  • Managing Diabetes During Hospitalization: Tips and Strategies for Inpatient Care. Welcome to our new course, in this video, we will start by understanding the Differences between Home and Hospital Care, then we will discuss the following:
    The best strategy for blood sugar monitoring, and discussing the different blood sugar targets,
    How to recognize, treat, and prevent hypoglycemia in inpatient settings.
    Insulin physiology and the different insulin formulations frequently used in inpatient settings, and discuss other diabetes medications classes
    The best inpatient blood sugar control strategies
    Basa/bolus insulin therapy, insulin sliding scale, how to adjust your regimen based on blood sugar values including difficult-to-control hyperglycemia, early morning hypoglycemia, persistent hypoglycemia, and how to transition into and from insulin drip. management of DKA followed by NKHH with common pitfalls in management I see all the time. And we will finish by talking about practical issues when a diabetic patient is discharged from the hospital.

    What do you if a patient is only like 40 units insulin at home with 20 units TID AC and they only requiring like 15 units qHS 5 TId AC partly because they are in the hospital , blood sugars goals are looser, they don’t like the food, they food is a diabetic diet.. when you’re discharging them do you tell them to go back on home insulin dose?

      Actually that will be the topic of of my last video in this course -yet to be released- but, in general, if the patient has a good oral intake and they are planning to go back to their home diet then we can resume back their home insulin regimen. If there’s any doubt and you are not comfortable transitioning them back to home insulin then keep the hospital regimen and have them follow up with their primary care physician within a week.
