    BIRTH CONTROL METHODS
    IUD (Mirena, Kyleena) - progesterone only. Last 5 years. Can cause cramping with insertion. Spotting can last anywhere from 3-6 months but usually you will have no periods after that. I usually tell people to try to wait at least 6 months. You can feel the string to ensure the IUD is still in place. Partners don't usually complain of feeling the strings unless they were cut too short. They actually recommended in teens!
    Copper IUD - no hormones! Last 10 years but can maker periods heavier
    Nexplanon - progesterone arm implant. Last 3 years. Insertion can cause bruising. You will and should be able to feel the implant through your skin. Some people complain of pain in their arm. Can cause spotting or no periods. Hard to tell who will have which effect.
    Depo Shot - most associated with weight gain. can have no periods. Hard to tell when you ovulate again after coming off depo (period won't come but you may still ovulate).
    Birth control Pills - estrogen/progesterone vs progesterone only. Have to take the pills everyday around the same time. Risk of blood clots with estrogen. Increases migraine headaches. Can make periods lighter and shorter in duration. Other common side effects: acne, bloating, breast tenderness, mood changes.
    Patch - estrogen/progesterone. Same side effects as the pill. Can cause skin irritation.
    Nuvaring - estrogen/progesterone. Same side effects as the pill. Can cause vaginal irritation. Needs to be kept in the fridge.

    Common myth about birth control pills (OCP) - you should stop once in a while because it’s not good to be on OCP for that long and you might not be able to get pregnant later. Totally false.
    What other things have you heard about birth control and that made you wonder if they’re true or not?!

      That it is not normal to not get a period on birthcontrol like nexplanon. But in reality its totally normal and totally okay. Its not normal to not get a period when not on any birthcontrol.

      Depending on your age but usually yes! There is some underlying issue of someone is not getting their period (if they’ve had one in the past) and should be worked up by a doctor.

      My gyno told me that the IUD is not for me because I was never pregnant and have PCOS. I know plenty of women that never had a baby and have an IUD. And I read a lot of sources that say it's ok to have an IUD if you were not pregnant before and even if you have PCOS. What do you think?

      Rose it’s absolutely okay to have an IUD before ever being pregnant. Just as long as your aren’t planning to get pregnant in the near future because IUDs are normally used for long term contraception that can last from 3/4 to 10+ years depending on the type. You could still get one and then maybe you changed your mind a few months down the road and decide to try for a child. Nbd it can be removed. As long as you do not have any other contraindications for IUD use then you could totally have one placed. Being nulliparous is not a contraindication to having an IUD. I would suggest a new Obgyn/PCP. :)

    Can you do a video on the difference between medical doctors and doctors with Phds/AuDs (Psychologists, Audiologists, Chiropractors, Dentists)? Explain the difference in education, role, and what they are/aren't allowed to do. I think this would help alot of students who are considering the medical field! Thanks ❤

      shelby danaé horrible choice! Nexplanon is the worst birth control there is in the way it makes you feel but it’s very good at preventing pregnancy. But it will make you hate your life. I’m sorry if this sounds rude but man let me tell ya I’ve gained 30lbs and my period disappears for months at a time once went 108 days without one and thought I was pregnant but turns out it was just my birth control. I used to have the most regular period that came on the exact day every month and only lasted 3 days but now I go without one for months and I hate it because I can never tell what my body is doing. Plus my acne was bad before but now it’s 10x worse and my boobs are always sore and my stomach used to be very flat but now it’s round and bloated all the time and I can’t eat without feeling sick to literally get every pregnancy symptom on this and for what? To prevent these things

      Alexa Pike I mean the nurse did tell me I could possibly not have a period which is fine with me. I have notice my acne getting way worse. Bloating too. I’m honestly probably going to get it removed here soon. Probably just going to do no birth control.

    Having a period seems like something that should naturally occur. Is there no harm at all when you use a birth control that stops your period completely? I can't wrap my head around the idea that it's safe or without consequence of some kind. Can you provide some clarity? Thank you!

      When you’re exclusively breastfeeding, you not have periods and that’s natural. The hormones trick your body so you’re not actually building up a lining and so there’s nothing to shed

      @Helen Hines there are many women who go many years without mensuration. When they breastfeed and become pregnant in a short period (more common in days without birth control). Decrease cancer rates are also shown to multiparty.

    I’ve been on the combined pill for 3 years now but may have to change contraception due to weight gain. This video was so informative but as a med student I can’t help feeling like there should be a better option!!

    I took the pill on and off for 14 years. I have 5 children now, (none due to the pill not working lol). I recently got the Nexplanon because at this point, I just don’t want to risk it. It works well, I had bleeding/spotting for 3 weeks after insertion. After that it’s just been light spotting irregularly. No other symptoms.

    Very informative video. Thanks! I’m currently on the Nexplenon aka the rod in my arm, I’ve been on it first 5 years. I do want it removed as it’s caused me to keep 15 pounds on my body fur the past 5 years. It also causes non stop spotting.

    Heard that Mirena can suppress the development of endometriosis. Some have said it's as effective as Lupron. So for women who don't want to get pregnant and have endometriosis, Mirena might be an option.

      I did pills after Wyatt’s pregnancy and got the IUD after London’s delivery. Neither has affected my milk supply

      The skyla is an IUD that last 3 years but it’s getting phased out because it’s the exact same size as Kyleena and Kyleena last 5 years.

    I got my IUD removed in February I had it 5 years it was effective I didn't got pregnant but here was my side of effects I gained 100 pounds and I had severe anxiety attacks. I rather not go through that again. Plus my flow of my period was jacked up now that I got removed I naturally slowly losing weight and I'm slowly getting g off my anxiety medication I'm not getting attacks often anymore....I'm trying try new method...I'm doing pull out and condom method now. I'm considering the pill.

      Fairly simple procedure but you do have to put to sleep. Not easily reversible.
      I personally think it’s easier for male partner to get vasectomy (can be done in the office under just local anesthesia). Much safer and less invasive.

      Vasectomies can fail that is why they recommend semen analysis 6-8 weeks post procedure. Once you’re clear, it should not fail. Not sure what you mean by backfire. They are reversible.

    I’m terrified of getting iud because I’m don’t feel comfortable knowing something inside me. I’m also concern of the Implant because I just lost 20lbs I’m afraid of gaining 20lbs back. What are your thoughts?

      Great job on losing all that weight!! Hormones can make it harder for you too lose weight but the studies have show that it should not make you suddenly gain the weight back. My patients who continue on their lifestyle changes do just fine.

      I think this is very individualized. Everyone tolerates medications differently so we need to ask what kid of reactions you had in the past and past medical history.

      Teens can totally get the Copper IUD. Just know that it can make your periods heavier and/or longer. If you’re ok with that, then great option

    This is so helpful, Jenny! I am 32 yo and I was never on any BC (no kids, I was lucky with the pull out method 😊), and I went on the combined (low dose) pill last October but I’ve just found out that my mom’s had a blood clot so I’m higher risk now and I’m so scared. Should I switch to the mini? Is there no blood clot risk with the mini? Even though taking it at the exact same time is so scary if an accident happens 😭
    Generally I hate hormonal BC but the idea of an IUD as in a foreign object inside me freaks me out so much! And also I know I’m supposed to check if it’s in place I’m not able to put my fingers in there 😅 so I know I’ll never check! Or I would try and get my GP to check it for me if they agree.
    It just sucks because all options seem so bad 😭😭😭 why isn’t there a health BC method? It’s so hard being a woman! Please help 🆘

      Progesterone only BC does not increase your risk of clots. It’s just the estrogen. You can also track your periods with an app and use condoms or just not have sex during ovulation

    I have nexplanon, spotting comes and goes but i rather have spotting then a full period. Like spotting goes away for a couple of months and then itllappear for like a week or so and goes away again for a few months. My nexplanon lasts 4 years. i got it from pph :)

    Is it safe to get IUD? I have decided to get that. I was deciding between the IUD or Implant in upper arm. I have fibroid, very heavy painful prolong periods each month and suffer so much I don’t want period anymore. My doctor recommend me the IUD even though I am scared of it. Implant in arm look less scary.

      The IUD is much better at controlling heavy periods. You are more likely to have constant spotting with the nexplanon

    After getting pregnant at 19 I got the depo shot and LOVED when I was on it. My periods dissapeared and weight gain wasn't a problem for me. But I still lived with my parents and they are against bc so sneaking out every 3 months to get it and school i couldn't keep up. After getting off it was a pain. I bled every other week for about 3 months or so. I got pregnant again because it was hard to track everything. Now im thinking of getting nexplanon.

      Late reply, but I’ve been on Nexplanon since 2016 and I’ve not had any periods, but no pregnancies :) I don’t have any issues with it other than it’s a little harder to lose weight it seems. Also, when you go into get it replaced, they can put in a new one in at the same time as well. Hope this helps.

    Could you go into more detail with the birth control pills? For instance I’m about to start the sprintec for 28 days, I don’t quite understand the 3 weeks active pills and the placebo pills

      That just means that 3 weeks of pills have hormones and the last week are water pills. So you don’t even necessarily need to take them. It just help you remember to take a pill everyday.

      Priscilla Huoy hi! You can either take the placebo pills for that week to keep up the habit or not take anything for a week (keep track of the days) and you will have your period or just start a new pack and skip your period that month. You still might have occasional spotting if you decide to skip your period that cycle. I wouldn’t recommend skipping your period very often because the uterine lining can build up over time and this increases your risk of developing issues like increased risk of uterine cancer. I wouldn’t skip more than 3 periods in a row. There are some hormonal pill options out there that you will only have 4 periods a year and the max amount of time in between periods is 3 months. Hope this helps!

      @@daniellis9234 this is exactly my thought. even all the articles that call it "Breakthrough or Withdrawal bleeding" - its like hello call it what u want but your body is telling u (breaking through and withdrawing) that its not normal to not have a period on a regular basis and ur taking stuff to force it not to just cuz its convenient not to have a period (not just when ur breastfeeding exclusively) - idk i could be wrong but thats just MHO (not to mention the mood swings and weight gain with added hormones)

      Tara Hassan yeah so breakthrough and withdrawal bleeding can be because either the uterine lining built up too much to sustain itself or maybe you missed a dose or took it too late and you are already experiencing “progesterone withdrawal.” The estrogen builds up the lining and the progesterone maintains it (the two hormones in combo oral birth control pills - other methods work different ways). When you stop taking the pills on your period week (placebo pills) or if you are close to your period week and you accidentally skip a dose or take a pill too late you can have bleeding. You can stop the bleeding earlier by starting a new pack. Hormonal forms of birth control are not bad and skipping a period isn’t dangerous but the recommendation as of right now is don’t go more than 3 months. Uterine cells are rapidly growing and dividing cells because they have such a quick turnover each month to maintain the uterus for egg implantation. Because they are constantly growing and dividing there is a higher risk of spontaneous mutation. If you let those cells live longer than they should (aka don’t shed them and have a period for a really long time) your risk of having a mutation that is cancerous increases. It’s not a black and white picture but just a recommendation. Birth control is amazing and so many women have been able to delay child bearing and treat many conditions because of it. It is not without risks and there are side effects but there are many more risks and side effects associated with pregnancy and child birth. It’s a give and take. I’m so happy we have options for women!

    I would never recommend the Mirena IUD after my personal experiences. I have never had children, and was told it would be perfectly fine. I had it inserted at a public hospital (in Australia) and had it for 10 months. Those 10 months were great. I had no bleeding, nothing. But at the 10 month mark I had severe bleeding, horrible cramping and pain in my coccyx, mental confusion etc. Went to my doctor who checked the strings and thought it was ok. But, considering my pain and bleeding, she ordered an ultrasound. Turns out It was partially expelled. Then, I went to a private gynaecologist who recommended we try the IUD again. He said that it is so rare for the IUD to partially or fully expel and so my first one must not have been inserted correctly. That is even though I had an ultrasound to confirm placement 6 weeks after insertion, and it was definitely in the correct place. I had my second one inserted, and 8 months later, the same thing happened. Never again. And I could not believe when another gynaecologist recommended I try AGAIN!
    I know this is only my experience and everyone is different, but I think it absolutely sucks when these things happen. The expulsions have wreaked havoc on my pelvic floor because they went into spasm trying to protect everything, and so I now have an overactive pelvic floor and horrible coccyx pain that flares up with my periods. I'm having to see a pelvic floor physiotherapist because of it.

      Usually we recommend progesterone only for breastfeeding because estrogen can decrease milk supply. But birth control shouldn’t affect milk supply of breastfeeding and milk supply is already well establish (no matter the type of BC).
      I started combo birth control pills at 4 months after Wyatt was born and still had an over supply. I got the Mirena IUD 6 weeks after London was born.

    Can breastfeeding moms have the iud? I don't want to see a drop in my milk.

      Yes because it’s progesterone only. I have the IUD. But any birth control is fine as long as breastfeeding and milk supply is already established. I started birth control pills when Wyatt was 4 months and still had a slight over supply

      @@drjennale How did you go about it with your partner when deciding to go on birth control? I don't plan on having a baby for the next 4 years while in grad school. I tried talking and educating my partner about being on the IUD or any birth controls but he's doesn't agree that I should. I know it's my body and choice. Feeling puzzled.

    What is the difference between the combo birth control pills vs the birth control pill with only progesterone? most obvious that one has estrogen and the other does not. However, does the combo have more side effects? Does the combo also have to be taken around the same time? Is one typically more effective than the other?

    So I got pregnant on the pill. I started at 148lbs and ended up at 210 when I got pregnant (7 weeks)
    And after I gave birth I went from 210 to 183. I got the Nexplanon, and now I'm back up to 200. What would you recommend for someone with a slow metabolism?

    Is there a waiting period after coming off of birth control to wait to get pregnant? I have the Kyleena IUD and will likely get it out in 3 years to have a baby, is it safe to TTC right after getting it out, or should I wait a few months to let my body get back to normal?

      I think skyla IUD is a great option. It’s a little bit smaller and releases a little less hormone daily as compared to the Mirena. And it last 3 years instead of 5 years. My only caution for people if that if you’re getting the IUD for heavy periods and hoping to get no periods, you’re more like get go that with Mirena and not skyla.

    The main reason I take the pill is because my piriod is very painful and heavy and unpredictable. The only problem is it negatively affects my sex drive down to nil and I have to take it every day which doesn't always happen. I'm looking into the other options available and want to know what is the best recommendation for me and my husband. We also don't want any unplanned pregnancy so we're super nervous about that as well. Any advice would be welcome.

      You can ask your doctor about a lower dose hormone pill. But if you are not planning to be pregnant within the next 3-5 years then there are longer birth control options like IUD or Nexplanon.

      Depo is a little harder to predict when you will ovulate after stopping the shot because it is stored in your fat cells. But in general, majority of people will be able to get pregnant after 3 months.

    Been on the depo shots for about two months and not a big fan of it. Been getting headaches on and off and spotting that can last for two weeks. Thinking of maybe switching to IUD or the insert. Not sure what to do.

      Definitely talk to your doctor about your goals. I personally have people complain more about the depo shot than other forms of BC. The Nexplanon (arm insert) is more likely to have spotting than the Mirena IUD.

      Jenny Le Thanks, I was able to talk to a nurse practitioner they had and she told me that the spotting was normal and to monitor my headaches. And if it continues then maybe I should schedule an appointment with my doctor to see if I can switch birth control and see my options.

    Which birth control is the best to get to start trying to conceive shortly after removal/ discontinuing? I have to get surgery and would like i removed shortly afterwards.

      All of them besides depo. Once stopping or after removal - you can become pregnant

      Just because an IUD can last 5 years, does not mean you have to use it that whole time. You can take it out any time and start trying for baby #3!

      All birth control that is progesterone only will not decrease your milk supply - IUD, nexplanon, depo shot, and progesterone mini pill.

    I’m surprised you failed to mention that Depo shot causes bone density loss so you shouldn’t take it for more than 2 years consistently and you can’t get pregnant immediately

    do patients ever have the option to be sedated before the insertion of an IUD?

      Sedation is more dangerous than IUD insertion itself. Unless it is coupled with a D&C then very rarely is that offered. Pre-medicating with 600-800 ibuprofen can help with pain. Or a Xanax or anxiety.

      The copper IUD is non hormonal. But IUD in general are associated with less weight gain.
      Personally I was only birth control pills for over a decade and it never inhibited me from losing weight

      Depo shots are every 3 months so it’s possible if you ovulated after 12 weeks! Always take a pregnancy test. Dollar tree ones work just as good.

    I was on nexplanon and would have two week long periods with only a one week break in between. I kept it in for the whole three years and got on the pill, lo loestrin to be exact, and it complete changed my mood. It made me really depressed and feel bad in general. I've been reading about the dangers on hormonal birth control in general and now I am scared to try anything else.

      Best they contain estrogen. Any birth control with estrogen have that risk. But the risk is low.

    Hi Jenny! I’m 18F and I’ve had my kyleena iud for also 5 months now, I thought this would be the holy grail after going through both the pill and patch and reacting very poorly (major nausea, due to high amounts of estrogen) but it’s been no stop cramping, 85% of my time is spent laying my bed with a heating pad, taking large amounts of Advil and Aleve to make the pain bearable. I’ve seen my gyno and many doctors and the iud is for sure in the right place, but I’m worried that there will be no end to this. Perhaps the iud is just not for me…I’m considering my opinions of switch to nexplanon or the nuva ring and any advice of yours would be greatly appreciated!❤️

      The IUD has no estrogen. But it is still possible to be causing some of your symptoms. Both the nexplanon and Nuvaring are great alternatives! Would talk to your doctor about risk and benefits of both

      I have the Mirena IUD. You can get the copper IUD and take it out whenever you want, when you’re ready to start trying to have children. It can last up to 10 years if you want it to (doesn’t mean it has to).
