  • In this video Dr. Angelish Kumar, a New York-based urology specialist, discusses the following:
    ● A hypertonic pelvic floor occurs when the muscles become too tense and are unable to relax. This can cause UTI-like symptoms. In this case, antibiotics will not work and pelvic floor therapy is required.
    ● In the case of pelvic floor laxity or a prolapse, physical therapy can help strengthen the pelvic floor. Alternatively, devices like pessaries can be used-these are a removable device placed in the vagina to support the weakened and prolapsed walls. Failing these options, surgery may be required.
    ● If a patient with pelvic floor laxity is prone to UTIs, it is recommended that the UTI is treated prior to pessary use as the insertion of a pessary may exacerbate the UTI.
    ● Finding a good pelvic floor therapist is crucial to receiving the appropriate treatment. In some cases, patients are given the wrong exercises for their particular condition-e.g. Strengthening exercises when they require stretching techniques-which exacerbates the issue.
    ● In the case pelvic floor treatment involves dilators or myofascial release (a manual therapy involving internal massage or stretching of the muscles), if a patient is prone to UTIs, Dr. Kumar may recommend preventative antibiotic treatment, similar to that recomended for after sex.

    Dr. Kumar is a board certified Urologist who graduated with honors from The Tufts University School of Medicine and is a member of the prestigious Alpha Omega Alpha Honor Society. She completed her urology training at NYU Langone Medical Center in New York City. In 2017, she was the recipient of the gold star teaching award from the Albert Einstein College of Medicine. In 2020, she founded Women’s Urology New York, a unique practice focused on providing compassionate and comprehensive urologic care exclusively to women. In addition to being a diplomate of the American Board of Urology, Dr. Kumar is a member of the American Urologic Association, Society for Urodynamics and Female Urogenital Reconstruction, and International Urogynecologic Societies.
