretinoin and it's bomb! I feel like it's what works the best. Salicylic acid worked fine atm you applied it but next morning it would be filled in again.

My skin is actually quite dry but in the past year I’ve been having sebaceous filaments on my nose. Would these recommendations still be appropriate for me?
I have always had larger pores and lots of stuff in my pores, when I was a teenager, and before I started this, I've had stuff that was more than just filaments. So I prefer to get the stuff out every week, since I found a gentle way doing it to all of the pores. It does cause a difference in how much of the filament stuff is in the pores at any one time. I gently do my nose and cheeks in large areas with a warm wet towel, after doing an oil cleanse (or not), but making sure not to do it too long at once or it will bruise or even rip the top layer. But I prefer to not have the stuff in my pores. This resets everything. It feels smooth the next day and then for a few days after the filaments are small. It just feels better for my peace of mind. I've tried Salycilic acid and it has not done much except dry me out a lot.
After I wash my face in a hot shower (without soap) and sometimes do the oil cleanse method to make it easier to get the stuff out, I go in front of the mirror and get the washcloth wet with warm water (keep it running to keep rinsing and wetting the washcloth). Then I do a squeezing pressure on large patches at a time (about the size of a dime or penny). I just gently but firmly squeeze repeatedly (about 2 squeezes per second) over a spot and can see the stuff coming out. I don't worry about whether I'm going to make it go back in with the washcloth. And I don't always look at it coming out, to get it done faster. But after I go over the whole nose a few times, I know the stuff is out. I do one side at a time, going from the corner of my nostril and squeezing the whole area of the crease part, then over the nostril and side of my nose, to the front. Then I do the same from the other side, and then do the upper sides and the front. (I do an area of cheek beside my nose, too.) One thing to realize is some won't have stuff in, and if nothing comes out with solid squeezing, to just move on and come back over and try the area again. But if nothing comes out, in my testing, they are usually empty. Remember not to do it too long, or it can bruise and peel skin off. Just put firm pressure in a repeated squeezing motion, about 2 squeezes per second, on an area about the size of a penny or dime. If nothing comes out, move on and then come back again. And oil cleansing with a sonic brush first may help.
I always thought they were blackheads. I have them on my nose and haven't been able to get rid of them. Also have whiteheads mainly on my chin. Will try the SA cleanser and the adapalene.

My cheeks are riddled with sebaceous filaments! 😓 Ever since Dr Dray's previous video on this issue, I started using the La Roche Posay cleanser and moisturiser, applying moisturiser while my skin is still damp, and I think I am seeing an improvement. I do still use a toner, which adds one extra step in between, but once I run out of it I will try to forgo it. Now I really want to try the La Roche Posay serum and adapalene cream, hopefully I can find them here in Europe.
Always thought my nose and small section under my eyes were very small blackheads that'd never went away. Didn't know what sebaceous filaments were until I saw a dermatologist tiktok reviewing a skin-care routine.

It was dry as hell during the accutane but damn it was nice afterwards 😩 low key wish I kept some of the accutane to take low dose now lol. However it was 4-5 years since I stopped and is still acne free so that’s something 👌🏼
I tried the PC BHA and it ruined my face. It did not help with my SF at all. It broke me out really bad and my skin is still trying to heal. I advise people buy the travel size first.

@@beatrix-ec1xf what I know (because I get miliaria pustulosa with sun light and heat) is bacteria can cause some milia to get infected, idk if fungus Can too (maybe because nothing work on getting rid of the pustules for me) but some fungal acne can look like milia, so I suggest you check that out
I have these damn things just on my nose and I’ve always tried the pore strips and they get worse, this video (that I found from a tik tok) is magic. I just ordered the stuff from Amazon. Seriously this was a decades long mystery solved for me lol
@@ineedabeachnow7205 innisfree pore clearing clay face mask, the face shop’s light rice water oil cleanser, and paulas choice 2% BHA Liquid salicylic acid, you can get them all on Amazon :) if you look up the tik tok it shows the order to put them on and the before/after it was crazy i had to get it when I saw that! lol it also said you can find cheaper/different brands for each as long as they’re basically the same thing.

Be careful, don’t use retinol and salicylic acid together! It can be super irritating and damages the skins moisture barrier

But I prefer to not have the stuff in my pores. This resets everything. It feels smooth the next day and then for a few days after the filaments are small. It just feels better for my peace of mind. I've tried Salycilic acid and it has not done much except dry me out a lot.
What do you do with a towel?
After I wash my face in a hot shower (without soap) and sometimes do the oil cleanse method to make it easier to get the stuff out, I go in front of the mirror and get the washcloth wet with warm water (keep it running to keep rinsing and wetting the washcloth). Then I do a squeezing pressure on large patches at a time (about the size of a dime or penny).
I just gently but firmly squeeze repeatedly (about 2 squeezes per second) over a spot and can see the stuff coming out. I don't worry about whether I'm going to make it go back in with the washcloth. And I don't always look at it coming out, to get it done faster. But after I go over the whole nose a few times, I know the stuff is out.
I do one side at a time, going from the corner of my nostril and squeezing the whole area of the crease part, then over the nostril and side of my nose, to the front. Then I do the same from the other side, and then do the upper sides and the front.
(I do an area of cheek beside my nose, too.)
One thing to realize is some won't have stuff in, and if nothing comes out with solid squeezing, to just move on and come back over and try the area again. But if nothing comes out, in my testing, they are usually empty.
Remember not to do it too long, or it can bruise and peel skin off. Just put firm pressure in a repeated squeezing motion, about 2 squeezes per second, on an area about the size of a penny or dime. If nothing comes out, move on and then come back again.
And oil cleansing with a sonic brush first may help.
I always thought they were blackheads. I have them on my nose and haven't been able to get rid of them. Also have whiteheads mainly on my chin. Will try the SA cleanser and the adapalene.
When you say our skin "has to do some additional steps" to retinol to get it to its active state, what exactly does our skin actually have do to the retinol?
Oxidise it to retinaldehyde then oxidise that to retinoic acid/tretinoin or a short simple way to show that. Retinol -> Retinaldehyde -> Retinoic Acid

My cheeks are riddled with sebaceous filaments! 😓 Ever since Dr Dray's previous video on this issue, I started using the La Roche Posay cleanser and moisturiser, applying moisturiser while my skin is still damp, and I think I am seeing an improvement. I do still use a toner, which adds one extra step in between, but once I run out of it I will try to forgo it. Now I really want to try the La Roche Posay serum and adapalene cream, hopefully I can find them here in Europe.

Ikr?? :( I miss the days of smooth non-oily polished skin. Although I did have the side effect of really dry lips and was really relying on lip balms lol
@@kipklop975 haha, yes my skin was perfect 6 months to 2 years after I stopped. It was dry as hell during the accutane but damn it was nice afterwards 😩 low key wish I kept some of the accutane to take low dose now lol. However it was 4-5 years since I stopped and is still acne free so that’s something 👌🏼

What would you suggest if you've been using SA + Differin already, but are still dealing with excess sebum / filaments? Stronger percentages?
I tried the PC BHA and it ruined my face. It did not help with my SF at all. It broke me out really bad and my skin is still trying to heal.
I advise people buy the travel size first.
Dr dray please please if you haven’t already make a video for products for milia around the mouth and chin I’ve had it for about 2 years and it’s still here if I pop it don’t pop it exfoliate or cleanse it won’t go down or go away and they are forming into clusters please please help 😥

@@beatrix-ec1xf what I know (because I get miliaria pustulosa with sun light and heat) is bacteria can cause some milia to get infected, idk if fungus Can too (maybe because nothing work on getting rid of the pustules for me) but some fungal acne can look like milia, so I suggest you check that out
BHA somehow never worked on my skin. But niacinamide is my holy grail with SF and LHA works on me.

oil cleanser, and paulas choice 2% BHA Liquid salicylic acid, you can get them all on Amazon :) if you look up the tik tok it shows the order to put them on and the before/after it was crazy i had to get it when I saw that! lol it also said you can find cheaper/different brands for each as long as they’re basically the same thing.
