Just closed today, spent 30 mins on lunch, clocked back in, waited 60 seconds for the min to change over, clocked out again for my last 15 min break. Just like that, last 45mins out of a total of 53 (we have a seven min rule where you can clock out 7 mins early and still get paid the full hour) spent eating and chit chatting with the boys in the break room. Don’t hesitate to play the system, lord knows they won’t hesitate to play you.

And it's no secret that residency is shit pay, and residents are basically slaves, beholden by their quarter of a million or more in debt to put up with the system until they "unlock" the higher wages of a competent specialist or attending. But given that nurses will make upwards of 60k for 36 hours of work weekly (many of them as new grads), they certainly aren't cheaper per hour if the residents are working 80 for the same pay. It is true that nurses are expanding their scope of practice, but this hardly constitutes "replacing" doctors. The entire point of nurses is to decrease the cost of healthcare and increase the availability, by decreasing the educational needs (and therefore costs) of the average worker. There's no reason doctors can't turn and ambulate their patients, wipe their asses, give meds, place IVs and foleys, monitor telemetry, feed patients, and do pretty much everything else that is the purview of nursing. But it would cost a hell of a lot more to hire all the doctors (with 8+ years of training) necessary to provide that care than it would to hire some nurses (with 2-4 years of training) to do the "lower tasks" for all the patients, and then have the doctor take care of only the tasks that explicitly require their advanced training, but for more patients. It's the same reason we have CNAs for our nurses. A 24 bed advanced care unit could run 8 nurses and their patients would be just fine. They could also run 6 nurses and 2 CNAs; the man hours per patient is the same, but the CNAs will do things like toiletting, turning, vitals, ambulating, etc. - things that only take about 2 weeks of full time training to do - so the nurses can spend more of their time doing things that actually require their 2-4 year degrees to do. Same outcome, but the delegation results in a lower price (as the average education per person in an 8 BSN unit will be 4 years/nursing staff, while in the latter case, it will be just slightly over 3 years/nursing staff. The latter case is far more common, exempting ICUs, where the critical condition of patients demands lower ratios - ideally 1 or 2 per RN - and therefore, leaves little for a CNA to do). For that matter, there's no reason CNAs (or even Docs) can't take out trash and mop the floors - but most facilities will have one housekeeper do this task for multiple units, because it is a lower risk, lower liability task, and can therefore be entrusted to someone with only a few hours of on the job training at a lower hourly rate. Cost and availability is behind a lot of the delegation. That's why PAs and NPs work under the supervision of doctors, why LPNs and CNAs work under RNs. These positions were formally created in wartime conditions, when we badly needed medical workers. Decreasing the educational requirements and delegating tasks accordingly allowed us to provide the same level of care for a lower average level of education - which costs less and allows people to enter the field quicker. Point being, changing your hiring practices to save money and allow for specialization is a reasonable thing to do, and exists on every level in every industry. Furthermore, I guarantee you that nurses are not replacing residents in a 1:1 replacement scheme; it's not the same scope of practice. Sure, nurses are expanding their scope. But everyone is. Today's NPs and PAs can do more than in the past. Today's CNAs/PCTs can do more than yesterday's candy stripers. That's just the way it goes. But nurses are not replacing residents outright. More likely, for every 10 residents you lose, you'll hire something like 1 attending, 4 midlevels and 5 nurses. This is in terms of delegation of work; in terms of cost, each of these positions is more costly than a resident per hour. So economics is not the driving factor here. And no, I'm advocating that companies rip off employees. I just don't think anyone is entitled anything. Anything - a product, a service, an idea, a company, information, education, and even your labor - are worth what someone is willing to pay for it. They have no intrinsic value in terms of dollars and cents. If you aren't being compensated the way you want to be for the work you do, ask for raise. If you don't get it (or don't feel it's worth asking), look for another position that pays better. If you find that you cannot land one of these positions, take good hard look in the mirror and ask yourself whether you really *deserve* the wage increase, or simply *want* it. Maybe you need to work harder or invest some time and/or money into learning a skill, acquiring a certificate, degree, or diploma, and thereby increase your odds of landing one of these positions. It could be true that nurses make up proportionately more hospital staff than attendings, now than at some point in the past. And it's no secret that residency is shit pay, and residents are basically slaves, beholden by their quarter of a million or more in debt to put up with the system until they "unlock" the higher wages of a competent specialist or attending. But given that nurses will make upwards of 60k for 36 hours of work weekly (many of them as new grads), they certainly aren't cheaper per hour if the residents are working 80 for the same pay. It is true that nurses are expanding their scope of practice, but this hardly constitutes "replacing" doctors. The entire point of nurses is to decrease the cost of healthcare and increase the availability, by decreasing the educational needs (and therefore costs) of the average worker. There's no reason doctors can't turn and ambulate their patients, wipe their asses, give meds, place IVs and foleys, monitor telemetry, feed patients, and do pretty much everything else that is the purview of nursing. But it would cost a hell of a lot more to hire all the doctors (with 8+ years of training) necessary to provide that care than it would to hire some nurses (with 2-4 years of training) to do the "lower tasks" for all the patients, and then have the doctor take care of only the tasks that explicitly require their advanced training, but for more patients. It's the same reason we have CNAs for our nurses. A 24 bed advanced care unit could run 8 nurses and their patients would be just fine. They could also run 6 nurses and 2 CNAs; the man hours per patient is the same, but the CNAs will do things like toiletting, turning, vitals, ambulating, etc. - things that only take about 2 weeks of full time training to do - so the nurses can spend more of their time doing things that actually require their 2-4 year degrees to do. Same outcome, but the delegation results in a lower price (as the average education per person in an 8 BSN unit will be 4 years/nursing staff, while in the latter case, it will be just slightly over 3 years/nursing staff. The latter case is far more common, exempting ICUs, where the critical condition of patients demands lower ratios - ideally 1 or 2 per RN - and therefore, leaves little for a CNA to do). For that matter, there's no reason CNAs (or even Docs) can't take out trash and mop the floors - but most facilities will have one housekeeper do this task for multiple units, because it is a lower risk, lower liability task, and can therefore be entrusted to someone with only a few hours of on the job training at a lower hourly rate.
Cost and availability is behind a lot of the delegation. That's why PAs and NPs work under the supervision of doctors, why LPNs and CNAs work under RNs. These positions were formally created in wartime conditions, when we badly needed medical workers. Decreasing the educational requirements and delegating tasks accordingly allowed us to provide the same level of care for a lower average level of education - which costs less and allows people to enter the field quicker.
Point being, changing your hiring practices to save money and allow for specialization is a reasonable thing to do, and exists on every level in every industry. Furthermore, I guarantee you that nurses are not replacing residents in a 1:1 replacement scheme; it's not the same scope of practice. Sure, nurses are expanding their scope. But everyone is. Today's NPs and PAs can do more than in the past. Today's CNAs/PCTs can do more than yesterday's candy stripers. That's just the way it goes. But nurses are not replacing residents outright. More likely, for every 10 residents you lose, you'll hire something like 1 attending, 4 midlevels and 5 nurses. This is in terms of delegation of work; in terms of cost, each of these positions is more costly than a resident per hour. So economics is not the driving factor here.
And no, I'm advocating that companies rip off employees. I just don't think anyone is entitled anything. Anything - a product, a service, an idea, a company, information, education, and even your labor - are worth what someone is willing to pay for it. They have no intrinsic value in terms of dollars and cents. If you aren't being compensated the way you want to be for the work you do, ask for raise. If you don't get it (or don't feel it's worth asking), look for another position that pays better. If you find that you cannot land one of these positions, take good hard look in the mirror and ask yourself whether you really *deserve* the wage increase, or simply *want* it. Maybe you need to work harder or invest some time and/or money into learning a skill, acquiring a certificate, degree, or diploma, and thereby increase your odds of landing one of these positions. If you are utterly unable to find a position paying what you think you deserve, it may be that you are less of an asset than you think you are.
Of course, the cynic will think of it as jumping through hoops for an employer. The cynic will therefore place all of the blame for his undercompensation (real or perceived) on his employer, stubbornly refuse to do anything to improve his situation or wages, and always resent his job, never deriving any pleasure from it.
If a person wants to make that choice, so be it. I advise against it. It is, generally speaking, a sad and unproductive way to live. Before you throw up your hands in frustration, consider whether there is anything you can do to improve your situation. Getting an education, learning a new skill, asking for a raise, or looking for a new job are not unreasonable things to do.

😌I worked at an impatient hospital counseling at risk youth, before it closed. I had to stay,because someone called out for 3-11 shift ,the worse. By YAHWEH'S grace, I figured out if I volunteered for 11pm shift and still work the my regular 7am shift, which gave me 16hrs, I couldn't stay for 3-11pm shift. Stay safe! Endless Blessings 😌🙏🏽
